중독 중환자에서의 초기 영양상태평가와 예후와의 관계 (Assessment of early nutritional state in critical patients with intoxication and the effect of nutritional status on prognosis)

  • Purpose: Nutritional status and support in critically ill patients are important factors in determining patient recovery and prognosis. The aim of this study was to analyze the early nutritional status and the methods of nutritional support in critically ill patients with acute poisoning and to evaluate the effect of nutritional status on prognosis. Methods: A retrospective study was conducted in tertiary care teaching hospital from January 2018 to December 2020. in an emergency department of university hospital, 220 patients who were stayed more than 2 days of poisoning in intensive care unit were enrolled. Results: 155 (70.5%) of patients with acute poisoning had low-risk in nutritional risk screening (NRS). Patients with malignancy had higher NRS (low risk 5.2%, moderate risk 18.5%, high risk 13.2%, p=0.024). Patients of 91.4% supplied nutrition via oral route or enteral route. Parenteral route for starting method of nutritional support were higher in patients with acute poisoning of herbicide or pesticide (medicine 3.2%, herbicide 13.8%, pesticide 22.2%, p=0.000). In multivariate logistic regression analysis, herbicide or pesticide intoxication, higher risk in NRS and sequential organ failure assessment over 4.5 were affecting factor on poor recovery at discharge. Conclusion: NRS in patients intoxicated with herbicide or pesticide were higher than that in patients intoxicated with medicine intoxication. Enteral nutrition in patients intoxicated with herbicide or pesticide was less common. Initial NRS was correlated with recovery at discharge in patient with intoxication. It is expected to be helpful in finding patients with high-risk nutritional status in acute poisoning patients and establishing a treatment plan that can actively implement nutritional support.

A review of canola meal as an alternative feed ingredient for ducks

  • This review provides an overview of the published data on the canola meal and its suitability for duck as an alternative plant-origin protein source to soybean meal. Canola meal is a legume origin protein source containing comparable amino acid profile to soybean meal and rich in essential minerals and vitamins. Nonetheless, it is known to contain less in energy content than soybean meal. Factors like field conditions and processing methods creates compositional variations among canola meal. Presence of anti-nutritional factors such as phenolic substances, phytate and glucosinolates which are known to reduce growth performance in livestock animals, are the major drawbacks for canola meal to be a competitive plant-origin protein source in the feed industry. This review is focused to address i) nutritional characteristics and feeding value of canola meal for ducks and ii) impacts of feeding canola meal on performances of ducks.

비만 아동과 관련된 환경적.심리적 요인 분석 (The Analysis of the Environmental and Psychological Factors in Obese Children)

  • This study was conducted to investigate the differences in environmental and psychological factors in children with obesity and to offer useful information for obesity treatment and prevention. 123 children of 5-6th grade in elementary school and their mothers were included in this study and divided into either the obese group and normal group(obese group: 61, normal group: 62) according to the Weight-Length Index(WLI). Results showed that between the two groups there no significant differences in family income, mother's occupation status, educational level, and anthropometric measurement. Nutritional knowledge in the obese group was significantly higher than that of the normal group(p<0.05). The mothers of children with obesity had a higher level of nutritional knowledge than the mothers of normal children(p<0.05). There were no significant differences in food behavior between the obese and normal groups. There were no significant differences in maternal child-rearing practices and in maternal attitude on flood behavior of children. Considering psychological factors ; 72.1% of the children with obesity appraised their body image as obese(p<0.05), and 54.1% of the mothers in obese group perceived their child's body shape as obese(p<0.05). The level of self-esteem were not significantly different when compared between the obese and normal groups. From these results, it was suggested that effective measures for obesity treatment and prevention should include nutritional monitoring, and such efforts need to be maintained in order to help the obese children and their mothers.

  • Alcoholic liver disease is defined by the development of three types of liver damage following chronic heavy alcohol consumption, namely, alcoholic fatty liver, alcoholic hepatitis, and alcoholic cirrhosis, The clinical features and laboratory tests often do not distinguish among these types of liver injuries. In addition, a considerable number of the patients who have clinical and laboratory features compatible with alcoholic liver disease are diagnosed on liver biopsy to have chronic viral hepatitis or other lesion. Because of these factors, liver biopsy is frequently needed to arrive a definite diagnosis of the disease, its activity, and its chronicity. Fatty liver is usually a benign and reverible condition that disappears on abstinence from alcohol. However, alcoholic hepatitis is usually regarded as a precursor of cirrhosis. The principle factors in the development of alcoholic hepatitis and cirrhosis are the quantity and length of ingestion of alcohol. women are much more susceptible than men to hepatic injuries. Since only 10 - 20% of alcoholics develop cirrhosis, however, it is conceivable that other factors, either genetic, environmental, or nutritional may contribute in the genesis of liver injuries. The most important factor in the treatment of alcoholic liver disease is prolonzed abstinence from alcohol, since abstinence by itself improves clinical status and survival, Nutritional support in patients with nutritional deficiency, and specific drug therapies such as corticosteroid or anabolic steroids for hospitaliged patients with severe alcoholic hepatitis also play an important role in devreasing morbidity and improving survival. Liver transplantation is a newer treatment modality in the patients with advanced cirrhosis, not responsible to medical treatment.

알코올성 간질환자의 음주실태 및 영양섭취실태 (A Study on the Alcohol Consumption and Nutrient Intake in Patients with Alcoholic Liver Disease)

  • The purpose of this study was to evaluate alcohol consumption and nutritional status in patients with alcoholic liver disease. The subjects were 80 patients with alcoholic liver cirrhosis and 12 patients with alcoholic fatty liver. Also 57 alcoholics without liver disease, 32 patients with viral liver cirrhosis and 194 normal men were included as control groups. Data on anthropometric index, socioeconomic status, alcohol consumption, dietary habits and dietary intakes were collected by individual interview. Alcoholic liver disease group had significantly lower triceps skinfold thickness and mid-uppr-arm circumferences than other groups. Socioeconomic status of alcholoci subjects was middle class or lower than that. The amount, duration and frequency of alcohol consumption were significantly higher and the quality of side dishes consumed with alcoholic beverage was significantly poorer in patients with alcoholic liver cirrhosis than others. Patients with alchololic liver disease ingested approximately 40% of daily caloric intake as alcohol and all alcoholic subjects had lower average intakes of protein, carbohydrates, fat, vitamins and minerals as compared with Korean adult average intakes. The results suggest that alcohol and poor dietary intake could cause malnutrition and might be two of the important risk factors to develop alcoholic liver disease in alcoholics. But other factors like genetic and immunological factors should be also considered in elucidating the causes of alcoholic liver disease. An extensive nutritional education should be emphasized for alcohol consuming population to prevent development of alcoholic liver disease.

중환자실 환자의 욕창을 예측하는 영양 관련 요인 (Nutrition-Related Factors Predicted Pressure Ulcers in Intensive Care Unit Patients)

  • Purpose: The purpose of this study was to identify the nutrition-related factors influencing the prevalence of pressure ulcers among patients admitted to an intensive care unit. Methods: Research participants were 112 patients who were admitted to the ICU of a university hospital. Data were collected through questionnaires which included general characteristics, diet-related characteristics, nutritional status, and pressure ulcer status. Multivariate logistic regression was used to identify independent factors association with prevalence of pressure ulcer Results: The prevalence of pressure ulcers was 58 patients (51.8%) at 10th day after ICU admission. In multivariate analysis, prevalence of pressure ulcers at 10th day in ICU was significantly higher in the nutritional risk group (OR=6.43), malnutrition group (OR=88.02), and deceased serum albumin group (OR=28.83). Conclusion: The results of this research indicate that scores on MNA (Mini Nutritional Assessment) and serum albumin were significant predictors of pressure ulcer prevalence in ICU patients. Therefore, regular MNA-SF and albumin checkups are needed to identify risk for pressure ulcer for ICU patients. In the case of decreased MNA-SF scores and serum albumin levels, more intensive pressure ulcer care is needed for ICU patients.

청소년기의 비타민.무기질 보충제 복용에 영향을 미치는 요인 (Vitamin.Mineral Supplement Use and Related Variables by Korean Adolescents)

  • This study was performed to investigate the factors related to vitamin & mineral supplement use by the adolescents. Nine hundred and seventy-two adolescent boys and girls attending middle or high schools were chosen from various cities and rural communities in Korea. In this study, the factors affecting vitamin & mineral supplement use were analyzed from a self-administered questionnaire. It was found that vitamin & mineral supplements were taken by 31.3% of subjects. High school students(p<.01), female girl students(p<.001), and rural-dwellers(p<.001) took vitamin & mineral supplements more frequently than their complementing groups. The socioeconomic status of the family (p<.01), and supplement consumption by parents and siblings respectively affected the supplement use by the subjects(p<.001). Vitamin·mineral supplements were consumed more often in subjects who perceived their health as poor(p<.001), skipped breakfast(p<.05), and received nutritional information from books or magazines(p<.05). However, food habits and nutritional knowledge score of subjects did not affect vitamin & mineral supplement use. These findings suggest that adolescents take vitamin & mineral supplements commonly, and they tend to use supplements without prescription. Vitamin·mineral supplement consumption seems to be affected by general characteristics, health related variables, and meal management attitudes of subjects. Therefore, nutritional understanding and education are required in regard to the used of vitamin & mineral supplements, and the relationship between balanced diet and good health for the adolescents.

  • The purpose of this study was to examine the relationships of nutritional and social factors among 100 children aged $5{\sim}6$ years. Forty five children (45%) were selected from families with low socioeconomic status, while 55 children (55%) were from those with high socioeconomic status. Cognitive performance of the children was measured by the Draw a man test and the degrees of hyperactivity were assessed by both children's mothers and their teachers using two different Check List (Behavior check list and Conner's rating scale). There were few associations between cognitive performance and nutritional variables. However, levels of hyperactivity were related positively to diversity of food intake and weight percentage for age, and negatively to animal and processed food preferences. These results indicate a possible role of nutrition on psychological development.

Association between Visual Impairment and Nutritional Risk among Older Adults with Diabetes: A Population-Based Cross-Sectional Study

  • Purpose: Despite the high prevalence of visual impairment caused by diabetic retinopathy and nutritional problems among older adults with diabetes, evidence regarding factors related to nutritional risk in this population is limited. Therefore, this study aimed to identify the correlates of nutritional risk among older adults with diabetes, focusing on visual impairment. Methods: This study was a secondary data analysis of the 2020 National Survey of Older Koreans aged 65 years and above. The sample comprised 2,376 older adults with diabetes, and complex sample ANOVA and Rao-Scott chi-square tests were used to compare the groups according to visual impairment. Complex-sample logistic regression analyses were conducted to verify the association between visual impairment and nutritional risk. Results: Older adults with diabetes, who also have severe visual impairment, are more likely to have nutritional risk status than those without impairment after controlling for covariates (odds ratio [OR] = 2.44, 95% confidence interval [CI] 1.16~5.13). Among the covariates, depression (OR = 3.58, 95% CI 2.60~4.94), dependent activities of daily living status (OR = 2.79, 95% CI 1.60~4.86), and experience of hospitalization during the past year (OR = 2.51, 95% CI 1.57~4.03) were strongly associated with nutritional risk. Conclusion: Severe visual impairment increases the nutritional risk among older adults with diabetes. Therefore, it is essential to prevent visual impairment due to exacerbation of diabetes through appropriate management. Additionally, tailored nutritional interventions for visually impaired older adults with diabetes that consider visual characteristics are required.

MNA (Mini Nutritional Assessment)를 이용한 건강증진센터 내원 노인의 영양상태 판정 (Nutrition Assessment of Older Subjects in a Health Care Center by MNA (Mini Nutritional Assessment))

  • Elderly people comprise an increasing proportion of the population, and nutritional impairments may contribute to health problems among this group. This study was conducted to evaluate the nutritional status by Mini-Nutritional Assessment (MNA) and to identify relationships among anthropometric measurements, biochemical indicators, bone Mineral Density (BMD) and MNA results among older adults (${\geq}$65 yrs, n=98, 66.7${\pm}$2.5 yrs; M=52, F=46, BMI 24.5${\pm}$2.8 $kg/m^2$) at a Health Care Center. A dietitian administered MNA and collected anthropometric measurements (weight, height, waist circumference), biochemical indicators (albumin, hemoglobin, hematocrit, TLC, glucose, lipids) and the BMD (spine, femur, F=46). Subjects were grouped into a normal nutrition group (0~2 risk factors of malnutrition) and a high risk of malnutrition group (>=3 risk factors of malnutrition) based on their risk factor status for malnutrition. The risk factors for malnutrition include age ${\geq}$65 years, PIBW <90%, albumin <3.5 g/dl, TLC <1,500%, Hgb <14 g/dl (men), Hgb <10 g/dl (women), loss of appetite and weight loss 1~3 kg/last 3 months. In addition, subjects were grouped into a normal, osteopenia and osteoporosis group by BMD. We found that 12% of the subjects were at risk of malnutrition (MNA score, 21.4${\pm}$2.1) and that 88% were well nourished (27.3${\pm}$1.5) according to the MNA. Full-MNA scores were positively and significantly (p<0.05) associated with BMI, mid-arm circumference (MAC), calf circumference (CC), albumin and hemoglobin, respectively. The full-MNA score of the high risk of malnutrition group (23.0${\pm}$3.8) was lower than that of the normal nutrition group (27.0${\pm}$2.1) (p<0.05). In addition, the Full-MNA score was negatively associated with the risk factor of malnutrition (r=-0.35, p=0.0004). We found that 39.1% of the subjects had osteoporosis, 45.7% had osteopenia and 15.2% were normal according to their BMD. The MNA score of osteoporosis group (24.58${\pm}$3.3) was lower than that of the normal (27.4${\pm}$1.1) and osteopenia group (26.9${\pm}$1.5) (p<0.05). These results suggested that MNA can be useful as a nutritional screening tool of older adults in Health Care Centers.
