STUDY ON THE RELATIONSHIP BETWEEN SEROTONIN SYSTEM AND PSYCHOPATHOLOGY IN TOURETTE'S DISORDER (Tourette씨병의 Serotonin계와 정신병리와의 상호관계에 관한 연구)

  • In order to elucidate the biological etiology and the effects of comorbidity on biological variables in tic disorders, plasma serotonin (5-hydroxlfryptamine, 5-HT) and 5-hydroxy- indoleacetic acid (5-HIAA) we.e measured in 87 tic disorders and 30 control subjects. The 87 tic disorder were composed of 45 Tourette's disorder(TS), 22 chronic motor tic disorders (CMT) and 20 transient tic disorders (TTD). Among these patients,43 patients were pure tic disorder (PT), 28 subject also had attention deficit hyperactivity disorder (T+ADHD) and 16 subjects had obsessive compulsive disorders (T+ OCD) as comorbid disorders. The results are summarized as follows : 1) Plasma 5-HT levels showed significant positive correlations with plasma 5-HIAA levels (Pennon r=0.77, p<0.05). 2) Plasma 5-HT and 5-HIAA levels showed no significant correlation with age in tic disorders. 3) Plasma 5-HIAA and 5-HT levels showed no significant correlations with age in control subjects. 4) There was significant difference in plasma 5-HT levels among TS, CMT, TTD and control groups (ANOVA F=34.48, df=3, 113, p<0.01), and post-hoc test using Scheffe method showed significant differences between control and TS, control and CMT, control and ITD groups. But, post-hoc test showed no significant differences between TS and CMT, TS and TTD, CMT and TTD groups. 5) There was significant difference in plasma 5-HIAA levels among TS, CMT, TTD and control groups (ANOVA F=26.48, df=3, 113, p<0.01), and post-hoc test using Scheffe method showed significant differences between control and TS, control and CMT, control and TTD groups. But, post-hoc test showed no significant differences between TS and CMT, TS and TTD, CMT and TID groups.f) There was significant difference in plasma 5-HT and 5-HIAA levels among PT, T+ADHD, T+OCD and contol groups (ANOVA 5-HT, F=37.59, df=3, 113, p<0.01, 5-HIAA, F=27.37, df=3, 113, p<0.01), and post-hoc test using Scheffe method showed signiscant differences between control and PT, control and T+ADHD and control and T+OCB. But, post-hoc test showed no significant differences between PT and T+ADHD, PT and T+ OCD and T+ADHD and T+ OCD. These results show that decreased 5-HT and 5-HIAA levels may play a role in the genesis of tic disorders, but these findings have no significant correlations with the severity of tic disorders. And the comorbid disorders of tics may have minimal effects on the biochemical abnormalities. Future studies must be focused on the effects of serotonin agonists and antagonists on tic disorders and molecular biological methodology may enhance to elucidate the mechanisms of these abnormal findings.

  • Objectives : The purpose of this study was designed to estimate the effects of Oriental medical treatment on posttraumatic stress disorder patient due to a series of traffic accidents by using SCL-90-R. Methods : We studied 20 patients who were admitted to Daegu Haany University Oriental Hospital from October2006 to May 2007. Results : 4 symptoms of SCL-90-R, somatization, obsessive-compulsive, depression and anxiety were significantly different when comparing after-treatment with before-treatment. But, 5 symptoms of SCL-90-R, interpersonal sensitivity, hostility, phobic anxiety, paranoid ideation and psychoticism showed no difference between after-treatment with, before-treatment measurements. The total impact or SCL-90-R was significant comparing after-treatment with before-treatment measurements. Conclusions : We concluded that there was a significant effectof Oriental medical treatment on posttraumatic stress disorder patients caused by a seriesof traffic accident. However, the findings of this study need to be confirmed in more patients with further studies.

  • Recently with the wide spread smart devices and the flexible forms of work such as working from home due to changes in work practices, people are getting various information easily. Many women with a baby get to be interested in foods, health, and growth of their baby and the new task of raising a baby comes with a new burden added to taking care of their family and can develop into obsessive compulsive disorder to be a great mother. So many mothers suffer from child-rearing stress. Also, as the balance of work and life in mobile environment emerged as an important issue, the need for integrated information management applications that give infant care assistance to new mothers was increased. This study analyzed preferences on infant care information apps and developed prototype apps for infant care information management that can provide integrated customized infant care information that is actually needed.

Antidiuretic Hormone Levels in Men with Burning Mouth Syndrome: A Pilot Study

  • Purpose: Burning mouth syndrome (BMS) is a disabling pain that mostly occurs in elderly women, but rarely in men. It is characterized by an unremitting oral burning sensation and pain without detectable oral mucosal changes. We investigated the clinical and hematologic features of middle-aged men with BMS, and compared the results to those of men with oral mucositis. Methods: Five men with BMS ($48.60{\pm}6.19years$) and five age-matched controls with oral mucositis ($49.80{\pm}15.26years$) underwent clinical and psychological evaluations and blood tests. Psychological status was evaluated using the Symptom Checklist-90-Revised. Cortisol, estradiol, progesterone, testosterone, follicle-stimulating hormone (FSH), luteinizing hormone (LH), adrenocorticotropic hormone (ACTH), and antidiuretic hormone (ADH) levels and erythrocyte sedimentation rate (ESR) were determined from the blood samples. Results: ADH level was significantly lower in men with BMS than in the controls. ADH levels correlated with testosterone (p<0.01), and ACTH levels strongly correlated with ESR (p<0.05). Progesterone level positively correlated with FSH and LH levels. Pain intensity on a visual analogue scale correlated with estradiol level only in men with BMS. Among psychological factors, the obsessive-compulsive disorder, interpersonal-sensitivity, and anxiety scores were higher in men with BMS than in the controls (p<0.05). However, no correlations were observed between the psychological and hematologic factors in both groups. The BMS symptoms presented only on the tongue, with the lateral border being the most prevalent area. Conclusions: Men with BMS may experience dysregulated endocrinologic or psychoneuroendocrinologic interactions, which might affect oral BMS symptoms, aggravating the severity of the burning sensation.

A Study on the Types and the Frequencies of Personality Disorders/Traits in the Patients with Somatoform Disorders (신체형 장애 환자에서 동반되는 인격 장애/성향의 유형 및 빈도에 대한 연구)

  • This study was conducted to identify the relationships between personality disorders/traits and somatoform disorders. After the patients were screened through self-rated SCID-II Questionnaire(Structured Clinical Interview for DSM-III-R, Questionnaire), the researcher got psychiatric history, performed clinician-rated SCID-P(Structured Clinical Interview for DSM-III-R, Patient Edition), classified and diagnosed mental disorders with SCID-P, and evaluated SCID-II(Structured Clinical Interview for DSM-III-R, Personality Disorders) by direct interview. The prevalencies of avoidant, obsessive-compulsive, schizotypal, narcissistic, paranoid, dependent, self-defeating, borderline personality disorders/traits in patients with somatoform disorders were diagnosed as 67.4%, 48.8%, 44.2%, 41.9%, 37.2%, 34.9%, 34.9%, 32.6%, respectively. The frequencies of self-defeating and schizotypal PD/traits were significantly higher than those of other neurotic control group. The results of this study could be regarded as replicating the results of previous studies that had reported most of all patients with somatoform disorder had presented with personality disorders/traits. This study, however, showed that the patients with somatoform disorders accompanied not with any specific types of personality disorders/traits but with various types of personality disorders/traits, which was much different from the previous usual clinical impressions. Thus, it is necessary for clinicians to approach the patients with somatoform disorders through more flexible and more supportive methods and attitude, in order that they should treat them more effectively.

Mediating Effects of Self-reassuring and Self-attacking on Relationship between Mental Health and Psychological Well-being among College Students (대학생의 정신건강과 심리적 안녕감 간의 관계에서 자기위안과 자기공격의 매개효과)

  • This study examined the mediating effects of self-reassuring and self-attacking in the relationship between mental health and psychological well-being among college students. The study subjects were 230 (98 males and 132 females), and the collected data were analyzed using the SPSS PROCESS Macro based on regression analysis. The results of the study were as follows. First, there were significant relationship among mental health, psychological well-being, self-reassuring and self-attacking. Second, depression, obsessive compulsive disorder, interpersonal sensitivity and psychosis had a significant negative effect on psychological well-being. Third, self-reassuring and self-attacking had a partial mediating effect on the relationship between mental health and psychological well-being. The indirect effect of mental health on psychological well-being through self-reassuring was significantly higher than the indirect effect of psychological well-being through self-attacking. Based on the results of this study, we discussed the role of self-reassuring in the relationship between mental health and psychological well-being.

Alcohol Use Disorder, Stress, Mental Health and Suicide among Seoul Citizens (서울시 지역주민의 알코올장애와 스트레스, 정신건강 및 자살)

  • Objectives: This study furthers the currents understanding of alcohol use disorders, stress, mental health status and suicide among Seoul citizens. This study is to identify differences according to socio-demographic variables and variables which affect stress, mental status and suicide for developing mental health programs. Methods: The study subjects using cluster-stratified sampling method were 1234 adults over 19 years old from 17 dong, S-Ku in Seoul City. A cross-sectional study with face-to-face interview was used to collect data. A questionnaire measuring socio-demographic variables, Symptom Checklist-90-Revised (SCL-90-R) and psychosocial wellbeing index-short form (PWI-SF) was utilized. Results: Three findings in this study were discussed: (1) The prevalence of problem drinking and alcohol dependence and AUDIT score were significantly higher among male, younger age, and high level of education and income; (2) AUDIT score were related with stress, all 9 symptoms of SCL-R and suicide plan; (3) Problem drinkers were more likely to have 2.5 times of anxiety, 3.0 times of hostility, and 2.4 times psychoticism than non alcohol problem drinkers. Alcohol dependence drinkers were more likely to have 1.9 times of high risk group of stress, 4.1 times of obsessive-compulsive, 4.7 times of anxiety, 5.5 times of hostility, and 2.3 times psychoticism than non alcohol dependence drinkers. Conclusion: The findings suggest that the future intervention of Mental Health programs should be carefully designed and tailored by socio-demographic variables.

Reliability and Validity of the Korean Version of the Hoarding Rating Scale-Self-Report (한국판 저장행동평가척도의 표준화 연구)

  • Objective : The Hoarding Rating Scale-Self-Report (HRS-SR) is a five-item scale that simply assesses the hoarding symptoms. We evaluated the factor structure, reliability, and validity of the Korean version of the HRS-SR (HRS-SR-K). Methods : A total of 144 individuals completed the self-administered questionnaires including HRS-SR-K, Obsessive-Compulsive Inventory-Revised-Korean version (OCI-R-K), Beck Depression Inventory (BDI), and Beck Anxiety Inventory (BAI). Psychometric properties of the HRS-SR-K were analyzed. Results : The Cronbach's α value for internal consistency of the HRS-SR-K was excellent (Cronbach's α=0.84). The construct validity was analyzed on the basis of principal component analysis and one-factor structure of the original scale was maintained. The HRS-SR-K total score and each item scores were more strongly correlated with the hoarding subscale score in OCI-R-K (convergent validity, r=0.71, p<0.01) than the corresponding scores of nonspecific depression or anxiety measures (discriminant validity). Conclusion : The HRS-SR-K is a simple and reliable self-report scale for examining the severity of hoarding symptoms.

Characteristics of Early Maladaptive Schemas and Associated Cognitive Functions in Visitors to the Psychiatric Department in a University Hospital for Military Designation Process (병역판정검사를 위해 일 대학병원 정신건강의학과에 내원한 환자들의 초기 부적응 스키마와 관련 인지 기능 특성)

  • Objective : The goal of this study was to investigate the characteristics of early maladaptive schemas (EMSs), and associated neurocognitive functions as seen in visitors for military designation process. Methods : This retrospective study included 111 males aged 18 to 24 years among three groups: 41 visitors for military designation process (VMD), 21 patients with obsessive-compulsive disorder (OCD), and 49 healthy subjects. We collected the results of the Young Schema Questionnaire, Symptom Checklist-90-Revised, three neurocognitive tests as well as their clinicodemographic data. We analyzed the differences in EMSs between these three groups, and the correlations among the identified EMSs and neurocognitive performances within the VMD group. Results : Compared with both the OCD and healthy groups, the evaluation of the VMD group showed significantly higher scores in mistrust/abuse (F=6.4, p=0.002), vulnerability to harm (F=6.6, p<0.0001) and negativity/pessimism schema (F=7.3, p<0.0001), even when controlling for depression scores and levels of education. These three schemas also exhibited significant negative correlations with the score of Stroop test with r ranging from -0.34 to -0.44. Conclusion : These findings suggest that people who are likely to have difficulties adjusting to living in a military life may have psychological vulnerabilities related to certain EMSs. Further studies are warranted to test the clinical potentials of these findings, such as a treatment target and a predictor factor.

Personality Characteristics and Those Influences on the Outcome of Cognitive Behavioral Therapy in Patients with Panic Disorder (공황장애 환자의 성격 특성과 인지행동치료의 결과에 미치는 영향)

  • The authors intended to investigate personality characteristics and those influence on the outcome of cognitive behavioral therapy in patients with panic disorder. 167 patients who met DSM-IV criteria for panic disorder were assessed by the PDQ-R(Personality Disorder Questionnaire-Revision) and various self-report tools for assessing symptoms of panic disorder. The effect of therapy was measured by the changes of scores and the end state functioning before and after 12-sessions of CBT. The patients with panic disorder were more likely showed obsessive-compulsive, avoidant and paranoid personality disorder and also Cluster C. If is needed when patients were divided into two groups according to total scores of PDQ-R(high or low personality disorder groups), high personality disorder group showed many evidences for increased psychopathology at the start of treatments, this suggested the close linkage between panic disorder and personality disorder. Interestingly, there were no significant differences between both groups in scores of clinical variables and the end state functioning. In conclusion, although patients with high tendency of personality disorder had more generalized problems at the beginning of treatments, they could improve as much as the patients with low tendency of personality disorder. They can be helped by cognitive behavioral therapy for panic disorder and seem to profit as much as patients with low tendency of personality disorder. If is needed to seek other factors in poor responders for cognitive behavioral therapy.
