The Disagreement of Discourse and Power Through the Experiences of Nursing for Caring of Long-term Hospitalized Patients (장기 입원환자 간호 갈등경험을 통해 본 담론 대립과 권력)

  • Purpose: This critical ethnography was performed to explore the experiences of nurses who are working with patients in an industrial disaster hospital. During the research process, I focused on the experiences of conflict in caring patients. Methods: Data for the study came from 13 informants with their corresponding patients through interview and observation from March 2002 to February 2004. The data was examined line by line; then compared and contrasted based on a critical discourse analysis. Results: Nurses' conflicts came from discrepancies of the world views from that of the patients. Such conflicts arose because of various issues as follows: Worker as an individual vs patients, nurse as young women vs the medical profession, hospital as an extended home vs health care setting, and hospitalization as a means to enhance work capacity vs a means of treatment. Conclusion: We need more study on the development of adaptive strategy for the nurses to overcome conflicts during their nursing career. Developing a nurses' and patient role intervention program is needed.

The Effect of Care-workers Work Environment on Organizational Commitment: The Mediation Effects of Job Satisfaction (요양보호사의 직무환경이 조직몰입에 미치는 영향: 직무만족의 매개효과 검증)

  • The purpose of this study is to identify the effect of the job environment of nursing care workers on organizational commitment and to verify the mediating effect of job satisfaction based on this. Data collection was conducted on 340 nursing care workers, centering on elderly care facilities in Gyeonggi-do, and the uncollected questionnaires and abnormalities were removed and the final 267 were selected as the subjects of the study. Data analysis results were derived through SPSS WIN 2.0 and PROCESS Macro V3.5, and Hayes (2013)'s Macro Model Number 4 was applied to verify the mediating effect of job satisfaction. As a result of the analysis, the partial mediating effect of the job satisfaction was confirmed in the effect of the job environment of the caregiver on organizational commitment. Based on the analysis results, practical and policy suggestions for improving the job environment and increasing job satisfaction of nursing caregivers, and follow-up studies were proposed to overcome the limitations of this study.

The experience of Long term Care Hospital Workers on Sexual Behaviors of the Elderly (노인 요양병원 종사자들의 노인 성행동 경험)

  • This study is an explorer study to investigate the experience of long term care hospital workers on the sexual behaviors of the elderly. The data were collected from August to October in 2019. The depth interview was implemented with using the semi structured questionnaires. The interview data collected from the participants were analyzed with using qualitative thematic analysis methods. The experience of long term care hospital workers regarding the elderly's sexual behaviors consisted of ten sub-categorized themes from the significant statements and was categorized into following four themes; 'Negative feelings related with elderlys' sexual behaviors', 'Difficulty of maintaining caring roles', 'Recognizing the absence of care for elderlys' sexual behavior', 'Accepting the therapeutic roles for elderlys' sexual behavior'. The findings could be the basic data for developing educational programs that help the long term care hospital workers understand the sexual behavior of the elderly.

Health Problems and Support from the Health & Welfare Service in the Elderly at Home (재가노인의 건강문제와 보건ㆍ복지 서비스 지원 실태에 관한 연구)

  • Purpose: This study was conducted to identify health problems and support received from the health and welfare service using MDS-HC(Minimun Data Set for Home Care) in the aged living at home. Method: Eighty-one elderly persons were selected from those listed in community-welfare service centers in Seoul and Kyunggi Province between December 2002 and January 2003. Result: Eleven health problems per elderly person on average were identified, and the frequent care needs were in order: preventive health measure, health promotion, visual function, depression & anxiety, communication disorders, social function, pain, environmental assessment, oral health, cognition and falls. The number of health problems by the level of ADL was ‘ADL 1(Independence)’ 9.87, ‘ADL 2(Partial independence)’ 12.78, ‘ADL 3(Dependence)’ 13.73. Utilization of formal health & welfare services among the elderly was ‘meals on wheels’ 40.7%, ‘home helper’ 38.2%, ‘visiting of social welfare worker’ 21.0%, ‘physical therapy’ 19.6%, ‘day care center’ 12.3%, ‘volunteer's service’ 9.9%, ‘home visiting care’ 3.7%, ‘occupational therapy’ 3.7%, and ‘speech therapy’ 2.5%. Conclusion: The results suggest that using the MDS-HC 2.0 is applicable to help decide criteria for both health and welfare service supplied to the elderly.

  • The purpose of this study was to evaluate the effects of the meaning of life, change to attitude toward the old and hospice perception through pre/post hospice education program of care worker trainees. Methods: This study is a quasi-experimental research using one group pretest-posttest design. The research period was from May to June, 2011. The subjects of the study were 53 care worker trainees in T city and S city. The data were analyzed by SPSS/WIN 17.0 program with descriptive statistics and paired t-test, mean, standard deviation. The period of this study was from May 9 to June 20 of 2011. Results: The meaning of life and hospice perception were significantly different between pre- and post-program. The attitude toward the old did not show a significant difference before and after the program. Conclusion: This study is need to improve hospice education program for comprehend meaning of life, change to attitude toward the ole and improve hospice perception.

The Study on the effects of Job Stress of Female Caregivers in Home-based Elderly Welfare Center on Turnover Intention: Self-elasticity as a controlling variable (재가노인복지센터 여성 요양보호사의 직무스트레스가 이직의도에 미치는 영향: 자아탄력성을 조절변인으로)

  • The purpose of this study was to understand the effect of job stress of female caregivers working in home-based elderly welfare centers on turnover intentions and to understand the moderating effect of self-elasticity in these relationships. The subject of the study was a self-written questionnaire survey of 170 nursing care workers at 28 home-based elderly welfare centers in Seoul, and 150 copies were collected and 133 people were finally analyzed excluding 17 unscrupulous responses. As a result of the study, first, the average value of turnover intention was high in the group with low experience of female caregivers at home-based elderly welfare centers and the group without religion. Second, the higher the job stress of nursing care workers, the higher the intention of turnover. Third, there was a moderating effect of self-elasticity between the job stress and the of turnover of nursing care workers. Therefore, it was necessary to consider measures to prevent job stress of nursing caregivers and improve self-resilience.

  • This study investigated the effect of calling on job burnout among nursing care workers, and a total of 230 copies were distributed to analyze 198 copies, excluding 32 copies of questionnaires with poor or missing answers. The correlation between factors on job burnout was found to have a positive correlation coefficient in the values of personal environment meaning and work meaning, and as a result of examining the effect of sub-factors of calling consciousness on job burnout, it was found that personal environment suitability had a significant effect. In order to reduce the job exhaustion of nursing care workers comprehensively, it is necessary to provide an environment for working at an institution. In addition, it will be necessary to activate the relevant institution in preparation for problems occurring in the workplace.

Nursing Students' Awareness and Performance on Standard Precautions of Infection Control in the Hospital (간호대학생의 병원감염관리 표준주의에 대한 인지도와 수행도)

  • Purpose: The purpose of this study was to investigate nursing students' awareness and performance on standard precautions and to provide meaningful information for nursing students' education regarding hospital infection control. Method: Four hundred forty seven nursing students at 6 universities in D-city participated in this study. Data collection was conducted from March to April 2011. Students' awareness and performance on standard precautions of infection control in hospitals were measured using the modified 2007 CDC standard precautions guidelines. Data analysis was performed using the SPSS WIN 18.0 program, descriptive statistics, t-test, and ANOVA. Results: The level of students' awareness in the standard precautions was higher than performance. The higher levels of students' awareness and performance on standard precautions included patient care equipment, safe injection practices, and worker safety. The lower levels of students' awareness and performance on standard precautions included hand hygiene and personal protective equipment. There was no difference in the students' awareness and performance of standard precautions according to their characteristics. Conclusions: Based on the findings of this study, hand hygiene and personal protective equipment need to be stressed more within the program to improve nursing students' infection control techniques.

Outcome Assessment of a Demonstration Project on Integration of Home Visiting Health Care and Social Welfare Services (보건소 중심의 방문보건.재가복지 통합시범사업 성과)

  • Purpose: The objectives of this study was to measure the outcomes of interventions on the health and social welfare of the elderly in a rural community in Korea. The project involved integrating services of one public health center with that of one social welfare agency, which were under different administrative structures. Method: A single group pretest-posttest design was used for this research. Seventy-five elderly residents living alone in a rural community participated in the study. All of them had coverage of free basic medical care and social welfare services by the government. Major activities for the intervention included: developing partnerships among community leaders/institutes; forming committees of community residents; educating care providers and volunteers; developing 8 integrated service programs and instruments; and organizing the networks. The 20-month intervention was care-managed by a public health nurse whom collaborated with social worker, and was assisted by volunteers. The t-test was utilized to analyze the outcome variables including the elder's health, social welfare and quality of life. A major limitation of this study was the lack of a control group. Results: The outcome of the intervention was shown by improved elder's health, social welfare needs, and quality of life. Integrating the services of public health centers with those of social welfare agencies is an effective way to improve the health of the elderly in the community. Conclusion: Developing community capacity with such integrated services will pay an important role in improving the health of the elderly who live alone.

Factors Influencing COVID-19 AstraZeneca (ChAdOx1) Vaccination and Side Effects among Health Care Workers in an Acute General Hospital (중소병원 의료기관 종사자의 COVID-19 AstraZeneca (ChAdOx1) 백신 접종 영향요인과 접종 후 이상 반응 실태조사)

  • Purpose: This study was conducted to identify factors affecting COVID-19 vaccination and associated side effects among health care workers in a small and medium-sized hospital. Methods: In May 2021, 301 workers out of a total of 670 working in a small and medium-sized hospital in Gyeonggi-do, were surveyed. The small and medium-sized hospital treats patients with COVID-19. Health care workers across different medical institutions responded with self-reported internet questionnaires. Factors influencing COVID-19 vaccination were analyzed via logistic regression. Results: Out of 301 subjects, 89.0% showed an intention to inoculate, and 85.7% were vaccinated against COVID-19. The most frequent reason for vaccination was responsibility as a medical worker. The fear of adverse reactions was the most frequent reason for non-vaccination. Adverse reactions after inoculation occurred in 70.9% of cases, and 30.6% were referred for treatment of adverse reactions. The factors ultimately influencing COVID-19 vaccination were vaccination intention, previous side effects from other vaccinations, occupation, and age. Conclusion: In order to improve the effectiveness of COVID-19 vaccination, a systematic approach is required to determine the risk factors associated with the young age of the administrative staff/personnel, subjects with a history of side effects associated with other vaccines, and health care workers who do not intend to be vaccinated. It is important to develop strategies to improve immunization. In addition, accurate and essential information regarding the side effects of vaccination is needed, along with appropriate education and publicity.
