진단방사선 및 핵의학 검사에 의한 한국인의 의료상 피폭 (Medical Exposure of Korean by Diagnostic Radiology and Nuclear Medicine Examinations)

  • 의료 목적으로 X선 촬영이나 CT, PET과 같은 진단방사선 피폭은 불가피하지만 선진국에서 의료 방사선이 최대의 인공 피폭원을 구성하고 있고 또 빠르게 증가하는 경향에 있음을 고려하면 의료상 피폭의 특성이나 그 결과로 인한 환자 선량 크기를 이해하는 것은 매우 중요하다. 이에 2002년도를 기준으로 단위 진료행위별 선량과 국내 의료보험 통계자료를 결합하여 방사선 진료절차별 집단선량과 1인당 유효선량 평가하였다. 절차의 유효선량 값은 NRPB 보고서, ICRP 80, MIRDOSE3.1 및 우리가 독립적으로 산출한 자료들로부터 편집하였다. 평가 결과 연간 집단선량은 진단방사선 22880man-Sv, 핵의학 4560man-Sv로서 총 27440 man-Sv로 나타났으며 따라서 인구 4770만 명을 나눈 1인당 연평균 의료상 피폭선량은 0.58mSv였다. 이 집단선량은 2002년 16기의 원전을 가동한 우리나라의 직업상피폭 70man-Sv보다 크게 많다. 특히 CT 촬영만의 집단선량도 9960man-Sv에 이름은 주목할 일이다. 이 결과는 국가의 방사선방호 정책이 의료에서 환자선량 최적화에 보다 주목해야 함을 시사한다.

방사성 의약품 자동 분주장치의 유용성 및 최적화된 분주방법에 관한 고찰 (A Study on the Usefulness of Auto Dispenser and Optimized Dispensing Method)

  • 최근 방사선 작업종사자의 외부피폭을 감소시키기 위하여 방사성 의약품 자동 분주장치를 사용하는 병원이 증가하는 추세이다. 방사성 의약품 자동 분주장치는 주사기의 피스톤을 잡아당기는 거리(mm)를 조절하여 방사성 의약품을 자동 분주 하도록 설계되어 있어 장비에 장착한 바이알 내 방사성 의약품의 비방사능이 클 때 소량의 원하는 단위 방사능의 방사성 의약품을 분주하기 어렵다. 따라서 본 연구에서는 방사성 의약품 자동 분주장치의 방사능 차폐능력을 평가하고 장비의 분주 재현성을 극대화하는 최적화된 분주방법을 찾고자 세 가지 실험을 진행하였다. 첫 번째 실험은 자동 분주장치의 사용여부에 따른 방사선 작업종사자의 손과 흉부의 피폭 비교 실험이다. 환자에게 $^{18}F-FDG$ 투여 시 30명은 자동 분주로, 30명은 수동으로 분주하여 투여하고 주사과정에서 방사선 작업종사자가 받는 손과 흉부의 피폭선량을 측정하여 비교하였다. 흉부 피폭선량은 독립표본 t-검정 결과, 분주방법에 따라 유의한 차이가 있었고 손의 피폭선량은 자동 분주 시 오른손 검지에서 11.5배 왼손 검지에서 4.8배 낮게 측정되었다. 두 번째 실험은 자동 분주장치의 분주 용량 재현성 평가이다. 생리식염수가 담긴 바이알을 장비에 장착하고 분주하고자 하는 용량을 1부터 15 mm까지 1 mm씩 증가시키면서 5회씩 분주하였을 때 주사기의 무게를 뺀 분주된 생리식염수만의 무게를 마이크로 저울로 측정하여 표준편차와 변동계수를 비교하였다. 그 결과, 표준편차는 분주하고자 하는 용량의 크기가 커짐에 따라 불규칙하게 나타났지만 변동계수는 감소하는 추세를 나타냈다. 또한 작은 크기의 주사기를 사용하였을 때 더 낮은 표준편차와 변동계수를 나타냈다. 세 번째 실험은 자동 분주장치의 분주 방사능 재현성 평가이다. 장비에 장착된 바이알 내 $^{18}F-FDG$의 비방사능이 다를 때 70 kg 환자를 기준으로 kg당 5.17 MBq (0.14 mCi)인 362.6 MBq (9.8 mCi)${\pm}10%$를 분주 후 동일 용량에서 4회 더 분주 하여 dose calibrator에서 방사능을 측정하고 그 표준편차와 변동계수를 비교 분석하였다. 그 결과, 장비에 장착된 바이알 내 $^{18}F-FDG$의 비방사능이 증가할수록 표준편차와 변동계수 모 두 증가하는 추세를 보였으며 작은 크기의 주사기를 사용할 때 더 낮은 편차를 보였다. 또한 바이알 내 $^{18}F-FDG$의 비방사능이 2 mL 주사기를 사용할 때에는 약 1,020 MBq/mL (27.6 mCi/mL) 이하, 5 mL 주사기를 사용할 때에는 약 915 MBq/mL (24.7 mCi/mL) 이하일 때 변동계수를 10% 미만으로 유지시키며 더욱 유용하게 사용할 수 있음을 확인하였다.

Use of Rank Sum Method in Identifying High Occupational Dose Jobs for ALARA Implementation

  • The cost-effective reduction of occupational radiation exposure (ORE) dose at a nuclear power plant could not be achieved without going through an extensive analysis of accumulated ORE dose data of existing plants. It is necessary to identify what are high ORE jobs for ALARA implementation. In this study, the Rank Sum Method (RSM) is used in identifying high ORE jobs. As a case study, the database of ORE-related maintenance and repair jobs for Kori Units 3 and 4 is used for assessment, and top twenty high ORE jobs are identified. The results are also verified and validated using the Friedman test, and RSM is found to be a very efficient way of analyzing the data.

Development of Moving Alternating Magnetic Filter Using Permanent Magnet for Removal of Radioactive Corrosion Product from Nuclear Power Plant

  • Radioactive Corrosion Products (CRUD) which are generated by the neutron activation of general corrosion products at the nuclear power plant are the major source of occupational radiation exposure. Most of the CRUD has a characteristic of showing strong ferrimagnetisms. Along with the new development and production of permanent magnet (rare earth magnet) which generates much stronger magnetic field than the conventional magnet, new type of magnetic filter that can separate CRUD efficiently and eventually reduce radiation exposure of personnel at nuclear power plant is suggested. This separator consists of inner and outer magnet assemblies, coolant channel and container surrounding the outer magnet assembly. The rotational motion of the inner and outer permanent magnet assemblies surrounding the coolant channel by driving motor system produces moving alternating magnetic fields in the coolant channel. The CRUD can be separated from the coolant by the moving alternating magnetic field. This study describes the results of preliminary experiment performed with the different flow rates of coolant and rotation velocities of magnet assemblies. This new magnetic filter shows better performance results of filtering the magnetite at coolant (water). How rates, rotating velocities of magnet assemblies and particle sizes turn out to be very important design parameters.

Assessment of occupational radiation exposure of NORM scales residues from oil and gas production

  • Radiological hazards from external exposure of naturally occurring radioactive materials (NORM) scales residues, generated during the extraction process of oil and gas production in southern Algeria, are evaluated. The activity concentrations of 226Ra, 232Th, and 40K were measured using high-purity gamma-ray spectrometry (GeHP). Mean activity concentration of 226Ra, 232Th and 40K, found in scale samples are 4082 ± 41, 1060 ± 38 and 568 ± 36 Bq kg-1, respectively. Radiological hazard parameters, such as radium equivalent (Raeq), external and internal hazard indices (Hex, Hin), and gamma index (Iγ) are also evaluated. All hazard parameter values were greater than the permissible and recommended limits and the average annual effective dose value exceeded the dose constraint (0.3 mSv y-1). However, for occasionally exposed workers, the dose rate of 0.65 ± 0.02 mSv y-1 is lower than recommended limit of 1 mSv y-1 for public.

Personal Dosimeters Worn by Radiation Workers in Korea: Actual Condition and Consideration of Their Proper Application for Radiation Protection

  • Background: Assessment of the radiation doses to which workers are exposed can differ depending on the placement of dosimeters on the body. In addition, it is affected by whether the placement is under or over a shielding apron. This study aimed to evaluate the actual positioning of personal dosimeters on the body, with or without shielding aprons, among radiation workers in Korea. Materials and Methods: We analyzed the survey data, which included demographic characteristics, such as sex, age, occupation, work history, and placement of the personal dosimeter being worn, from a cohort study of Korean radiation workers. We assessed the use of personal dosimeters among workers, stratified by sex, age, working period, starting year of work, and occupation. Results and Discussion: Overall, high compliance (89.1% to 99.0%) with the wearing of dosimeters on the chest was observed regardless of workers' characteristics, such as age, sex, occupation, and work history. However, the placement of dosimeters, either under or over the shielding aprons, was inconsistent. Overall, 40.1% of workers wore dosimeters under their aprons, while the others wore dosimeters over their aprons. This inconsistency indicates that radiation doses are possibly measured differently under the same exposure conditions solely owing to variations in the placement of worn dosimeters. Conclusion: Although a lack of uniformity in dosimeter placement when wearing a shielding apron may not cause serious harm in radiation dose management for workers, the development of detailed guidelines for dosimeter placement may improve the accuracy of dose assessment.

임금근로자의 고용형태에 따른 건강수준의 차이 분석 - 유해위험요인 노출을 경유한 영향 경로 - (Analysis of health inequality by the type of employment on health among employees - Pathway of the effect via the exposure of hazardous factors -)

  • Flexible production system has been expanded to all sectors of economy with the change of labor market and diversification of employment. The unstable employment with irregular work has replaced stable employment with regular work. This study has investigate the impact pathway of the type of employment on health status especially ill-health symptoms experiences. Among the first Korean Working Conditions Survey data, the employee's response data was used to analyze the path way with multiple regression analysis. The result has shown the direct effect of the type of employment on ill-health symptoms experience. Indirect effect of the type of employment was found the pathway via the exposure to noise, high temperature, low temperature, dust, skin contact to chemicals, painful posture, heavy material handling, standing position, repetitive movement of hands. However the exposure to the other hazardous factors such as organic solvent, wholebody vibration, radiation, lifting people, infectious materials were not influenced by the type of employment.

항공 승무원의 방사선 지식에 영향을 미치는 요인 분석 (Analysis of Factors Affecting Radiation Knowledge among Aircrew)

  • Objectives: This study identified factors impacting radiation knowledge among aircrew, who are affected by cosmic radiation exposure due to their occupational environment. Methods: In September 2019 we conducted an online survey of aircrew through a Google link. We evaluated the level of radiation knowledge using a ten-item (10 points) questionnaire. The following exploratory variables were evaluated in relationship with the level of radiation knowledge using univariable linear regression models: sex, age, duration of employment, position level, company, marriage, education level, personal/family history of disease, and the number of times acquiring information on radiation through various channels (internet searching, watching television, reading newspaper, conversation about radiation with aircrew/non-aircrew, in-house training). With a p of 0.2 in univariable models, we built a multivariable linear regression model using a stepwise selection method. Results: The average radiation knowledge score of the 356 respondents was 7.22. Univariable linear regression analysis showed that radiation knowledge of the aircrew was associated with their company, position level, age, and number of conversations with other aircrew members. Our multivariable model showed that the radiation knowledge level of aircrew decreased as they had more conversations about radiation with other aircrew members and as their age increased. Conclusions: Korean air crew showed a lower level of radiation knowledge as their age and the number of conversations with colleagues increased. The study suggests that more education is needed in order for aircrew to gain accurate radiation knowledge.

선량한도 하향이 방사선작업인력 및 집단선량에 미치는 영향예측 (Estimated Additional Number of Workers and Additional Collective Dose by Reducing Dose Limits)

  • 개인 선량한도를 하향조정 하였을 경우 추가적으로 요구되는 방사선작업 자 수와 집단선량을 예측하기 위한 연구가 수행되었다. 연구대상은 한국원자력안전기술원에서 선택한 기관 모두를 포함하였고 해석기준의 선량한도는 ICRP Publ. 60. 및 BEIR V 보고서가 권고한 값으로 하였다. 해석자료로 사용된 데이터베이스는 원자력법에 의거 원자력발전사업 허가자와 비파괴검사 허가자가 한국방사성동위원소협회에 제출한 개인피폭선량 기록의 편집자료에 근거하였다. 연간 개인선량한도의 하향 설정에 따른 영향은 상관모델을 사용하여 계산하였다. 본 연구모델의 민감도는 종사자가 방사선작업환경 내에 있는 동안의 비생산성을 가정하여 분석하였다. 최상의 방사선작업환경조건에서도 종사자의 최저 비생산성 인자에 접근하는 것이 대단히 어렵기 때문에 최저 비생산성 가정은 보수적인 가정이라는 주장을 쉽게 변호할 수 있다는 것이 장점이다. 개인선량한도가 연평균 2cSv로 하향 조정되고 개인 작업자의 비생산성율이 0.1이라고 가정하면 1997년 동안 방사선시설에서 추가로 요구되는 방사선작업종사자수는 231명으로부터 269명으로 증가되고, 집단선량 man-cSv는 근사적으로 14%만큼 증가된다.

Determination of Exposure during Handling of 125I Seed Using Thermoluminescent Dosimeter and Monte Carlo Method Based on Computational Phantom

  • Background: The thermoluminescent dosimeter (TLD) and Monte Carlo (MC) dosimetry are carried out to determine the occupational dose for personnel in the handling of 125I seed sources. Materials and Methods: TLDs were placed in different layers of the Alderson-Rando phantom in the thyroid, lung and also eyes and skin surface. An 125I seed source was prepared and its activity was measured using a dose calibrator and was placed at two distances of 20 and 50 cm from the Alderson-Rando phantom. In addition, the Monte Carlo N-Particle Extended (MCNPX 2.6.0) code and a computational phantom with a lattice-based geometry were used for organ dose calculations. Results and Discussion: The comparison of TLD and MC results in the thyroid and lung is consistent. Although the relative difference of MC dosimetry to TLD for the eyes was between 4% and 13% and for the skin between 19% and 23%, because of the existence of a higher uncertainty regarding TLD positioning in the eye and skin, these inaccuracies can also be acceptable. The isodose distribution was calculated in the cross-section of the head phantom when the 125I seed was at two distances of 20 and 50 cm and it showed that the greatest dose reduction was observed for the eyes, skin, thyroid, and lungs, respectively. The results of MC dosimetry indicated that for near the head positions (distance of 20 cm) the absorbed dose rates for the eye lens, eye and skin were 78.1±2.3, 59.0±1.8, and 10.7±0.7 µGy/mCi/hr, respectively. Furthermore, we found that a 30 cm displacement for the 125I seed reduced the eye and skin doses by at least 3- and 2-fold, respectively. Conclusion: Using a computational phantom to monitor the dose to the sensitive organs (eye and skin) for personnel involved in the handling of 125I seed sources can be an accurate and inexpensive method.
