An Analysis of Nurse's Perception of Internal Marketing Activities Affecting on Nurse's Turnover Intention, Nursing Task Performance and Nursing Productivity (간호사가 지각하는 내부마케팅활동 정도가 간호사의 이직의도, 간호업무수행 및 간호업무생산성에 미치는 영향)

  • Purpose: The purpose was to analyze the effects of internal marketing activity factors on nurse's turnover intention, nursing task performance and nursing productivity. Methods: The subjects were 355 nurses who were working at the 3 universities hospital over 1 year. The instruments were used of internal marketing activity factors(Lee, 2001), turnover intension(Lee, 1995), nursing task performance(Park, 1988) and nursing productivity(McNeese-Smith, 1996). Results: The mean score of internal marketing activity factors was 2.79, education and training 2.97, individualization 2.93, communication 2.87, promotion 2.76, work environment 2.63, reward system 2.62, and management vision for employee 2.61. The turnover intention was 3.12, nursing task performance 3.49, and nursing productivity 3.38. The internal marketing activity factors were negatively correlated with turnover intention(r=-0.37, p<0.0001), and positively correlated with nursing task performance(r=0.29, p<0.0001) and nursing productivity(r=0.30, p<0.0001). The key predictor of turnover intension was reward system, education and training, communication, and salary. They explained 35.0% of the total variance. In nursing task performance, communication, management vision for employee, salary and unit explained 26.0% of the total variance. In nursing productivity, communication, reward, education and training, salary, and position explained 24.0%. Conclusions: To increase nurse's nursing task performance and nursing productivity and to decrease turnover intention, it is necessary to concentrate on improving communication and reward system in the internal marketing activity factors. Through these activities, the effectiveness of internal marketing strategies will be enhanced and finally, nursing organizational outcome will be increased.

A Study on the Satisfaction of Web-Based Health Education Programs (웹기반 건강교육 프로그램에 대한 만족도 연구)

  • Purpose: This study was to present the process of web-based educational program (WEP) development and to identify factors affecting satisfaction with WEP for the certificate of healthcare managers working at the National Health Insurance Corporation (NHIC). Methods: Subjects were healthcare managers and voluntary participants of WEP. A total of 1,449 respondents were surveyed through an online questionnaire about their satisfaction with the educational contents and system. Results: The mean contents satisfaction was 3.75 (SO 0.54), and system satisfaction 4.68 (SD 0.54). According to statistical analysis, the type of certification, experience and professional career of health care management affected contents satisfaction. And factors affecting system satisfaction were the type of certification and gender. Conclusion: WEP was utilized as a pre-requisite course for the certificate program of healthcare managers. However, the development of advanced WEP is suggested to meet the educational needs of healthcare managers who have certificate or license and their job related to healthcare management.

Nursing Case Management Program Development for the Elderly at Hospital (입원노인환자의 사례관리 프로그램 개발 연구)

  • Purpose: This study was performed to develop the case management program that can provide the qualitative health care service to satisfy the increasing needs for health care elderly and prevent the increase in medical cost cased by prolonged and repetitive hospitalization. Method: The survey was completed as follows. First, 290 elderly patients who would be soon discharged in 4 hospitals are participated in the survey on needs of nursing care and case management programs. Second, 70 professionals working at 5 hospitals including doctors, nurses, social workers and so on are participated in the survey on the case management team operation. Result: Level of needs to Nursing care and case management programs showed so high. Professionals suggested on the case management team operation. Finally, based on the survey and literature review, the case management program was developed. Conclusion: In this study, the specific models for the five case management services were suggested. It is thought that the service provision by the case management team based on the multi-disciplinary case management program model suggested in this study would be suggested as a constituent of the effective health care delivery system for the hospitalized elderly patients.

A Comparative Study Recognition of Future Career and Nurse's Characteristics According to Nursing School System (학제에 따른 진로인식, 간호사자질인식의 비교연구)

  • This study was to compare the recognition of future career and nurse's characteristics according to nursing school system. This study was based on cross sectional descriptive method. The data were analyzed by $x^2$-test, t-test, and ANOVA using PASW WIN 18.0 program. The data represented that students who were doing associated degrees or bachelor degrees, showed the similar level recognition of future career and nurse's characteristics. However they showed differences in recognition of the career where they could create and new things(t=2.933, p=.004) and working part time(t=2.328, p=.021). In regards to recognition of nurse's characteristics bachelor degrees students had higher professional ethics($4.59{\pm}.44$). This study proposed that these research results could be used for improving methodology of nursing education.

Applying document routing mode of information access in nursing diagnosis process (문서 라우팅 기법을 이용한 간호진단 과정에서의 정보접근)

  • Nursing diagnosis process is described as nurses assessing the patients' conditions by applying reasoning and looking for patterns, which fit the defining characteristics of one or more diagnoses. This process is similar to using a typical document retrieval system if we consider the patients' conditions as queries, nursing diagnoses as documents, and the defining characteristics as index terms of the documents. However, there is a small fixed number of nursing diagnoses and infinite number of patients' conditions in a typical hospital setting. This state is more suitable to applying document routing mode of information access, which is defined as a number of archived profiles, compared to individual documents. In this paper, we describe a ROUting-based Nursing Diagnosis (ROUND) system and its Natural Language Processing-based query processing component, which converts the defining characteristics of nursing diagnoses into query representations.

Comparative Study of Home Nursing Care Services under the Long-term Care Insurance System in Four Nations (방문간호의 국내외 현황분석 - 한국, 미국, 일본, 독일의 사례를 중심으로 -)

  • Purpose: This comparative study analyzed information systems including manpower, contents of service, clients, and costs among four nations. Methods: A literature review of relevant publications from Korea, United States, Japan, and Germany supported the use of several plans to activate home nursing care services under the long-term care insurance system in Korea. Results: Korean home nursing care services require quality improvements. The results indicated that a rule is necessary that rations simple visiting service and home nursing care services under the long-term care insurance system, that an integrated management system for elderly care is required, and that the revised delivery of services needs to establish and reflect on various factors to estimate the value in a home visiting nursing care cost system. Conclusion: The data should be valuable in establishment of home nursing care services under the long-term insurance system in Korea.

A Comparison between Home Care Nursing Interventions for Hospice and General Patients (가정 호스피스 대상자와 일반 가정간호 대상자에게 제공된 간호중재 비교)

  • The purpose of the study was to compare home care nursing intervention activities analyzed by the Nursing Intervention Classification (NIC) system for hospice and general patients. Method: For the descriptive survey study, data was collected by reviewing charts of 151 hospice patients and 421 general patients who registered in the department of home health care nursing at K Hospital. Results: According to the NIC system application, there were 2380 total nursing interventions used for the hospice patients and 8725 for the general home care patients. For both sets of patients (hospice vs. general), the most frequently used nursing intervention in level 1 was the Physiological: Complex domain (40.13 vs. 31.06 percent), followed by the Safety domain; in level 2, the Risk Management class (28.4 vs. 27.70 percent), followed by Tissue Perfusion Management; and in level 3, Vital Sign Monitoring (6.18 vs. 4.84 percent), followed by Health Screening. Conclusion: The study showed that there was a lack of specialized hospice nursing interventions such as emotional, family and spiritual support, and care for dying hospice patients.

Pain Management in Cancer Patients Who Are Registered in Public Health Centers (보건소 등록 재가암환자가 지각하는 통증관리와 통증관리 방해 정도)

  • Purpose: This study was designed to describe outcomes of pain management, to identify pain intensity, pain management and barriers to pain management, and to test correlation among the variables in cancer patients who are registered in public health centers. Methods: By using a descriptive survey design, 3 instruments were used to collect data: the Numeric Rating Scale for pain, the Barriers Questionnaire-Korean version, and a one-item self-report tool about patient satisfaction. A sample of 190 patients with cancer was recruited from a public health center. Results: The mean rating for pain during the past 24 hours was mild and the mean score of barriers to pain management was 3.20. Patients were satisfied with pain management but they also had concerns it. A negative correlation was found among pain severity, pain relief and satisfaction of pain management. However, there was not significant correlation between the patient-related barriers to pain management and other variables. Conclusion: These results suggest that the intervention for cancer patients should focus not only on patient-related barriers to pain management, but also address health-care system related barriers.

Financial Ratio Analysis for Developing Nursing Management Strategies in University Hospitals (대학병원에서의 간호관리 전략 수립을 위한 재무비율 분석과 활용)

  • Purpose: The purpose of this study was to analysis the financial statements of university hospitals and to apply the results to build nursing management strategies. Methods: Data on the financial statements of university hospitals were collected each hospital's homepage or internet search from February to June, 2010. Financial statements of 11 hospitals were analyzed using the 4 categories of financial ratio analysis method: liquidity, performant, growth and turnover. Results: Overall results showed that the financial status the university hospitals were unstable, and many financial indicators did not meet financial standard ratios. Only 8 financial indicators of total 19 indicators satisfied financial standard ratios. Conclusion: The results of financial statements analysis suggest that nurse managers should develop the blue ocean strategy for diversification of nursing services to improve financial ratios of liquidity, performance, and growth. Using a unit-based just-in-time system for effective supply management would help to increase profits and to decrease costs of hospital by improving financial ratios of turnover.

The Development of patient classification system for hemodialysis (혈액투석환자 중증도 분류체계 개발)

  • Purpose : This study was conducted to develop a patient classification system for hemodialysis and to test its validity and reliability. Method : The process of the system development was as below. The lists of hemodialysis nursing activities were collected from literature and hemodialysis practice guideline and they were classified into 10 factors and 16 elements. And then, 4 classification levels were identified for each element. The content validity and interrater reliability of developed patient classification system were tested. Result & Conclusion : 10 factors of patient classification system for hemodialysis were consisted of psychosocial support, mobility, access, teaching, assessment, stability, supportive therapy, test, general nursing during hemodialysis, hemodialysis room management. According to validity and reliability results and experts' opinions, 4 classification levels revised to 3 classification levels and 2 elements were deleted. Finally, patient classification system were consisted of 10 factors, 14 elements, 3 classification levels, 3 categories.
