Effects of Occupation-Based Bilateral Upper Extremity Training for Chronic Stroke: Pilot Study (뇌졸중 환자의 작업기반 양측성 상지훈련 효과: 예비연구)

  • Objective : The purpose of this study is to observe effects of Occupation-Based bilateral UE training for chronic stroke. Methods : 4 patients with hemiplegic stroke were divided into two groups: experimental group and control group. From May to August 2016, we conducted a occupation - based bilateral UE training and a traditional bilateral UE training for 5 weeks, 3 times a week, and 60 minutes per session for each subject. EMG, accelerometer, ARAT, Y-BAT, SIS, and COPM were performed before and after the intervention. Results : The experimental group showed a large changes before and after intervention in ECR, DA and all items except for hand function and memory of SIS, compared with the control group. Also, the experimental group showed a large change before and after the intervention compared to the control group in the accelerometer measuring the amount used of bilateral hand and grasp, pinch, and gross movement items of ARAT and COPM, Y-BIT. Conclusions : Occupation-based bilateral upper extremity training was effective in the recovery of upper limb function and social participation through neurological changes. Future research will be required development studies to ensure a high content validity of the Occupation-based bilateral upper extremity training protocol.

Effects of the Education about the Self-application of the Kinesio-Taping for the Adult Women with Chronic Low Back Pain on Pain, Satisfaction and Gait (키네시오 테이핑 자가적용 교육이 만성요통 성인 여성들의 통증, 만족도 및 보행에 미치는 영향)

  • We aimed to analyze the effects of Kinesio-Taping on pain, satisfaction, and gait, and to provide basic data on the self-application of this method. The participants were 30 adult women with chronic low back pain. Kinesio-Taping was applied for 2 weeks, followed by 4 weeks of self-application education and self-application. The participants' pain levels were analyzed using the visual analog scale, satisfaction was analyzed using the Canadian Occupational Performance Measure, and gait was analyzed in terms of stride length, step length, step width, and gait time using the Walkway MG-1000 gait analyzer. The results of the pain showed the statistically significant decrease steadily (p<0.001) and the results of the satisfaction showed the statistically significant increase steadily (p<0.001) from the application of the Kinesio-Taping. Also, according to the In-Object verification between pre-test, taping application (after 2 weeks) and Self-application (after 4 weeks), the results of the satisfaction showed statistically significant difference (p<0.001). According to the gait analysis, the results of the stride length and step length showed the statistically significant increase steadily (p<0.001) and the results of the step width and gait time showed the statistically significant decrease steadily (p<0.001) from the application of the Kinesio-Taping. Also, according to the In-Object verification between pre-test, taping application (after 2 weeks) and Self-application (after 4 weeks), the results of the step width showed statistically significant difference (p<0.001). Thus, the self-application education about the Kinesio-Taping should be considered to reduce the pain and improve the performance level of the activities of daily life through the improvement of the self-management ability and to induce the active participation in the self-management activities and increase the satisfaction.

Effects of Participating in Tele-rehabilitation Service on Health Promotion and Quality of Life for Individuals with disability (원격재활서비스 참여가 지역사회 장애인의 건강증진 및 삶의 질에 미치는 영향)

  • The aims of this study was to investigate the effect of the Tele-rehabilitation service on health promotion and quality of life for individuals with disability. The subjects were 3 men who agreed with participating in service and were assessed of health promotion level with physical/psychological scales and quality of life. The service consists of 20 sessions for 13 weeks. The results are as follows: First, all participants improved psychosocial health promotion (subjective health level) factor. Second, physical health promotion (upper limb function, strength, range of motion) maintained and improved, but there was little change. Third, the quality of life was higher than before. In addition, qualitative research showed that participants experienced a positive change in subjective health status, emotional stability, benefit, and self efficacy, and they were satisfied with the service. Based on the results, it was shown that participation helps improve the health and quality of life of the individuals with disability in the community. This study can be used it as a basic data for establishing a Tele-rehabilitation service for individuals with disability in local community.

Translation and Content: Validity Verification Study of the Korean Version of the School Setting Interview (K-SSI) (School Setting Interview(SSI)의 번안 및 내용타당도 검증)

  • Objective : This study was conducted to validate the content validity of the Korean version of the school setting interview (SSI) that assesses the adequacy of school environments for students aged 9-19. Methods : The Korean version of the SSI, which was completed via a translation-reverse translation process, was verified by conducting a questionnaire on category suitability two times with 35 experts. Results : The content validity index (CVI) for the entire K-SSI tool was .90 in the first test. The "Maps and diagrams" measurement in reading items and the "Taking a shower" measurement in sports activities were both below .75. A second round of verification was conducted after the CVI was modified. As a result, the secondary verification CVI results were further raised to .93. Conclusion : This study confirmed that the K-SSI contains items that are appropriate to Korean culture; it has been proven to have high content validity. Future continuous research and clinical use are required to study the validity and reliability of the K-SSI. Further, this tool should also be applied to students with diverse disabilities who face difficulties in school settings.

The Definition of Frail Elderly and the Frailty Screening Assessment Tool: A Systematic Review (허약노인의 정의 및 허약 선별 평가도구에 관한 체계적 고찰)

  • Objective : The objective of this study was to present the components of frailty by organizing the definitions of frail elderly and analyzing the tools used to screen them. Methods : This study searched for articles at involved frailty screening assessments in the elderly. Databases including CINAHL, Embase, Medline Complete, and PubMed were searched. The search terms were "assess" AND "frailty" AND "screening" AND ("frail elderly" OR "elderly"). Results : A total of 539 articles were identified by the search and 11 articles were selected. Frailty occurs due to the depressed function of multidimensional factors, and a frail elderly person is defined as one at high risk of health degeneration, functional impairment, and occurrence of disability, and having a high level of threat to life. Seven tools were selected from 11 articles. The most frequently used tool was the frailty phenotype, which was used in five articles (45.4%). The identified components of frailty were physical, activity participation, nutrition, psychological, social, overall health, and age. Conclusion : The results confirmed the definition and components of frailty. This study is expected to contribute to the future development of standardized evaluation tools for screening frail elderly individuals and intervention programs for the management of the frail elderly.

A Comparative Study on Vascular Conditions and Stress on Leisure Activity in the Elderly (노인의 여가활동에 대한 혈관상태와 스트레스 비교연구)

  • Objective : The purpose of this study was to find out the stress of vascular conditions and leisure activities for the elderly and the effect of stress problems on the vascular conditions of the elderly. Methods : The study surveyed 172 elderly people, conducted a validity and reliability analysis to organize items of leisure activities, analyzed vascular conditions and stress for leisure activities, and conducted a regression analysis to analyze stress factors affecting the blood vessel conditions of the elderly. Results : First, there was a significant difference in comparing the average value of vascular conditions for leisure activities in the elderly (p=.012) and sports activity was the most stable in vascular conditions. Second, there was a significant difference in comparing the average value of stress for leisure activities of the elderly (p=.035) and rest and other activities were higher than other activities. Third, the effect of stress problems in older people on vascular conditions has been shown to be influenced by financial difficulties and health problems. Conclusion : Leisure activities in older people can act as a factor in changing the quality of life. Older adults can expect improvement of vascular conditions and reduction of stress through leisure activities, especially sports activities have a high relationship of vascular conditions, and rest and other activities have a high relationship with stress. As a result, this study confirmed the importance of selecting and continuing participation in leisure activities for the elderly.

Factors Affecting Activity Restriction in the Elderly with Chronic Disease: Using data from the 8th period of the National Health and Nutrition Examination Survey (만성질환 노인의 활동 제한에 영향을 미치는 요인: 국민건강영양조사 제 8기 자료를 활용하여)

  • In this study, a complex sample logistic regression analysis was performed to identify the factors affecting the activity restriction of 2,701 normal elderly and chronically ill elderly aged 65 and over using raw data from the 8th period of the National Health and Nutrition Examination Survey. It was found that the elderly with chronic disease felt more restricted in their activities than the normal elderly. Activity limiting factors in stroke and hypertension patients are subjective health status, economic level, stress perception, and moderate-intensity work and leisure. The factors limiting activity in patients with heart disease were subjective health status and economic level, and factors limiting activity in patients with joint disease were subjective health status and high-intensity work and leisure. Activity limiting factors for lung disease patients are education level, high intensity work and leisure, and endocrine system activity limiting factors include subjective health status, stress perception, high intensity work and leisure, and activity limiting factors for cancer patients. is subjective health status, stress perception, moderate-intensity work and high-intensity leisure. Rehabilitation programs and policy support are needed for the continuous participation of the elderly with chronic diseases.

Survey About Current Status of Pediatric and Adolescent Physical Therapy: Focus on Pediatric and Adolescent Rehabilitation Hospitals in Seoul and Gyeonggi Province (소아 청소년 물리치료 실태 조사: 서울 경기 지역 소아 청소년 재활병원을 중심으로)

  • Objective : This study aimed to investigate the current status of physical therapy in children and adolescents. Methods : Sixty questionnaires from physical therapists treating children and adolescents with disabilities were analyzed. The questionnaire consisted of questions on physical therapy, participants, satisfaction, and the assessment of pediatric and adolescent physical therapy. Descriptive statistics and frequencies were used to investigate the current status, participants, and satisfaction. Differences between physical therapy participation difficulty, importance-ability of major aspects of pediatric and adolescent physical therapy, and therapy goal frequency were analyzed using paired T-test. Results : 11 to 15 cases (66.7%) and one-on-one treatment (95.0%) were performed independently (95.0%). The main ages of the subjects were preschool and school, the diagnoses were brain lesions and developmental delay, and treatment was conducted for up to 20 years or older. Satisfaction with pediatric and adolescent physical therapy was high (70.0%), as was the intensity of work (71.7%). Neurodevelopmental therapy, gait training, and goal-directed rehabilitation were the main treatments, and Gross Motor Function Measures of 88 and 66, respectively, were used. Respondents said that current fee system is inadequate (66.1%) and appropriate fee system is needed. Conclusion : This study extensively investigated the content of and factors related to pediatric and adolescent physical therapy. Based on the current situation, efforts to improve the expertise and continuity of pediatric and adolescent physical therapists and apply the latest treatment techniques are required.

A Study on the Dose Constraints for Occupational Exposure: Focusing on Expert Opinions by Field of Ridiation Industry (직무피폭의 선량제약치에 관한 연구: 분야별 전문가 의견 중심으로)

  • A Study on the Introduction of Dose Constraints for Occupational Exposures: Focusing on Experts' Opinions by Field of Radiation Industry. The International Commission on Radiological Protection suggests Justification, Optimization, and Dose Limits as the three principles of radiological protection, among which, as a means of protection optimization, ICRP 103 recommends to set dose constraints. In this study, opinions are collected from experts in each category of radiation industries for stakeholder participation to qualify dose constraints. A guidance and questionnaire for analyzing the dose constraints have been developed for their collection, and opinions were collected from radiation protection experts in selected categories. 20 out of 22 experts, consisted with 91%, have assessed the dose constraints setting is necessary, and 2 experts, consisted with 9%, assessed it is unnecessary. The average of dose constraint presented by experts for RI production institutions is to be the highest level of 15.3 mSv, and light-water reactors (14.6 mSv), non-destructive inspection (14.4 mSv), heavy-water reactor and medical institutes (13.9mSv) is to be above the overall average dose constraint. In case of public institutions, the average dose constraint is to be 8.6mSv, and research institutions (8.8mSv), educational institutions (9.6 mSv), waste disposal sites (9.7 mSv), and general industries (10.6 mSv) are resulted to below the overall average dose constraint. As for the means of setting dose constraints, 8 experts out of 22 suggested setting dose constraints for each specific industry or task. And, 5 experts especially suggest setting dose constraints for the specific groups with relatively high exposure, such as workers with above the record levels. As a countermeasure for workers who exceed the dose constraints, 15 experts out of 22 expressed that the cause analyses for them and preparation for a plan of reducing them are necessary.

  • The Korea Government adopted the gender mainstreaming system to realize gender equality. The active participation of members, organizations and groups of the society is the most important, for the systematic and continuous working of the gender mainstreaming system. We suggest a model for constructing gender governance with the women, the neglected classes, the educational group, the occupational group, the research group, NGOs, the medias, the government, the council, and the gender impact analysis and assessment center, etc.
