유ㆍ무선 및 방송 그리고 인터넷의 망 통합을 염두에 둔 차세대 통신망은 다양한 유형의 단말기 보급과 더불어 새로운 서비스 창출에 대한 필요성이 대두되었고 인터넷 기반의 응용 소프트웨어를 개발하는 방법에 대한 진화가 정보 기술 쪽에서 진행되고 있다. 소프트웨어를 통합하여 비용을 절감하며, 서로 다른 도메인에서 개발된 기능들이 결합된 새로운 응용들을 개발하는 방법론으로서 Web Service 개념이 도입되고 있다. 인터넷에서의 Web Service 기술 적용 시도에 비슷한 시점에 전기통신 분야에서도 표준화 기관들인 ETSI. 3GPP 및 Parlay를 중심으로 통신망 자원들을 사용하기 쉽도록 개방하여 통신 시장에서의 잠재적 신규서비스들을 창출하려는 활발한 노력의 결과 개방형 Parlay/OSA API들을 표준으로 규격화하였으며 최근에 전기통신 분야에 친숙하지 않은 IT 개발자들을 통신 시장에 대거 흡수하여 IT 기술을 전기통신 분야에 접목시키려는 노력의 일환으로 Parlay X Working Group은 Parlay/OSA API들을 더욱 추상화시키기 위한 시도를 하고 있다. ［9］ 본 논문에서는 Parlay Gateway 와 Parlay X Gateway 그리고 Web Service를 개발하여 웹 서비스 기반의 개방형 통신 서비스의 활성화방안에 대해서 기술한다.

Objectives The purpose of this case study is to report the effect of Gwakyangjeonggisangamibang on adenoidal hypertrophy. Methods The subject was a child with adenoidal hypertrophy. This patient was treated with oriental herbal medicine. The improvement was observed by subjective symptoms and A/N ratio, and OSA-18 survey was performed to compare quality of life before and after the treatment. Results After the treatment, the symptoms of adenoidal hypertrophy were relieved. A/N ratio of this child went down from 0.9 to 0.65. Also, the significant OSA-18 score change demonstrated that the patient's quality of life was improved. Conclusions This study showed that a conservative treatment using herbal medicine can be an effective choice for adenoidal hypertrophy before deciding to performing an adenoidectomy.

As the obesity patients increase recently, breath disease such an OSA(obstructive sleep apnea) is also increasing. When the breath disease occurs, the risk comes to be high. Dependence degree the PAP(positive airway pressure) is also coming to be high. The mechanical simulator is composed cylinder, valve, ball screw and the motor that they correspond to the lung and airway, the diaphragm of the human. In order to confirm the characteristic of the motor and the valve, it accomplished an test. The simulator traces breath pattern against the normal breath and the OSA.

Schizophrenia is a chronic, currently incurable, and devastating syndrome. Although sleep disturbances are not primary symptoms of schizophrenia, they are important aspects of schizophrenia. Difficulties initiating or maintaining sleep are frequently encountered in patients with schizophrenia. Many schizophrenics report low subjective sleep quality. Measured by polysomnography, increased sleep latency as well as reduced total sleep time, sleep efficiency, slow wave sleep, and rapid eye movement sleep latency (REM latency), are found in most patients with schizophrenia and appear to be an important aspect of the pathophysiology of this disorder. Some literatures suggest that worsening sleep quality precedes schizophrenic exacerbations. Co-morbid sleep disorders such as obstructive sleep apnea (OSA) and restless legs syndrome (RLS), and sleep-disrupting behaviors associated with schizophrenia may lead to sleep disturbances. Clinicians should screen the patient with sleep complaints for primary sleep disorders like OSA and RLS, and carefully evaluate sleep hygiene behaviors of all patients with schizophrenia who complain of sleep disturbances.

Towards the deployment of 3rd generation systems, Virtual Home Environment (VHE), stemming from 3GPP, appears to be a powerful concept advocating the provision and delivery of personalized services to roaming users across network and terminal boundaries. This paper aims to expose the pathway for materializing the VHE concept, via the specification, implementation and evaluation of a prototype. Core aspects for realizing the concept such as the business and roaming models are presented, while a VHE enabling architecture utilizing key technologies (OSA/Parlay, MExE, and USAT) is introduced. The proposed architecture has already been evaluated by performing an initial trial demonstration. Currently it is being extended to support VHE service provision to roaming users, an aspect to be addressed by the final trial.

The paper describes the study of concurrent subspace optimization(CSSO) for coupled multidisciplinary design optimization (MDO) techniques in mechanical systems. This method is a solution to large scale coupled multidisciplinary system, wherein the original problem is decomposed into a set of smaller, more tractable subproblems. Key elements in CSSO are consisted of global sensitivity equation(GSE), subspace optimization (SSO), optimum sensitivity analysis(OSA), and coordination optimization problem(COP) so as to inquiry valanced design solutions finally, Automatic differentiation has an ability to provide a robust sensitivity solution, and have shown the numerical numerical effectiveness over finite difference schemes wherein the perturbed step size in design variable is required. The present paper will develop the automatic differentiation based concurrent subspace optimization(AD-CSSO) in MDO. An automatic differentiation tool in FORTRAN(ADIFOR) will be employed to evaluate sensitivities. The use of exact function derivatives in GSE, OSA and COP makes Possible to enhance the numerical accuracy during the iterative design process. The paper discusses how much influence on final optimal design compared with traditional all-in-one approach, finite difference based CSSO and AD-CSSO applying coupled design variables.

이동 망 및 인터넷의 폭발적인 확장과 망 통합의 가속화에 따라 다양한 서비스 기능 및 데이터의 결합이 필수적인 서비스가 요구되고 있으나, 기존의 지능 망 기반 구조로는 이러한 서비스에 대한 대처하기가 현실적으로 불가능하다 이에 대한 해결책으로써 국제 표준화 포럼인 Parlay 를 중심으로 개방형 기술 기반의 서비스 API(Application Programming interface)를 개발해 오고 있다. 본 논문에서는 개방형 서비스 API 의 표준 규격인 Parlay/OSA API 구조에 대해서 살펴본다. IP 기반의 통합 망에서의 본 API 적용을 위한 망 구조를 제시하고, 실제 서비스 예를 대상으로 망 내각 노드 간의 처리 흐름 및 Parlay/OSA API와 망 내 신호 방식간 연동방법 등을 제안하였다.

Considering psychosocial needs of patients, it is not surprising that surgery-first approach (SFA) is becoming more popular than ever. Although the concept of SFA was introduced a few decades ago, the limitation of analysis method based on two-dimensional images makes surgeons reluctant to choose SFA. Recently, the advancement of three-dimensional technology allows us to perform SFA even without minimal pre-surgical orthodontic treatment, and the prediction of surgical outcome became more accurate, especially in obstructive sleep apnea (OSA) patients to whom the advantages of SFA should be more significant. Here, we describe the current trend of SFA and its implication in OSA patients.

Periodic limb movements during sleep (PLMS) are prevalent in various sleep disorders, such as restless legs syndrome (RLS), periodic limb movements disorder, obstructive sleep apnea (OSA), REM sleep behavior disorder (RBD), and narcolepsy. PLMS has been hypothesized to be related to the decline of dopaminergic transmission. In RLS, PLMS is suggested to be related to iron deficiency and symptom severity. PLMD is a rare sleep disorder, and the role of PLMS in PLMD has not been clearly investigated yet. PLMS in OSA, which remain after proper PAP therapy, may need further management. The clinical relevance of PLMS in RBD and narcolepsy have not been investigated thoroughly and need further studies. Whether PLMS are to be considered as a mere symptom of individual sleep disorders or not can be elucidated through studies investigating the efficacy of therapeutic approaches to reduce PLMS in various sleep disorders.
