A Study on Community Senior Citizen Center as the Senior Health Care Center Vitalization Plan Measures

  • Purpose. This study aims to explore Situations and Problems of the Community Senior Citizen Center as the senior health care and the Elderly's Leisure status. Then, aims to arrange Activation Vitalization Plan of the senior's leisure in Community Senior Citizen Center. Methods. The literature and data used in this study was based on a questionnaire survey, mostly from Gyeongki-Do Community Senior Citizen Center Branch and statistical research data. Literature review and analysis frequency was by reference to the paperback and academic papers related to the senior health welfare. Results. First, the period of the seniors with the Community Senior Citizen Center as health facilities has appeared in 6-10years(32.8%), followed by the response showing that more than 10years(32.4%). Therefore, it reveals that the senior live in the same region in the long term. Second, the number of days that the senior health care the Community Senior Citizen Center has been used by the senior was over 5days. This result was supported by 608people(61.7%). Both men and women replied that they use the health center more than five days. Third, the number of the senior who responded that they use the Community Senior Citizen Center as health facilities 629people(63.9%). They replied that they use the facilities mostly afternoon. The senior use the facilities all day appeared to 263people(26.7%). Conclusions. It seems like that there needs to be a variety of personalized programs that can be added to increase the life satisfaction of the senior participation in leisure programs for the Community Senior Citizen Center as senior health facilities in the future. Additionally, the government needs to require a wide range of financial support for the Community Senior Citizen Center as senior health care and devise the strategies that will lead the health center for the senior need to be actively utilized.

지역사회 통합돌봄 제도 연구 동향 분석: 주제 범위 문헌 고찰 (Research Trends in integrated Community Care Systems: A Scoping review)

  • 본 연구는 국내 지역사회 통합돌봄 제도에 관한 연구 동향을 파악하고 향후 연구 방향을 제시하고자 수행되었다. 주제 범위 문헌고찰방법으로 JBI방법론적 지침에 따라 진행하였다. 문헌검색은 5개의 DB(RISS, DBpia, NDSL, KISS, NAL)에서 실시하여 최종분석에는 34편의 논문이 선정되었다. 지역사회 통합돌봄과 관련된 이용자와 제공자 측면으로 분류하여 분석하였으며 이용자 측면 결과는 서비스 만족도, 신체적 건강상태와 관련된 변수가 가장 많이 사용되었고 긍정적인 결과가 나타났다. 제공자 측면 결과는 서비스 제공으로 인한 문제점과 개선점을 주로 연구하였으며 기관 간 협업체계 활성화의 필요성과 서비스 수행 인력 부족이 많은 연구에서 나타났다. 본 연구결과를 토대로 향후 지역사회 통합돌봄 제도의 확장에 있어 대상자 선정기준 확립 뿐 아니라 서비스 제공자의 전문성 확보, 지역 간 및 기관 간 협업체계 및 서비스 편차해소를 위한 대책마련이 필요할 것이다.

장기요양기관(시설급여) 평가의 효과성 검증 (Verification of effectiveness of evaluation of long-term care institution)

  • 본 연구에서는 장기요양기관 관리자, 요양보호사, 입소노인을 대상으로 하여 2015년도 장기요양기관의 평가(평가등급, 평가등급의 변화, 평가지표의 적합성), 요양보호사의 서비스의 질, 입소노인의 서비스 질 만족도, 삶의 만족도, 시설 선택에 미치는 영향을 파악하고자 하였다. 이를 위해 IRB를 받은 뒤 2018년 1월과 2월에 장기요양기관 관리자 79명, 요양보호사 381명 및 입소 노인 381명을 대상으로 설문조사를 완료하였다. 설문조사는 2015년도 장기요양(시설급여) 평가결과를 반영한 할당표본을 실시하였다. 연구결과 장기요양기관 평가의 주된 목적이라 할 수 있는 입소노인의 삶의 질 향상에는 요양보호사의 서비스 질 보다 기관평가(평가등급, 평가등급의 변화, 자표의 적합성 인식)가 더 큰 영향을 미치는 것으로 나타났다. 그러나 퇴소 후 시설재선택의향은 요양보호사의 서비스의 질에 더 큰 영향을 받았다. 따라서 시설재선택의향을 제외하고 보면, 국민건강보험공단의 평가의 호과성이 부분적으로나마 검증되었다고 볼 수 있다.

혈액투석 환자의 자기효능이론 기반 식사관리 프로그램이 식사관리이행, 신체상태 및 삶의 질에 미치는 효과 (Effects of Dietary Program based on Self-efficacy Theory on Dietary Adherence, Physical Indices and Quality of Life for Hemodialysis Patients)

  • Purpose: The purpose of this study was to examine effects of a dietary program based on self-efficacy theory on dietary adherence, physical status and quality of life (QoL) in hemodialysis patients. Methods: A non-equivalent control group pre-post test design was used. The intervention group received the dietary program for 8 weeks from August 4 to September 26, 2014. The control group received only usual care. Results: ANCOVA showed that dietary adherence (F=64.75, p <.001) was significantly different between the two groups. Serum albumin (F=12.13, p =.001), interdialytic weight gain (F=56.97, p <.001), calories (F=15.80, p <.001) as physical status indices were significantly different, but serum potassium (F=2.69, p =.106) and serum phosphorus (F=1.08, p =.303) showed no significant difference between the two groups. In terms of health-related QoL, the physical component scale (F=10.05, p =.002) and the mental component scale (F=16.66, p <.001) were significantly different between the two groups. In addition, in terms of diet related QoL, diet level (F=35.33, p <.001) and satisfaction level (F=15.57, p <.001) were significantly different between the two groups, but dietary impact level (F=1.23, p =.271) was not significantly different. Conclusion: Findings show that the dietary program based on self-efficacy theory is an effective nursing intervention program to improve adherence to diet, and to maintain physical status and QoL for hemodialysis patients.

부산지역 결혼이주여성의 건강관련 실태 (Health Status of Married Immigrant Women in Busan)

  • Purpose: By focusing on immigrant women's problems and considering human rights, resettlement plans and marital-life adaptation, health status is often overlooked. It was examined in this study. Method: The participants were 110 immigrant women who lived in Busan. Data were collected from September to November 2007 using a questionnaire. Results: One of five of the women had been hospitalized for a delivery, about 25% were pregnant, and most (80.9 %) have never had a pap-smear test. The average score for anxiety was 3.0 (${\pm}2.50$) and for depression 2.9 (${\pm}2.71$). Most were in the normal range for blood pressure, body mass index(BMI), blood sugar and urine test. There was a significant difference in level of anxiety according to education (F=3.42, p=.020) and in level of depression according to age (F=4.05, p=.020), number of children(t=-.444, p=.009), financial status (F=3.75, p=.027) and satisfaction with life (F=6.38, p=.002). There were significant differences in BMI according to age (F=7.73, p=.001), native nation (F=18.38, p=.000), period of stay in Korea (F=3.11, p=.049), education (F=3.82, p=.012), number of children (t=-4.26, p=.040). Conclusion: Although the health status of the immigrant women was good, procreative care protocols should be developed to support immigrant women who are pregnant.

산후 우울 수준과 분만전후 관련 요인에 관한 연구 (Postpartum Depressive Score and Related Factors Pre- and Post-delivery)

  • Purpose: This study aimed to identify the scores of postpartum depression(PPD) on the first day, 1st week, and 6th week after the delivery and to explore their related factors before and after delivery in postpartum women. Methods: With a survey design, 293 postpartum women were recruited from a postpartum unit, Ilsin Christian hospital in Pusan via convenience sampling and were followed at 1st week and 6th week in the outpatient clinic. Results: Results showed that the scores of PPD(EPDS score) were low at postpartum 1st day, 1st week and 6th week but prevalence of PPD(EPDS ${\geq}13$)was 3.1%at 1st day, 8.2%at 1st week and 7.5%at 6th week, respectively. The pre-delivery factors were experience of depression, and the post-delivery factors were baby's sex(1st day), no caregiver for baby(1st week), and no help and concern for taking care of baby from husband and family(1st day and 6th week). The greater satisfaction with becoming a mother and her life, and greater maternal attachment were related to lower level of PPD at the three time points. Conclusion: Regular screening for postpartum depression and supportive and informative education is needed for postpartum women visiting the outpatient clinic for follow-up.

노인요양시설 간호사와 요양보호사의 직무관련 소진 영향요인 (Factors Influencing Work-related Burnout of Nurses and Caregivers in Nursing Homes)

  • 본 연구는 노인요양시설에 근무하는 간호사와 요양보호사의 직무관련 소진에 영향을 미치는 요인을 확인하기 위한 서술조사연구이다. 연구의 대상은 서울과 경기지역 소재 5개소 노인요양시설의 간호사 77명과 요양보호사 111명으로 구조화된 설문지를 이용하여 자료를 수집하였다. 수집된 자료는 IBM SPSS Statistics 19.0의 독립표본 t-test, Pearson's correlation coefficients, and multiple stepwise regression으로 분석하였다. 간호사의 직무관련 소진은 직무만족(r=.333, p=.003), 스트레스(r=.484, p<.001)와 양의 상관관계가 있었으며, 요양보호사의 직무관련 소진은 스트레스(r=.458, p<.001)와 양의 상관관계가 있는 것으로 나타났다. 간호사의 직무관련 소진에 영향을 미치는 요인은 대인관계 스트레스였으며 이는 간호사의 직무관련 소진의 27.0%를 설명하고 있었다. 요양보호사의 경우는 직무관련 스트레스로 나타났으며 이는 요양보호사의 직무관련 소진의 21.2%를 설명하고 있었다. 이와 같은 연구결과를 바탕으로 근무환경의 개선과 체계적인 스트레스 관리 프로그램의 개발이 이루어져야 할 것이다.

Current Status of Patient Safety Regulations, Guidelines and Support Mechanisms in Korean Hospitals

  • Purpose: This study was conducted to investigate patient safety regulations and guidelines in order to understand their current status, and to examine support measures to improve patient safety in Korean hospitals. Methods: The participants were the safety officers from hospitals with 200 or more beds and 112 hospitals responded to the online survey. The questions covered patient safety regulations, the performance level of patient safety activities, patient safety incident reporting systems, the dedicated professional, training, support mechanisms, and expectations of reporting systems. Results: Among preventative measures, fall prevention and hand hygiene were reported to be most widely practiced (92% and 91%, respectively). Time-out for invasive procedures showed a relatively low practice rate at 70%. Among patient care activities, transfusion, surgery and sedation, medication, and infection management were performed by 84, 74, 93 and 93% of the hospitals, respectively. Patient safety activities included patient safety committee, patient safety cooperation between decision-making bodies, patient safety workshops, seminars, lectures, and training for employees. Conclusion: Patient safety regulations and guidelines have not yet been sufficiently prepared, and a public institution such as a certification authority is of crucial importance to enforce these guidelines.

보건소를 실습한 간호대학생의 코로나19에 대한 예방적 건강행위 연구: 건강신념 모형을 중심으로 (A study of nursing students practicing nursing at public health centers on preventive health behavior research on COVID-19: Focusing on the health belief model)

  • 본 연구는 보건소 실습을 한 간호대학생들의 '코로나19' 대한 지식과 건강신념 그리고 '코로나19'의 예방적 건강행위의 관계를 알아보기 위한 연구이다. 연구대상자는 231명이며, 자료수집은 2021년 10월 27일부터 11월 4일까지설문조사 하였다. 수집된 자료는 SPSS 23.0 프로그램을 이용하여 분석하였다. 간호대학생의 '코로나19'에 대한 예방적 건강행위에 영향을 미치는 요인을 알아보기 위하여 위계적 회귀분석을 실시하였다. 회귀모형 적합도는 (F=9.71, p<.005)이며, 코로나19에 대한 예방적 건강행위에 영향을 미치는 요인으로는 전공만족도(β=.18, p=.009)와 코로나19 지식(β=.20, p=.001), '코로나19' 건강신념(β=.36, p=.000)으로 나타났고, 설명력은 30.6%이다. 따라서 본 연구결과를 통해 간호대학생들에게 '코로나19'에 대한 예방적 건강행위를 증진시킬 수 있는 보건교육 프로그램의 전략을 개발해야 할 것이다.

이부교대(二部交代) 근무제도(勤務制度)에 대한 임상간호사(臨床看護師)의 요구도(要求度)에 관한 연구(硏究) (A Study on the Need of Clinical Nurses about the System of 12-Hour Work Shift)

  • The 12-hour work shift can be influenced on the nurse's job satisfaction, effective personnel administration, and quality of patient care. The purpose of this study was to explore the perception on the 12-hour work shift by nurses. A total of 516 nurses were selected by convenience sampling from six general hospitals in Seoul. Based on the literature review, the questionnaire on the need of the 12-hour work shift was designed by the investigator. The data were analyzed using descriptive statistics, ANOVA and Duncan's method as post-hoc test. The results of this study are as follows ; 1. The need of 12-hour work shift The mean needs of 12-hour work shift in the nursing practice, psychological, physical, socioeconomic, and environmental perspectives were 3.05, 2.72, 2.66, 3.08, and 4.22. 2. The need of 12-hour work shift by demographic data For the nursing practice, there was a statistically significant relationship between marital status(p=.021), satisfaction on the 8-hour work shift(p=.038), the perception on the 12-hour work shift(p=.001) and the need of 12-hour work shift. For the psychological perspectives, there was a statistically significant relationship between marital status(p=.016), the perception on the 12-hour work shift(p=.0001), and the need of 12-hour work shift. For the physical perspectives, there was a statistically significant relationship between age(p=.002), marital status(p=.005), the number of children(p=.005), the duration of job career(p=.014), the job position(p=.002), the work shift(p=.030), the perception on the 12-hour work shift(p=.0001) and the need of 12-hour work shift. For the socioeconomic perspectives, there was a statistically significant relationship between the job position(0=.002), the work shift(p=.006), the perception of the 12-hour work shift(p=.002) and the need of 12-hour work shift. For the environmental perspectives, there was a statisitically significant relationship between the traffic method(p=.05), the duration of the job career(p=.026), the perception on the 12-hour work shift(p=.309) and the need of 12-hour work shift. 3. The need of 12-hour work shift by the demographic data There was a statistically significant difference between marital status(p=.014), the number of children(p=.038), the job position(p=.007), the work shift(p=.037), and the perception on the 12-hour work shift(p=.0001) for the need of 12-hour work shift.
