강원도 일부 농촌지역 노인의 사회경제적 위치와 구강건강관련 삶의 질과의 연관성 (The association between oral health related quality of life(OHRQoL) and socio-economic position in the elderly in rural area of Gangwon province)

  • Objectives : The objective of this study was to assess a level of oral health related quality of life(OHRQoL) for rural communities elderly and to determine the association between OHRQoL and socio-economic position. Methods : The study population was elderly(60+year-old) residents of PyeongChang county, Jeongseon county, Yeongwol county, Gangwon province. A total of 171 people were invited to participate. Oral health related quality of life was measured using the GOHAI. The data were analyzed with Mann-Whitney U test or Kruskal-Wallis test and to assess socio-economic inequalities in OHRQoL(GOHAI), we used multi-variable logistic regression models. We used models adjusting for age, sex, family status factors(Model I) and compared them to models additionally adjusting for stress level(Model II). Results : There were significant differences in lower score of GOHAI at lower age group and live alone group. And we found that lower SES was significantly associated with lower score of OHRQoL. Social gradient in the score of OHRQoL persisted when adjusted for age, sex, family status, stress level. Conclusions : We recommend that oral health promotion program should be developed after due consideration SES for rural communities elderly because OHRQoL of rural communities elderly was low and association between SES and OHRQoL for rural communities elderly.

구강건강에 이용되는 보완대체의학 치료: 무작위대조임상연구를 통한 문헌적 고찰 (Complementary and Alternative Medicine for Oral Health : a Literature Review of Randomized Controlled Clinical Trials)

  • Objective : This review aims at analyzing Complementary and Alternative Medicine (CAM) therapies, using for oral health to know current use of CAM in dental area by analyzing randomized controlled clinical trials (RCTs). Method : We searched the following 6 electronic databases until 1 July 2014. : Pubmed, Medline, Embase, Allied and Complementary Medicine Database(AMED), Cumulative Index to Nursing and Allied Health Literature(CINAHL), the Cochrane Library. We included parallel RCTs and cross-over RCTs that assessed the efficacy of CAM regardless of blinding and language. Results : A total of 154 RCT articles were included. There were 2-arm parallel study design(69.5%), 3-arm parallel study design(20.8%), 4-arm parallel study design(4.5%) and Cross-over design(5.2%). Complementary and alternative medicine RCTs in oral health tend to increase in the early-2000s and in the mid-2010s. In addition, 154 citations were classified according to diseases and interventions that categorized Natural Products studies(68.2%), Mind and Body Practices studies(31.8%). We classified in detail that vitamin and mineral therapies(29.9%), dietary supplements(24.7%), acupuncture(23.3%), Herbal medicine(13.6%), homeopathy (2.6%), energy therapies(2.6%), Massage(1.9%), biofeedback(0.6%), traditional medicine(0.6%). Conclusion : Increasing publications and diverse interventions regarding CAM for oral health is observed by analyzing RCTs from the literature review. Further studies are needed to be performed as systematic reviews to verify their effectiveness and the research to inquire into side effect.

Midfacial soft tissue changes after maxillary expansion using micro-implant-supported maxillary skeletal expanders in young adults: A retrospective study

  • Objective: The aim of this retrospective study was to assess the midfacial soft tissue changes following maxillary expansion using micro-implant-supported maxillary skeletal expanders (MSEs) in young adults by cone-beam computerized tomography (CBCT) and to evaluate the correlations between hard and soft tissue changes after MSE usage. Methods: Twenty patients (mean age, 22.4 years; range, 17.6-27.1) with maxillary transverse deficiency treated with MSEs were selected. Mean expansion amount was 6.5 mm. CBCT images taken before and after expansion were superimposed to measure the changes in soft and hard tissue landmarks. Statistical analyses were performed using paired t-test and Pearson's correlation analysis on the basis of the normality of data. Results: Average lateral movement of the cheek points was 1.35 mm (right) and 1.08 mm (left), and that of the alar curvature points was 1.03 mm (right) and 1.02 mm (left). Average forward displacement of the cheek points was 0.59 mm (right) and 0.44 mm (left), and that of the alar curvature points was 0.61 mm (right) and 0.77 mm (left) (p < 0.05). Anterior nasal spine (ANS), posterior nasal spine (PNS), and alveolar bone width showed significant increments (p < 0.05). Changes in the cheek and alar curvature points on both sides significantly correlated with hard tissue changes (p < 0.05). Conclusions: Maxillary expansion using MSEs resulted in significant lateral and forward movements of the soft tissues of cheek and alar curvature points on both sides in young adults and correlated with the maxillary suture opening at the ANS and PNS.

아로마 용액을 이용한 구강간호가 뇌졸중 노인의 구강상태와 구강 세균 집락에 미치는 효과 (Effects of Special Mouth Care with an Aroma Solution on Oral Status and Oral Cavity Microorganism Growth in Elderly Stroke Patients)

  • Purpose: This study was conducted to examine the effect of oral care with an aroma solution on oral status and oral cavity microorganism growth in elderly patients with stroke. Methods: A non-equivalent control group, with a pretest-posttest design was used in this study. The participants were assigned to the experimental group (n=30) that received oral care with an aroma solution or the control group (n=31) that received 0.9% saline solution. To identify the effect of the experimental treatments, objective/subjective assessments of oral status and oral cavity microorganism growth were performed using the oral assessment guide, oral perception guide, and oral swab culture. Data were analyzed using Chi-square test, Fisher's exact test, and t-test with the SPSS version 21.0 program. Results: The objective oral status was significantly lower in the experimental group than in the control group (t= -3.64, p<.001). There was no significant difference between the subjective oral status of the experimental group and control groups (t= -1.24, p=.109). Oral microorganism growth was significantly lower in the experimental group than in the control group (t= -7.39, p<.001). Conclusion: These findings indicate that special mouth care using an aroma solution could be an effective oral health nursing intervention for elderly patients with stroke.

Risk Factors for Oral Cancer in Northeast Thailand

  • Oral cancer is a common site of head and neck cancer, and is relatively frequent in Northeast Thailand. The objective of this hospital-based, case-control study was to determine associations with risk factors. A total of 104 oral cancer cases diagnosed between July 2010 and April 2011 in 3 hospitals were matched with control subjects by age, sex and hospital. Data were collected by personal interview. There were significant associations between oral cancer and tobacco smoking (OR=4.47; 95%CI=2.00 to 9.99), alcohol use among women (OR=4.16; 95%CI=1.70 to 10.69), and betel chewing (OR=9.01; 95%CI=3.83 to 21.22), and all three showed dose-response effects. Smoking is rare among Thai women (none of the control women were smokers), but betel chewing, especially among older women, is relatively common. We did not find any association between practicing oral sex and oral cancer.

Impact of skeletal divergence on oral health-related quality of life and self-reported jaw function

  • Objective: To investigate the differences in oral health-related quality of life (OHRQoL) and self-reported jaw function between patients with hyperdivergent and normodivergent facial types. Methods: Eighty patients with a distinctively hyperdivergent facial type (mandibular plane angle greater than 2 standard deviations, or $42^{\circ}$) and 80 controls were individually matched according to age, sex, ethnicity, and treatment stage. Data were collected using self-report questionnaires such as the Oral Health Impact Profile (OHIP-14) and Jaw Functional Limitation Scale (JFLS-8). Results: The mean age of the patients was $17.2{\pm}4.6years$ (range, 12-49 years), with most (65.0%) being female and of New Zealand European origin (91.3%). Individuals with hyperdivergent facial types had higher overall and social domain scores on the OHIP-14 (p < 0.05) than did the ones with normodivergent facial types. However, the intergroup differences in JFLS-8 scores were not significant (p > 0.05). Conclusions: Jaw function appears to be similar in individuals with hyperdivergent and normodivergent facial morphologies. However, those with hyperdivergent facial types are more likely to self-report poorer OHRQoL than are those with normal faces, especially in relation to social aspects.

  • This study discusses the effects of infrared thermography in dentistry. Infrared thermography allows the quantification of infrared rays and provides useful data for dental care and research. It has been used for the diagnosis of temporomandibular dysfunction (TMD) and proposed as an objective measure of myofascial pain. It is closely related to temperature and produces beneficial objective data. A study of the heat emissions from facial structures has the potential to benefit dental diagnosis of TMD, postoperative inferior alveolar damage, and endodontic and conservative treatments. In this paper, we assessed infrared thermography in dentistry and consider future developments of infrared thermography.

일부 노인장기요양환자의 구강실태 및 구강기능 (Dental Status and Oral Function in Some Long-term Care Elderly Patients)

  • Objective: This study aims to investigate the relationship between dental status and oral function by analyzing those in some long-term care elderly patients. Methods: It performed oral examination and 4 oromotor function examinations such as repeated swallowing function, correct pronunciation function, saliva secretion rate and maximum mouth opening to 91 elderly patents aged 65 and older in the municipal geriatric hospital located in Cheonan. Results: Dental status of patients such as the number of dental caries, treated teeth, retained teeth and function teeth were better in mobile elderly patients than in immobile elderly patients. Attachment rate of dental plaque and the number of teeth being extracted were more in mobile patients than in immobile patients. More retained teeth, the number of function teeth and dental caries and the score in pronunciation status test were significantly higher. With more function teeth, the score in pronunciation status test was significantly higher. As saliva secretion rate is higher, repeated swallowing function was significantly better. repeated swallowing function rate is higher pronunciation status was significantly better. Conclusions: With the results of this study, it was found that among long-term care elderly patients, oral function was worse in immobile patients than in mobile patients. Therefore, it may be necessary to plan and perform an oral function improvement program preferentially for elderly patients requiring long-term care.

학교구강보건사업의 보건지리학적 분석을 위한 지리정보체계의 활용 (A GIS-Based Public Health-Geographical Analysis of Oral Health Programs for Primary School Students)

  • 본 연구는 학교구강보건사업을 통해 구강보건서비스를 제공받고 있는 초등학생들이 전국의 각 시 군 구별로 어떻게 서로 다르게 분포되어 있는지를 파악하기 위하여 GIS기법 중에서도 단계구분도를 이용하여 5단계로 나누어 지도를 제작하였다. 서울에서는 학교구강보건사업을 실시하는 초등학교가 전혀 없어서 본 연구에서는 제외되었고 205개 시 군 구를 대상으로 하여, 구강보건서비스에 대한 공간적 지리적 접근성을 비교의 기준으로 삼아 다음과 같은 결과를 얻었다. 통계적 인접성에 의해 5단계 중에서 기본구간은 0.1~20.0%로 설정하였다. 1. 전국 각 시 군 구 초등학교 학생 수 대비 학교구강 보건실을 운영하는 초등학교 학생 수 비율이 기본 구간에 해당하는 지역은 전국적으로 113개 시 군 구에 해당하며 33개 시 군 구는 0%에 해당한다. 2. 전국 각 시 군 구 초등학교 학생 수 대비 학교구강 보건실을 운영하는 초등학교에서 구강보건교육과 바른양치교육을 수행했던 학생 수의 비율이 기본 구간에 해당하는 지역은 전국적으로 101개와 102개 시 군 구에 각각 해당한다. 구강보건교육이 전혀 실시되지 않는 시 군 구는 34개, 바른양치교육이 전혀 실시되지 않은 시 군 구는 35개이다. 3. 전국 각 시 군 구 초등학교 학생 수 대비 학교구강 보건실을 운영하는 초등학교에서 예방서비스를 받았던 학생 수의 비율이 기본 구간에 속하는 지역은 전국적으로 80개 시 군 구에 해당한다. 34개 시 군 구의 초등학생들은 어떤 예방서비스도 제공받지 않았다. 4. 전국적으로 학교구강보건실을 통해 치료서비스를 제공 한 초등학교 중에서 기본 구간에 해당하는 시 군 구는 153개이다. 51개 시 군 구의 초등학생들은 치료서비스를 전혀 제공받지 않았다. 이와 같이 학교구강보건사업을 통하여 구강보건서비스를 제공받는 초등학생들은 전국적으로 볼 때 지리적 공간적 편차가 매우 크다는 사실을 알 수 있다. 다시 말해서, 초등학생들의 구강보건서비스에 대한 공간적 지리적 접근성은 전국의 각 시 군 구에 따라 격차가 매우 크다는 사실을 확인할 수 있다. 또한, 본 논문은 GIS 기법을 활용하여 학교구강보건사업의 전국적 분포를 비교 분석함으로써 공공 부문에서의 국가구강보건정책을 입안하고 시행하는데 정책적 근거자료로 활용될 수 있을 것이며, GIS를 활용하여 보건지리학, 구강보건학, 치과위생학 사이의 학제간 연구를 수행하였다는 데 의의를 갖는다.

The Association Between Masticatory Function Assessment and Masseter Muscle Thickness in the Elderly

  • Purpose: This study investigated the association between the objective indicator of masticatory function assessment and the masseter muscle thickness (MMT) using ultrasound imaging. Methods: A total of 99 subjects (males: 24, females: 75, mean age: 76) were analyzed. The maximum bite force (MBF) was measured with a pressure-sensitive sheet and an image scanner. The mixing ability index (MAI) was calculated by image analysis after asking the subjects to chew a wax specimen. The MMT during rest and clenching were obtained with a diagnostic ultrasound system, and the difference in MMT during rest and MMT during clenching was defined as the difference in masseter muscle thickness (DMMT). Multiple regression analysis was performed to determine the independent variables affecting MBF and MAI. Results: The MBF showed correlation with the number of remaining teeth (β=0.346, p=0.002) and DMMT (β=0.251, p=0.011). The MAI correlated with only the number of remaining teeth (β=0.476, p<0.001). Conclusions: The DMMT reflects the state of masseter muscle contraction, and can be used as a predictor as well as the number of teeth when assessing masticatory function.
