체외막산소화장치 (ECMO) 적용 중환자의 영양지표 비교 (Comparison of the nutritional indicators of critically Ill patients on extracorporeal membrane oxygen (ECMO))

  • 본 연구는 부적절한 영양지원이 ECMO 환자의 영양상태 뿐만 아니라 사망률 등에도 부정적인 영향을 미칠 수 있음을 보여주었다. 대부분의 ECMO 환자는 중환자실 입실 초기부터 영양상태가 이미 영양불량이거나 영양불량 위험군이었고, 재원기간이 경과하면서 악화되는 것을 관찰하였다. 이는 ECMO 환자들이 가장 위중할 때에 필요로 하는 칼로리와 단백질의 영양 지원이 충분하지 않았음을 시사하는 것이며, 특히 PN과 EN의 개시 시점이 영양관리지침의 권고보다 지연되지 않도록 노력하는 개선이 시급하다. 또한, ECMO 환자의 영양상태 평가만 아니라 공급한 영양지원에 대한 환자의 반응까지 고려한 영양관리가 필요한데, 이를 위해서는 영양관리가 중환자 간호에서 우선시 되어야 하며, 영양사를 포함한 다학제간 접근이 반드시 포함되어야 한다. 점차 증가하는 ECMO 적용 사례를 고려한다면, 중환자실에서의 집중치료 과정에 영양관리도 비중있게 반영되어야 할 것이다. 차후에는 전향적 무작위통제 연구로 환자의 질환 유형이나 ECMO 배치형태에 따른 영양지원방법과 효과를 비교할 것을 제안한다. 더 많은 대상자 수 확보를 위하여 여러 병원의 자료를 취합하여 분석하거나 환자 상태에 따라 영양공급의 적정량을 평가하고 비교하는 연구를 제안한다.

A STUDY ON NUTRITIONAL STATUS OF MACRO MINERALS OF CATTLE IN JAVA IN INDONESIA

  • Nutritional status of cattle in Java in Indonesia was investigated by evaluating P, Ca, Mg, K and Na concentrations in forage and plasma inorganic phosphorus (Pi), Ca and Mg, hematocrit and hemoglobin concentrations of cattle during both rainy and dry seasons in 1988. Investigations were conducted at Jonggol (West Java), Malang (East Java) and Majokerto (East Java). Low P content in forage averaging 0.11% on a dry matter basis, high Ca:P ratio in forage in the rainy season (7.7:1) and low plasma Pi concentrations averaging 4.71 mg/dl were observed in Jonggol. In all three places, Ca content in forages was higher than the requirement according to the NRC standard and plasma Ca concentrations were above the critical level. During the rainy season forty percent of cattle in Malang and Mahokerto showed plasma Mg concentrations below 2mg/dl in spite of normal Mg and K content in forage. Forage samples from each place showed a wide variety in Na content, and Na content of two thirds of forage samples was lower than the requirement according to the NRC standard. Hematocrit and hemoglobin concentrations in Malang and Majokerto showed higher values than the normal range.

  • 공시균의 배양여액으로부터 독성물상을 chloroform 에 전용하여 농축한 후 avicel column chromatography 및 avicel TLC로 정제하였다. 독성물질 생성에 요구되는 영양원은 생육에 필요한 영양첨과 일치하지 않았으며 Valine, asparagine, arginine, 요소, D-glucose, D-mannose, L-rhamnose, D-ribose, D-xylose, D-arabitol, starch 등이 독성물질생성에 요구됨이 인정되었다. 또한 오탄당의 대사과정중 ribose, xylose, fructose, glucose가 대사되는 과정이 본 물질생성의 주된 경로로 추정된다. Vitamin류는 무관함에 비해 Mg가 필수적으로 요구되었다. 물질의 생성은 균의 증식에 비례되었으며 6일간 3$0^{\circ}C$에서 배양했을때 가장 많이 생성되었다. 독성물질 처리후의 잉어 조직은 아가미 신장 췌장등에서 핵진의 굴곡과 더불어 접의 심한 수축과 세포질의 괴사가 현저하였다. 본 물질의 화학식은 $C_{38}$ $H_{66}$ $NO_4$로 추정되며 UV조사시 회청색형광을 나타내었다.

Dietary Reference Intakes of sodium for Koreans: focusing on a new DRI component for chronic disease risk reduction

  • Sodium is a physiologically essential nutrient, but excessive intake is linked to the increased risk of various chronic diseases, particularly cardiovascular. It is, therefore, necessary to accomplish an evidence-based approach and establish the Korean Dietary Reference Intakes (KDRIs) index, to identify both the nutritional adequacy and health effects of sodium. This review presents the rationale for and the process of revising the KDRIs for sodium and, more importantly, establishing the sodium Chronic Disease Risk Reduction Intake (CDRR) level, which is a new specific set of values for chronic disease risk reduction. To establish the 2020 KDRIs for dietary sodium, the committee conducted a systematic literature review of the intake-response relationships between the selected indicators for sodium levels and human chronic diseases. In this review, 43 studies published from January 2014 to December 2018, using databases of PubMed and Web of Science, were finally included for evaluating the risk of bias and strength of evidence (SoE). We determined that SoE of the relationship between dietary sodium and cardiovascular diseases, cerebrovascular disease, and hypertension, was moderate to strong. However, due to insufficient scientific evidence, we were unable to establish the estimated average requirement and the recommended nutrient intake for dietary sodium. Therefore, the adequate intake of sodium for adults was established to be 1,500 mg/day, whereas the CDRR for dietary sodium was established at 2,300 mg/day for adults. Intake goal for dietary sodium established in the 2015 KDRIs instead of the tolerable upper intake level was not presented in the 2020 KDRIs. For the next revision of the KDRIs, there is a requirement to pursue further studies on nutritional adequacy and toxicity of dietary sodium, and their associations with chronic disease endpoint in the Korean population.

여자 청소년 음식 기반 권장식사패턴의 제안과 이를 적용하여 작성한 식단의 평가 (Proposition and Application of a Dish-Based Target Pattern for Korean Adolescent Girls)

  • Objectives: Maintaining a balanced diet and thus health is crucial for adolescents, and the first step for balanced diet practice is meal planning. Adolescents, however, find it difficult to plan their meals. This study thus was set out to design an easier way of planning meals for adolescent girls. Methods: A dish-based target pattern for adolescent girls was tabulated, and validity of this was examined. Meal plan applying a dish-based target pattern was prepared by 150 female middle school students, and nutritional adequacies of those meal plans were examined. Validity and adequacy were tested by energy content, energy contribution ratio, nutrient adequacy ratio (NAR), probability of nutrient inadequacy, index of nutritional quality (INQ) calculation. Results: A dish-based target pattern with 11 dish groups was validated for nutritional adequacy. Though the NAR of calcium was 0.96, the INQ of calcium was 1.00. The average energy supply from the meal plans was 2,379 kcal, higher than the estimated energy requirement of a female middle school student, but the energy contribution ratio of carbohydrates, proteins, and fats were all adequate according to the acceptable macronutrient distribution range (AMDR). NAR of all nutrients examined were 1.0, except for calcium. The NAR and INQ of calcium were 0.87 and 0.75, respectively, and the meal plans at risk for calcium inadequacy was 19.30%. Conclusions: A dish-based target pattern proposed for adolescent girls was valid, but the meal plan prepared by female middle school students using this approach was high in energy and low in calcium supply. To cut down the energy supply from the meal plan, it is necessary to recommend dishes low in fat and use low fat cooking methods. To increase the calcium supply, it is important to recommend seaweed and legume group dishes with higher Ca INQ food items.

경남 일부 지역 여자 노인의 건강 및 영양 상태 조사 (A Study on Health Conditions and Nutritional Status of Elderly Women in Gyeongnam)

  • This study was performed to assess the nutritional status of low income elderly women aged ${\geq}$65 years residing in Gyeongnam Masan (n=124). Nutrition intakes, food intake frequency, and health-related behaviors including smoking, drinking, and exercise were investigated. Nutrition intake was calculated by the 24-hour recall method using CAN-pro (ver. 3.0). Average daily intakes of energy were $1,142.3{\pm}39$ kcal (71.4% of EER) in subjects aged 65~74 years and $1,071.0{\pm}41.7$ kcal (66.9% of EER) in subjects aged ${\geq}$75 years and the subjects consumed energy less than both 75% of estimated energy requirement (EER). The proportions of energy derived from protein, fat, and carbohydrate were 15.4:15.5:70.6 (aged 65~74 years), and 15.3:13.4:70.8 (aged ${\geq}$75). Nutrients consumed at less than estimated average requirements (EARs) were Ca (60.4%), P (98.4%), Zn (91%), vitamin E (48% of adequate intake, AI), vitamin $B_1$ (63.3%), vitamin $B_2$ (54%), niacin (87.7%), vitamin C (62.5%), and folate (50.5%). Especially, the intakes of Ca (58%), vitamin E (41% of AI), vitamin $B_1$ (60%), vitamin $B_2$ (50%), folate (46.5%), and vitamin C (54%) were 75% less than the EAR for people aged ${\geq}$75 years. According to the food intake frequency survey, the intakes of calcium, milk, fruits, and vegetables were very poor. In conclusion, this study suggests that a nutritional support program for elderly women of low socioeconomic class must be provided by the government to improve the quality of remaining life.

대전지역 수유기 여성의 영양섭취 상태와 식행동 (Nutritional Status and Eating Behavior of Lactating Women in Daejeon)

  • Nutrients intake status of 73 lactating women, that is 45 breast feeding (BF), 13 formula feeding (FF) and 15 mixed feeding (MF), living in Daejeon was investigated. Self-recorded food intakes for two weekdays and eating behaviors using questionnaires were surveyed from May to August 2008. Subjects aged $29.2{\pm}3.4$ years and their infants aged $8.2{\pm}3.2$ months. Body mass index of the subjects was $21.0{\pm}3.2$. Of the subjects 23.3% were employed. Daily energy intake was $1953{\pm}391$ kcal. Mean adequacy ratio (MAR) was $0.77{\pm}0.14$ and was higher in FF ($0.86{\pm}0.13$) than in BF ($0.76{\pm}0.11$) and MF ($0.72{\pm}0.18$). Nutrients that over 50% of the subjects took less than estimated average requirement were vitamin A, riboflavin, folate, vitamin C and calcium. And index of nutritional qualities of those 5 nutrients were below one Forty six and sixths percent (46.6%) of the subjects showed GMFVD = 11111 pattern of five food group intakes and 57.5% took meals three times daily, 56.2% skipped often breakfast, and 64.4% dined out two times and more per week. Subjects having lower MAR (< 0.72, n = 24), compared with those having higher MAR (${\geq}$ 0.83, n = 24), showed more skipping daily meals and less frequency of eating-out, and took less legumes, vegetables, fishes, and milk. As the results, intakes of calcium, vitamin A, riboflavin, folate, and vitamin C were insufficient in lactating women, especially in BF and MF mothers. Meal skipping and low intakes of dairy foods, legumes, vegetables, and fishes might have adverse influences on nutritional status of lactating women. Accordingly, nutrition care program for lactating women should be focused on intake of three meals daily and a variety of food.

운동 목적에 따른 식행동 및 식이섭취 상태 (Eating Behaviors and Nutritional Status According to the Purpose of Exercise)

  • 본 연구는 젊은 성인을 대상으로 운동 목적에 따른 영양섭취 실태를 조사하고자 수행되었다. 규칙적으로 운동하고 있는 20~30대 남녀를 대상으로 운동 목적에 따라 근육증가군 107명과 체중감소군 107명을 나누어 비교 분석하였다. 그룹별 성별구성을 분석한 결과, 근육증가군은 주로 남성(75.2%)이 많은 반면 체중감소군은 여성(74.3%)이 많이 분포하고 있었다. BMI 판정결과 두 그룹간의 유의적인 차이는 없었으며, 체중감소군의 절반 이상(56%)이 저체중 또는 정상체중으로 나타났다. 근육증가군의 하루 운동시간 및 규칙적으로 운동한 기간이 체중감소군에 비하여 유의적으로 많았다(p<0.01). 영양지식의 상태는 근육증가군의 점수가 체중 감소군에 비하여 유의적으로 높은 것으로 나타났다(p<0.05). 식행동 또한 근육량 증가군의 점수가 체중감소군에 비하여 유의적으로 높았다(p<0.05). 영양섭취 상태를 살펴보면, 한국인영양섭취기준과 비교하였을 때 근육증가군의 에너지 섭취비율이 에너지필요추정량의 83.72%로 체중감소군의 74.53%에 비하여 유의적로 높았다(p<0.01). 권장섭취량에 대한 단백질 섭취비율은 근육증가군이 199.62%, 체중감소군은 136.36%로 나타났으며 근육증가군이 체중감소군에 비해 유의적으로 매우 많은 양을 섭취하고 있었다(p<0.001). 권장섭취량에 대한 칼슘 섭취비율은 근육증가군이 67.75%, 체중감소군은 68.64%로 조사되었으며, 두 군 모두 권장섭취량과 비교하여 매우 부족한 수준으로 나타났다. 이와 같이 본 연구에서는 근육증가를 목적으로 운동하는 경우 과량의 단백질을 섭취하고 있었으며, 체중감소를 목적으로 운동하는 경우 필요량에 미치지 못하는 에너지 섭취를 하는 것으로 나타났다.

Nutritional Requirements of Actinomyces Isolated from Rumen of Goat

  • The objective of this work was to investigate the nutritional requirements for the growth of Actinomyces sp. 9RCC5 isolated from the rumen of a native goat in Korea. The growth of strain 9RCC5 on the basal medium or the medium minus certain ingredients from the basal medium demonstrated that strain 9RCC5 showed absolute requirement of vitamin B complex mixture, while hemin and volatile fatty acids (VFA) were stimulatory to growth to some extent. The 9RCC5 strain grew well with casein hydrolysate as the sole added nitrogen source. However, neither a complex of 18 amino acids nor ammonium sulfate effectively replaced casein hydrolysate. Vitamins such as riboflavin and pantothenate were essential for growth, while thiamin and biotin were stimulatory. With regard to VFA, the growth was stimulated by acetic acid but inhibited by valeric acid. Relatively large quantities of $Na^+$, $K^+$ and $Ca^{2+}$ were absolutely required for growth. Supplementation of clarified rumen fluid to the basal medium in a range of 0-10% (vol/vol) resulted in an increased rate of growth as well as an increased extent of growth.

Recent Advances of Nutritional Physiology and Artificial Diet of the silkworm, Bombyx mori, in Japan

  • Recently, the cocoon production is reducing very steeply in Japan. The main reason for this cocoon reduction is due to high labor cost and the production expense is apt to rise year by year. Accordingly, the improvement of cocoon productivity by retrenchment of the production expense is intensively required in my country. Therefore, the silkworm rearing on the artificial diet is largely expected as the innovation techniques of sericulture. Since rearing of the silkworm on artificial diets has first been achieved in 1960, and the silkworm larvae were reared throughout all the instars on artificial diets(Fukuda et al., 1960, Ito & Tanaka, 1960). The artificial diets used at this time contained 50% or more of dried, pulverized mulberyy leaves as one of the main ingredients, and the composition was rather simple. When reared on these diets, larval growth and development were retarded, small cocoons appeared, and the adults laid only a small number of eggs. At first, we would like to study about nutritional requirement of the silkworm, using the chemically defined diet which prepared artificially. Subsequently, attempts were made to improve the diets by replacement of crude ingredients with possibly purer compounds, as mentioned later. Before giving the details on individual nutrient, I would like to summarize about the outline of utilization flow of various nutrients through the larval-pupal development and these daily quantitative requirements per body weight in the silkworm when they were reared on mulberry leaves.
