일본 노인전문요양시설 치매노인의 공용공간 이용 특성에 관한 연구 (A Study on the Space Use Pattern of the Elderly with Dementia In Common Space of Nursing Home in Japan)

  • The purpose of this study was to clarify the space use pattern and the daily living activities of the elderly with dementia in the common space (dining room, day corner) of nursing home in Japan. The subjects were six units of the 2 nursing homes in Tokyo. For this, researcher visited from September 5 to December 5 and collected data by observations of behavior of the elderly with dementia in common space and interviews with staff for the information about basic characteristics of the elderly with dementia and administration and operation of the facilities. The data were analyzed using the frequency and percentage with SPSS 11 program and recorded in the form of behavior map. The results revealed as follows: first, 6 types (self-concentration type, physical environment-interest type, passive others-interest type, staffs-interaction type, other elderly-interaction type, visitors-interaction type) of the daily living activities of the elderly with dementia in common space were founded. Second, the daily living activities of the elderly with dementia in common space tended to focus on self-concentration type and physical environment-interest type. Third, physical environmental characteristics of common space affected space usage pattern of the elderly with dementia. And it was perceived like as living oriented space, social interaction space, facility space, staff oriented space. Conclusively it was suggested to plan dining space as semi-private space, day comer as semi-public space.

파킨슨병 환자의 자가간호수행에 영향을 미치는 요인 (Factors Influencing Self Care Performance in Patients with Parkinson's Disease)

  • Purpose: This study aims to find the factors that affect the degree of self-care performance in Parkinson's disease (PD) patients. Method: This study used a descriptive correlational design. The data were collected using a sample of 80 PD patients, from the university affiliated Parkinson Center in Busan. Pearson's correlations and multiple regression analyses were conducted using the SPSS 18.0. Results: The mean score of the self-care performance was 3.61 (${\pm}0.40$). The highest score observed of self-care performance subscale was medication subscale ($4.55{\pm}0.50$) and the lowest score was observed in the exercise subscale ($3.03{\pm}0.64$). The self-care performance had significant correlations with depression (r=-.32, p<.01), self-care knowledge (r=.28, p<.05), function of motion (r=.25, p<.05), ability of daily activity (r=.22, p<.05), self-efficacy (r=.24, p<.05), and support from medical staff (r=.24, p<.05). The significant predictors of self-care performance included depression (${\beta}=-.28$), status of employment (${\beta}=-.27$), self-care knowledge (${\beta}=-.21$), support from medical staff (${\beta}=-.28$), and educational level (${\beta}=-.28$), accounting for 34% of the variance in the self-care performance. Conclusion: Depression, self-care knowledge, medical support are significant predictors which affect the self-care performance with PD patients.

Increased breast cancer incidence among nurses in a tertiary university hospital in South Korea

  • Background: A series of breast cancer cases were recently reported in a tertiary university hospital in South Korea. Nurses are generally exposed to risk factors for breast cancer such as night shift work, antineoplastic agents, and job strain. However, the epidemiological evidence of excess incidence among nurses remains lacking. This study aims to investigate the excess incidence of breast cancer among nurses in a tertiary university hospital and provide epidemiological evidence of occupational risk factors. Methods: A retrospective cohort was developed using personnel records of female workers in the nursing department who worked from January 2011 to June 2021 in a tertiary university hospital in South Korea. Sick leave records were used to identify cases of breast cancer. The standardized incidence ratio of breast cancer among nurses was compared to the general population. Results: A total of 5,509 nurses were followed up for 30,404 person-years, and 26 breast cancer cases were identified. This study revealed a significantly increased breast cancer incidence among all included nurses, with a standardized incidence ratio of 1.65 (95% confidence interval [CI]: 1.08-2.41), compared to the general population. Workers, who handle antineoplastic agents in their representative department and current and/or former department, had significantly elevated breast cancer standardized incidence ratios of 2.73 (95% CI: 1.008-5.94) and 3.39 (95% CI: 1.46-6.68), respectively. Conclusions: This study provides significant evidence of increased breast cancer risk among nursing staff in a hospital setting, particularly those who handle antineoplastic drugs. Measures that reduce exposure to risk factors should be implemented, especially anticancer drugs, to protect healthcare professionals. Further research at a national level that focuses on healthcare workers is necessary to validate breast cancer incidence and its contributing factors.

간호사 임상경력관리체계 모형 개발 - 상급종합병원 중심 - (Development of Clinical Ladder System Model for Nurses: For Tertiary Care Hospitals)

  • Purpose: The purpose of this study was to develop a Clinical Ladder System (CLS) model for staff nurses working in inpatient units of tertiary care hospitals in Korea. Methods: The study was carried out in 2 steps. First, a nursing competence evaluation scale was developed. Second, evaluation of the nursing competences, qualifications, and professional activities of 230 nurses from five tertiary care hospitals was done by 49 head nurses between Feb. and Dec., 2014. Nurses were selected by head nurses according to their clinical experience and expected behavioral characteristics at each level of the ladder. Results: A nursing competence scale was developed consisting of 5 subcategories (clinical practice, ethical practice, education, leadership, and professional development) and 11 elements, and 5 levels of behavioral indicators for each element. Cronbach's alphas for the entire tool and subcategories were over .853 and stability of the scale was confirmed. There were significant differences in nursing competence according to the 5 levels of the ladder. Conclusion: The findings indicate that the proposed CLS model with a standard score for nursing competence, recommended or obligatory criteria for qualifications and professional activities provides a good tool for developing nurses' competences and retaining excellent nurses in clinical practice.

상급종합병원과 종합병원의 근거기반 임상간호실무지침의 활용도 (Utilization of Evidence-Based Clinical Nursing Practice Guidelines in Tertiary Hospitals and General Hospitals)

  • Purpose: The purpose of this study was to investigate the actual utilization of clinical practice guidelines developed by Hospital Nurses Association. Methods: The subjects were 70 nurses who were in charge of guideline distributions in 70 advanced general hospital and general hospitals with 500 beds or more nationwide. Data were collected between June and August, 2020 by mail (return rate: 88.6%). Data were analyzed using descriptive statistics, t-test, and ANOVA with SPSS/WIN 24.0. Results: Among the clinical practice guidelines developed by Hospital Nurses Association, 72.9~90.1% were placed with book and electronic file in nursing department and 24.3~35.8% were placed with book and electronic file in each nursing unit at hospital. The average number of utilized clinical practice guidelines were 3.96±3.88, and average score of guideline utilization was score 2.85±0.79 which means 'use sometimes'. Conclusion: To improve the distribution and utilization of the clinical practice guidelines, it is necessary to enhance the recognition of values of evidence based nursing practice targeting head of nursing department and to stimulate the distribution and utilization of the clinical practice guidelines using diverse education programs for staff nurses.

일선 간호관리자를 위한 리더십 훈련 프로그램 개발 (A Study on the development of leadership training program for first-line nurse managers)

  • The health care environment becomes more competitive every day. It has fallen to nurse managers - from vice presidents of patient care to nurse managers and their assistants - to recruit and develop a workforce that successfully meets the needs of both patients and the organization. This means employees who demonstrate advanced critical thinking skills, creative problem solving, and sound decision making skills combined with clinical skills and patient advocacy. The environment which nurse managers create and the way they relate to their workforce, are pivotal to organizational viability. Especially leadership of first -line nurse managers contributes to the success of their organizations. First-line nurse managers are deserved to be one of the most administrative supervisors through the middle stratum in a hospital organization as being a manager in the field service if assessed from the overall aspects of hospital, as being an interim managers in the nursing department as well as being a supreme supervisor in a unit in terms of an organizational structure in the hospital. Similarly, as a compete leader, the first-line nurse managers have not only a professional which is qualified to perform a role of appropriate coordination with medical staff and key personnel but also hold an important key position a being responsible for performing his or her given role. The first-line nurse manager is expected to manage human and fiscal resources in ways not required before. While an identified need for well-prepared first-line nurse manager continues to plague the profession, first-line nurse managers often have difficulty providing the leadership required. The need leadership training to function effectively in their positions. But we hardly find a useful leadership training program for first-line nurse managers, therefore the purpose of this study was to developed the leadership training program for them. The steps of leadership program development were below: 1st step, 2 studies were done before develop a leadership program. One was done to ask to first-line nurse managers what they want to learn through leadership training, the other one was to ask the staff nurses what their opinions are for their first-line nurse managers leadership. 2nd step was searching other leadership programs contents. The results of this study were below: The total amount of hours is 24. Leadership training program contents are : Future of nursing profession (210min), understanding basic factor's of leadership and leadership theories(310 min), self understanding as first- line nurse managers(320 min), basic principle and practice of interpersonal relationship(210 min), assertiveness training, conflict management (180min), and group study(210min). This is challenging time to be a leader, especially in nursing. As nurse managers look toward the new millennium, it seems as through the same struggles are ahead that are behind. So nurse managers need to embrace change with a positive attitude. They need to demonstrate risk taking and support it in their staffs. All these things are possible that after they participate the leadership training program.

노인요양시설의 인간 중심 케어 (Person-centered care)에 대한 연구 (A Review of Person-centered Care in Nursing Homes)

  • 본 연구는 미국 장기요양에서 급속히 확산되고 있는 인간 중심 케어의 구체적인 모델 및 이와 관련한 실무에 대한 고찰을 통하여 우리나라의 장기요양제도에 적용 가능한 모델과 구체적인 실무 적용방안을 밝히기 위해 수행된 통합적 문헌고찰 연구이다. 대표적인 인간 중심 케어 모델로는 에덴 대안 모델, 그린 하우스 모델, 웰스프링 모델이 있고 자기결정권, 친밀한 관계, 집 같은 환경, 직원 임파워먼트 향상이라는 공통점이 있다. 인간 중심 케어에 대한 연구 현황을 살펴보기 위해 38개의 연구 논문을 분석하였다. 10개의 모델 적용 효과 연구를 분석한 결과 3가지 모델들은 전반적으로 거주자들의 지루함, 무력감, 우울, 만족감 등 주로 심리사회적인 측면에서 긍정적인 효과를 나타내었다. 인간 중심 케어에 기반한 중재 효과 평가 연구는 28개가 분석되었는데, 자기결정권을 촉진시키는 식사, 목욕, 케어활동 등은 거주자들의 참여, 상호작용, 자아인식, 문제행동 등 정서와 인지적 측면에서 긍정적인 효과를 나타냈고 친밀한 관계는 거주자, 가족, 직원 모두에서 삶의 질이나 선호, 서비스 질 문제에 대한 긍정적인 인식을 나타냈다. 집 같은 환경은 만족감, 활동 빈도에서 긍정적이었으며 직원 임파워먼트, 팀 운영 및 팀워크, 참여적이고 협력적인 관리자 스타일 등은 특히 직원 및 조직에서 업무에 대한 인식과 능력, 이직, 만족감 등의 긍정적인 효과를 나타내었다. 이러한 고찰을 바탕으로 우리나라 장기요양시설에서 인간 중심 케어를 증진시키기 위한 방향에 대해 논의하고 인간 중심 케어를 증진시킬 수 있는 정책과 제도의 변화 방향에 대해 제언하였다.

간호교육을 위한 대학 공간 재구조화 방안 탐색 : A대학교 사용자 인식과 요구를 중심으로 (Exploring College Space Restructuring Plans for Nursing Education: Focusing on the Perceptions and needs of Users at A College)

  • 본 연구는 간호교육 맥락에서 대학교육 공간을 개선하기 위해 사용자의 요구를 파악하고, 이를 바탕으로 개선 방안을 도출하는 것을 목적으로 한다. 이러한 목적을 위하여 서울소재 1개 간호대학교에서 교수자(7명), 학습자(9명), 행정담당자(8명)를 대상으로 집단 인터뷰(대상별 3~5명)를 실시하고, 그 결과를 분석하였다. 인터뷰는 대면 인터뷰로 실시하였으며, 인터뷰에는 약 60~90분가량이 소요되었다. 교수자, 학습자, 행정담당자 별로 인터뷰 결과를 분석하고, 각 집단의 인터뷰 결과를 종합하여 다음과 같은 시사점을 도출하였다. 첫째, 교수·학습의 질을 향상시키기 위해서는 물리적 환경의 개선뿐만 아니라 교수자와 학습자 간의 상호작용의 질도 함께 개선하는 것이 중요하다. 둘째, 학습자들이 자신감을 가지고 다양한 기술을 연습할 수 있도록 잘 설계된 실습 환경을 마련하는 것이 필요하다. 셋째, 학습 효과를 극대화하기 위해 간호학 교육과정과 교육 활동을 반영한 강의실 구성 및 배치가 중요하다. 넷째, 간호대학생들의 스트레스 관리 및 심리적 안정감을 제공하기 위해 생활 및 휴식 공간 측면에서의 공간 재구성이 필요하다. 다섯째, 강의실 배정 기준과 공간 사용 규정 마련에 있어 간호학 교육과정을 반영한 행정적 지원이 필요하다. 마지막으로, 본 연구의 제한점과 후속연구를 위한 제언을 논의하였다.

목회간호사의 업무활동분석 (Analysis of Church based parish nursing activities in Teagu city)

  • The concept of parish nursing began in the late 1960s in the United States when increasing numbers of churches employed registered nurses (RNs) to provide holistic, preventive health care to the members of their congregations. Parish nursing role was developed in 1983 by Lutheran chaplain Granger Westberg, and provides care to a variety of church congregation of various denominations. The parish nurse functions as health educator, counselor, group facilitator, client advocate, and liaison to community resources. Since these activities are complementary to the population-focused practice of community health' CNSs, parish nurses either have a strong public health background or work directly with both baccalaureate-prepared public health nurses and CNSs. In a Midwest community in U.S.A., the Healthy People 2000(1991) objectives are being addressed in health ministries through a coalition between public health nurses and parish nurses. Parish nursing is in the beginning state in Korea and up untill now, there has been no research was conducted on concrete role of korean parish nurses. The main purpose of this study was to identify, classify and analyze activities of parish nurses. The other important objective of this study was to establish an effective approach and direction for parish nursing and provide a database for korean parish nursing model through analysis and' classification of the content of the nursing record which included nursing activities. This study was a descriptive survey research. The parish nurses were working in churches where the demonstration project developed on parish nursing. The study was done on all nursing records which were working in churches where the demonstration project developed on parish nursing. The study was done on all nursing records which were documented by parish nurses in three churches from March, 1995 to February, 1996. Namsan, Taegu Jeei and Nedang presbyterian churches in Taegu and Keimyung nursing college incooperated together for the parish nursing demonstration project. The data analysis procedure was as follows: First, a record analysis tool was developed and second, the data was collected, coded and analyzed, the classification for nursing activities was developed through a literature review, from which the basic analysis tool was produced and cotent validity review was also done. The classification of the activities of parish nurses showed 7 activitity categories. 7 activity categories consisted of visitation nursing, health check-ups, health education, referring, attending staff meetings, attending inservices and seminar, volunteers coordinating. The percentage of activities were as follows: Visitation nursing(A: 51.6%, B: 55%, C: 42.6%) Health check-ups(A: 13.5%, B: 12.1%, C: 22.3%) Health education(A: 13.5%, B: 13.2%, C: 18.2%) Referring(A: 1.4%, B: 4.2%, C: 2.4%) Attending staff meeting(A: 18.8%, B: 13.0%, C: 12.2%) Attending inservices and seminar(A: 1.5%, B: 2.2%, C: 2.1%) Volunteers coordinating(A: 0.3%, B: 0.4%, C: 0.0%) To establish and develope parish nursing delivery network in Korea, parish nurses role, activities and boundaries of practice should be continuously monitored and refined every 2 years. Also, It is needed to develope effective nursing recording system based on the need assessment research data of various congregation members. role, activities and boundaries of practice and arrangement of the working structure, continuing education, cooperation with community resources and structuring and organizing parish nursing delivery network. 경영수지분석을 통한 종합병원의 간호관리료 차등지급제 개선방안 (A Proposal to Improve Nursing Fee Differentiation Policy for General Hospitals Using Profitability-Analysis in the National Health Insurance)

  • Purpose: The purpose of this study was to propose optimal hospitalization fees for nurse staffing levels and to improve the current nursing fee policy. Methods: A break-even analysis was used to evaluate the impact of a nursing fee policy on hospital's financial performance. Variables considered included the number of beds, bed occupancy rate, annual total patient days, hospitalization fees for nurse staffing levels, the initial annual nurses' salary, and the ratio of overhead costs to nursing labor costs. Data were collected as secondary data from annual reports of the Hospital Nursing Association and national health insurance. Results: The hospitalization fees according to nurse staffing levels in general hospitals are required to sustain or decrease in grades 1, 2, 3, 4, and 7, and increase in grades 5 and 6. It is suggested that the range between grade 2 and 3 be sustained at the current level, the range between grade 4 and 5 be widen or merged into one, and the range between grade 6 and 7 be divided into several grades. Conclusion: Readjusting hospitalization fees for nurse staffing level will improve nurse-patient ratio and enhance the quality of nursing care in hospitals. Follow-up studies including tertiary hospitals and small hospitals are recommended.
