Evaluation of a Community-Based Program for Breast Self-Examination Offered by the Community Health Nurse Practitioners in Korea

  • Background. Breast cancer is the most common form of cancer among Korean women. Only 14 % of urban women and 10% of rural women in Korea, however, participated in breast cancer screening behavior in 1998 (Korean Ministry of Health & Welfare, 1999). Purpose. The aim of this study was to evaluate the effect of community-based breast self-examination (BSE) education programs in Korea. Methods. First, breast cancer risk appraisals were done with 1,977 rural women. Of the 1,977 women, nearly 30% (n=494) had a higher or equal to borderline risk of developing breast cancer. This quasi-experimental study was conducted to target these women with a high or equal to borderline risk of breast cancer. The risk appraisal feedback and breast self-examination education were used as an intervention for breast cancer prevention and early detection. Results. After a 3-month follow-up, 30.5% of the women in the intervention group performed regular BSE compared to 10.2 % of women in the control group. The mean knowledge score related to breast cancer and BSE was significantly higher for the women in the intervention group than that in the control group.

Effects of Health Care Nursing Policy Education on Nursing Students' Political Efficacy, Political Participation, and Political Interest (보건의료 간호정책 교육이 간호대학생의 정치효능감, 정치참여 및 정치 관심도에 미치는 효과)

  • This study attemped to examine the effects of health care nursing policy education on nursing students' political efficacy, political participation, and political interest. It attempted to guide the direction of policy education within nursing curriculum. The subjects consisted of 89 nursing students of G-university from March 8, 2023, to June 21, 2023, including 44 in the experimental group and 45 in the control group. The health care nursing policy class was developed using the ADDIE(Analysis, Design, Development, Implementation, Evaluation) model of instructional design. Data analysis used the SPSS 25.0 program through mean, standard deviation, and independent sample t-test. The experimental group that participated in this education showed statistically significant improvement in political efficacy(t=2.34, p<.05) and intrinsic political efficacy(t=2.75, p<.05), as well as passive political participation score(t=2.22, p<.05) compared to before the intervention. Based on the findings of this study, it is suggested that health care nursing policy education should be structured to enhance external political efficacy and promote active political participation in future nursing curriculum.

The Relationship among Menopausal Symptoms, Competency of Self-Care Agency and Social Support in Middle-aged Woman (중년 여성의 폐경 증상경험과 자가관리 역량, 사회적 지지와의 관계)

  • Purpose: This descriptive correlational study was intended to analyze the relationship among menopausal symptoms. competency of self-care agency and social support in middle-aged women. Methods: Participants in the study were 140 women who were 45-60 years of old. Data were collected using structured questionnaires and analyzed using descriptive statistics., t-test, ANOVA, $Scheff{\grave{e}}^{\prime}s$ test and Pearson correlation coefficients. Results: The frequency and discomfort of menopausal symptom were low, but the mean discomfort score were higher than frequency score. Middle-aged women with lower competency of self care experienced more frequent menopausal symptoms (t=-0.28, p=.001) and more discomfort (t=-0.24, p=.004). However, There was no significant difference in menopausal symptoms according to social support. Conclusion: Nursing interventions to enhance competency of self-care agency in middle-aged women are required in order to reduce the discomfort of menopausal symptoms.

An Exploratory Study on the Policy for Facilitating of Health Behaviors Related to Particulate Matter: Using Topic and Semantic Network Analysis of Media Text (미세먼지 관련 건강행위 강화를 위한 정책의 탐색적 연구: 미디어 정보의 토픽 및 의미연결망 분석을 활용하여)

  • Purpose: This study aimed to analyze the mass and social media contents and structures related to particulate matter before and after the policy enforcement of the comprehensive countermeasures for particulate matter, derive nursing implications, and provide a basis for designing health policies. Methods: After crawling online news articles and posts on social networking sites before and after policy enforcement with particulate matter as keywords, we conducted topic and semantic network analysis using TEXTOM, R, and UCINET 6. Results: In topic analysis, behavior tips was the common main topic in both media before and after the policy enforcement. After the policy enforcement, influence on health disappeared from the main topics due to increased reports about reduction measures and government in mass media, whereas influence on health appeared as the main topic in social media. However semantic network analysis confirmed that social media had much number of nodes and links and lower centrality than mass media, leaving substantial information that was not organically connected and unstructured. Conclusion: Understanding of particulate matter policy and implications influence health, as well as gaps in the needs and use of health information, should be integrated with leadership and supports in the nurses' care of vulnerable patients and public health promotion.

Dissemination of Nursing Science and Nursing Training Policy during the Japanese Colonial Period (일제강점기 간호학의 보급과 간호사 양성 정책)

  • This study attempted to present the direction that nursing should have for the future nursing development by looking at the process of trying to protect the subjectivity of nursing in various events during the Japanese colonial period. In the early days of modern nursing, from 1910 to 1919, nursing education institutions were expanded and nursing-related systems were created. From 1920 to 1937, as a development of modern nursing, not only expanded the field of nursing, but also revised nursing-related laws and organized two nursing associations. From 1939 to 1945, the stagnation period of modern nursing, amidst the social disaster of war, a training policy for mobilizing war personnel was unfolded, and another change was experienced. In order to expand the field of nursing and continue to develop it, continuous interest and research on the role and spread of nursing that change in various social events from the past to the present are needed.

Correlates of Physical Activity among Korean Navy Personnel: An Ecological Approach (함정 근무 장병의 신체활동 관련 요인: 생태학적 접근)

  • Purpose: This study was conducted to measure the relationships between ecological factors and Navy personnel's physical activity (PA) based on McLeroy's Ecological model. Methods: A cross-sectional survey was conducted with a convenience sample of 184 Navy personnel working in 10 Navyships. A self-reporting questionnaire consisted of measures of intrapersonal, interpersonal, organizational and community factors related to Navy personnel` s PA. Data were analyzed by descriptive statistics, $x^2$-test, t-test, analysis of variance, and hierarchical multiple regression using SPSS/WIN 17.0 programs. Results: Their mean PA level was $2,848.1{\pm}3,344.5$ MET-min/week, and mostly moderate level (50.5%). Hierarchical multiple regression analysis showed that religion, working department, working type, perceived health status and community environment were significant PA correlates. Conclusion: Community environmental factors as well as intrapersonal factors were significantly associated with Navy personnel's PA, indicating that community health nurses should expand an approach for individual-level behavioral change to incorporate Navy personnel specific community environmental barriers into PA interventions.

Factors Related to Nursing Home Institutionalization of Elderly using Home Care Services (노인장기요양 재가서비스 이용자의 시설 입소 영향 요인)

  • Purpose: Ageing in place may improve the quality of life of frail elderly and decrease their costs of services. The purpose of this study was to examine the factors that influence the institutionalization of elderly using home care services in a Korean long-term care insurance system. Methods: This study used the data of '2009 Satisfaction survey of Korean long-term care system'. The survey proceeded to use a sampling data based on region, level of long-term care need, and insurance type among the beneficiaries between August and September 2009. The onset dates of institutionalization of 1,095 participants were ascertained from long-term care insurance claim data. This study calculated the hazard ratio through the Cox Proportional Hazard Model. Results: A total of 176 subjects who were institutionalized in nursing homes were included. There were higher risks in the group that included those who were 85 years and over, had dementia or fracture, used home-visit nursing service, and were not supported by direct family. Conclusion: The results of this study have policy implications to supplement the home care service system and postpone nursing home institutionalization of elderly.

Research Trend of Women's Health in Korean Nursing Journals (2010~2015) (한국 간호저널에서 여성건강 연구경향 분석: 최근 6년간의 논문분석[2010~2015])

  • Purpose: To explore trends of women's health in nursing research by analyzing articles on women aged 13 years or older that were published in Korean Journal for Women Health Nursing from 2010 to 2015. Methods: Seven focus areas were identified and modified to reflect integrative conceptual models of women's health: maternity care, menstrual concerns, health problems in women, gender influences on health risks, social influences on women's health, women and health care policy, and sexual health and violence against women. A total of 383 studies were analyzed according to these seven focus areas. Results: Health problems in women, maternity care, and societal influences on women's health were the most widely studied topics in Korean women's health. There was increased attention to societal influences on women's health and gender influences on health risk. However, these areas are still limited in nursing research. Only 1% of these studies were in area of women's health policy. Conclusion: More studies in area of sexual health and violence against women are needed. Studies in area of women and health care policy are also needed to improve women's health in Korea.

Construct Validity and Reliability of the Korean Version of the Practice Environment Scale of Nursing Work Index for Korean Nurses (한국어판 간호근무환경 측정도구의 타당도와 신뢰도)

  • Purpose: To develop and test the validity and reliability of the Korean version of PES-NWI measuring nursing work environments in hospitals. Methods: The Korean version of the PES-NWI was developed through forward-backward translation techniques, and revision based on feedback from focus groups. An internal consistency reliability and construct validity using confirmatory factor analysis were conducted using SPSS WIN (16.0) and AMOS (18.0). Survey data were collected from 733 nurses who worked in three acute care hospitals in Seoul, South Korea. Results: The Korean version of PES-NWI showed reliable internal consistency with a Cronbach's alpha for the total scale of .93. Factor loadings of the 29 items on the five subscales ranged from .28 to .85. The five subscales model was validated by confirmatory factor analysis (RMR<.05, CFI >.9). Conclusion: The findings of this study demonstrate that the Korean version of PES-NWI has satisfactory construct validity and reliability to measure nursing work environments of hospitals in Korea.

Health Education and Health Care in Daycare Centers for Preschoolers (어린이집 보건교육과 건강관리 현황 분석)

  • Objective: The purpose of this study was to understand the needs of teachers in daycare centers for preschoolers in providing health education and health care services and to identify competencies and barriers to health education among the teachers. Method: A total of 410 teachers from 496 public daycare centers were recruited. Participating daycare centers were selected using a stratified sampling method. Data were collected from June to August 2002 using mailed questionnaires. Result: About 37% of the daycare centers provided health education more than 6 times during the past year and 92% provided annual health screenings. Traffic safety, personal hygiene, and sexuality education were most frequently provided. Regarding in-service education for daycare teachers, 62.7% had obtained health related education. Sexuality education, traffic accident, and injury prevention were the main topics for the in-service education. The teachers had relatively higher competencies and lower barriers to health education, while they also had proper knowledge related to health of children. Conclusion: Based on the study results, health professionals could plan and develop health promotion programs to meet the needs of teachers and children in daycare centers.
