Assessment of Estimated Daily Intakes of Preservatives in the Korean Population (한국인의 식이를 통한 보존료의 일일추정섭취량과 안전성평가)

  • In this study, the daily intake of preservatives in the Korean population was estimated, using data from national health and nutrition studies regarding the concentration of preservatives in each type of food, food intake by sex, age group, and weight. The level of safety of these preservatives was evaluated via comparisons with the acceptable daily intake (ADI) data established by the Joint FAO/WHO expert committee on food additives (JECFA). The Estimated daily intake (EDI) of sorbic acid is 495.85 ${\mu}g$/kg bw, benzoic acid is 215.34 ${\mu}g$/kg bw and parahydroxybenzoate is 5.01 ${\mu}g$/kg bw per day each. Their percentages in relation to ADI are 2, 4.3, and 0.1%, respectively. The comparatively high-consuming groups were taking sorbic acid 1,012.06 ${\mu}g$/kg bw, benzoic acid 229.56 ${\mu}g$/kg bw, and parahydroxybenzoate 11.58 ${\mu}g$/kg bw per day, and their intake levels as a percentage of ADI were 4.1, 4.6, 0.1%, all within safe levels. Males were found to have taken more benzoic acid than women, whereas we detected no sighificant differences in the intakes of sorbic acid and parahydroxybenzoate between males and females. The EDI of sorbic acid, benzoic acid, and parahydroxybenzoate were lower than the ADI in all age groups studied.

Comparing greenhouse gas emissions and nutritional values based on Korean suggested meal plans and modified vegan meal plans

  • Producing animal products from farm to table emits massive amounts of greenhouse gases (GHGs). Modified meal plans, mainly including vegetables and grains, have been recommended to reduce GHG emissions. However, these meal plans have not been developed with regard to the micronutrient content, but rather with regard to the energy requirements of grains and vegetables, which could result in a nutritional imbalance. For this reason, we investigated a common Korean suggested meal plan (SMP) from the National Institute of Agricultural Sciences, in which nutritional conditions were considered, and evaluated its GHG emissions using the Life Cycle Assessment Inventory Database and nutritional values. The SMP, which included meat, was based on the Korean Nutrition Society for adult men age 19 to 29, and was changed to a vegan meal plan (VMP). Animal-based protein sources were substituted for meat alternatives, such as beans and tofu, for which carbon footprint data was available. To compare the nutritional differences, the 9th Korean Food Composition Tables I and II were consulted. To calculate GHG emissions, the carbon footprint data of the food was converted to a CO2 equivalent (CO2e) using a procedure from the Foundation of Agriculture Technology Commercialization and Transfer. It was found that GHG emissions per calorie were 18% lower for the VMP when compared to the SMP. However, if GHG emissions per total amino acids were evaluated, the VMP GHG emissions per total amino acids were 0.12 g CO2e/mg, while the corresponding value for the SMP was 0.06 g CO2e/mg. The Korean daily meat intake reported by the Korea Agricultural Statistics Service was 37.1% lower than in the SMP, but when converted to a protein intake the figure was 17.0% lower. It was found that each SMP resulted in more GHG emissions than the VMP, but when considered as GHG emissions per total amino acids, the opposite pattern was apparent. There is a need to conduct more detailed studies of the variation in GHG emissions with different meal plans, using the daily meat intake per person.

Assessment of Self-purification Capacity along the Midstream of Nakdong River (낙동강(洛東江) 중류수계(中流水系)의 자정능력(自淨能力) 평가(評價))

  • Dissolved oxygen, BOD and flow pattern of the Nakdong River stream were measured for 3 years from 1978 to 1980 at quarterly intervals of January, April, June and October at 12 sites along the main stream from Sangju to Imhaejin and at 2 sites of Geumho and Nam River tributaries. With these data, the self-purification factors of the river were computed to obtain the following results: 1) The average BOD loads per day at the tributary of Geumho River were 94 tons in January, 39 tons in April, 60 tons in July and 54 tons in October, and these are considered to be the main source of water pollution toward the main stream of the Nakdong River. 2) Self-purification factors for the Hwawon-Hyunpung region of the main stream after receiving Geumho River water were computed to give $0.21{\sim}0.59$ of deoxygenation $constant(K_1)$ and $0.56{\sim}2.27$ of reaeration $constant(K_2)$. The oxygen-sag curves constructed for the main stream showed a remarkable decline at Hwawon and a quick recovery at Hyunpung, indicating a rapid decomposition of pollution loads received from the Geumho River. It was confirmed that the self-purification capacity of the Nakdong River was relatively high.

A New Bioluminescent Rat Prostate Cancer Cell Line: Rapid and Accurate Monitoring of Tumor Growth (효과적인 항암효능측정을 위한 발광 전립선 세포의 개발 및 평가)

  • Caliper measurements of tumor volume have been widely used in the assessment of tumors in animal models. However, experiments based on caliper data have resulted in unreliable estimates of tumor growth, due to necrotic areas of tumor mass. To overcome this systematic bias, we engineered a new luciferase-expressing rat prostate cancer cell line (MLL-Luc) that produces bioluminescence from viable cancer cells. MLL-Luc cells showed a strong correlation between bioluminescence intensity and cell number ($R^2$=0.99) and also accurately quantified tumor growth, with reduced bioluminescence signals caused by necrotic cells in a subcutaneous MLL-Luc xenograft model. The accurate quantification of tumor growth with bioluminescence imaging (BLI) was confirmed by a better antitumor effect of combination chemotherapy, compared to that based on caliper measurements with a correlation between the bioluminescence signal and tumor volume ($R^2$=0.84). These data suggest that bioluminescent MLL xenografts are a powerful and quantitative tool for monitoring tumor growth and are useful in evaluating the efficacy of anticancer drugs, with less systematic bias.

Risk Factors of Arthritis in Middle-aged Women in Korea (중년여성의 관절염 발생 영향요인 분석)

  • Purpose: To investigate the risk factors of arthritis, this study was carried out with 1,829 Korean women aged between 40-64 who responded to a health behavior survey. Methods: Data were extracted from the results of the 2001 National Survey on Health and Nutrition conducted by the Ministry of Health and Welfare. Results: 1 Financial difficulty and continuous anxiety about health were directly associated with arthritis. The odd ratios of these two factors of arthritis were 1.43 and 1.82 respectively. 2. With regard to the presence or absence of arthritis according to demographic characteristic, the incidence of arthritis was higher in the age between 50-64, in the group whose education level was middle school or lower, and in those who experienced the death of spouse, divorce or separation. Nevertheless, it was not significantly different according to financial status and employment status. 3. Concerning the presence or absence of arthritis according to psychological characteristic, the incidence of arthritis was found to be higher in those who were not healthy by subjective health assessment, in those who were always anxious about their health, in those who were always depressed and sad, and in those who felt heavy stress. 4. With regard to the presence or absence of arthritis according to the health behavior, the incidence of arthritis was higher in postmenopausal cases and cases who were having hormone therapy, and the incidence of arthritis was higher in those who did not drink alcohol, those with BMI higher than 25, and those who slept shorter than 7 hours a day. Conclusion: Based on the above results, it is required to provide a means applicable to communities by developing preventive education that can prevent the incidence of arthritis in middle-aged women as well as intervention programs.

Nutritional Status of Recipients of Allogeneic Hemopoietic Stem Cell Transplantation by Types of Conditioning Regimen (동종 조혈모세포 이식환자의 이식 전 처치 형태에 따른 영양상태)

  • This study was conducted for 39 patients who are recipients of allogeneic hemopoietic stem cell transplantation at BMT ward of St. Mary's hospital affiliated to Catholic University of Korea from April to September 1999. The subjects were devided into two groups; those who received both TEl and chemo therapy as conditioning regimen (TEl group). and those who used chemo agents as singular conditioning regimen (chemo group). The oral intake status of the two groups were compared through physical assessment and blood chemistry exam of the subjects, and factors influencing their nutritional change and oral intake were explored in each stage of the transplantation (six stages: admission, conditional stage, date of transplantation, one week after transplantation, two weeks after transplantation, and three weeks after transplantation). The prior aim of the study was to provide baseline data to minimize delayed treatment from nutritional deficiency of the subjects. The results were as follows: 1. TBI group was significantly decreased of oral calorie intake in two weeks after transplantation compared to admission and conditioning stage while that of chemo group was significantly decreased on the date of transplantation. 2. TBI group was significantly decreased of protein intake in two weeks after transplantation compared to admission and conditioning stage. In chemo group, protein intake was significantly decreased on the date of transplantation compared to admission. It was remarkable that TBI group showed lesser protein intake than chemo group. 3. Both group were significantly decreased of BMI in one week and three weeks after transplantation compared to admission. TBI group showed significantly higher BMI than chemo group. 4. Both group were significantly decreased of Triceps Skinfold Thickness (TST)on the date of transplantation compared to admission stage. 5. TBI group was significantly decreased of mid-arm muscle circumference (MAMC) in two weeks after transplantation compared to admission, conditioning, date of transplantation. 6. TBI group was significantly decreased of albumin level in two weeks after transplantation compared admission stage. In chemo group, it was significantly decreased on the date of transplantation compared to admission, three weeks after the transplantation. 7. TBI group was significantly decreased of transferrin level in two weeks after transplantation compared admission, conditioning, date of transplantation and one week after transplantation. In chemo group, it was decreased of transferrin level in 3 weeks after transplantation. 8. Oral intake of TEl group was impacted by vomiting before transplantation and gingivitis after transplantation. In chemo group, it was impacted by vomiting before transplantation and by two factors, gingivitis and nausea, after transplantation. The results showed oral calorie intake was not different between the two groups while protein intake was significantly lower in TBI group than chemo group. Oral intake was significantly impacted by vomiting before transplantation in both groups, but affected by oral gingivitis in TBI group and gingivitis and nausea in chemo group after transplantation. This findings present that standardized strategies to manage nutrition and gingivitis more effectively are desperately needed to enhance oral intake and protein intake of patients who receive TBI as conditioning regimen.

Monitoring of Heavy Metals in Vegetables in Korea (국내 유통 중인 채소류의 중금속 모니터링)

  • This study was carried out as a survey on the level of lead (Pb), cadmium (Cd), arsenic (As) and mercury (Hg) in 5 items of agricultural products (lettuce, pumpkin, head lettuce, cabbage and crown daisy) for which safety guidelines are not yet established in Korea. The 407 samples were collected from local markets in 9 regional places and the levels of metals were measured by ICP-MS and mercury analyzer. The average levels of Pb, Cd, As and Hg were 0.026, 0.018, 0.008 and 0.003 mg/kg for lettuce, 0.002, 0.001, 0.001 and 0.0006 mg/kg for pumpkin, 0.002, 0.005, 0.001 and 0.0005 mg/kg for head lettuce, 0.001, 0.002, 0.0002 and 0.0006 mg/kg for cabbage and 0.022, 0.014, 0.006 and 0.004 mg/kg for crown daisy. For risk assessment, the daily dietary exposures of Pb, Cd and Hg by intake of these agricultural products were calculated and compared with PTWI (provisional tolerable weekly intake) established by JECFA. The daily dietary exposure of heavy metals by intake of these agricultural products were 0.13, 0.56 and 0.09% of PTWI, respectively. These monitoring results will be utilized as fundamental data for the establishment of Korean standards of Pb, Cd, As and Hg in 5 items of agricultural products and valuable source for DB construction for science-based safety control of heavy metals in foods including agricultural products.

Preliminary Study on Developing Test Items of Swallowing & Communication Screening Protocols for Patients with Head and Neck Burns (안면부 및 경부 화상 환자의 삼킴 및 의사소통능력 선별 프로토콜 개발을 위한 예비 연구)

  • We have to consider two parts of the evaluation and treatment for the patients with head and neck burns. The primary consideration is swallowing function for nutrition supply for them and the next is speech function for efficient communication and aesthetic impression of them. The purpose of this study is to summarize the preliminary questions of Communication Screening Protocols which can help understand comprehensively on swallowing disorder, motor speech disorder and voice disorder of patients with head and neck burns. We divided the evaluation into 4 evaluation areas including 'oral mechanism', 'respiration/voice', 'articulation', and 'swallowing' by referring to overseas studies dealing with various communication disorders caused by burns, and prepared the final questionnaires by conducting the content validity verification by five expert (speech & language pathologist). The range of Content Validity Index was shown relatively appropriate with .50~.84. There was a conflict of opinions in experts whether the items in the areas of respiration/voice and swallowing may be appropriate, whereas there was no different view of the oral mechanism and articulation area. Through the different characteristics of communication difficulties of patients with head and neck burns, we expect it will be modified appropriately according to the patients through evaluation of burn patients by type and severity.

Association between physical activity and health - related quality of life in Korean patients with COPD (만성폐쇄성폐질환의 신체활동수준과 삶의 질 간의 관련성)

  • Objective: Patients with COPD have a lower overall quality of life than normal people. If patients with COPD do not regularly perform physical activities, their exercise capacity is reduced. It could lead to muscle loss, and negatively affect their general physical, social, and psycho-social status. The purpose of this study was to examine association with physical activity, sedentary life time and health-related quality of life in patients with COPD. Method: Of the total of 22,948 participants surveyed in Korean National Health and Nutrition Examination Survey (KNHANES 2013-2015), 8,626 participants were used for this study. Of these, under 40 years, over 80 years, missing values and abnormal values were excluded. Study variables included physical activity level, sedentary time and health-related quality of life variables. The physical activity level assessment tool was measured using International Physical Activity Questionnaire (IPAQ). Sedentary time referred to the amount of sitting time without moving the body. EQ-5D (Euro Quality of Life-5 Dimensions) was used as an index of health-related quality of life. Control variables were age, gender, income level, education level, marital status, comorbidity, smoking, BMI, cough, sputum, COPD severity. For this study, descriptive analysis, T-test, ANOVA and multivariate regression analysis were performed. Results: Of the 1,092 patients with COPD, 76.1% (n=831) were male and 23.9% (n=261) were female, while 39.0% (n=2,939) were male and 61.0% (n=4,595) were female in the comparison group without COPD. The COPD group with high level of physical activity showed a high level of EQ-5D scores ($0.9349{\pm}0.11$, p <0.001). Among patients with COPD, after adjusting for control variables, physical activity and sedentary time (physical activity level, Β=0.047, p <.001), (sedentary time, Β=-0.017, p <.05) were associated with health-related quality of life. Conclusion: Patients with COPD have a higher quality of life as their physical activity increases and the quality of life decreases as the time spent sitting increases. This study suggests that public health experts should consider improving COPD patient physical activity.

The Definition of Frail Elderly and the Frailty Screening Assessment Tool: A Systematic Review (허약노인의 정의 및 허약 선별 평가도구에 관한 체계적 고찰)

  • Objective : The objective of this study was to present the components of frailty by organizing the definitions of frail elderly and analyzing the tools used to screen them. Methods : This study searched for articles at involved frailty screening assessments in the elderly. Databases including CINAHL, Embase, Medline Complete, and PubMed were searched. The search terms were "assess" AND "frailty" AND "screening" AND ("frail elderly" OR "elderly"). Results : A total of 539 articles were identified by the search and 11 articles were selected. Frailty occurs due to the depressed function of multidimensional factors, and a frail elderly person is defined as one at high risk of health degeneration, functional impairment, and occurrence of disability, and having a high level of threat to life. Seven tools were selected from 11 articles. The most frequently used tool was the frailty phenotype, which was used in five articles (45.4%). The identified components of frailty were physical, activity participation, nutrition, psychological, social, overall health, and age. Conclusion : The results confirmed the definition and components of frailty. This study is expected to contribute to the future development of standardized evaluation tools for screening frail elderly individuals and intervention programs for the management of the frail elderly.
