Purpose. With a sample of cognitively impaired nursing home residents and nursing staff, the following were examined 1) the proportion and nature of aggressive behavior, 2) the frequency and types of aggressive behavior, 3) the difference between the residents who demonstrate aggressive behavior and those who do not demonstrate aggressive behavior (age, mental status, functional status, and pain, length of nursing home stay), and 4) nursing staff responses to aggressive behavior by residents. Methods. A cross-sectional descriptive study design was used. Data were collected from cognitively impaired nursing home residents (N=205) and nursing staff (N=60) at two nursing homes using Ryden Aggression Scale I and II, Mini-Mental State Exam, Modified Barthel Index, Verbal Descriptor Scale, and aggressive behavior management questionnaire. Data were analyzed using descriptive statistics including t-test. Results. About $62.9\%$ residents were found to be aggressive and $38.5\%$ were both physically and verbally aggressive. Pushing, making threatening gestures, hitting, slapping, cursing/obscene/vulgar languages, making verbal threats were occurred frequently. Aggressive residents were significantly older, had more cognitive impairment, had more pain, and stayed longer in the nursing home when compared with non-aggressive residents. Considerable proportion of nursing staff responded to aggressive behaviors inadequately. Conclusion. Aggressive behavior among cognitively impaired nursing home residents is prevalent thus needs to be prevented and reduced. Along with environmental modification, educational programs for nursing staff and family caregivers need to be developed and implemented so that they can have extensive knowledge and skills to manage aggressive behaviors.
Purpose: The purposes of this study were to examine the relationships among activities of daily living, self-efficiency, nursing home care quality and nursing home adaptation, and to identify the influencing factors of nursing home adaptation in nursing home residents. Methods: The study employed a descriptive correlational design. The data were collected from 148 older adults without dementia by interview from six nursing homes in three cities from February 1, 2008 to February 28, 2008. Results: Levels of nursing home adaptation in older adult residents were different by type of decision maker of nursing home admission, reason of nursing home admission, type of payment and length of nursing home stay. The nursing home adaptation was significantly correlated with self-efficiency and nursing home care quality. The strongest predictor of nursing home adaptation was type of decision maker of nursing home admission followed by the self-efficiency. Conclusion: The study suggested that during the decision making period of nursing home admission, older adults should have enough time and careful considerations of their families to decide by themselves in positive ways. Nursing home staff should be able to identify reasons of nursing home admission and demands or expectations of older adults and their families.
Purpose: Nurses' management of Nursing Home residents' remaining functional ability is phenomena emphasizing as the main practice focus in long term care and has specific meanings within the context of quality of life for fragile and disabled elderly people in nursing homes. This study was conducted to clarify and to conceptualize the phenomena of nurses' management of nursing home residents' remaining functional ability. Method: The Hybrid Model of concept development was applied to develop a concept of nurses' management of residents' remaining functional ability. Results: This concept of nurse' management emerged as a phenomenon having meanings in two different dimensions, deliberation and enactment for five types of residents: residents with declining function and dementia requiring nursing assistance, bed-ridden residents with moderate dementia, residents with advanced dementia and behavior problems, but good physical health, residents at the end of life stage, and bed-ridden residents with intact mental health. Conclusions: Results indicate that nurses' management of remaining functional ability of residents in nursing homes is a concept having types of needs that should be treated in specified ways. Interventions should have a positive impacts on practical applications in nursing homes and on enhancing residents' remaining function.
Purpose: The purposes of this study were to examine the amount of daily fluid intake among nursing home residents and to explore the caregiver's perceived barriers to elderly's fluid intake. Method: Data was collected from 111 nursing home residents and 64 caregiver's in 4 nursing homes. A random, non-consecutive three days of 24 hour fluid intake was measured and recorded. The caregiver's perceived barriers to elderly's fluid intake was assessed using a structured questionnaire. Results: The average amount of daily fluid intake was 1,035(SD=359)ml with the range of 210m1 to 2,050ml. About 52% (n=58) of the subjects had a less than adequate fluid intake. The amount of daily fluid intake was significantly associated with age, mental status, physical functioning, and the number of oral medications ordered. The most frequently mentioned caregiver's perceived barrier was elderly's concern about incontinence with increased fluid intake. Conclusion: Inadequate fluid intake among nursing home residents is prevalent. To enhance adequate hydration of nursing home residents, an institution wide nursing intervention is necessary.
Purpose: The purposes of this study were to examine the relationships among the sense of control, social support, depression and nursing home adjustment, and to identify the influencing factors of nursing home adjustment in nursing home residents. Methods: The study design was a cross-sectional descriptive study. Data were collected from May to June of 2010 using structured questionnaires from 212 elderly residents without dementia from 15 nursing homes. Results: The levels of nursing home adjustment depended on the self-rated health, the voluntary institutionalization, and the length of stay. The nursing home adjustment was significantly correlated with a sense of control, social support and depression. The five most influential factors affecting adjustment were self-rated health, placement decision maker, the length of stay, a sense of control and depression, which explained about 51.5% of the variances. Conclusion: The findings reflect the importance of voluntary institutionalization, a sense of control, avoiding depressive mood and social support which facilitate residents' adjustment. These results can provide guidance for nursing interventions to facilitate nursing home adjustment.
This study was peformed to assess the nutritional status of female nursing home residents by dietary intake. The subjects were 60 nursing home residents aged over 60 from 3 different nursing homes in Seoul, Cheonan and Chunchon. Dietary intake was assessed by 24-hr recall and nutrient intake was calculated using CAN-Pro and database for vitamin E established by authors. To establish vitamin E database, we analyzed vitamin E concentration using HPLC in several foods consumed frequently by nursing home residents. The results of nutrient intake were as follows; 1) Average daily intakes of energy, protein, phosphorous, iron, vitamin $B_1$ and niacin were higher than RDA, but calcium, vitamin $B_2$ and niacin of Chunchon were lower than RDA. 2) Intakes of protein, sodium and niacin of Cheonan residents were significantly higher than those of Seoul. 3) Intakes of phosphorous, potassium, vitamin $B_1$ and dietary fiber of Cheonan residents were significantly higher than Seoul and Chunchon. 4) Intakes of sodium and cholesterol of Seoul residents were higher than Cheonan and Chunchon. 5) Average daily intake of vitamin E was lower than RDA. Especially vitamin A intake of Cheonan residents was significantly lower than other two cities.
The purpose of this study was to describe the overall physical environment's features of nursing homes for Korean American elders in the United States. This investigation was conducted in Korean American nursing homes on the East Coast and in the Midwest with multiple qualitative data collection methods, such as interviews, observations, document review, and field notes. The findings of this study are as follows: 1) Although almost all respondents were unable to speak and understand English at all, most written materials in nursing homes were in only English; 2) The nursing homes were placed near or within the Korean communities; thus, it contributed to close ties with religious Korean American groups and other groups in the ethnic communities; 3) Whether the facility's physical features looked like those of a hospital or a home, many residents did not consider a nursing homes as a real home; 4) A fenced garden in the nursing home in the Midwest was the residents' favorite public space; 5) Due to being forced to room with residents who had dementia and were bedridden, they were more likely to have conflicts with their roommates and feel uncomfortable living in their room; and 6) The facilities seemed to be designed without consideration to protect residents' privacy.
Purpose: This study attempted to adapt evidence-based oral health care guidelines for nursing home residents in South Korea. Methods: The manual for guideline adaptation, version 2.0, developed by the Korean National Evidence-based Healthcare Collaborating Agency was applied. On the basis of a comprehensive literature review of current research and multidisciplinary panel discussion on adapting such guidelines, we developed evidence- based guidelines for oral health at nursing homes. Results: The guidelines have three domains: oral assessment, oral hygiene care, and support and facilities. Oral assessment includes recommendations on the frequency of and tools for conducting oral health assessment. Oral hygiene care involves recommendations for care of natural teeth, denture care, xerostomia, and behavioral problems. Support and facilities refer to six components of the support system provided by facilities to encourage oral hygiene care among nursing home residents. Conclusion: These adapted guidelines could be an effective method to improve oral hygiene among nursing home residents.
Purpose: The purpose of this study was to develop a health promotion program for frail elderly nursing home residents, and to analyze the effects of the program. Methods: The research was a nonequivalent control group pretest-posttest design. Data collection was performed from February 26, 2012, to April 26, 2012. The subjects were 28 residents for the experimental group, who were selected at A nursing home, D city, in Korea and 27 residents for the control group, who were selected at B nursing home. Data were analyzed using descriptive statistics, ${\chi}^2$-test, independent t-test, ANCOVA and Cronbach's ${\alpha}$ with SPSS/Win 17.0 program. Results: There was a significant improvement in health promoting behaviors (F=64.26, p<.001), Lt. Grip strength (F=39.76, p<.001), Rt. Grip strength (F=38.06, p<.001), Static balance (F=3.98, p=.050), TUG (F=18.60, p<.001), oral status (F=26.75, p<.001), depression (F=18.79, p<.001), and subjective health status (F=10.75, p<.002) in the treatment group compared to the comparison group. Conclusion: The health promotion program improved the health promoting behavior, physical fitness, oral status, depression, and subjective health status of the frail elderly nursing home residents. Therefore, health promotion programs for frail elderly nursing home residents could be helpful.
