The Relationship between Patient Characteristics and Satisfaction with Hospital Care (환자특성에 따른 의료이용에 대한 환자만족도 비교)

  • Purpose: The purpose of this study was to identify the relationship between patient characteristics and patient satisfaction. Methods: A cross-sectional questionnaire survey was conducted in an acute care hospital. The subjects were 317 patients discharged from general medical and surgical nursing care units during September, 2005. Patient satisfaction was measured using the short-form satisfaction scale of Hwang and Park(2001). Additional information about patient characteristics, including general demographics and health care utilization variables, was collected from the hospital information systems. Multiple regression analysis was performed to determine patient characteristics influencing patient satisfaction. Results: Patients were satisfied with hospital care with an average of 4.10 on a five-point Likert scale. Patient characteristics explained 13.5% of the variance of patient satisfaction. The significant factors influencing patient satisfaction were patients' age and perceived health status. There was no significant relationship between structural variables and patient satisfaction. Conclusion: This study showed that patients' characteristics were significant factors explaining patient satisfaction. Therefore, these characteristics should be adjusted in reporting patient satisfaction as an indicator for hospital-level or department-level rating.

Organizational Culture and Workplace Bullying of Intensive Care Unit Nurses (중환자실 간호사가 인식한 간호조직문화와 직장 내 괴롭힘 경험과의 관계)

  • Purpose: The purpose of this study was to identify the relationship between organizational culture and workplace bullying of intensive care unit (ICU) nurses. Methods: A secondary analysis was conducted using three different data sets on workplace bullying of hospital nurses. This analysis focused on the relationship between organizational culture and workplace bullying of 240 ICU nurses. The original data were collected using self-reporting questionnaires and were analyzed using hierarchical multiple regression. Results: ICU nurses were aware of their organizational culture as being rank-oriented, relationship-oriented, innovation-oriented, and task-oriented in that order. The results showed that 15.0% of the ICU nurses reported being victims of workplace bullying, mostly work-related bullying. The perception of a relationshiporiented culture explained a variance of 29% in the workplace bullying of ICU nurses. Conclusion: The findings suggest that ICU nurses who are strongly aware of a relationship-oriented culture would experience less workplace bullying. Further research is needed to develop interventions that can foster a relationship-oriented culture in ICUs.

The Effects of Implementation of Ventilator-Associated Pneumonia Prevention Bundles (인공호흡기 관련 폐렴 예방 번들 적용의 효과)

  • Purpose: The purpose of this study was to evaluate the effectiveness of a ventilator-associated pneumonia (VAP) bundle. Methods: This was a retrospective study that was carried out between June 2010 and October 2015. In this study, 3,224 intubated patients were included. The VAP bundle which was applied to Group 1 patients (n=470) included head-of-bed elevation to 30 degrees, cuff pressure monitorization, prophylaxis of peptic ulcer, and prophylaxis of deep vein thrombosis. The VAP bundle for Group 2 patients (n=1,914) included all the elements of the VAP bundle for Group 1 patients and one additional element which was oral care with 0.12% chlorhexidine. The VAP bundle for Group 3 patients (n=870) added sedative interruption and assessment of readiness to extubate to the VAP bundle for Group 2. Results: The numbers and incidences of VAP were significantly different among the three groups. Moreover, there were significant differences among groups in ICU length of stay and mortality. Conclusion: Three different VAP prevention bundles made different effects in patient outcomes.

A Comparative Study of the Factors Influencing Burden of Primary Family Caregivers according to the Degree of Serious Illness of Elderly Patients Admitted in an Intensive Care Unit (중환자실 입원 노인 중증도별 주 부양가족의 부담감 영향요인 비교)

  • Purpose: This study was to compare and examine the factors influencing burden of primary family caregivers according to the severity of illness of elderly patients admitted in an intensive care unit. Methods: Subjects were the families of elderly patients in intensive care units of K, S and Y hospitals in Seoul. Data were collected from March to October 2007. Subjects were 108 persons over age 65. Data were analyzed by SAS statistics. Results: First, groups 5 and 3 showed higher burden than that of group 4. Second, high correlation was found between stress and burden, stress and anxiety, and burden and anxiety. Third, factors influencing family burden were found to be stress for group 5, stress, anxiety, and monthly income for group 4, and stress and patient age for group 3. Conclusion: Specific nursing interventions to decrease the stress of primary family caregivers of serious ill elderly patients in an intensive care unit are needed. Additionally, more effective and systematic activation of a long-term medical insurance system for seriously ill seniors is considered necessary to mediate the burden of primary family caregivers.

Characteristics Influencing the Occurrence of Respiratory Medical Device-related Pressure Ulcers in the Pediatric Intensive Care Unit (소아중환자실 환아의 호흡기계 의료장치 관련 욕창 발생 관련 특성)

  • Purpose: This prospective study was conducted to determine the incidence and related characteristics of respiratory medical device-related pressure ulcers (MDRPU) in children admitted to a pediatric intensive care unit (PICU). Methods: The participants were 184 children who were admitted to the PICU of P University Hospital from April 2016 to January 2017. Data were collected on the occurrence of respiratory MDRPU and characteristics regarding the application of respiratory medical devices. Results: Respiratory MDRPU occurred in 11.9% of participants (58.3%: stage I ulcers, 37.5%: mucosal ulcers). The devices associated with respiratory MDRPU were endotracheal tubes (54.2%), high-flow nasal cannulas (37.5%), and oximetry probes (8.3%). Respiratory MDRPU associated with an endotracheal tube were significant differences according to the site and strength of fixation, the use of a bite block and adhesive tape, skin dryness, and edema. In high-flow nasal cannulas, significant differences were found according to the site of fixation, immobility after fixation, and skin dryness. Conclusion: The occurrence of respiratory MDRPU is significantly affected by the method and strength of fixation, as well as skin dryness and edema. Therefore, appropriate consideration of these factors in nursing care can help prevent respiratory MDRPU.

The Relationship between Person-Centered Nursing and Family Satisfaction in ICUs (중환자실에서의 인간중심 간호와 가족 만족도 관계)

  • Purpose : This study aims to identify the relationship between person-centered critical care nursing (PCCN) and family satisfaction for intensive care unit (ICU) nurses. Methods : This research used a cross-sectional survey. The participants were 142 nurses and 121 family members from 8 ICUs in 4 hospitals in B city. The questionnaire survey was conducted between December 2018 and March 2019. The eight ICUs were divided into those that scored high and those that scored low for person-centered care relative to the average score for PCCN, and the difference in family satisfaction between the ICUs with high and low scores was compared using t-test. Results : The mean score of PCCN was $3.68{\pm}0.40$ out of 5. In the subcategories of PCCN, "comfort"was the highest at $3.95{\pm}0.49$, followed by "respect" at $3.73{\pm}0.57$, "compassion" at $3.59{\pm}0.57$, and "individuality" at $3.47{\pm}0.54$. The average score of family satisfaction with the ICUs was $3.45{\pm}0.67$ out of 5. In its subcategories, "emotion" was the highest at $3.69{\pm}0.72$, followed by "information" at $3.61{\pm}0.75$, "participation" at $3.30{\pm}0.79$, and "resources" at $3.20{\pm}0.80$. The family satisfaction ($3.75{\pm}0.54$) with the ICU of the highest-scoring PCCN was significantly higher than the family satisfaction ($3.25{\pm}0.53$) with the ICU with the lowest-scoring PCCN (t=4.97, p<.001). Conclusion: The results suggest that the PCCN scores of ICU nurses should be promoted to improve family satisfaction. It is necessary to pursue a variety of ways to secure the provision of person-centered nursing practice in ICUs.

Influence of Partnerships with Nurses and Social Support on Readiness for Discharge among Mothers of Premature Infants (미숙아 어머니의 간호사와의 파트너십, 사회적 지지가 퇴원준비도에 미치는 영향)

  • Purpose: The purpose of this study was to investigate partnerships with nurses, social support and readiness for discharge among mothers of premature infants in the neonatal intensive care unit (NICU) and to examine the factors associated with readiness for discharge. Methods: A survey was conducted among 85 mothers of premature infants hospitalized in a NICU in Seoul, Korea. The collected data were analyzed with descriptive statistics, the t-test, one-way analysis of variance, Pearson' correlation coefficients, and multiple linear regression using SPSS version 25.0. Results: The results of the regression analysis showed that partnerships with nurses (${\beta}=.32$, p=.011) and parenting experience (${\beta}=.32$, p=.001) were significantly associated with readiness for discharge. Conclusion: To improve the readiness for discharge among mothers of premature infants, developing strategies to strengthen their partnership with nurses and to provide family-centered care will be needed.

Analysis of Nursing Interventions Performed by Orthopedic Surgery Nursing Unit Nurses Using NIC (간호중재분류(NIC)에 근거한 정형외과 간호단위의 간호중재 수행분석)

  • Purpose: The purpose of this study was to identify the frequency with which nursing interventions according to domains and classes, and core nursing interventions of the Nursing Intervention Classification (NIC) were performed by nurses on orthopedic surgery nursing units. Method: For this purpose, the third edition of NIC was used. Of the 486 nursing interventions, 424 were selected at 75% consent by experts. Data were collected from June, 2003 to July, 2003 5 hospitals and 69 nurses(return rate : 95.8%) in Gwang-ju and Chonnam region. 5 point Likert scale describing frequency was used. Results: The most frequently performed domain was 'physiological : basic'($2.97{\pm}.60$), followed by 'health system'($2.65{\pm}.65$) and 'physiological : complex'($2.55{\pm}.46$). The most frequently performed class was 'activity and exercise management'($3.82{\pm}.89$), followed by 'immobility management'($3.64{\pm}.62$), 'skin/wound management'($3.41 {\pm}.60$), 'physical comfort promotion'($3.23{\pm}.68$) and 'thermoregulation'($3.01{\pm}.91$). The most frequently performed nursing intervention was 'medication administration' ($4.96{\pm}.21$), followed by 'medication administration : intravenous'($4.93{\pm}.31$), 'analgesic administration'($4.91{\pm}.51$), 'pain management'($4.87{\pm}.34$) and 'medication administration : intramuscular'($4.78{\pm}.68$). Conclusion: In conclusion, the third edition of NIC was found to be a general and comprehensive classification system for application on orthopedic surgery nursing units. These findings will help in building of a standardized language for orthopedic surgery nursing units and enhance the quality of nursing care.

Evaluation on the Performance of Nursing in according to the Nursing grade of Hospitals (병원 간호등급에 따른 간호수행 정도)

  • As a cross-sectional study, this study was aimed to investigate and compare the job efficiency and satisfaction of nurses according to the hospital grade. Survey was conducted by mail on June 2009, and the respondents were 1,016 nurses working in 15 hospitals which are 9 high-grade general hospitals and 6 general hospitals. The percent of nurses acknowledging their hospital grades is 34.5%, and that is 20.5% at high-grade general hospitals. As the result of review of studies, it is concluded that under the circumstance that differential rates are contracted to calculate fees for hospital services and copayment of patients are according to nursing grades and hospital grades, the degree of nurses' awareness of insurance fees impact on their performance like recording of care and prescription. In order to improve nurses' performance, they need to be educated about the national insurance fee system. In hospitals with higher nursing grade and more beds, the levels of nursing quality and faithfulnes and their job satisfaction were higher. Nurses' awareness of their hospital nursing grade was related to the quality of nursing but not the faithfulness. Nurses working in higher nursing-grade hospital are more self-respect and satisfied at their jobs, and their job efficiencies are not significantly different. The current nursing fees based on the proper number of nurses per beds of nursing units should be changed to be based on the amount of job per nurse by their nursing protocol, and the nurse staffing standard should be differentiated between nursing grades. As the aspect of nursing, 24-hours patient care, it is difficult to improve nurses' job satisfaction, and in the other hand, that tends to depend on their income level. In the current circumstance, comprehensive research is required to investigate the propriety of 25% of the inpatient fees as the nursing management charge.

An Analysis of Nursing Practice in an Oriental Medicine Hospital (일 대학부속 한방병원 간호사의 직무내용 및 직무량분석)

  • This study was directed at analysing nursing practice in one oriental medicine hospital. In this study, the nurses were chosen from two units at one oriental medicine hospital. Using the worksampling method, a total of 780 series of nursing actions were collected over thirteen days. this study had two findings. First, the percentage for the amount of actual nursing practice was 79.5% which is lower than the amount of actual nursing practice in other western style general hospitals. Second, the content of nursing practice included nursing actions(43.8%), nursing management (35.5%), and education /research (0.1%), indicating that only the special care of oriental nursing and education /research were not observed in nursing practice. These findings indicate a deficit knowledge in oriental medicine and oriental nursing care by nurses in oriental medicine hospitals. Since Oriental Nursing has not been included in Nursing curriculum, there is a need for research to develop basic education in Oriental Medicine and pharmacology as well as the development of a sgstematized approach to Oriental Nursing so as to provide an appropriate background for nurses working in this field. Further research is needed to define nursing in the Oriental Medicine system.
