The purpose of this study was to analyse the relations among the residents' characteristics, the policy of the facility, and the physical characteristics of activity areas, and to understand the elements for the lively use of activity areas in the elderly nursing facilities. The results of the study are as follows: First, there was a relationship between the characteristics of semi-public activity areas and the residents' management-control policy. The facilities having semi-public areas of living room type showed more flexibility in time and place and more respects for the residents. Second, the activity areas of connection type were much more used than the compartment type, especially in the facilities having more residents incapable of walking. On the other hand, the facilities having high control for the residents showed high using rate in public activity areas, and all those did not have semi-public areas or showed low using rate in there. Third, in the analysis of the relationship between the physical characteristics and the using rate of activity areas, the public activity areas having compartment type in the spatial composition and hall as a form were likely to have multiple usages. On the whole, the various form of activity areas, the area per residents, openness, accessibility and centrality were having interactions each other for high using rate in activity areas. Especially, many stimulations and the furniture arrangements that were good for the residents' interactions had a function of compensation for the poor physical characteristics of activity areas. This study investigated only the using rates to understand the lively use of the space. For the future study, it is suggested that the detail using behavior of the residents should be examined to understand the influential elements.
The purpose of this study was to provide basic data for the development of educational material in Korean for patient3 being discharge after open heart surgery. The subjects were 45 adult patients who were seen in the out -patient department of the thoracic surgery unit of P University hospital between February 1993 and May 1993. The data were collected through a chart review and interview. Nursing needs, and the patients status related to diet, activity, medication, physical and psychoemotional limitations were analyzed. The results are summarized as follows ; 1. Mitral valve replacement was the most frequent type of surgery(91.2%). In most cases, a mechanical valve was used (91.0%) . The duration of admission after the operation was two to four weeks for most of the patients(48.9%). The follow-up period was between one and two years for 28.9% of the patients and below 6 years for all of the patients. 2. Many Patients didn’t know the importance of a low salt diet(57.8%), and did not get any education on low salt diet (66.7%). Gimchi was included as one of the most commonly ingested foods (77.8%). All of the patients indicated nursing needs related to education about low salt diets and further they indicated a major interest in foods that are harmful (57.7%) or foods that are safe(51.1% ). 3. Most patients did not recognize the need for limitations on physical activity(84.4%). Further, 31.1% of them could not return to work at the time of the study. All patients had nursing needs related to physical activity, with the most frequent questions being about the permissble degree of activity and special cautions dictated by their illness (60.0%). 4. Many patients were ignorant of the necessity of medication(55.6%) . Forty percent of the patients were taking additional drugs, usually herb drugs. The time and duration of medication and the side effects of drugs were common concerns (57.7%) related to nursing. needs about medication and were mentioned by all of the patients. 5. All of the patients complained of physical discomfort including the following : memory disturbance(62.2%), weight gain(60.0%), chest tightness (55.6%), hair loss(51.1%), sleep disturbance(46.7%) and other symptoms in that order of frequency. Nursing needs related to physical condition were a concern for all of them. The viability and function of the replaced valve(53.3%) and weight gain(60.0%) were mal or concerns. 6. Looking at their psychoemotional condition it was found that 36 patients(80.0%) were emotionally unstable. The causes were physical discomfort(17 patients), insufficient knowlege of open heart surgery(6 patients), fear of death(6 patients), familial over protection(2 patients ) and lack of support (5 patients).
The competence of newly graduated nurses is based on various clinical expriences gained when they were students. Therefore, instructors of nursing students, professors in nursing schools or directors in hospitals must play a critical role in assisting them to obtain various knowledge and experienced nursing skills. The purpose of this study was to investigate nursing care activities and nursing care hours practiced by nursing students in a general hospital. The subject students were total 214 nursing students, 2nd graders(sophomores) and 3rd graders(juniors) from 5 Junior Nursing Colleges in Seoul and they practiced at S general hospital to gain clinical experience. The data were collected for 4 days. The tools for this study were the direct nursing care activity list consisted of 15 nursing areas and the indirect nursing care activity list consisted of 9 nursing areas. The subject students were supposed to record their own score. The results of this study are ; 1. The nursing care hours per nursing student 1) The average total nursing care hours a day per each nursing student are 362.65 mins(6.04hr), the direct nursing care hours per each nursing stuent are 202.09 mins(direct nursing care rate 56.0%) and it is higher than the indirect nursing care hours, 159.75mins(indirect nursing care rate 44.0%). The direct nursing care rate of each student by a team approach in the evening shift(56.3%) is higher than that in the day shift(55.8%). 2) The hours of checking vital signs are the longest(47.35mins) among the direct nursing care activites and next is in order of counseling 8l emotional support, nurse rounds, and accompaning a patient during examination. The hours of reporting are the longest(32.39mins) among the indirect nursing care activites, and next is the activities related to education such as reviewing chart, looking up references, etc. 3) The freqency of checking vital signs practiced by nursing student is the highest(the average of 55.7 times) among the direct nursing care activities and next is in order of nurse rounds, assistance of feeding, and counseling & emotional support. The required time for nursing students to accompany their patient during examination is the longest(20.7mins) and next are in order of restriction on patient' activity, orientated by a head nurse, skin care, sitz bath, bathing & hair shampoo, and assisting with patients' exercise. 2. The nursing care hours per grader 1) The average hours of total nursing care per a nursing student are 369.2mins(6.2hrs) to 2nd graders, 355.9mins(5.9 hrs) to third graders. The direct nursing care rate per each nursing student to 3rd graders(59.3%) was higher than that to End graders(52.8%). 2) For 2nd graders, the highly marked nursing activities are teaching associated with direct nursing care activities such as drawing up papers, looking up references, reviewing charts, and being orientated by staff nurses. For 3rd graders, measurments, observations, and nurse rounds in indirect nursing care activities are taken highly 3) The most frequent practice of the nursing care activites is checking vital signs : 65 times to 3rd graders and 46.5 times to 2nd graders. Our suggestions based on the results of this study are : 1. It is recommanded to repeat the same designed study in a variety of clinical fields for further study. 2. It is recommanded to collect data not by self-record method but by observated method. 3. It is needed for nursing instructors in Nursing Schools and in hospitals to develop the guidelines and check-list of clinical practice courses.
Changing concepts of health care, are stimulating the demand for health care, thereby orienting society to health care rights to such an extent that they are deemed as fundamental ones inalienable to man. Concomitantly, qualitative as well as quantative improvement is being sought in the nursing service field. Today, efforts are being made in various areas, especially to qualitatively improve nursing services. A second issue concerns proper staffing. It is important to study staffing, in as much as it continues to be the most persistent and critical problem facing hospital nursing administrators today. It involves quantity, quality, and utilization of nursing personnel. A great deal of attention has been focused on this problem since mid 1930's when nursing services began to be felt as an important segment of hospital operation representing the largest single item of hospital budgets. Traditionally, the determination and allocation of nursing personnel resources has relied heavily on gloval approaches which make use of fixed staff-to-patient ratios. It has long been recognized that these ratios are insensitive to variations between institutions and among individual patients. Therefore, the aim of this thesis is to point to the urgent need for the development of methodology and criteria suited to the reality of Korea. The present research selected one place, the W Christian Hospital, and was conducted over a period 10 days from January, and nurses who were them on duty in their unit. The total num-her of patients surveyed was 1,426 and that of 354. The research represents many variables affecting the direct patient care time using the result from the direct observation method, then using a calculation method to estimate the relationship between the patients care time and selected variables in the hospital setting. The amount of direct patient care time varies with many factors, such 89 the patients age. diagnosis and time in hospital. Differences are also found from hospital, clinic to clinic, ward to ward, and even shift to shift. In this research, the calculation method of estimating the required member of nursing staff is obtained by dividing the time of productive patient care activity(with the time of patient care observed), by the sum of the productive time that each the staff can supply, i.e., 360 minutes, which is obtained by deducting the time for personal activities. The results indicate a substantial difference between the time of productive patient care observed directing and the time of the productive patient care estimated using calculating method. If we know accurately the time of the direct patient care on a shift, there required number of staff members calculated if the proper method can be determinded should be able the time of the direct patient care be estimated by the patient classification system, but this research has shown this system to be in accurate in Korea. There are differences in the recommended time of productive patient care and the required number of nursing staff depending upon which method is used. The calculated result is not very accurate, so more research is needed on the patient classification system.
This study was done in an attempt to reveal those factors interfering with the observation study. Three trained research assistants collected data by observing the two occupational health nurses working in the small scale enterprises on the 3rd and 10th of February, 1999. The first observation was performed on the 3rd of February without observation guidance. Guidance was given during the second observation on the 10th of February. These data were compared between reporters in nursing services of 'Workplace visiting', 'Health counselling' and 'Blood pressure checking' according to nursing time and frequency. These areas were chosen because these activities were regarded as clearly identifiable by observers in contrast to the other performances. The data showed inconsistency between reporters. However, they were reduced when reporters observed nurses with guidance, although there were still minor differences between data in time and frequency. Discussions were carried out to find possible reasons for these differences. The causes were mainly attributed to the difficulty in workplace conditions to observe, the work specificity of nurses visiting each workplace and the delicate situation between nurses and observers when reporters were travelling in the nurse's car. Therefore, the results suggest that the observation method applied to the nursing service research at small-scale enterprises should be more concerned about the selection of appropriate workplace and operational definition of nursing activity in order to more precisely measure nurses' attitudes.
It is an important task to give adequate nutritions to infants and to wean properly for rapid growth. This study was conducted to survey on doing the weaning activities of the first-time mother. This study was to provide basic data of health teaching and nursing intervention for the promotion of ideal weaning activities. Survey was done from January to February in 2001. Total 444 mothers who have first-time baby under twelve month old were interviewed in five community health center around contry (Seoul, Choung-ju, Asan, Cheon-an, Jeju). The results are summarized as follows; Starting time of weaning was common in four month old(40.4%). Most common daily weaning times by age showed once(under 3 months), twice(4 to 7 months), three times(8 to 12 months). The younger, the more commercial weaning products(p<0.01). Education level showed relatively positive relation with home-made weaning food(p<0.05). Items showed low scores in 4 to 7 months group were starting new food item, waiting at least one week interval to new food, feeding by milk bottle, allowing grasping spoon, consulting pediatrician for problems after weaning, starting from morning, regular schedule. Items showed low scores in 8 to 12 months group were feeding whole egg rather than yolk only up to 12 months, keeping weaning interval were increased, feeding by milk bottle, trying various cooking method, not giving commerical cooking and instant food and unproper raw milk feeding. In weaning activities by general factors, mother age, health status, marriage duration, monthly income were statistically significant (p<0.05). To improve good weening activities, targetting on low maternal age and poor health status, and low income group, developing teaching materials and training program for items showed low scores and proper weaning time, type of easy home-made food and easy cooking method should be provided. Further study on effect of nursing intervention to improve weaning activities are needed.
The purpose of this study is to clarify the correlation between the degree of patients daily activities and that of a sense and social support of families. This study covers families of 252 patients under home health nursing care at 6 University Hospitals during the period of March to May. 1999. The data were collected by using three different questionnaires. Data were analysed by utilizing SAS program such as frequency. average. percentage. t-test. ANOVA. Scheffe test and Pearson Correlation Coefficient. The results were as follows: 1. The average score of patients' daily living activity was $2.36\pm0.67$. $97.2\%$ patients turned out to be. in part. dependent on their family's care. $66.5\%$ of the patients were still dependent on their families in walking: $66.3\%$ In bathing: and $61.0\%$ in using toilet. 2. The average score burdened family caregivers had felt was 2.25 in 4 full point which can be interpreted as average. Among the six burden dimensions. time was said to be the No. 1 burden-yielding factor. When it comes to the characteristics of patients. there were tendencies for patients with lower incomes residing in communal housing. with cerebrovasculal diseases and with higher score of daily living activity to show a higher burden scores. 3. The average score of social support was 2.49 in 5 point. which represents a average score. Among other items. the opportunity of social integration was 2.66. topping the list whereas a chance of upbringing was starkly low at 2.42. The higher scores of social support were shown in cases where the caregiver was male spouse. total care giving duration exceeded 25 months and malignant patients daily living activity scores were low. 4. The score of patients daily activity was positively correlated to the degree of the family burden (r=0.1942). Data indicates there was direct correlation between daily living activity and burden of time consuming, self-improving and physical impairment. 5. The score of the patients daily living was negatively correlated to the social support (r=0.3414), As a whole. there was negative correlation between social support and the formation of intimacy and self-confidence.
The purpose of this study is to analyze physical characteristics, spatial composition, and using behavior of lounges in elderly skilled nursing facilities as spaces for activity programs and also to evaluate the spaces with the framework based on the indices for performing activity programs in elderly facilities. The results of the study were as follows: First, the lounge of an independent type ensured the privacy to perform programs, and so activities were managed more systematically. On the other hand, an expanded corridor type made the elderly feel difficult to pay attention to activities due to co-use of lounges and corridors. In a lounge of a hall type, the elderly had easy access to the place, but it also had weak home-like atmosphere because the space was used as a lobby entrance. Second, the facilities with western types of tables in whole lounges showed big changes in spatial composition, such as moving all the tables for any activities. It resulted in more preparation time and created obstacles in passages and space use. Third, in the evaluation of lounges based on the framework, most of lounges had accessibility of good quality, but they needed to improve home-like atmosphere and flexibility. To create home-like atmosphere, various spatial compositions and classifications in the lounge should be tried. Moreover, enough space and easy movable furniture can be considered for flexible spatial compositions.
Objectives: This study aimed to examine the association between sitting time and BMI-defined low weight and overweight/obesity in Korean adolescents. Methods: Data were from Adolescent Health Behavior Online Survey (KYRBS) in 2015 including 63,345 adolescents aged 12-18 years. The analysis investigated low-weight and overweight/obesity odds ratio(OR) according to study sitting time and leisure sitting time adjusted for sex, school grade, social-economic status(SES), physical activity and sleeping time. Results: After adjusting for sex, school grade, SES, physical activity and sleeping time, study sitting time and leisure sitting time were not significantly associated with low-weight. However, leisure sitting time was positively associated with overweight/obesity. Conclusions: Health program to less sitting time should be established with consideration for differences in risk according to sex, age, SES and active leisure time through the school physical class should be provided to adolescents. Additionally, strategies to promote healthy weight are required to integrate sitting time, physical activity and sleeping time in Korean adolescents.
Purpose: The purpose of this study was to examine the effects of a daily life-based physical activity enhancement program performed by middle-aged women at risk for cardiovascular disease. Methods: This study used a randomized control group pretest-posttest design. Middle-aged women aged 45 to 64 were recruited from two outpatient cardiology departments, and randomly assigned to an experimental group (n=28) and a control group (n=30). For the experimental group, after providing one-on-one counseling and education, we provided customized text messages to motivate them in daily life. To monitor the practice of physical activity, they also used an exercise diary and mobile pedometer for 12 weeks. Subjects' physical activities (MET-min/week) were measured using the International Physical Activity Questionnaire (IPAQ). Their physiological data were obtained by blood tests using a portable analyzer, and the data were analyzed using the SPSS 21.0/WIN program. Results: There were significant differences in exercise self-efficacy, health behavior, IPAQ score, body fat, body muscle, and fasting blood sugar between the two groups. However, there were no significant differences in total cholesterol, hemoglobin A1c, high-density lipoprotein cholesterol, and waist-to-hip ratio. Conclusion: Strengthening physical activity in daily life without being limited by cost burden and time and space constraints. Therefore, it is essential to motivate middle-aged women at risk for cardiovascular disease to practice activities that are easily performed in their daily lives.
