Database Design in Ward Nursing Information System (병동간호업무 전산화를 위한 데이터베이스구축;간호업무기록지를 중심으로)

  • In the complexity and diversity of modern society, there is an urgent need for an information system which can systematically collect, manage and analyze data. Especially in the discipline of nursing, a nursing informarion system is necessary to maximize nursing resources and improve nursing care in the present system which is faced with increases in client needs and multiple changes in hospital environments. This research was done to provide a basis for the development of an integrative nursing information system for the future, by designing dababases items which were extracted from an analysis of the ward nursing information system on general wards excluding the OPD, ICU, OR and CSR with functions using a different system from the wards, and the design of output screen used the database items. The ward nursing information system was analysed through analysis of nursing practice related to recordings, such as the worksheet, kardex, and other nursing practice recordings, on 25 wards. The development of the database was the part of the construction of hospital information system and used the database development life cycle which is related to the system development life cycle. The database development steps included selection of database management system and design of a physical database following the principles of the order communication system which is been developing at Y University Hospital. Conceptual database and Logical database were designed using the base of 25 data items and fields derived from analysing the worksheet, the data items and fields derived from the kardex and other nursing practice recording, from these 19 data base tables were framed through transforming the relational database. Through this process, four types of output material for nursing practice recording which nurses can carry and use during their nursing practice were produced.

Postpneumonectomy Syndrome after Left Pneumonectomy -one case report- (좌측 전폐절제술후 발생한 Postpneumonectomy Syndrome의 치험 1례)

  • Postpneumonectomy syndrome is a rare and delayed complication of left pneumonectomy in most patients with normal mediastinal vascular anatomy. This syndrome is characterized by dyspnea and recurrent pulmonary infection in the remaining right lung that typically occur within the first postoperative year. The condition is believed to be secondary to postsurgical changes that include a marked shift of the mediastinum to the left, clockwise rotation of the heart and great vessels, and herniation of the right lung into the left anterior thorax. These changes lead to compression of the trachea or right main bronchus among the thoracic spine and the right pulmonary artery. We report a case of postpneumonectomy syndrome in 15 year-old girl that followed by left pneumonectomy for bronchiectasis 6 years ago. We have inserted an expandable prosthesis in the left thoracic cavity posterior to the heart. After implantation of an expandable prosthesis, an anatomic reposition of the shifted mediastinum was achieved, which resulted in instantaneous and sustained relief. The post-operative course was uneventful and the patient was followed in OPD from after discharge to now.

Taussig-Bing Anomaly with Coarctation of Aorta (대동맥 축착을 동반한 Taussig-Bing 기형의 수술 치험 1예)

  • The patient was a 30-day-old female infant with symptoms of severe dyspnea and cyanosis, when she was admitted to the ER. The echocardiography revealed DORV with subpulmonary VSD, and the diagnosis of Taussig-Bing anomaly was made. Two days after admission, an urgent operation was performed. The operation consisted of intraventricular tunnel repair and arterial switch operation. She was discharged, and after checking her chest X-ray through OPD, there was no interval change of cardiomegaly. She was then re-admitted, and the angiography revealed coactation of aorta. We performed a resection and end-to-end anastomosis of aorta. She is currently in good condition 11 months postoperatively.

The Effects of the Nurses' Knowledge to Breast-Feeding on the Nursing Activities for Breast-Feeding (간호사의 모유수유에 대한 지식정도가 모유수유 간호활동에 미치는 영향 - 산과외래, 분만실, 산과병실 및 신생아실 간호사를 중심으로)

  • This study was attempted to identify the knowledge to breast-feeding and the performance of nursing activities for breast-feeding and to test 'how does the degree of knowledge to breast-feeding influence the nursing activities for breast-feeding?' The subjects were 180 nurses working in delivery rooms, nursery, obstetrics & pediatrics wards or OPD of obstetrics & gynecology of 8 general hospitals in Pusan as of August 3 through 13, 1996. The results are abstracted as follows. 1) Subject nurses' age, 25-29 was 45.6%(the major), education levels graduates from junior college were 95.0%, unmarried status was 62.2%, 76.5% of married nurses had children, 39.7% in-service education for Breast-Feeding, 33.7% did nursing activities for breast-feeding actively, the reason for inadequate activities for breast-feeding was 'too much other tasks.' 2) The degree of knowledge to breast-feeding ; mean score was 13.54, the degree of performance of nursing activities for breast-feeding : 92.38±20.93 points out of possible 145 points (3.19±.74 out of possible 5 points) moaned that it was a low level. 3) The hypothesis 'the nurses who have higher degrees of knowledge to breast-feeding will show higher degrees of performance of nursing activities for breast-feeding than the nurses who have lower degrees of knowledge to breast-feeding' was tested by t-test(t=-.01, P=.9888), but rejected because it turned out statistically not significant at the level of P<.05. Above results suggested the degrees of knowledge to breast-feeding and the degrees of performance of nursing activities for breast-feeding were generally low and the degree of knowledge didn't influence the nursing activities. Researchers believe that the education for breast-feeding by the nurses need to be performed systemically & practically and new method of breast-feeding education program need to include hospital managers as well as nurses related mothers and their family. In audition, researchers propose the introduction of &lactation specialist system&, for the specialist can change the attitude of feeding-mothers positively with their specialty and authority.

A Clinical Report of the Patient with Limited-Stage Small Cell Lung Cancer (제한병기 소세포폐암 환자의 치험례 임상보고)

  • Objectives : This study was designed to evaluate the effects of oriental herb medicine therapy on a limited stage small ell lung cancer(SCLS) patient. Methods : The medical record of this case of SCLS was researched. The patient had been treated with oriental herb medicine for five months continuously in OPD. Clinical data was analyzed and the mass of small cell lung cancer before and after oriental herb medicine therapy was compared using X-ray and CT scan of the whole body in order to determine metastasis. Result : After 5 months of oriental herb medical treatment, most of symptoms disappeared or improved. The small cell lung cancer patially receeded and improvements were mainly seen in sputum, coughing, insomnia, diarrhea, and performance status. Conclusion : The study suggests that oriental herb medicine therapy effects the tumor size by causing remission and Improving health and relieving symptoms, and also positively effects the quality of life as a supportive or curative therapy for SCLC patients.

Analysis of the Factors Regarding Work-related Musculoskeletal Disease by Company Size (사업장 규모별 업무상 근골격계질환 요양 실태와 영향 요인)

  • Purpose: This study was constructed in order to examine factors that influence work-related musculoskeletal disease (WMSD) approvals and current status according to the company size. Method: This is a descriptive study that utilized Industrial Accident Compensation Claim Data. Workplaces with over 35,811 workers derived from the 2012 claim data, which comprised approximately 91.5%, were selected for this study. Then workplaces were divided into three groups according to the number of workers: less than 5, 5~299, and 300 and over. Results: Since 2008, the number of small sized workplaces has increased. The 2012 data showed that 32.5% of workers at small sized workplace had WMSD. However, workplaces with 5~299 workers showed WMSD approval rate of 60%. Of note most WMSD approved workers were employed by manufacturing and construction companies, regardless of the workplace size. Most of them were engaged in elementary tasks. The days of medical treatment at OPD and IPD were most prevalent among workers at the largest workplaces. Conclusions: It is certain from this study that WMSD has been polarized by the company size. More policy attention should be paid to the WMSD status of workers at small sized workplaces which usually do not have their own health office.

The Financial Support of Regional Bank for SMEs and Regional Financial Policy (지방은행의 지역중소벤처기업지원과 지역금융정책)

  • This study investigates regional financial markets and proposes some policy measures for boosting up the regional financial system. Financial supports for start-ups, small and medium sized enterprises(SMEs) in early stage growth have been constrained due to expanded financial gaps among regions during financial restructuring period. The bank consolidation through M&A is associated with widening financial gaps between Seoul area and others, and between regions where regional banks survives or not. Loans to small firms are associated with relationship lending techniques that may be better supported by smaller regional banks. The loan rate to SMEs in locals where regional banks are activated has tended to be 10%point higher than the locals where regional banks closed, and also drive the greater contribution for preventing local capital outflow and promoting local capital investment by local penetrating strategy. Government should develop regional financial policies to boost up regional financial system, and expand the business area of regional financial institutions for supporting start-ups and SMEs.

Surgical Treatment of Aortic Aneurysm - Review of 37 cases between 1984 and 1987 - (대동맥류의 외과적 치료 -37례 보고 (1984-1987) -)

  • Thirty-seven patients of aortic aneurysm underwent operations during January 1984 December 1987 at our hospital. Twenty-six patients had aneurysms involving ascending aorta, three patients had aneurysms involving both ascending aorta and abdominal aorta. and eleven patients had aneurysms involving descending thoracic or abdominal aorta. Among the patients who had aneurysms involving ascending aorta, annuloaortic ectasia with aortic regurgitation were thirteen and all of these underwent ascending aorta graft replacement + AVR with composite graft. The patients who had aortic regurgitation due to ascending aortic dissection were three and all of these underwent intraluminal ringed graft insertion at ascending aorta + aortic valve resuspension. Intraluminal ringed graft insertion was safe, simple, and fast method in the operation for aortic aneurysm. Eleven patients were underwent this operation and the results were good. Major causes of death of the patients who underwent aortic aneurysm operation are underlying cardiovascular diseases or delayed rupture of the aneurysm or complications related newly appeared aneurysm. Among our patients, dissection progressions were appeared in two but neither severe nor complicated. And no patient died from delayed rupture of aneurysm or complications related newly appeared aneurysm. All patients were followed up via OPD and were controlled hypertension or heart failure if present. Operative mortality is 18.9\ulcornera in all, 23% in patients who had aneurysms involving ascending aorta and 7.6` who had aneurysms involving descending thoracic or abdominal aorta. Comparing with other reports, our operative mortality is still high but improved steadily. So we recommend aggressive surgical management of the aortic aneurysm.

A Study on Loose Laboratory Reports in A Hospital (일개(K) 병원의 누락 조직검사결과지에 관한 조사연구)

  • Background : The medical record is a compilation of pertinent facts of a patient's life and health history, including past and present illness and treatment. It is written by the health professionals contributing to that patient's care. And the medical record is the permanent, legal document which must contain sufficient information to identify the patient, justify the diagnosis and treatment, and record the results. As such, it must be accurate and complete. So we try to analyze the medical record especially a kind of incomplete record, loose laboratory reports. Methods: During the one-year period(from January to December 1988), a medical record practitioner examine and analyze the record of laboratory reports at K Hospital in Seoul. A total of 320 loose laboratory reports for 3,818 admitted laboratory reports. And a medical record practitioner and a physician review and analyze the influencing factors for the various reasons of clinical and laboratory aspects. Result: The loose percentage by department is the highest in obstetrics(40.4%) but the highest loose rate is in pediatrics(25.0%). The most of omission is occurred in operation room(80.3%) than OPD(19.7%). The change of diagnosis is according to duration of laboratory and more changable in cancer patient. Conclusion : Regular analysis of the documentation in the medical record so it fulfills its purposes of communicating patient care information. So it serves as evidence of the patient's course of illness and treatment for various legal, reimbursement, and peer evaluation review. And it is very important aspect of quality assurance in medical activities.

The Clinical observation of acute Bell's palsy 80 Case (급성기 안면마비 (Bell's palsy) 환자의 예후 및 치료율에 대한 임상고찰 80례)

  • Objective : Bell's palsy is common and has many clinic study. but bell's palsy prognosis is not enough specific. So this study was evaluated bell's palsy prognosis, treatment number, sequela of normal group and bad prognosis group. Methods : From June 2009 to June 2010, patients who visited Dong-seo Oriental Medicine ENT. A clinic study was done on patient who were diagnosed bell's palsy, onset 2weeks within when first visited OPD and treated 3 times over in Dong-seo Oriental Medicine Cental. To evaluate grade of paralysis, House-Brackman Scale was used. We classified treatment numbers of each HB-Scale group, normal gIVroup and bad prognosis group. Results : The distribution of Onset HB-Scale : Gr II 26.25%, Gr III 67.5%, Gr IV 6.25% Onset HB-Scale Gr II patients completely recover 100% Onset HB-Scale Gr III patients completely recover 64.8%, improved 27.8%, nothing change 7.4% Onset HB-Scale Gr IV patients completely recover 40%, improved 60% Onset HB-Scale Gr II & IV patients recovery percentage make no difference of normal group (Group A) and bad prognosis. Onset HB-Scale Gr III patients completely recover Group A 66.7%, Group B 52.9%, improved Group A 23.2%, Group 35.3%, noting change Group A 5.1%, Group B 11.8% Onset HB-Scale Gr II patients has no sequela. Onset HB-Scale Gr III & IV patients has tendency that they treat more times, more improving and less sequela probability Conclusion : Onset HB-Scale is the indicator of acute bell's palsy prognosis.
