  • Objectives : The concurrent use of Korean Medicine (KM) and Western Medicine (WM) for the management of disease are increasing. In this study, the factors determining the choice of concurrent use of KM and WM was investigated based on national survey data for the public. Methods : Survey data from 1239 people with experience treating diseases with KM were utilized for this study. The national survey data were gathered using e-mail and face-to-face interviews between December 2007 and January 2008. Results : Among people who had been treated with KM, 819 (66%) used both KM and WM to the treat same disease. Multivariable analysis revealed that concurrent users of KM and WM were significantly associated with age 40-69 (OR=1.43, 95% CI=1.12-1.82), university education or higher (OR=1.34; 95% CI=1.05-1.71), enhanced health status (OR = 0.47, 95% CI=0.35-0.62) and unsatisfied with WM (OR = 1.71, 95% CI=1.02-2.87). The respondents that reported KM was more effective than WM among concurrent users of KM and WM were significantly associated with being an office worker (OR=1.78, 95% CI=1.25-2.53), age 40-69 (OR = 0.62, 95% CI=0.44-0.86), unsatisfied with WM (OR=2.61, 95% CI=1.51-4.50) and overall satisfied with KM (OR=8.38, 95% CI=5.65-12.44) Conclusions : This study showed that two-thirds of KM user were concurrent user of KM and WM, and some influential factors determined the choice of a concurrent use of KM and WM. Therefore, Korean Medicine Doctors should consider the possibility of concurrent use, need to know the information on the benefits and risks of concurrent use.

  • Objectives: This study aimed to evaluate the effect of improper positioning single-peak and multi-peak lights on color change, microhardness of bottom and top, and surface topography of bulk fill and incremental composites after artificial aging for 1 year. Materials and Methods: Bulk fill and incremental composites were cured using multi-peak and single-peak light-emitting diode (LED) following 4 clinical conditions: (1) optimal condition (no angulation or tip displacement), (2) tip-displacement (2 mm), (3) slight tip angulation (α = 20°) and (4) moderate tip angulation (α = 35°). After 1-year of water aging, the specimens were analyzed for color changes (ΔE), Vickers hardness, surface topography (Ra, Rt, and Rv), and scanning electron microscopy. Results: For samples cured by single-peak LED, the improper positioning significantly increases the color change compared to the optimal position regardless of the type of composite (p < 0.001). For multi-peak LED, the type of resin composite and the curing condition displayed a significant effect on ΔE (p < 0.001). For both LEDs, the Vickers hardness and bottom/top ratio of Vickers hardness were affected by the type of composite and the curing condition (p < 0.01). Conclusions: The bulk fill composite presented greater resistance to wear, higher color stability, and better microhardness than the incremental composite when subjected to improper curing. The multi-peak LED improves curing under improper conditions compared to single-peak LED. Prevention of errors when curing composites requires the attention of all personnel involved in the patient's care once the clinical relevance of the appropriate polymerization reflects on reliable long-term outcomes.

  • The objective of this study was to assess beliefs (e.g. advantages, disadvantages, benefits, barriers) and knowledge about eating peanuts and tree nuts. Personal interviews based on the Theory of Planned Behavior were conducted to elicit beliefs about eating nuts. Then, a cross-sectional survey was administered to WIC participants from one county in North Carolina. One-hundred-twenty-four WIC participants (mean (SD) age=28.39 (8.09) completed the study. Most were Caucasian (51.6%) females (96%). About one third believed that eating nuts may help to lower cholesterol level. However, only about one forth believed that nuts can lower a risk of a heart attack or diabetes. More than one third believed that eating nuts will cause weight gain. The knowledge of respondents' about nutrient content of nuts was low with correct answers to most questions about 20% or below. The mean (SD) positive attitude, negative attitude, benefits and barriers scores, based on a range from 1 to 5, were 2.53 (0.91), 3.25 (0.89), 2.97 (0.85) and 2.90 (0.76), respectively. WIC participants' beliefs regarding the health effects of nuts are inconsistent with the most recent research findings. They are in a need to education about benefits of eating nuts.

  • Use of the reconstructed fuel assemblies from partially burnt nuclear fuel pins is analyzed. This reutilization option is a potential candidate technique to make better use of the nuclear resources. Standard two step method is used to calculate node i.e. fuel assembly average burnup and then pin by pin ${\eta}$ values are reconstructed to ascertain the residual reactivity in the used fuel pins. Fuel pins with ${\eta}$ > 1:0 are used to reconstruct to-be-reused fuel assemblies. These reconstructed fuel assemblies are burnt during the cycle 3, 4, 5 and 6 of a 1000 MW PWR core by replacing fresh, once burnt and twice burnt fuel assemblies of the reference core configurations. It is concluded that using reconstructed fuel assemblies for the fresh fuel affect dearly on the cycle length (>50 EFPD) when more than 16 fresh fuel assemblies are replaced. However, this loss is less than 20 days if the number of fresh fuel assemblies is less than eight. For the case of replacing twice burned fuel, cycle length could be increased slightly (10 days or so) provided burnt fuel pins from other reactors were also available. Reactor safety parameters, like axial off set (< ${\pm}10%$), Doppler temperature coefficient (<0), moderator temperature coefficient at HFP (<0) are always satisfied. Though, 2D and 3D pin peaking factors are satisfied (<1:55) and (<2:52) respectively, for the cases using eight or less reconstructed fuel assemblies only.

  • This talk presents the overview of the author's research and development activities on fuzzy inference hardware. We involved it with two distinct approaches. The first approach is to use application specific integrated circuits (ASIC) technology. The fuzzy inference method is directly implemented in silicon. The second approach, which is in its preliminary stage, is to use more conventional microprocessor architecture. Here, we use a quantitative technique used by designer of reduced instruction set computer (RISC) to modify an architecture of a microprocessor. In the ASIC approach, we implemented the most widely used fuzzy inference mechanism directly on silicon. The mechanism is beaded on a max-min compositional rule of inference, and Mandami's method of fuzzy implication. The two VLSI fuzzy inference chips are designed, fabricated, and fully tested. Both used a full-custom CMOS technology. The second and more claborate chip was designed at the University of North Carolina(U C) in cooperation with MCNC. Both VLSI chips had muliple datapaths for rule digital fuzzy inference chips had multiple datapaths for rule evaluation, and they executed multiple fuzzy if-then rules in parallel. The AT & T chip is the first digital fuzzy inference chip in the world. It ran with a 20 MHz clock cycle and achieved an approximately 80.000 Fuzzy Logical inferences Per Second (FLIPS). It stored and executed 16 fuzzy if-then rules. Since it was designed as a proof of concept prototype chip, it had minimal amount of peripheral logic for system integration. UNC/MCNC chip consists of 688,131 transistors of which 476,160 are used for RAM memory. It ran with a 10 MHz clock cycle. The chip has a 3-staged pipeline and initiates a computation of new inference every 64 cycle. This chip achieved an approximately 160,000 FLIPS. The new architecture have the following important improvements from the AT & T chip: Programmable rule set memory (RAM). On-chip fuzzification operation by a table lookup method. On-chip defuzzification operation by a centroid method. Reconfigurable architecture for processing two rule formats. RAM/datapath redundancy for higher yield It can store and execute 51 if-then rule of the following format: IF A and B and C and D Then Do E, and Then Do F. With this format, the chip takes four inputs and produces two outputs. By software reconfiguration, it can store and execute 102 if-then rules of the following simpler format using the same datapath: IF A and B Then Do E. With this format the chip takes two inputs and produces one outputs. We have built two VME-bus board systems based on this chip for Oak Ridge National Laboratory (ORNL). The board is now installed in a robot at ORNL. Researchers uses this board for experiment in autonomous robot navigation. The Fuzzy Logic system board places the Fuzzy chip into a VMEbus environment. High level C language functions hide the operational details of the board from the applications programme . The programmer treats rule memories and fuzzification function memories as local structures passed as parameters to the C functions. ASIC fuzzy inference hardware is extremely fast, but they are limited in generality. Many aspects of the design are limited or fixed. We have proposed to designing a are limited or fixed. We have proposed to designing a fuzzy information processor as an application specific processor using a quantitative approach. The quantitative approach was developed by RISC designers. In effect, we are interested in evaluating the effectiveness of a specialized RISC processor for fuzzy information processing. As the first step, we measured the possible speed-up of a fuzzy inference program based on if-then rules by an introduction of specialized instructions, i.e., min and max instructions. The minimum and maximum operations are heavily used in fuzzy logic applications as fuzzy intersection and union. We performed measurements using a MIPS R3000 as a base micropro essor. The initial result is encouraging. We can achieve as high as a 2.5 increase in inference speed if the R3000 had min and max instructions. Also, they are useful for speeding up other fuzzy operations such as bounded product and bounded sum. The embedded processor's main task is to control some device or process. It usually runs a single or a embedded processer to create an embedded processor for fuzzy control is very effective. Table I shows the measured speed of the inference by a MIPS R3000 microprocessor, a fictitious MIPS R3000 microprocessor with min and max instructions, and a UNC/MCNC ASIC fuzzy inference chip. The software that used on microprocessors is a simulator of the ASIC chip. The first row is the computation time in seconds of 6000 inferences using 51 rules where each fuzzy set is represented by an array of 64 elements. The second row is the time required to perform a single inference. The last row is the fuzzy logical inferences per second (FLIPS) measured for ach device. There is a large gap in run time between the ASIC and software approaches even if we resort to a specialized fuzzy microprocessor. As for design time and cost, these two approaches represent two extremes. An ASIC approach is extremely expensive. It is, therefore, an important research topic to design a specialized computing architecture for fuzzy applications that falls between these two extremes both in run time and design time/cost. • Cardiac arrest, hypoxia, shock or seizure has been known to induce cerebral ischemia. This study was designed to investigate the effect of ischemia on hippocampal pyramidal layer induced by transient bilateral occlusion of the common carotid arteries. Mature Mongolian gerbils were sacrificed at days 2, 4, and 7 after carotid occlusion for 10 minutes. Sham-operated gerbils of control group were subjected to the same protocol except for carotid occlusion. During operation for ischemia, body temperature was maintained $37{\pm}0.5^{\circ}C$ in all gerbils. Paraffin-embedded brain tissue blocks were cut into coronal slices and stained with H-E stain or immunostain by TUNEL method. Neurons with the oval and prominent nucleus and without the eosinophilic cytoplasm in the subfield of hippocamapal pyramidal layer were calculated as to be viable neurons. Their chromatins were condensed or clumped. Their nuclei appeared multiangular or irregularly shrinked. The width of the pyramidal layer was reduced due to the loss of nuclei. At day 2 after reperfusion, some neurons in the CA1 subfield were slightly eosinophilic. But most neurons in the CA2 subfield were strongly eosinophilic. At day 4 day, most neurons in the CA1 subfield were severely damaged and at day 7 day, only a few survived neurons were observed. Survived neurons per longitudinal 1mm sector in the CA1, CA2, CA3, and CA4 subfields of pyramidal layer were investigated. At day 2, the mean numbers of pyramidal neurons in CA1, CA2, CA3, and CA4 subfiedls were 104.5/mm (54.3%), 51.0/mm (33.8%), 105.5/mm (85.6%), and 124.3/mm (93.5%) compared to the nonischemic control group, respectively. At day 4, the mean numbers of pyramidal neurons in CA1, CA2, CA3, and CA4 subfields were 3.2/mm (1.7%), 51.5/mm(34.2%), 95.3/mm (77.4%), and 112.5/mm (84.6%), respectively. At day 7, the mean numbers of pyramidal neurons in CA1, CA2, CA3, and CA4 subfiedls were 0.8/mm (0.4%), 5.7/mm(3.8%), 9.8/mm (8.0%), and 5.0/mm (3.7%), respectively. The mean numbers of apoptotic positive neurons in the CA1 subfield at day 2, 4, and 7 after reperfusion were 67.8/mm, 153.2/mm and 123.7/mm, respectively. These results suggest that the transient cerebral ischemia cause severe damages in most neurons at day 7 and that the prosminent apoptotic positive neurons in hippocampal pyramidal layer are the delayed neuronal death induced by ischemia.

  • 1. Introduction Community medicine with the concept of comprehensive medical care and an ideal medical care delivery system not only for an individual or family but for the whole community has emerged. In April 1970, the Presbyterian Medical Center started a hospital based community health service project in order to improve the health of the people in rural areas. Prior to commencing a comprehensive medical care system, a family survey was needed. The major objective of this survey was to obtain information concerning the people and their environment so as to be able to plan and implement a comprehensive medical care program in Soyang-Myun. 2. Survey Method An interview using a family record form was carried out for each household. This family record form was designed to get information about demography, family planning, environmental sanitation and vital statistics. Prior to beginning, the members of the survey team were trained in interviewing techniques for three days. The team consisted of a public health nurse, four nurse-aides, a sanitarian and four health extension workers who are working in our project, The survey was carried out during the period November 1971 to March 1972. 3. • The main purpose of this study was to investigate attitudes toward mental illness and the mentally ill of psychiatric nurses in Korea and to identify the relationships between the nurses' attitudes and demographic variables. Subjects in this study were 122 psychiatricnurses and 111non-psychiatric nurses in active service at 12 hospitals in Seoul. The insrtument used for measuring attitudes was a questionaire developed by Cohen and Struening (1962) referred to as the“Opinions about Mental illness(OMI) Scale”made up to 51 Likert-type items. The findings of this study indicated hatnurses showed negative attitudetoward mental illness and the mentally ill: Very high on social restrictiveness (factor D), low on Mental Health Ideology (factor C), and Interpersonal Etiology (factor E). Since the high score on Factor A, B and the low score on Factor B, C, E reflect a negative altitudes toward mental illness, this study population related extremely negative attitudes compared to any other surveys. And of the demographic variables that related to their attitudes the education degree and the kind of the hospital in active service showed very significant differences. On the basis of the investigation the findings indicated the followings; 1) Althohg attitudes toward mental illness and the mentally ill of psychiatric nurses were not significant differences from non-psychatric nurses, there was a trend that attitudes of psychaitric nurses did show more negative responses rather than those of non-psychiatric nurses. 2) Demographic variables that relate to their attitudes on the OMI scale were the education degree of the respondent and the kind of their hospital in active service. 3) About attitudes toward mental illness and the mentally ill baccalaureate nursesshowed more positive attitudes in Factor A (Authoritarianism), Factor C (Mental Health Ideology) and Factor D (Social Restrictiveness) than diploma program murses. 4) Nurses in active Service in the private hospital revealed more positive attitudes(A,B,C,D) except Factor (E) than those in the national or public hospital. 5) The ages, duration of work, wanted or unwantedG roup of psychiatric ward and satisfactory level of psychiatric nursing service were non-significant.
