The Development and Simulation of Training Cost Estimating Model for the Operation of the Nurse Residency Program (신규간호사 교육 프로그램(Nurse Residency Program) 운영을 위한 교육비용 산출 모형 개발 및 모의 적용)

  • Purpose: This study aims to develop a cost model for NRP (Nursing Residency Program) operation and ultimately provide evidence for financial factors for NRP operation in the future by simulating a cost model. Methodology: This study developed a model for the NRP education cost calculation model based on the review of Hansen's model, which has systematically reported on the development and operation of NRP, and discussions with nursing education experts at a university-affiliated hospital. With the simulation, it was intended to predict nurses' supply and demand in the long term and to calculate changes in long-term education costs. Findings: Firstly, turnover model, term model, cost model necessary for calculating a model for the NRP education cost calculation model was set up. Secondly, the simulation showed the following results; 1) the proportion of newly graduated nurses less than 5 years of working decreases gradually over time, which will make the composition of nurses more balanced. 2) In the first year of the partial introduction of NRP, the cost of training new nurses was about 2.1 times higher than before. After the introduction, the training cost in the 13th year began to be lesser than before the introduction, and in the 25th year, it decreased by 28.1% compared to before the introduction. Practical Implications: Firstly, NRP would be an effective way to solve the higher turnover and frequent departure of new nurses and the imbalance of nurses' composition. Secondly, although the costs of NRP are incurred in the early stages, in the end, NRP training costs are reduced compared to before the introduction of NRP. It is necessary to systematically understand the contribution effect of NRP by analyzing the economic value of NRP considering financial and non-monetary returns in the future and providing a basis for decision-making related to NRP implementation.

A Study on the Informal Cost Burden of the Patients Admitted to the Hospital (입원환자의 비공식적 의료비용 부담에 관한 연구)

  • To estimate total burden of hospital admission over patient of gamily, we need to know the unofficial private expenses in addition to explicit hospital admission fees. This study was conducted from June 29, 2,000 to August 10, 2,000. Subjects were 104 patient at university hospital located at chungnam province. After thorough explanation of purpose and procedures, notebooks are given to each patient or guardian. They are requested to recorded all relevant expenditures occurred during hospital stay. Incomplete records were filled-up by direct personal contact or phones. Datas were summarized and analyzed using SAS statistical package. P-value less than 0.05 was considered significant. The results of the study are as follows: 1. In 96.1% of the patient, guardians stayed at hospital to take care of patients. In 38,8% one of the family members get work-leave or temporary resting from job. Average date of leave was 7.5days. 2. Average informal cost burden per patient was 204,467 won (14,330 won${\sim}$1,594,870 won). Average hospital cost paid by the patient was 1,061,807 won. The ratio of informal cost burden to hospital cost paid by the patient was 0.327. 3. According to the regression analysis, the relevant factors affection informal cost burdens were distance from home to hospital(p=0.018), and duration of hospitalization(p=0.0001). 4. Informal cost burden was composed of expenses for personal expense of care giver (126, 720 won/patient), meal (86,924 won/patient), transportation (77,648 won/patient), necessaries of life (18,789 won/patient), tests and treatments not covered by insurance (17,289 won/patient), medical supplies not covered by insurance (15,280 won/patient), treat for visitors (14,757 won/patient), TV coin (8,247 won/patient), and others (7,582 won/patient). In addition to the hospital cost paid by the patient for hospital admission, the informal cost burdens should be recognised explicitly because it is not small. Significant proportion of informal cost burden is composed of care-giver's personal expense, transportation, meal. It is suggested that some polices are to be devised and implemented enabling that this portion of informal expenses be directed to formal professional nursing care. Thus we can improve the quality of care and decrease discomfort of patient's relatives.

Comparative Study of Home Nursing Care Services under the Long-term Care Insurance System in Four Nations (방문간호의 국내외 현황분석 - 한국, 미국, 일본, 독일의 사례를 중심으로 -)

  • Purpose: This comparative study analyzed information systems including manpower, contents of service, clients, and costs among four nations. Methods: A literature review of relevant publications from Korea, United States, Japan, and Germany supported the use of several plans to activate home nursing care services under the long-term care insurance system in Korea. Results: Korean home nursing care services require quality improvements. The results indicated that a rule is necessary that rations simple visiting service and home nursing care services under the long-term care insurance system, that an integrated management system for elderly care is required, and that the revised delivery of services needs to establish and reflect on various factors to estimate the value in a home visiting nursing care cost system. Conclusion: The data should be valuable in establishment of home nursing care services under the long-term insurance system in Korea.

  • Purpose: This study was conducted to compare the hypertension management between a non-elderly group and elderly group of hypertension patients in Community residents. The study also sought to generate strategies for increasing the hypertension management of residents using Community health center. Methods: Data on the general characteristics and hypertension management from 381 hypertension patients between non-elderly and elderly, living in P city, Gyeonggi Province and C city, Chungnam Province. South Korea, were collected based on a structured questionnaire, The data were analyzed using the SPSS 20.0 statistics program. Results: The use of a Community health center in the non-elderly and elderly groups showed a statistically significant difference in facility excellence and cheaper cost. Hypertension management was measured every day, The daily blood pressure and physician counseling was performed according to the changes in blood pressure. The management of hypertension medication in a community health center provided for hypertensive patients can be evaluated as an efficient service. Conclusion: The self-management ability of hypertension needs to be improved. In particular, especially, the elderly managed by the Community health center have good accessibility and a good alternative for the treatment cost. Therefore, it is necessary to provide support and measures to make hypertension management safer.

  • This study investigated the Infant Institutional Household (IIH) for the care of infants. The purpose was to provide basic information needed to raise Korea's birth rate and to design higher level nursing facilities which can satisfy working mothers who are presently offered few facilities. To achieve this purpose. IIM was divided into 7 sub-functional stations : planning-management, marketing, materials-management, business-management, man power-management, financial-management and control etc. The results are as follows : 1. Kind of facilities : Infants'(full responsibility) Institutional Household. 2. Number of infants : total 19 babies. 3. Ratio of infants to teachers : one to one (Contained assistance teachers) 4. Installing region : convenient traffic place - in Seoul. 5. Building site and kind : the first floor space of 60pyung (198㎡) in the Apartment complex which . contained many apartments of small size. 6. Nursing time : 24-hour day care in weekdays Age of object : age from 1month to 24months old babies(after his/her birth). 7. Kind of services and Cost : mother-like care, cost includes nursing-fee, a deposit and admission-fee. 8. Economic Condition and managing direction : pursuit limited profits with nursing-fee, admission-fee, a loan and national (or self -government of direct) supplementary funds.

A Study on Health Service Utilization for the Low Income Elderly in Korea (전국 저소득층노인의 보건의료이용과 영향요인 분석)

  • The purpose of this study was to analyze the relation amongst health status, health care costs. health service utilization among the low income elderly who were 60 years of age or older, earning a half of the average Korean family income. The cross-sectional descriptive survey research we conducted used families randomly sampled nationwide. The data were collected from July 12 to August 7, 1999 and the total sample was 1.259 household members (421 households). These were the major findings. 1. As for the health status. 72.4% of respondents fell ill in the last 1 month; 54% of respondents had chronic disease. 2. As for the health care cost. the cost of hospitalization and the medical treatment were 1.069,000 won and 226.000won. respectively. 59.3% of respondents experienced a burden from the monthly health care expenses. 3. As for the health service utilization for the last 3months. 28.5% of respondents didn't utilize the health service. In addition, 22.2% of respondents gave up a medical treatment because of economic situation (88.8%). 4. The statistically significant determinants of health service utilization are old age, female, living with a spouse, unemployed state, medicare, and more days sick. 5. It is shown conclusively that equity and efficacy of the health care policy are to be considered for lower income older adult.

Experience of Alopecia in Adults: A Grounded Theory Approach (성인의 탈모경험: 근거 이론적 접근)

  • Purpose: The purpose of this study was to investigate the alopecia experience in adults and to explain the process of their experiences. Methods: Using a grounded theory methodology, 18 interviews were performed with fourteen men and four women, 34~57 years of age, suffering from alopecia. Data were analyzed using the constant comparative analysis method. Results: The core category emerged as "inescapable fetters". adults with alopecia engaged in three stages: embarrassment, seeking solution, and acceptance phase. Causal conditions were a vicious cycle of stress, biological factors and poor life style. Contextual conditions were recognition of irreversibleness, negative social awareness, and marriage. The central phenomenon of the adaptation process among the adults with alopecia was withdrawn life due to negative body image. Action/Interaction strategies included rely on medical treatment, efforts to take good care of hair, research for information treatment, efforts to cover up hair loss, and mind control. Intervening conditions were time cost, economic cost, support of surrounding people. Consequences was burden of unfinished lifetime homework. Conclusion: When caring for these adults, it is important to identify needs, allow patients to express what they want at that moment and support them in maintaining a daily life.

The Analysis of Research on the Home-Based Pulmonary Rehabilitation Program (가정 호흡재활 프로그램 관련 연구 논문 분석)

  • Purpose: The purpose of this study was to analyze the trend of research on the use of home-based pulmonary rehabilitation. Method: Using Medline and CINAHL search engine, experiemental research, titled as 'home-based pulmonary rehabilitation' and 'chronic lung disease', statistics were collected. The total 29 experimental studies published between 1981 to 2000 were selected and analyzed according to the ATS guidelines. Result: 1) The study samples in research were mostly of men, whose lung function was moderate to severe. 2) The total period of the intervention program varied from 5 to 12 week. The program included excercise intervention and educational intervention. Psychosocial intervention was minimal. 3) The outcome of the home-based pulmonary rehabilitation had been measured mostly by physical aspects. Quality of life and cost were less frequently measured. 4) In terms of effectiveness of the home based pulmonary rehabilitation program, physiologic outcome, such as lung function and hematologic markers, there was more 'no effect' than 'positive effect', whereas there was more 'positive effect' in decreasing dyspnea, improving excercise capacity, and improving quality of life. Conclusion: Based on these findings, future research on home-based pulmonary rehabilitation should be emphasized and provide standardized protocol, including psychosocial intervention, and analyses on cost and quality of life.

A Study about Developing Care Delivery System of Providing Efficient Patient Care (효율적인 환자간호를 위한 간호전달체계 모형 개발에 관한 연구)

  • The purpose of this study was to develop a nursing care delivery system which can provide patient focused care and increase satisfaction for both patients and nurses. This study was a quasi-experimental study to test the effectiveness of direct / indirect nursing care hours, patient satisfaction, nurse's job satisfaction, and the difference of the length of stay between model and control group. Control group data were collected from 100 patients who had lumbar disease in the neuro-surgical unit and model group data were from 66 patients who didn't have family residents from August 21, 1995 to February 29, 1996. Model unit was operated by modified PPM(Professional Practical Model) system for 3 months from December 1, 1995 to February 29, 1996. Working committee empowered nurses to make a decision to provide increased opportunities for autonomy, accountability, and control over the environment in which they deliver care. Satisfaction survey was done twice and analyzed by the SPSS program. The results of the study were as follows; 1. Mean direct nursing care hours of the model group was 90.47 minutes as it was 28.04 in the control group which shows significant statistical difference (t=-18.0, P=0.000). 2. Patient satisfaction to nursing care of in the model group was significantly higher than the control group (t=-6. 77, P=0.000) and it apples to the all subcategories, too. 3. Nurse's job satisfaction shows the significant increase in the model group than the control group(Z=-3.0405, p=0.0024). 4. There was no statistical difference between 2 groups in average length of stay except for the post-op patient which shows 5.4 days less in the model group. This study shows that patient satisfaction and nurse's job satisfaction were increased and length of stay was decreased in the model group. Even though this new developed Patient - Nurse Oriented Model can provide nursing with the opportunity to improve patient care to increase productivity and highly valued, it is very difficult to practice due to the limited nursing resources, higher acuity levels and nursing care cost. It is strongly recommended that the government needs to look at the unreasonable reimbursement system and recognize the nursing care costs to the high acuity level patients. Otherwise, make the patient responsible for the nursing care cost so that we can provide more efficient and patient focused care.

Utilizing Video vs Simulation Practice for Handoff Education of Nursing Students in Pediatric Nursing (간호대학생을 위한 시뮬레이션 기반의 아동간호 인수인계에 관한 동영상 실습교육과 시뮬레이션 실습교육의 효과)

  • Purpose: The purpose of this study was to develop a model for handoff education for nursing students based on simulation using video and to identify educational effects of a simulated situation in pediatric care units. Methods: Data were collected from May 1 to 30, 2016. Participants were 84 senior nursing students in Seoul (video group: 43, simulation group: 41). Both groups were given a lecture and pre-briefing on handoff education. The simulation group had nursing practice on resolving health issues for respiratory distress using a high-fidelity baby simulator. The video group watched a video recording of a scenario based simulation, and used a summarized handoff situation to practice patient handoff to another student. Results: There was no significant difference between the two groups for handoff self-confidence, problem solving ability, handoff competence (self-assessment of students), or learning satisfaction. Self-confidence increased significantly in both groups. Handoff competency evaluated by the instructor was higher in the video group compared to the simulation group (t=2.33, p=.022). Conclusion: Nursing student education for handoff practice utilizing a video in the pediatric unit was more cost effective. Therefore, it could be a useful educational method for students in learning patient handoff practices and helpful for related research.
