Systematic Review of Meaning-centered Interventions for Adolescents

  • Purpose: This study analyzes meaning-centered intervention studies conducted in Korea and abroad to compare and confirm their characteristics and results. Methods: A literature search using keywords in English and Korean was performed using nine electronic databases in December 2017. Search participants included adolescents, and interventions conducted in meaning-centered intervention studies were selected for evaluation. A Risk of Bias Assessment tool for non-randomized studies was used for quality assessment. Results: All studies were based on quasi-experimental designs. The semantic intervention included topics such as freedom, choice, responsibility, pain, death, finding value of life, purpose and meaning of life, and becoming a master of my life. All studies used logotherapy, and included meaningful interventions that influenced the meaning and purpose of the life of adolescents. The life satisfaction of adolescents changed significantly when there is a positive and open relationship with their parents. Therefore, an educational program for parents based on meaning therapy is desperately needed. Conclusion: Meaning-centered interventions were found to be effective interventions for exploring the meaning of life not only for adolescents with problems but also for healthy youth, and as such could be used as basic data for the development of an appropriate intervention for enhancing their life.

기관내관을 삽입 받은 중환자의 구강간호 방법에 따른 구강상태 비교 (Comparison of Oral Care Interventions on the Oral Status of Intubated Patients in Intensive Care Units)

  • Purpose: The purpose of this study was to evaluate the effects of three different oral care treatments on the oral state of patients with intubation in intensive care units. Methods: The research design was a nonequivalent control group design with repeated measures. The patients were assigned a normal saline, chlorhexidine or toothbrushing group. Each group received its own oral care treatment for 5 minutes, twice a day and for 8 days. The oral assessment guide, hygiene performance index and pathogenic microorganisms. Data were collected from patients before the experiment, 4 days after, and 8 days after completion and were evaluated. Results: The chlorhexidine group and tooth brushing group showed significant improvement on the oral assessment guide and decrease in the hygiene performance index, compared to the normal saline group. Similarly, pathogenic microorganisms were significantly decreased in the chlorhexidine group and tooth brushing group, when compared to the normal saline group. Conclusions: Oral treatments with chlorhexidine and toothbrushing improve the oral health state of patients, therefore use of chlorhexidine and toothbrushing could be an effective nursing intervention for intubated patients in intensive care units.

한국과 독일 방문요양의 등급별 급여 비교 연구 (A Comparative Study on the Care Services According to Care Levels in the Korean and German Home Care System)

  • Purpose: This study aims to classify multiple forms of home care services from a service user's perspective. By comparing the German system to the Korean one, this study seeks to find better ways of enhancing client-centered care. Methods: The data of 121 home care users were collected from six home care centers that had under the management of nursing managers for more than five years. The researcher used a German instrument to estimate the level of care. Results: High correlation was found between German and Korean assessment criteria (r=0.81, p<.001). However, compared to their German counterparts, Korean home care providers allocated more time towards provision of domestic help per daily visit. German home care providers allocated more time towards services relating to provision of support for physical activities, including personal hygiene. It was confirmed that the level of care in Korea does not correlate with either the categories of home care services and duration. Conclusion: Therefore, care services should be offered according to long-term care needs. This study suggests a need for systematic care aimed at strengthening the client's autonomy which should be properly planned, implemented, and managed.

중환자실 간호사의 죽음 인식, 임종간호 태도, 정신건강이 임종간호 스트레스에 미치는 영향 (Influence of Death Perception, Attitude Toward Terminal Care, Mental Health on the Terminal Care Stress of Intensive Care Unit Nurses)

  • Purpose: This study aimed to identify the factors affecting the terminal care stress of nurses in intensive care units in terms of their death perception, attitude toward terminal care, and mental health. Methods: This descriptive study collected data from 118 nurses in intensive care units in one tertiary referral hospital and three general hospitals. The instruments used in the study were the Terminal Care Stress Assessment Tool, the View of Life and Death Scale, the Frommelt Attitudes toward Nursing Care of the Dying Scale (FATCOD), and the Mental Health Assessment Tool. The data were analyzed by t-test, ANOVA, Pearson's correlation coefficient, and multiple regression. Results: There was a significant positive correlation between terminal care stress and death perception (r=.31, p<.001). The factors significantly influencing the terminal care stress of the participants included gender (β=.33, p<.001), religion (β=.24, p=.004), and death perception (β=.35, p<.001), and the overall explanatory power was 23.1% (F=12.73, p<.001). Conclusion: To decrease terminal care stress among nurses, establishing the death perception of nurses based on value clarification about death may be necessary. Furthermore, this study suggests an intervention study examining the effect of an education program on terminal care stress among ICU nurses.

항암화학요법을 받는 소화기암 환자의 피로, 불안, 우울, 인지기능이 삶의 질에 미치는 영향 (Impacts of Fatigue, Anxiety, Depression, and Cognitive Function on the Quality of Life in Gastrointestinal Cancer Patients Receiving Chemotherapy)

  • Purpose: This study aimed to investigate the effects of fatigue, anxiety, depression, and cognitive function on the quality of life of gastrointestinal cancer patients receiving chemotherapy. Methods: Data were collected from a total of 141 participants. The measurements used were Functional Assessment of Chronic Illness Therapy for fatigue (FACIT-F), Hospital Anxiety and Depression Scale (HADS) and Functional Assessment of Cancer Therapy for cognitive function (FACT-Cog). Results: Significant correlations were found among fatigue, anxiety, depression, cognitive function, and quality of life. The mean score of quality of life was 59.60 out of 108, and 68% of the variance in QOL was explained by fatigue, anxiety, depression, and cognitive function. Cognitive function was the most influential factor (β=.30), followed by anxiety (β=-.27), depression (β=-.24), and fatigue (β=-.18). We found that the better the cognitive function, the lower the anxiety and depression, and the lower the degree of fatigue, the higher the quality of life. Conclusion: A nursing program for managing the changes in fatigue, anxiety, depression, and cognitive function should be provided to enhance maintenance and the improvement of the quality of life for gastrointestinal cancer patients who receive chemotherapy.

건강신념모델을 기반한 소규모 산업장 생산직 남성근로자의 대사증후군 감소 프로그램 개발을 위한 요구사정 (Health Belief Model-based Needs Assessment for Development of a Metabolic Syndrome Risk Reduction Program for Korean Male Blue-collar Workers in Small-sized Companies)

  • Purpose: This study aimed to comprehend the real context of metabolic syndrome-related factors of Korean male blue-collar workers from small-sized companies based on the health belief model. Methods: A total of 37 workers from three companies were interviewed, and three series of focus group interviews were conducted. Data were analyzed using deductive content analysis. Results: Data were classified into four categories: knowledge, perceived susceptibility and severity, perceived barriers, and beliefs. Knowledge referred to low knowledge level; perceived susceptibility and severity referred to unawareness of susceptibility and severity; perceived barriers referred to shift work, overtime work, and a social context including having no choice but to drink; and beliefs referred to believing that health promotion behaviors do not relate to preventing metabolic syndrome, believing that one cannot prevent metabolic syndrome oneself, and believing that professional help is required. Conclusion: To prevent and reduce the risk of metabolic syndrome among Korean male blue-collar workers, interventions should focus on strategies to increase metabolic syndrome-related knowledge and perceptions, social support, and self-efficacy for practicing health behaviors. In addition, it is necessary to develop policies for establishing a healthy drinking culture in companies.

Patient and Clinical Variables Account for Changes in Health-related Quality of Life and Symptom Burden as Treatment Outcomes in Colorectal Cancer: A Longitudinal Study

  • Objective: The aim of the current study was to evaluate changes in treatment outcomes in terms of health-related quality of life (HRQoL) and symptom burden at zero, one, three, and six months after an initial diagnosis of colorectal cancer. The demographic and clinical characteristics that account for outcome changes in patients were investigated using a repeated measures framework. Methods and Materials: A cohort study was performed of 134 colorectal cancer patients followed from diagnosis to 6 months post-treatment in Central Taiwan. HRQoL and symptoms were assessed at diagnosis and one, three, and six months thereafter. The Functional Assessment of Cancer Therapy-Colon (FACT-C) questionnaire, VAS pain, and the Memorial Symptom Assessment Scale (MSAS) were used for data collection. A generalized estimating equation (GEE) was applied for statistical analysis. Results: The majority of the patients were male (55%) and married (91.5%). The mean age was 60.4 years (SD = 11.71). Most were diagnosed stage III and IV colorectal cancer (54.5%). All underwent surgery; some also received chemotherapy (CT) or concurrent chemoradiation therapy (CCRT). The results of the GEE showed that overall, the HRQoL, pain, and symptoms of the patients significantly improved over the treatment period. Patients with stage IV disease who had received surgery and CCRT showed the worst HRQoL. Females, patients with comorbidity, and stage IV patients had higher pain scores over time. Female and stage IV patients had more severe physical symptoms, whereas stage II and IV patients had worse psychological symptoms over time. Conclusion: The patients' HRQoL, pain, and symptoms significantly improved over the 6-month treatment period. Certain patient and clinical variables accounted for changes in treatment outcomes regarding HRQoL and symptom burden in colorectal cancer patients.

국내 운동 관련 메타분석 논문의 질 평가 (Review of Meta-analysis Research on Exercise in South Korea)

  • Purpose: The purpose of this study was to evaluate the quality of meta-analysis regarding exercise using Assessment of Multiple Systematic Reviews (AMSTAR) as well as to compare effect size according to outcomes. Methods: Electronic databases including the Korean Studies Information Service System (KISS), the National Assembly Library and the DBpia, HAKJISAand RISS4U for the dates 1990 to January 2014 were searched for 'meta-analysis' and 'exercise' in the fields of medical, nursing, physical therapy and physical exercise in Korea. AMSTAR was scored for quality assessment of the 33 articles included in the study. Data were analyzed using descriptive statistics, t-test, ANOVA and ${\chi}^2$-test. Results: The mean score for AMSTAR evaluations was 4.18 (SD=1.78) and about 67% were classified at the low-quality level and 30% at the moderate-quality level. The scores of quality were statistically different by field of research, number of participants, number of databases, financial support and approval by IRB. The effect size that presented in individual studies were different by type of exercise in the applied intervention. Conclusion: This critical appraisal of meta-analysis published in various field that focused on exercise indicates that a guideline such as the PRISMA checklist should be strongly recommended for optimum reporting of meta-analysis across research fields.

수용개작방법을 활용한 당뇨인의 발관리 실무지침 개발 (Development of Clinical Practice Guideline by Adaptation: Diabetic Foot Care)

  • Purpose: This study was done to use a guideline adaptation process to develop a Korean evidence-based diabetic foot care clinical practice guideline for diabetic foot prevention and management. Methods: The guideline adaptation process was conducted according to the guideline adaptation manual developed by the National Evidence-Based Healthcare Collaborating Agency. The process consists of three main phases, with 9 modules including a total of 23 steps. Results: The newly developed diabetic foot care clinical practice guideline consisted of an introduction, description of diabetic foot, summary of recommendations, recommendations, references, appendices, and glossary. There were 165 recommendations in 4 sections (risk assessment for diabetic foot ulcers, prevention of diabetic foot ulcers, wound assessment of diabetic foot ulcers, and management of the diabetic foot). In grading by recommendations, for A there were 30 (18.2%), B, 8 (4.8%), C, 30 (18.2%) D, 97 (58.8%). Conclusion: This guideline can be used as educational material for healthcare workers and diabetic patients. It can also be utilized as a practice guideline for healthcare workers in the hospital and community setting.

텍스트네트워크분석을 적용한 통증관리 간호연구의 지식구조 (Identification of Knowledge Structure of Pain Management Nursing Research Applying Text Network Analysis)

  • Purpose: This study aimed to explore and compare the knowledge structure of pain management nursing research, between Korea and other countries, applying a text network analysis. Methods: 321 Korean and 6,685 international study abstracts of pain management, published from 2004 to 2017, were collected. Keywords and meaningful morphemes from the abstracts were analyzed and refined, and their co-occurrence matrix was generated. Two networks of 140 and 424 keywords, respectively, of domestic and international studies were analyzed using NetMiner 4.3 software for degree centrality, closeness centrality, betweenness centrality, and eigenvector community analysis. Results: In both Korean and international studies, the most important, core-keywords were "pain," "patient," "pain management," "registered nurses," "care," "cancer," "need," "analgesia," "assessment," and "surgery." While some keywords like "education," "knowledge," and "patient-controlled analgesia" found to be important in Korean studies; "treatment," "hospice palliative care," and "children" were critical keywords in international studies. Three common sub-topic groups found in Korean and international studies were "pain and accompanying symptoms," "target groups of pain management," and "RNs' performance of pain management." It is only in recent years (2016~17), that keywords such as "performance," "attitude," "depression," and "sleep" have become more important in Korean studies than, while keywords such as "assessment," "intervention," "analgesia," and "chronic pain" have become important in international studies. Conclusion: It is suggested that Korean pain-management researchers should expand their concerns to children and adolescents, the elderly, patients with chronic pain, patients in diverse healthcare settings, and patients' use of opioid analgesia. Moreover, researchers need to approach pain-management with a quality of life perspective rather than a mere focus on individual symptoms.
