Tuberculoma in a 3-year-old child (3세 소아에서 발생한 결핵종)

  • Tuberculomas of the lung are rare in children and one of the more common lesions presenting a solitary pulmonary nodule roetgenorgraphically. We are reporting of a 3-year-old child with a tuberculoma in left upper lobe. The patient was initially diagnosed as the benign mediastinal tumor but in the end as tuberculoma in left uper lobe. Wedge resection including the mass was done. The tumor had brown smooth external surfaces on sectioning including the mass was done. The tumor had brown smooth external surfaces on sectioning show pale gray and soft cut surface was shown. In light electromicroscopy chronic granulomas with multinucleated giant cells and central caseous necrosis were observed which are the characteristics of tuberculoma. The postopeative course was smooth and uneventful and patient has been well for 4 months postoperatively.

  • Extrapelvic endometriosis is a rare disease. The majority of extrapelvic endometriosis cases involve scar tissue following obstetric and gynecologic procedures. We have treated two cases of extrapelvic incisional endometriosis. A 39 year old female patient with cyclic vaginal spotting after laparoscopic assisted vaginal hysterectomy due to uterine myoma and a 35 year old female patient with a painful palpable abdominal mass after cesarean section. Both underwent complete excision and were proven to have endometriosis by pathology. Here we report on both cases and review the medical literatures.

  • Kartagener`s syndrome is a clinical entity comprising a combination of situs inversus, bronchiectasis, and sinusitis or nasal polyposis. This syndrome is rare and is usually seen in a young age group. The syndrome is punctated by recurrent upper respiratory tract infection and pneumonia. This is a report of Kartagener`s syndrome found in 18 years old male and 21 years old female patients who were received surgical treatment of bronchiectasis. The male patient was performed right transposed lingular segmentectomy and lower lobectomy and female patient was perforated left transposed middle lobectomy and lower lobectomy. Both patients were discharged with good results.

Norwood Procedure with Home-made Bovine Pericardial Patch (우심낭 조직편을 이용한 Norwood 술식 -2예 보고-)

  • A patch design using bovine pericardial patch for aortic enlargement in the Nowood procedure has been introduced to avoid problems related to homograft availability. We report 2 successful cases of Norwood procedure with home-made bovine pericardial patch. The first case was a 23-day-old (2.2 kg) patient with multi-level left ven-tricular outflow tract obstruction with ductal-dependent systemic circulation. The other case was a 9-day-old (3 kg) patient with hypoplastic lefi heart syndrome. This technique was relatively easy to perform, reproducible and quite effective like homograft patch.

Clinical Analysis of Chest Trauma; Analysis of 247 patients (흉부 손상의 임상적 고찰)

  • Clinical analysis were performed on 247 cases of thoracic trauma, those were admitted & treated at the department of thoracic & cardiovascular surgery,Hanyang University Hospital during the period from Jan,1989 to June,1992. Age distribution of those was from 2 to 80 years old & mean age was 38 years old. The ratio of male to female patient was 186:61 [3:1].This ratio revealed high incidence in male patient. The most common cause of trauma was traffic accident in this series.The modes of injury were as follows: traffic accident 124 cases[50.2%],fall down 52 cases[21.05%], stab wound 47 cases[19.03%] and gun-shut wound 1 case.Ellapse time from accident to admission were 141 cases [57.09%] under 6 hr.Rib fracture were observed in 159 cases[64.37%], hemo or pneumothorax were observed 134 cases[54.25%] of total cases and location distributed Right:Left:Both[74:112:37], in left predominant. Conservative,non-operative treatment were performed in 128 cases and operation[open thoracotomy] 32 cases.Mortality was 1.6%[4 cases] & most common cause of death were due to irreversible shock with brain edema. Conclusively, more evaluation & co-operation of other department were expected treatment & better prognosis.

Case Reports of Rieger's Syndrome (Rieger증후군의 증례)

  • The authors observed the two cases of Rieger's syndrome in 21-year-old male and 13-year-old female with a chief complaint of Partial anodontia of permanent teeth on both jaws. We had done the laboratory and ophthalmic examinations and had taken radiographs. Final diagnosis was established as Rieger's syndrome. We obtained the results as follows, 1. There was no peculiar hereditary tendency in them. 2. The patients had no disturbance of general physical activity and mentality. 3. The male patient had pseudoprogenathism, concave profile, congenital missing on upper anterior and second premolar teeth, and conical shaped crown of upper len celral incisor. The female patient had also pseudoprognathism, concave profile, thickened upper labial frenum, decreased upper dental arch width, congenital missing on upper anterior teeth, and delayed eruption of second premolars. 4. The ophthalmic symptoms that were ins abnormalities, decreased visual acuity, and increased intraocular pressure were present. 5. The abnormalities except dental and ophthalmic abnormalities were not round.

Tracheoinnominate Artery Fistula after Tracheal Reconstruction and Tracheostomy (기관재건술 및 기관절개술 후 발생한 기관무명동맥루)

  • Tracheoinnominate artery fistula is a rare but a catastrophic complication after tracheostomy or tracheal reconstruction. We experienced two cases of tracheoinnominate artery fistula after tracheal reconstruction and tracheostomy. The first patient was a 11 year old girl with cerebral arteriovenous malformation who maintained tracheostomy for 6 months before undergoing tracheal reconstruction. Three days after tracheal reconstruction, massive bleeding occurred through the intubation tube. She underwent emergency reoperation of repair the innominate artery with 5-0 Prolene and reconstruction of trachea. She died of bleeding 3 days after the reoperation. The second patient was a 68 year old man who underwent right upper lobectomy due to lung cancer. After operarion MRSA Pneumonia was developed and tracheostomy was performed 10 days after intubation. Twelve days after tracheostomy, massive bleeding occurred and emergency operation of ligation of innominate artery was performed. He died of sepsis 7 days after reoperation.

  • The postoperative respiratory complications such as pneumonia and ARDS, are poor prognostic factors after esophagectomy in patients with esophageal cancer. To avoid these complications, there have been attempts to use minimally invasive approach. Recently introduced daVinciTM S surgical system is used in esophagectomy because of its advantages of minimal invasiveness, clear 3-dimensional imaging and precise handling of robotic arms. We report a 83-year-old esophageal cancer patient who underwent daVinciTMS robot-assisted esophagectomy, laparoscopic stomach mobilization followed by cervical esophagogastrostomy.

  • Objectives This study is report the effect of Asian Medical treatments for acne. Methods 25 years old mixed Hispanic female in California resident patient who was suffering from acne was treated with the Chung-We mix Extract powder, Houttuyniae Herba Extract and comedone extractor. She was taken a picture to compare with before and after treatments. Results Her acne was decreased and global acne grading system by 35 points to 7point improvement on the after treatments. Conclusion This case has the effectiveness with Herbs. According to Asian medicine, the caused of acne is internal problems and the acne is diagnosed as several kind of heats in the internal organ. Further clinical case of acne is needed for better results and to develop the ways of accurate diagnosis.

Sepsis Developed from an Odontogenic Infection (근막간극감염에 의한 패혈증 연구)

  • Mortality associated with maxillofacial infection is relatively low due to the development of antibiotics, and improved oral care. However, inappropriate treatment, delayed treatment, old age, underlying systemic disease, and drug-resistant micro-organisms can potentially result in life threatening situations such as cavernous sinus thrombosis, mediastinitis, and sepsis. Sepsis is the most dangerous state with high mortality, ranging from 20~60%. The treatment of sepsis involves properly monitoring vital functions, fluid resuscitation, surgical drainage, and empirical use of high doses of antibiotics until culture results are available. Ventilatory support maybe be required as well. We encountered a 64-year-old patient who died from sepsis that developed as the result of an odontogenic infection. The initial diagnosis was right temporal, infraorbital, buccal, pterygomandibular space abscess. Despite surgical and medical supportive care, the condition progressed to sepsis and after four days the patient died due to multiple organ failure.
