The effects of a maternal nursing competency reinforcement program on nursing students' problem-solving ability, emotional intelligence, self-directed learning ability, and maternal nursing performance in Korea: a randomized controlled trial

  • Purpose: The purpose of this study was to develop a maternal nursing competency reinforcement program for nursing students and assess the program's effectiveness in Korea. Methods: The maternal nursing competency reinforcement program was developed following the ADDIE model. This study employed an explanatory sequential mixed methods design that applied a non-blinded, randomized controlled trial with nursing students (28 experimental, 33 control) followed by open-ended interviews with a subset (n=7). Data were analyzed by both qualitative and quantitative analysis methods. Results: Repeated measures analysis of variance showed that significant differences according to group and time in maternal nursing performance; assessment of and intervention in postpartum uterine involution and vaginal discharge (F=24.04, p<.001), assessment of and intervention in amniotic membrane rupture (F=36.39, p<.001), assessment of and intervention in delivery process through vaginal examination (F=32.42, p<.001), and nursing care of patients undergoing induced labor (F=48.03, p<.001). Group and time improvements were also noted for problem-solving ability (F=9.73, p<.001) and emotional intelligence (F=4.32, p=.016). There were significant differences between groups in self-directed learning ability (F=13.09, p=.001), but not over time. The three main categories derived from content analysis include "learning with a colleague by simulation promotes self-reflection and learning," "improvement in maternal nursing knowledge and performance by learning various countermeasures," and "learning of emotionally supportive care, but being insufficient." Conclusion: The maternal nursing competency reinforcement program can be effectively utilized to improve maternal nursing performance, problem-solving ability, and emotional intelligence for nursing students.

Analysis of Obesity Intervention Programs in Adolescents: Focused on Endothelium Functions (비만 청소년의 비만 중재 프로그램에 대한 국내외 연구 분석: 혈관내피 기능을 중심으로)

  • Purpose: The purpose of this study was to determine the current status of obesity intervention programs, and its effects on endothelium function in adolescents. Method: This was a descriptive research study, that investigated domestic and international literature 2009-2018. Using databases inside and outside of Korea to search for 'adolescent', 'obese adolescent', 'exercise', 'program', or 'intervention', and 'endothelium function', a total of 14 literature have been selected for 6 domestic research and 8 international research, excluding overlapping, case studies, literature reviews, and unrelated studies. Results: Interventions for enhancing endothelium function were exercise program, diet, vitamin D3 treatment, and Orlistat intake relative to international research, and combined exercise and purple sweet potato intake, in most domestic research. Dependent variables included FMD, RHI, NMD, EPCs, and EMPs, for measuring endothelium function in international research, and PWV, FMD and RHI, in domestic research. Conclusion: Results of this study confirmed that application of obese intervention programs, may improve endothelium function in adolescents. Further studies are required, to develop nursing intervention, that would enhance endothelium function in adolescents.

The Effect of a Dementia Preventive Intervention based on Motivational Interviewing among the Elderly over 75 Years of Age in Nursing Homes (요양시설입소 후기노인의 동기면담 기반 치매예방 프로그램 효과)

  • Purpose: The purpose of this study was to examine the effects of the motivational interviewing (MI) dementia preventive intervention on dementia preventive behaviors, depression and cognitive function among elderly over 75 years of age in nursing homes. Methods: A nonequivalent control group pretest-posttest design was used. A total of 57 participants were divided into three groups; 18 in the MI dementia preventive intervention (Group A), 20 in the dementia preventive program (Group B) and 19 in the control group. Dementia preventive behaviors, depression, and cognitive function were assessed at pre, post, and 1 month after the intervention. Data were analyzed using descriptive statistics, t-test, ANOVA and repeated measure ANOVA. Results: The results reported that there were significant differences in dementia preventive behaviors and depression among Group A, B, and C. Also, there were significant differences in dementia preventive behaviors and cognitive function during time periods. There was a significant interaction between groups and times in relation to depression. Results suggested that the effects of MI dementia preventive program was persistent after 1 month following the intervention. Conclusion: Further research needs to develop dementia preventive programs considering physical and mental traits of the elderly in late years staying at nursing homes.

The Effects of a Self-Management Program on Successful Aging (자가관리 프로그램이 성공적 노후에 미치는 효과)

  • Purpose: The purpose of this study was to gain knowledge of successful aging for the Korean elderly and to identify the effects of an intervention program on the change of the knowledge Method: Data were collected from 134 elders from 3 schools for the elderly in H City. The Successful Aging Scale was used to collect data and modified CDSMP was applied as the intervention. Descriptive statistics, t-test, ANOVA and ANCOVA were used. Results : The total score of the knowledge of successful aging among the elderly cohort was 2.44 out of 3, and the scores of the factors making up old age were 2.66 for the success of children, 2.63 for self-control, 2.56 for self-efficacy, and 2.05 for happy marriage. Life satisfaction from the success of children and self-control were strongly correlated(r=0.7036, p=0.000). Conclusion: The self-management program brought positive changes to the knowledge of successful aging through the self-control parameter, and although it could not change the degree of knowledge of the overall successful aging among the elderly cohort, the program represents the first Korean nursing intervention focused on self-management techniques needed for successful aging.

Effects of Multifactorial and Follow-up Programs Applying a Capacity Building Strategy: Focusing on Older Adults Living in a Urban-rural Complex Area

  • Purpose: The purpose of this study is to examine effects of a multifactorial program for preventing the frailty of older adults and effects of a follow-up program applying a capacity building strategy. Methods: A quasi-experimental pretest-posttest design was used for the nonequivalent control group. The follow-up group (n=75) and non-follow-up group (n=68) received the same multifactorial program comprising muscle strength exercise, cognitive training, and psychosocial programs for 12 weeks. After completion of multifactorial program, the follow-up group took follow-up programs applying the capacity building strategy for following 12 weeks. The data of physical function, cognitive function, and psychological function, and self-rated health were collected from both groups three times: before intervention, after intervention, and 12 weeks after intervention. The data were analyzed using $x^2$ test and t-test. Results: In comparison with the non-follow-up group, the scores of Timed Up & Go Test, and physical activities energy expenditure were significantly improved in the follow-up group. Conclusion: These results indicate that a multifactorial program with follow-up adapting the strategies of capacity building for the older adults group is feasible to prevent the physical frailty in community.

Effects of a Cognition Activation Program for the Institutionalized Old-Old in Korea (장기요양서비스 수급 후기노인의 인지향상 프로그램 중재 효과)

  • Purpose: This study was conducted to test effects of a cognition activation program on ADL, cognitive functions, depression, and QOL in the institutionalized elderly. Methods: The subjects were 40 elders who were admitted to a nursing home, with 20 of them in the experimental group and the other 20 in the control group. Data were collected from September 1 to December 17, 2012. The experimental group participated in a 12 week intervention program. ADL, cognitive functions, depression, and QOL were measured before and after the intervention. Data were analyzed by t-test and regression analysis of the SPSS and AMOS programs. Results: ADL (t=2.468, p=.017), depression (t=-2.625, p=012), and QOL (t=2.428, p=.020) were more significantly improved in the experimental group than in the control group, but there were no statistical differences in cognitive function between the two groups. Cognitive functions had positive effects on ADL (CR=-2.263, p=.024) and depression affected QOL (CR=-7.248, p<.001). Conclusion: The intervention program had positive effects on ADL, depression, and QOL except cognition. Cognitive functions of elders dramatically decrease after age 85, and it takes too much time to improve it. Therefore the program for the improvement of cognitive functions should be provided as early as possible.

Developing a Community Capacity Builded Exercise Maintenance Program for Frail Elderly Women (지역사회 역량강화 전략을 이용한 재가 허약여성노인의 운동유지 프로그램 개발)

  • Purpose: This study was conducted to develop a community capacity builded exercise maintenance program for frail elderly women. Methods: As a guideline to develop the exercise maintenance program, the intervention mapping framework, including needs assessment, setting program goals, selecting theory-informed intervention methods, producing program components, planning program implementation and evaluation, was used. Focus group interviews with public health nurses and frail elderly women were conducted for needs assessment. Intervention strategies and components were formulated based on community capacity theory. Results: The developed exercise maintenance program consisted of strategies focusing on leadership development, partnership construction, organization development, community systematization of dimension of community capacity. A exercise maintenance program using health leader, health contract, exercise pocketbook, rhythmic activity suiting song and self-help group was included. Conclusion: The intervention mapping method was found to be useful to develop theory-based valid and community capacity builded exercise maintenance strategies for frail elderly women.

The Effect of a Yoga Program on the Stress Levels of Professionals (요가 프로그램이 전문직 종사자의 스트레스에 미치는 효과)

  • Purpose: The aim of this study was to examine the effects of a yoga program on perceived stress, stress response and heart rate variability in professionals. Methods: The research design was a quasi-experimental intervention study. The participants were 47 adults with professional jobs consisting of an experimental group with 23 adults and a control group with 24 adults. The duration of the yoga program was eight weeks from July 4 to August 30, 2012. Results: There were significant decreases in the stress response score after the yoga program. No significant changes in perceived stress and heart rate variability were observed between the experimental group and control group. However, there were significant decreases in perceived stress, stress response scale, and heart rate variability for some participants whose stress levels were high. Conclusion: An eight-week-long yoga program could be considered as an intervention to decrease the stress response of professionals even though it might not reduce their perceived stress and heart rate variability. To further analyze the effects of a yoga program in professionals, changes in intervention strength (such as longer periods and frequency) and the development of physiological measures, which would show the co-relationship between perceived stress and physiological response, are necessary in future studies.

The Effect of Supportive Nursing Intervention on the Anxiety and Nursing Satisfaction of Mothers with Children Undergoing Open-Heart Surgery (지지적 간호중재가 개심술 환아 어머니의 불안 및 간호만족도에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effect of supportive nursing intervention on the anxiety and nursing satisfaction of mothers with children undergoing open-heart surgery. Methods: The research design involved a non-equivalent control group pretest-posttest non synchronized design. The subjects consisted of 43 mothers of children who were expected to undergo open heart surgery, and were divided into two groups; an experimental group of 22 and a control group of 21. The experiment was administered to the experimental group a total of four times, following supportive nursing intervention protocols. The research tools used were Spielberger's State Anxiety Inventory for anxiety and Park Jung-Eun's nursing satisfaction. The data were analyzed using the $x^2$-test, and t-test with SPSS/WIN 12.0 program. Results: Hypothes 1, 'The experimental group who are given supportive nursing intervention will have a lower anxiety score than the control group' was supported(t=5.658, p=.000). Hypothesis 2, 'The experimental group who are given supportive nursing intervention will have a higher nursing satisfaction score than the control gropu wasl supported(t=-9.549, p=.000). Conclusion: The supportive nursing intervention protocol was effective in reducing anxiety and increasing the nursing satisfaction of mothers with children undergoing open-heart surgery.

The Effect of Mother-fetus Interaction Promotion Program of Talking and Tactile Stimulation on Maternal-fetal Attachment (태담과 촉각 자극의 모-태아 상호작용 증진 프로그램이 모-태아 애착에 미치는 효과)

  • The purpose of this study is to investigate the effect on mother-fetus attachment through mother-fetus interaction promotion program of talking and tactual stimulation aimed at enhancing sensitivity of primipara Non-equivalent control group posttest design was used. Fifty primiparas (26 mothers for intervention group and 24 mothers for control group) were recruited from three general hospital and an OBGY clinic located in Gang Nung city. Data was collected from January 30th to December 20th in 2001. For the intervention group, programed education which focused on mother-fetus interaction promotion of talking and tactile stimulation in the 1st trimester. Telephone counselling was provided with interval of two weeks. For two groups, home visiting for data collection of mother-fetus attachment was conducted at 36 to 38 weeks of gestational age. The data were analyzed using chi-square test and Fisher' Exact test to test the equivalence of two groups, and the effect of intervention program was determined with t- test. The result was as follow: The significant difference was found in mother-fetus attachment between two groups(t= 2.772, P= 0.0079). It indicated that intervention program was effective in improving mother-fetus attachment. In conclusion, this study has shown that the applied nursing intervention aimed at enhancing sensitivity of primiparas to fetus's movement promoted mother-fetus attachment. Therefore, this study suggests that this nursing intervention to increase maternal sensitivity to the fetus's movement should be broadly applied to primiparas, which can be beneficial for formation of mother-infant relationship, and for promotion of the social, affective, and cognitive developments of their children.
