Pain Evaluation in Patients with Knee Osteoarthritis (퇴행성슬관절염 환자의 통증에 대한 연구)

  • We performed this study to assess pain in patients with knee osteoarthritis. Participants were 120 Patients(men 26 and women 94) with knee osteoarthritis. Pain evaluations were conducted using survey composed of 12 items by interview with oriental medicine doctor. Collected data were analyzed by IBM SPSS Statistics 19.0. Women were 3.8 times more than men(21.7% vs 78.3%). Approximately 80% of participants were overweight and obesity. Descending stair is most painful action for knee osteoarthritis patients. The average degree of knee pain was more than moderate. 5. People diagnosed as arthritis before had the much higher knee pain than never diagnosed before. This study showed female gender, overweight and obesity were associated with the risk of knee arthritis and participants complained knee pain more than moderate. It is hoped that our findings provide information that help the treatment of knee osteoarthritis.

Correlation between Short Stature and Obese Degree - Intended for the Case of 236 Patient in the Name of Short Stature (저신장과 비만도의 상관성 분석 - 저신장을 주소로 내원한 환아 236명을 대상으로)

  • Objectives The purpose of this study was to investigate correlation between short stature and obese degree. And we also wanted to know the characteristics of patients visited clinic in the name of 'short stature'. Methods Height, body weight, BMI, fat mass, lean body mass, percent body fat were taken intended for 236 patients in the name of short stature. To all the patient questionnaire for growth clinic was drawn up, and ultrasound scan was taken through calcaneous of the right foot. The patients were classified to three groups - short, average, tall group - according to relative position of mid-parental height. It was analysed that the differences between groups in obese degree. Also was investigated correlation between position of short stature and obese degree, and between obese degree and bony maturity. Results & Conclusion 1. The average ages of patients in the name of 'short stature' were $12.69{\pm}3.93$ years old in boys, $10.66{\pm}3.67$ years old in girls. And it seemed to be just before second rapid maturing period. 2. The average BMI were $20.58{\pm}4.07kg/m^2$ in boys, $18.65{\pm}2.85kg/m^2$ in girls, and average percent body fat were $21.99{\pm}7.35%$ in boys, $26.01{\pm}6.35%$ in girls. 3. The numbers of obese children were 34(31.2%) in boys, 19(14.9%) in girls on the basis of BMI. And the numbers were 39(35.8%) in boys, 53(41.7%) in girls on the basis of percent body fat. There was a big difference in case of girls. 4. The numbers of AG(average group) were 48(44.0%) in boys, 60(47.2%) in girls, the numbers of SG(short group) were 35(32.1%) in boys, 31(24.4%) in girls, and numbers of TG(tall group) were 26(23.9%) in boys, 35(27.6%) in girls. 5. There were no significant differences among the groups in BMI, lean body mass, fat mass, percent body fat. Only significant difference in DI(disease index). 6. There were no significant correlation between PH(percent height) and BMI, leanbody mass, fat mass, percent body fat. Only significant correlation in DI(disease index). 7. There were no significant differences between DA(difference between bone age and chronorogical age) and BMI, leanbody mass, percent body fat. Only significant correlation in fat mass.

A Study on the Level of Perception of the Health Condition of Koreans and Americans in Virginia, USA and Their Preference for Salty Taste (미국 버지니아주 거주 한국인과 미국인의 건강상태 인식도 및 짠맛 기호도 비교)

  • This study was conducted as a comparative analysis, using the SPSS 17.0 program for a survey conducted from March to July 2013, on the level of perception and preference factors of Koreans and Americans residing in Virginia, USA(89 male and female adults of 20 years of age or above). For the information on the degree of obesity, a survey was used and the subjects provided with their heights and weights. The researcher used this data to calculated their BMI and distinguished the degrees of obesity. For the BMI, the obesity diagnosis of Korean Society for the Study of Obesity was used for Koreans, and the WHO's obesity diagnosis on adults were employed for Americans. First, regarding the Korean participants, the level of obesity appeared in the order of normal (79.6%), overweight (14.3%), and underweight (6.1%). Also, the level of perceptions of their health conditions appeared in the order of average (51.7%) and good (34.7%). No one perceived their health to be in poor condition. Meanwhile, for the American participants, the order was as follows: overweight at about 45%, normal weight at about 35%, obesity at about 12.5%, and underweight at about 7.5%. In terms of the level of perception of health during ordinary times, about 55% of the American participants answered that they are in good health. Therefore, the Americans were found to perceive their health to be in relatively better condition than the Koreans residing in the USA. Also, concerning the level of interest in health while eating food at ordinary times, the American participants were discovered to be more interested (about 62.5%) than the Korean participants. The research on their preference for salty taste was conducted as a survey, and the subjects were asked to choose an item according to their subjective feeling. Regarding the preference for salty taste, both Koreans and Americans residing in the USA responded as moderate. However, a higher proportion of Americans than Koreans responded to favor salty taste. Concerning the reason for their preference for salty taste, Koreans responded with various reasons whereas Americans mostly responded that it is because they eat salty food frequently. For any changes in dietary behavior related to eating salty food, both Korean and American respondents thought that the amount of their daily intake of salt is appropriate and that they consider taste as most important in selecting a snack. Based on the above data, the results of this research indicate a need for actual improvement in the diet with regard to salty food along with diverse policies for promoting life with a low salt diet in future.

Antioxidant and Anti-Obesity Activity of Ethanol Extracts from Fermented Arctium lappa L. (발효 우엉 에탄올 추출물의 항산화 및 항비만 활성)

  • This study was conducted to investigate the antioxidant and anti-obesity activity of ethanol extracts from fermented Arctium lappa L. Arctium lappa fermented twice with Phellinus linteus ($2^{nd}$ FA) showed more DPPH radical scavenging activity than non-fermented Arctium lappa (NFA), and Arctium lappa fermented once ($1^{st}$ FA) at a concentration of 62.5 ppm. The $2^{nd}$ FA showed the highest level of ABTS radical scavenging activity at concentration range of 31.5~125 ppm. The ABTS free radical scavenging activities of $1^{st}$ FA and $2^{nd}$ FA were similar to that of BHA, a synthetic antioxidant, at a concentration of 250 ppm. The total polyphenol content of $2^{nd}$ FA was higher than those of NFA and $1^{st}$ FA. The flavonoid content was significantly increased in $1^{st}$ FA and $2^{nd}$ FA than NFA. During adipocyte differentiation, lipid accumulation in 3T3-L1 cells was significantly decreased in the order of NFA, $1^{st}$ FA, and $2^{nd}$ FA at all concentrations. In conclusion, the antioxidant and anti-obesity activities of A. lappa were increased depending on the degree of fermentation. It is suggested that fermented Arctium lappa, especially $2^{nd}$ FA, could be used as a natural ingredient for functional foods and medicine.

Analysis of Anthropometric and Clinical Data in Obese Children in Changwon (창원지역 일부 비만아의 체격지수와 생화학적 지표에 관한 연구)

  • In this study, we investigated the effects of obesity index on the risk factors of chronic diseases in obese children. The subjects were male and female children aged 9 to 12. The average obesity index was 147.7$\pm$13.8% in boys and 147.6$\pm$16.5% in girls. The average percentages of body fat was significantly higher in boys(33.9$\pm$5.7%) than in girls(30.7$\pm$4.4%). It was found that 26% of subjects had abnormal serum GOT and GPT values. Percentages of subjects at risk of cardiovascular disease(CVD) based on corresponding criteria of TG, T chol, HDL chol, and LDL chol were 28.6%, 28.6%, 45.5% and 42.7%, respectively. The risk of CVD was increased with the degree of obesity, which was not significant. The AI(atherogenic index) was higher in boys(4.6$\pm$2.3) than in girls(3.6$\pm$1.2). The serum T chol and LDL chol levels of subjects whose mother are in thirties were significantly higher than those of subjects whose mother are in fourties. The T chol levels of subjects who had mother with job were significantly higher compaired to those of subjects who had housewife mother. The results indicated the urgent need of nu tritional management for the obese children to prevent further devel opment of childhood obesity and hyperlipidemia. Based upon this study, devising method and media for the mother nutritional education as well as for the children nutritional education is required for better growth and health promotion of primary school children.

A Study on the Relationship between Self-Efficacy and Health Promoting Lifestyle Profile of Childhood Obesity (학령기 비만아동의 자기효능감과 건강증진 생활양식과의 관계연구)

  • Purpose: The purpose of this study was to offer basic data concerning the prevention of obesity for obese children to identify the relationship between their self-efficacy and the degree of performance on health promoting lifestyle profile. Method: The subjects were 148 fourth to sixth grade elementary school students in Gwang-ju, Korea during the period from September 10 to September 25, 2001. The collected data were analyzed by mean, standard deviation, t-test, ANOVA, Pearson's Correlation Coefficient and Stepwise Multiple Regression by using the SPSS program. Result: The relationship between general characteristics of subjects and self-efficacy were significant differences according to eating time(F=5.338, p=.006). The relationship between general characteristics of subjects and health promoting lifestyle profile were significant differences according to grade(F=5.615, p=.005), eating time(F=8.591, p=.000), concern about weight control(F=3.915, p=.022), experience of weight control(t=7.632, p=.006). Health promoting lifestyle profile was showed significant positive correlations with self-efficacy(r=.537, p=.000). Self-efficacy(27.6%) was the highest factor predictor health promoting lifestyle profile of obesity students. In addition to eating time, concern about weight control, explained for 33.3% in health promoting lifestyle profile. Conclusion: Results of this study point to self-dfficiency as a major factor in the health promoting behavior of childhood obesity. According to the results of this study, There is a reason to develop a health program to promote self-efficiency.

The Clinical Study on the Effect of Reduction in Body Weight and the Change of Body Composition after Fasting Therapy (절식요법의 체중감량효과와 체성분 변화에 대한 관찰)

  • Objectives : This study was aimed to investigate the change of the reduction in body weight and the change of body composition after fasting therapy, Chulsikyopub, which was put into practice in Dept. of oriental rehabilitation medicine at Kyung Hee oriental hospital. Method : We measured body weight, body composition, and the degree of obesity 2 weeks after fasting therapy and 10 weeks after fasting therapy with body composition analyzer(Inbody 2.0). Statistical comparisons between pre-therapy group and post-therpay group were done by paired t-test and Wilcoxon's signed rank test. Results : 2 weeks after fasting therapy, body weight was decreased 7.33kg(9.18%), body fat was decreased 2.47Kg(7.24%), muscle weight was decreased 4.66kg(11.37%), body fat ratio was increased 1.08%, and body metabolic rate(BMR) was decreased 118.23kcal(8.22%). 10 weeks after fasting therapy, each of body weight and fat was decreased 10.4Kg, muscle weight was increased 0.3Kg(0.7%), body fat ratio was decreased 7.9%, and BMR was increased 6.8Kcal(0.47%). But muscle weight and BMR did not satisfy statistical significance. Conclusions : fasting therapy for 10 weeks reduced body weight and body fat statistically significant without loss of muscle weight and BMR.

Relationship between Quantitative Sonographic Measurements and Serum Biochemical Parameters in Childhood Obesity

  • Purpose: We investigated the relationship between sonographic measurements of fatty liver and body mass index standard deviation score (BMI-Z score), abdominal wall fat thickness (AWFT), and serum biochemical parameters in childhood obesity. Methods: Anthropometric, laboratory, and ultrasonography data were obtained from 174 children with BMI-Z score >1. After the qualitative grading of hepatosteatosis (grades 0-3), the quantitative liver-kidney echogenicity ratio (LKER) was calculated using a software tool. Groups according to sex, age (AG-I to AG-III), BMI-Z score (BMG-I to BMG-III), and hepatosteatosis degree (HS-I and HS-II) were formed. The differences and distributions of the variables were statistically analyzed and compared among the groups. Results: Serum transaminase and glucose levels showed a positive correlation with LKER, whereas the HDL level showed a negative correlation. BMI-Z score and AWFT showed a positive correlation with fasting insulin level and HOMA-IR value. LKER was significantly higher in girls than in boys (p=0.008). In the AG-I group (age 3-8.9 years), the BMI-Z score was significantly higher, whereas AWFT was significantly lower than in the other age groups (p<0.001). The cutoff point of LKER for predicting grade 2 or higher steatosis (HS-II group) was determined to be 1.83. Cardiovascular disease risk was significantly higher in the HS-II group (p=0.035). Conclusion: As a valuable quantitative measurement tool, LKER can be used for the sonographic screening of fatty liver. AWFT, on the basis of its correlation with fasting insulin level and HOMA-IR value, may be a useful sonographic parameter in the management of childhood obesity.

Korean Medicine with Self-monitoring for Weight Control and Factors Associated with Weight Loss: a Retrospective Observational Study (자기감시를 병행한 한의비만치료에서 체중감량 및 감량에 영향을 미치는 요인 분석: 후향적 관찰연구)

  • Objectives: The purpose of this study was to investigate the effect of weight loss in Korean Medicine (KM) obesity treatment combined with self-monitoring and to identify the relevant factors which are related to successful weight loss. In addition to this, adverse events reported by the patients were analyzed to assess the safety of KM treatments for obesity Methods: This was a retrospective observational study that analyzed the medical records of the patients that participated in the 10-week of KM treatment. All patients took herbal medicine for weight loss and were instructed to reduce calorie intake and adhere to self-monitoring. Clinical data including body weight were collected for evaluating the effect of the treatments. Behavior factors affecting weight loss were collected. The correlation between each factor and weight loss was analyzed. Adverse events (AEs) were collected at each time to evaluate the safety of the intervention. Results: One hundred eighteen patients' records were included in this study. The average weight loss was 7.64kg(74.59kg to 66.95kg). 94.9% of patients lost 5% or more of their initial weight. The degree of weight loss was identified to be related to self-assessment of diet control and self-assessment of appetite control. The average self-weighing frequency significantly increased(2.45 times/week before treatment and 4.70 times/weeks during treatment). Constipation, insomnia, and dizziness were found to be the most frequent AEs, and except for one case, most AEs were mild cases. Conclusion: From this study, we found that KM obesity treatment combined with self-monitoring showed significant weight loss without serious AE.

Slacks pattern development for the female adolescents with lower-body obesity using virtual simulation system (가상착의 시스템을 활용한 하체 비만 여자 청소년의 슬랙스원형 설계)

  • This study analyzes the body shapes of lower-body obese female adolescents and proposes a slacks pattern suitable for their body type. Lower-body obesity is a prevalent type of teenage obesity, and our proposals aim to improve consumer satisfaction in ready-to-wear clothes across this demographic. We first observe characteristics of obese lower bodies, noting significantly above-average thigh and hip circumference. These figures indicate a high degree of curvature in obese lower bodies, along with a large drop value. Leveraging this data, we develop a novel slacks pattern using 3D avatars in a virtual simulation system. The formulas for the main areas of the pattern are as follows: front waist girth W/4+0.75cm+0.5cm, back waist girth W/4+0.5cm-0.5cm, front hip girth H/4+1.25cm-0.5cm, back hip girth H/4+2cm+0.5cm, front crotch extension H/16+0.5cm, back crotch extension H/8+1cm. Results from appearance evaluations show that this pattern minimizes strain rate on the waist and hips, and its average score is significantly higher than that of an alternative pattern that was also evaluated. The minimized strain rate and high average score indicate that our pattern assigns a sufficient amount of space to the appropriate areas. Based on these results, we expect our research to inform slacks pattern development and production for obese consumers of all types.
