The associations between dietary behavior and subjective measurements of serious dental diseases in nursing home staff (일부 병원종사자의 식행동과 주관적 중대 구강병과의 연관성)

  • Objectives : The objective of this study is to determine the associations between dietary behaviour and subjective measurements of dental caries and periodontal disease in a cohort of nursing home staff. Methods : A self-reported survey was carried out in 280 nursing home staff in Jeollabukdo Province, Korea. The collected data were analyzed using SPSS Version 19.0 program. Multiple regression analysis was conducted to examine the effects of dietary behavior and food intake on subjective measurements of the two serious dental diseases. Results : The irregular meal tended to increase dietary imbalance and periodontal diseases in the nursing staff. For example, it had influences on the imbalance of sugar, vegetable, and safood intake. Conclusions : It is important to take regular meal because irregular eating behavior tended to increase dietary imbalance and periodontal diseases in the nursing staff.

Risk Assessment of Physical Hazards in Greek Hospitals Combining Staff's Perception, Experts' Evaluation and Objective Measurements

  • Objectives: The promotion of health and safety (H&S) awareness among hospital staff can be applied through various methods. The aim of this study was to assess the risk level of physical hazards in the hospital sector by combining workers' perception, experts' evaluation and objective measurements. Methods: A cross-sectional study was designed using multiple triangulation. Hospital staff (n = 447) filled in an H&S questionnaire in a general hospital in Athens and an oncology one in Thessaloniki. Experts observed and filled in a checklist on H&S in the various departments of the two hospitals. Lighting, noise and microclimate measurements were performed. Results: The staff's perception of risk was higher than that of the experts in many cases. The measured risk levels were low to medium. In cases of high-risk noise and lighting, staff and experts agreed. Staff's perception of risk was influenced by hospital's department, hospital's service, years of working experience and level of education. Therefore, these factors should be taken into account in future studies aimed at increasing the participation of hospital workers. Conclusion: This study confirmed the usefulness of staff participation in the risk assessment process, despite the tendency for staff to overestimate the risk level of physical hazards. The combination of combining staff perception, experts' evaluation and objective measures in the risk assessment process increases the efficiency of risk management in the hospital environment and the enforcement of relevant legislation.

The Use of Nursing Diagnosis in Practice (간호진단의 임상적용 현황, 문제점 및 개선방안)

  • This study was conducted to investigate the present situation and problems related to the use of nursing diagnosis in practice. The data were obtained from 332 subjects (27 director of nursing service, 302 staff nurses) who worked in university hospitals in Korea from July through August 1988 using a mailed questionnaire. Data were analyzed by frequency, X$^2$ test and t-test. The findings were as follows ; 1, Clinical use of nursing diagnosis by directors of nursing service and staff nurses. 1) The majority of the nursing departments (88.9%) conducted group education on nursing diagnosis during the last 5 years and 81.5% of them kept a record format for nursing diagnosis : 88.9% of them had had prior experience with the nursing diagnosis. 2) Most of nurses (97.0%) had received education on nursing diagnosis. 2. Factors related to the clinical use of nursing diagnosis in nursing service departments and by staff nurses. 1) The one factor related to the use of nursing diagnosis in the nursing service department was the existence of a record. 2) Factors related to the use of nursing diagnosis by the staff nurses were the organization style of the nursing service department, group education during the last 5 years, existence of a record, the attitude of the director of nursing service, and prior experience of the use of the nursing diagnosis as characteristics of nursing service department and educational experience of nursing diagnosis as a character of nurse. 3. Problems with the use of nursing diagnosis. 1) The primary problem was the lack of time and personnel (mean : 3.757) ; the second problem was the lack of knowledge and will to use nursing diagnosis in practice by the staff nurses(mean : 3.546). 2) There was no significant difference in problems expressed by the director of nursing services and the nurses. The majority of nurses who worked in the university hospitals expressed interest in and concern about the use of nursing diagnosis. Most of the nurses had had education about on nursing diagnosis but use in practice was limited. The primary problem was lack of time and manpower. Strategies for improving use of the nursing diagnosis in practice : 1) Strengthening the education about nursing diagnosis and a holistic approach to understanding human beings. 2) Develop protocols for the use of nursing diagnosis. 3) Eliminate the language barrier regarding nursing diagnosis by translation into in Korean. 4) Decentralization of the nursing service to promote accountability by individual nurses for use of nursing diagnosis.

Perception and Practice of Hospital Infection Control of Nursing Staff depending on the Supplementation of Nurses in Long-term Care Hospitals (간호사 확보에 따른 노인요양병원 간호인력의 병원감염관리 인지도 및 실천도)

  • Purpose: The purpose of this study was to describe the perception and practice of hospital infection control of nursing staff in long-term care hospitals by the level of supplementation of nurses. Methods: The participants were 212 nurses and nurse assistants in 13 long-term care hospitals in a metropolitan city and the data were gathered by self-reported questionnaires during August 2011 and analyzed by SPSS/WIN program. Results: The beds per a nurse were 15, and the proportion of nurses among nursing staff in long-term care hospitals was about 33%. In general, the level of infection control in practice was lower than that of perception. The highest perception and practice domain was 'Management of disinfection/contamination', and the lower level domains were 'Personal hygiene' and 'Hand washing' There were statistically significant differences in the hospital infection control of perception and practice depending on age, education, career in long-term care hospital, job position, the quantity of beds, nurse, and nurse assistant, beds per a nurse and proportion of nurses in hospitals. Conclusion: According to these results, the systematic and continual education on hospital infection control of the nursing staff in long-term hospitals should be carried out. In addition, the policy to add more nurses into long-term care hospitals must be implemented.

The Influence of Violence Experience and Emotional Intelligence of Nursing Staff in Long-Term Care Hospitals on the Quality of Nursing Service (요양병원 간호인력의 폭력경험과 감성지능이 간호서비스 질에 미치는 영향)

  • The present descriptive study investigated the influence of violence experienced by nursing staff in long-term care hospitals and their emotional intelligence on the quality of nursing service. The study participants included 167 nursing staff from 9 different long-term care hospitals in G and C Provinces. Data collected from questionnaires were analyzed using SPSS 23.0 software. While slight differences were found among the subtypes of violence experience, it was found that verbal violence was the most common form in violence, experienced by the nursing staff, followed by physical threat and physical violence. A hierarchical regression analysis performed to investigate the degree of influence of violence experience and emotional intelligence on the quality of nursing service found that violence experience did not significantly affect the quality of nursing service when the general characteristics were controlled whereas emotional intelligence had a significant influence on the quality of nursing service. The results of this study show that, although it is commonly believed that violence experience is a major factor compromising the quality of nursing service, emotional intelligence, which reflects one's ability to utilize and control one's emotions, may actually have a more significant impact on the quality of nursing service. Emotional intelligence can be improved through education and training; therefore, it is necessary to explore ways to improve emotional intelligence of nursing staff such as development of various programs.

Influencing Factors on Symptoms of Stress among Hospital Staff Nurses (임상 간호사의 스트레스 증상에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to identify the factors influencing Symptoms of Stress among hospital staff nurses. Method: Data was collected by questionnaires from 249 hospital staff nurses in three General Hospital. The data were analyzed using descriptive statistics, pearson correlation coefficients, and stepwise multiple regression. Results: The score of the symptoms of stress showed a significantly positive correlation with the score of work stress(r=.22, p=.00). The symptoms of stress showed a significantly negative correlation with the score of social support(r=-.28, p=.00), self efficacy(r=-.31, p=.00), and hardiness(r=-.24, p=.00). The most powerful predictor of symptoms of stress was social support and the variance explained was $16\%$. A combination of social support, ways of coping, and work stress account for $32\%$ of the variance in symptoms of stress among hospital staff nurses. Conclusion: This study suggests that social support, ways of coping, self efficacy, hardiness, and work stress are significantly influencing factors on symptoms of stress among hospital staff nurses.

Influence of Authentic Leadership and Coworker Support on the Psychological Well-being of Staff Nurses (수간호사의 진성리더십과 동료지지가 간호사의 심리적 안녕감에 미치는 영향)

  • Purpose: This descriptive study investigated the effects of authentic leadership and coworker support on psychological well-being of staff nurses. Methods: The participants were 166 staff nurse working in university hospital located in D city. Data were collected from July 1 2017 to 31, 2017. Data analysis was performed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and hierarchical regression. Results: The regression model that included demographics and coworker support explained about 40.0% of psychological well-being. Marital status, nurse satisfaction, turnover experience, and coworker support were the predictors of psychological well-being in staff nurses. Conclusion: Coworker support was an important predictor of the psychological well-being of staff nurse. Therefore, strategics to improve coworker support are needed to increase psychological well-being in the clinical field.

Analysis of Nursing Activities and Cost of Nursing Service Based on the ABC System (활동기준원가계산(ABC)을 이용한 간호활동 분석 및 간호서비스 원가분석;일 산부인과 간호단위를 중심으로)

  • The purpose of this study is to examine a possibility of applying the ABC system to analyze the cost of nursing service on one obstetrics and gynecology (OB/GYN) unit in a major medical center. The activities of nurses on an OB/GYN unit were analyzed for 4 days for 96 hours. 2 week days and 2 days on weekend. Total cases included in the study were activities of 12 charge nurses and 12 staff nurses. Activities were categorized into direct and indirect activities in order to calculate the cost of activities. Nursing activities were defined multidimesionally in order to utilize the ABC system. The 60 nursing activities were classified into the direct nursing activities and the indirect nursing activities based on a literature review, interview, and survey results. The direct activities were further categorized into the single nursing activities and the multiple nursing activities. The indirect activities were classified into the indirect nursing service activities and the general management activities. The major findings of this study were as follows : 1. There were differences in activities according to the positions and duties of the nurses. The charge nurses mostly performed the indirect nursing service activities and the general management activities. Almost all of their indirect nursing activities spent on each patient were similar. The staff nurses performed the single nursing activities and the multiple nursing activities. 2. The activities of staff nurses included medication, patient assessment, rounding and organizing the unit. patient education, nursing treatment, admission and discharging of patients. There was no differences in types and amount of time spent between the nurses on day-time duty and evening-time duty, but the patient assessment activity increased during the night-time. 3. The cost of post partum nursing services for women who had a normal vaginal delivery using the ABC system included the cost incurred by direct nursing activities provided by the staff nurses (85.9%), indirect nursing activities incurred by the staff nurses(19.0%) and the nursing activities by the charge nurses and the head nurse(14.1%). The ABC system is a relatively new method of cost analysis. The results of this study can provide the nursing and hospital managers with useful information on cost control. It is suggested that more studies should be done using the ABC system and extend the scope of studies to include value analysis to aid the Activity-Based Management(ABM) and/or the reengineering of hospital process.

Development and Analysis of the Effects of Caregiver Training Program on Aggressive Behavior m Elders with Cognitive Impairment

  • Purpose. The purposes of this study were to 1) describe the type and frequency of aggressive behavior of cognitively impaired nursing home resident, 2) develop a caregiver training program on prevention and management of aggressive behavior, 3) examine the effects of caregiver training program on the incidence of aggressive behavior of cognitively impaired nursing home resident, and 4) examine the effects of caregiver training program on nursing staff's aggressive behavior management skills. Methods. One-group, time series, quasi-experimental design with a pre-test and two post- tests was used. Data were collected from cognitively impaired home residents (N = 32) and nursing staff (N = 36) in a proprietary nursing home using Ryden Aggression Scale I, II, and Aggressive Behavior Management Scale. Data were entered and analyzed by descriptive statistics and repeated measures ANOVA. Results. Incidence of aggressive behavior was high with a mean score of 3.09 (SD = 3.11) at baseline. Caregiver training program was developed based on Progressively Lowered Stress Threshold (PLST) model and gerontological and psychiatric literature. The mean scores of aggressive behavior at baseline, Post I, and II did not differ significantly although the difference approached to the significant level (F = 2.925, p = .066). Nursing staff's aggressive behavior management skills increased at Post I, and at Post II when compared to baseline, and the difference was significant (F=12.736, p=<.00l). Conclusion. Caregiver training program showed potential impact on reduction of aggressive behavior in elders with cognitive impairment and was effective in increasing nursing staff's aggressive behavior management skills.

Development and Effect of a Web-based Child Health Care Program for the Staff at Child Daycare Centers (보육교사를 위한 웹기반 영유아 건강관리 프로그램 개발 및 효과)

  • Purpose: The purpose of the study is to develop a web-based program on child health care, and to identify the effect of the program on knowledge of, attitudes towards child health care, and health care practice in staff of daycare centers. Methods: The program was developed through the processes of needs analysis, contents construction, design, development, and evaluation. After the program was developed, it was revised through feedback from 30 experts. To identify the effect of developed program, onegroup pretest-posttest design study was conducted with 64 staff members from 12 daycare centers in Korea. Results: The program was developed based on users' needs and consisted of five parts: health promotion, disease and symptoms management, oral health, injury and safety, sheets and forms. This study showed that the total score of staff who used the program was significantly higher in terms of knowledge, attitudes, and their health care practice compared with pretest score (p<.05). Conclusion: These results suggest that this Web-based program can contribute to the child health promotion as well as can provide the staff with the insightful child health information. Therefore, it is expected that this program will be applied to staff of other child care settings for children's health.
