Related Factors of Quality of Life in Male Patients with Chronic Obstructive Pulmonary Disease (남성 만성폐쇄성폐질환자의 삶의 질 관련요인)

  • Purpose: The purpose of the study was to examine the related factors of quality of life (QOL) among patients with Chronic Obstructive Pulmonary Disease (COPD). Methods: Patients diagnosed with COPD (N=230) were recruited from four hospitals in Kyeong-Nam province, from March 2 to November 30, 2010. The data collection instruments were the Short Form 36, perceived dyspnea measure by Modified Medical Research Council, COPD and Asthma Sleep Impact Scale, COPD Self-efficacy Scale, and Center for Epidemiologic Studies Depression Scale were used. Following the completion of the data collection instruments Pulmonary function was tested. Data were analyzed with descriptive statistics, Pearson correlation and simultaneous multiple regression using SPSS/WIN. Results: The mean QOL of this study was 68.24. Using a multivariate approach, the significant correlates of QOL were depression (${\beta}$=-.37), dyspnea (${\beta}$=-.28), self-efficacy (${\beta}$=.20), and a sufficient degree of household income (${\beta}$=.16). These variables explained 49% of variance in QOL. Conclusion: The study suggests that psychological aspects are an important factor in explaining QOL of the patients. Screening and minimizing depression could be effective strategies in enhancing QOL of patients with COPD and further investigation to reduce depression could warrant the improvement of QOL in patients with COPD.

A Grounded Theory-Based Approach to Practice Adaptation Process of Hemodialysis Unit Nurses (혈액투석실 간호사의 실무적응과정에 대한 근거이론적 접근)

  • Purpose: The purpose of this study was to generate a grounded substantive theory for the practice adaptation process of hemodialysis unit nurses. Methods: Participants in this study were 10 nurses working in one of two hemodialysis units. Data were collected through tape recorded in-depth interviews done between December, 2011 and February, 2012. Data were analyzed using grounded theory methodology. Results: From the participants' statements, 43 concepts, 18 subcategories and 10 categories were extracted through the open cording process. The 10 categories were: "Burden", "Role conflict", "Fearful", "Conflict of emotion", "Lack systematic job training", "Lack support system", "Compassion", "Rapport created", "Sense of duty", and "Growth". The core category was discovered to be 'recognized growth'. Phenomenon was identified as 'burnout' and this series of processes was categorized as having three stages: 'conflict', 'acceptance', 'growth'. Conclusion: The results of this study provide useful information about the needs of Hemodialysis Unit Nurses during the practice adaptation process based on their stages and types of practice adaptation. Finally, this study contributes data for the development of intervention programs that support the Hemodialysis Unit Nurses' practice adaptation.

The Effect of Social Stigma on Suicidal Ideation of Male HIV Infected People: Focusing on the Mediating Effect of Hope and Depression (남성 HIV 감염인의 사회적 낙인이 자살생각에 미치는 영향: 희망과 우울의 매개효과)

  • Purpose: The purpose of this study was to identify the mediating effect of hope and depression applied on the influence of social stigma on suicidal ideation of 108 HIV infected males. Methods: This study was a descriptive, crosssectional design that used a survey approach. Data collection was one-on-one interviews by a counseling nurse from July 2012 to January 2013. The survey included questions about social stigma, hope, depression, and suicidal ideation. Results: Fitness of the hypothetical model was appropriate ($x^2/df=1.97$, TLI=.97, CFI=.98, RMSEA=.07, SRMR=.04). Social stigma had no direct effect on suicidal ideation but had a significant indirect effect on suicidal ideation via hope and depression. Hope had a mediating effect the relationship between stigma and depression but no direct effect on the relationship between stigma and suicidal ideation. Hope had an indirect effect on suicidal ideation via depression. Stigma and hope accounted for 41% of depression was where as suicidal ideation was explained 56.3% by depression, hope, and stigma. Conclusion: In order to reduce depression and suicidal ideation of HIV infected people, stigma improving strategies are required. And hope intervention for HIV infected people may decrease their suicidal ideation and depression.

Attitude of Infant and Toddler′s Mother towards Massage Intervention (마사지 중재에 대한 영유아 어머니의 태도)

  • The purpose of this study was to understand and analyze the subjective structure of attitude, opinion of infant and todder's mother, regarding the massage intervention in order to yield fundamental data for the activation scheme of the massage intervention. This research was based on a Q-Methodological approach. According to the results of this study, there were four categories of mothers' opinion about the massage intervention. The first type accepts the wholistic effect of massage intervention very positively that it improves infant's health in various aspects: prevention of diseases, promotion of the growth, settlement of sentiments, etc. The second type describes the effect of the intervention in the emotional aspect, such that the intimacy plays a very significant role in the intervention. The third type explains the complementary effect of the intervention that it eases symptoms rather than actually cures diseases. The fourth type emphasizes the healing effect of the intervention that it is required to go through professional training to utilize the effect. For example, a Kyongrak massage is very effective for curing diseases. In consideration of various types of massage intervention, it is very necessary to have a specialty for the massage intervention and develop unique mothers education programs depending upon different applications of the massage intervention in order to reorganize and activate the massage intervention as a nursing intervention for infant and toddlers.

The Present and Future of Children′s Hospice Care in Korea (한국 아동 호스피스의 현황과 전망)

  • Purpose: This study is to identify the present situation of children's hospice and to find the developing strategies for child hospice system in Korea. Method: The data was collected from both literatures and the recent data provided by the government. The direction of future of children's hospice cared in Korea was predicted based on the literature analysis and the report and policy of government. Result: In Korea, the system of the child hospice is not processing. There are the importance differences between children and adult in that the characteristics and approach of the hospice care. All medical personnel and the people related to hospice care including children and their family should be recognized the necessity of the children's hospice care. The following strategies is needed for setting up the child hospice :the principles and standards, recognizing of the necessities, developing of educational program for the specialist and the systemic children's hospice program, and the organization of child hospice. Conclusion: Directions suggested from this study have the importance of child hospice to establish and develop well in Korea for both children with life-threatening disease and their families.

Childhood Fever Management: Current Practice vs Evidence (아동의 발열관리: 현황 및 과학적 근거)

  • Purpose: The purposes of this review were to address misconceptions of childhood fever and fever management practice among parents and health care providers, and to identify the scientific evidences against such misconceptions and practices. Methods: Journal databases and clinical guidelines from 2000 to 2015 were searched. The search terms were fever, fever management, misconception, myth, fiction, fact, fever phobia, child, antipyretics, tepid bath, alternating use/combined use of antipyretics, and physical cooling method. Results: There are significant gaps between current concepts and practices, and the scientific evidence. Misconceptions and unrealistic concerns about childhood fever still exist among parents and even health care providers, worldwide. The evidences suggest that antipyretics should be given carefully with the aim of relieving discomfort or pain rather than decreasing the temperature itself. Alternating use of antipyretics should be discouraged due to the risk of confusion and error. Antipyretics do not prevent febrile convulsions. Moreover, the scientific evidence does not support tepid sponge massage. Conclusion: Evidence-based childhood fever management interventions should be targeted toward parents and health care providers. By adopting an evidence-based approach to nursing interventions, pediatric nurses can ensure children receive appropriate and safe fever management.

Critical Discussion on Smoking During Pregnancy as a Form of Fetal Abuse: An Approach to Advocate for Fetal Right to Life (임신 중 흡연에 의한 태아학대: 태아생존권 옹호를 위한 접근)

  • Purpose: Smoking during pregnancy contributes to the risk of negative health outcomes in mothers and babies. The purposes of this study were to review the harmful effects of maternal smoking during pregnancy on fetal and child development, to discuss if maternal smoking should be criminalized as a form of child abuse, and to explore advocating for fetal rights. Methods: A variety of published literature and legal documents including the Korean constitution, criminal laws, and children's welfare laws were reviewed and critically analyzed. Results: Women who smoke during pregnancy are more likely to experience abortion related to placental dysfunction. Their unborn risk premature birth, fetal growth restriction, low birth weight, neurobehavioral disturbances, and/or other complications and newborn babies are also at risk for complications. The advocates for fetal rights can assert that maternal smoking should be regarded as a crime. Conclusion: Findings show that maternal smoking during pregnancy is a major risk factor for many adverse pregnancy outcomes. Effective strategies and health policies for smoking cessation during pregnancy are required to protect pregnant women and their babies.

A Study on the Job Satisfaction of the Physician Assistant (PA) Male Nurses: A Mixed-method Design (남자 전담간호사(Physician Assistant)의 직무만족: 혼합적 연구방법 적용)

  • Purpose: This is a mixed method study to understand the experience regarding the job satisfaction of Physician Assistant (PA) male nurses. Methods: First, factors influencing job satisfaction were analyzed using quantitative approach and job adaptation processes by qualitative research was done through interviews. Finally both of the collected data were comprehensively compared. Results: The factors which had the greatest influence on job satisfaction in the regression analysis were job stress (${\beta}=-.49$, p<.001) and job identity (${\beta}=.46$, p<.001) and the explanatory power explaining these variables was 58% (Adj. $R^2=.58$). The results of qualitative research were categorized using the same four variables with qualitative research variables. Five themes in job satisfaction, three themes in job identity, five themes in job stress and six themes in job value were identified by content analysis. Conclusion: This finding identified that the job stress and job identity were important variables related to job satisfaction. Therefore, the factors influencing job identity have to be strengthened, and a systematic management and interest to develop them are necessary.

The Effects of Child Care Helpers' Emotional Labor and Job Stress on Their Burnout (아이돌보미의 감정노동과 직무 스트레스가 소진에 미치는 영향)

  • Purpose: This study aims to understand baby care helpers' degree of emotional labor, job stress, and burnout and analyze the effects of emotional labor and job stress on their burnout. Methods: Data were collected from August 16 to September 6, 2016 for three weeks with child care helpers working in C city, D city, and S city as subjects. Multiple regression analysis was used in order to examine the factors influencing the subjects' burnout. Results: Burnout in general characteristics differed significantly according to age (F=4.81, p=.011). As the most influential factors for burnout, emotional labor (${\beta}=.43$) had statistically significant correlation with it, along with job stress (${\beta}=.16$) and age (${\beta}=.14$). These variables explained burnout ($Adj.R^2=0.29$, p<.001) at a rate of 29%. Conclusion: It is judged that strategies and continuous management aimed at reducing child care helpers' emotional labor and job stress are necessary. As an approach from different aspects, it is required that variables related to emotional labor, job stress, and burnout are identified and there would be more researches in order to establish some national policies for child care helpers.

ICU Nurses' Work Experience for Attempted Suicide Patient by Drug Ingestion: A Focus Group Study (일 대학병원 중환자실 간호사의 음독자살시도 환자 간호 경험: 포커스 그룹 연구)

  • Purpose: This study to describe experiences of nurses who work in intensive care units (ICUs) where they frequently encounter patients with attempted suicide by drug ingestion. Methods: Data were collected by 2 focus group interviews with 9 ICU nurses. The interview were recorded and transcribed, and data were analyzed using qualitative content analysis. Results: The results of data analysis, five themes were drawn: 'Confusion about for the attempted suicidal patient care', 'Helplessness for dying with attempted suicidal patients', 'Guilty for insufficient care', 'Ambivalence for the attempted suicidal patients', 'Recognition of need for professional approach'. Conclusion: The results of this study help us to understand patients who attempted suicide through the ICU nurses' experience. It would be useful to develop effective education programs for ICU nurses to preventive strategies for patients who are high risk of recurrence of suicidal crisis situation.
