사회보장플랫폼과 비대면 돌봄에 관한 고찰 (A Study on Social Security Platform and Non-face-to-face Care)

  • COVID-19가 전 세계를 휩쓸면서 지금까지만 해도 4,500만명 이상의 확진자와 100만 명이 넘는 사망자가 발생했고, 당분간 이런 상황이 계속될 것으로 보인다. 특히 이탈리아나 스페인 등을 비롯한 유럽의 경우에는 감염자의 절반 이상이 요양시설에서 발생했고, 미국에서도 노인요양시설에서 4천여 명이 사망한 것으로 보고되고 있다. 때문에 포스트코로나 이후에 당면하게 될 문제는 집단돌봄에 대한 근본적 해결 요구와 가정 중심 서비스로의 이동에 있게 될 것이다. 특히 ICT 등 4차 산업 기술을 접목한 가정중심돌봄, 즉 초 기술을 기반으로 하는 커뮤니티 케어의 정착과 확장에 관한 논의가 점점 더 활발해질 것으로 보인다. 이로 인하여 4차 산업과 사회보장·사회복지를 어떻게 구체적으로 결합할 것인가라는 과제가 주어진다. 이른바 스마트웰페어시티를 어떻게 만들어 국민의 사회보장과 복지에 기여할 것인가가 바로 그것이라고 할 것이다. 이러한 상황에서 본 논문은 선행연구에서 다루었던 사회보장플랫폼에 관한 내용 중 그 기능과 범위 및 스마트웰페어시티 개념의 확정과 확장가능성을 통해 비대면 돌봄의 개념과 범위 및 내용 등을 고찰하였다. 이는 우리 사회가 지향하고자 하는 사회보장·사회복지영역에서의 스마트시티를 구현함으로써 커뮤니티 케어나 Aging in Place를 실현하고자 하는데 나름의 중요한 의미를 가지는 것이라고 판단된다. 다만 비대면 돌봄서비스의 구현을 위한 정책적, 법·제도적 관점에서의 구체적 방안이 무엇인지에 대해 다루지 못하고 있는 점은 아쉬운 부분이며 향후 이에 대한 연구가 필요하다는 점에서 한계가 있음을 밝혀둔다. 본고에서 다루고 있는 내용들이 스마트웰페어시티 뿐 아니라 행정·실천·법제 등을 비롯한 사회보장·사회복지체제 내지 전달체계 등에서의 방향과 미래상을 제시하고, 궁극적으로는 인간의 삶의 질 향상이라는 목표를 달성하는데 기여할 수 있기를 기대해 본다.

교사의 성 지식.태도.실태 연구 -50세 이상 교사를 중심으로- (Knowledge, Attitude & Practice for Sexuality of Teachers -Based on over 50 year old teachers-)

  • To correctly plant the value of sex to the juvenile, we cannot overemphasize the importance of the sexual education in schools. Therefore, the study for teachers' consciousness and view of sex is also necessary, because they are subjects of education and role-models of sex of their students. Especially, if teachers are over fifty in age, it is more important, because it is obvious, as they are in positions of managers and directors of education, that their view of sexual value and consciousness are influencing the aim of education and the course of education. Hereby this study was conducted to check their sexual consciousness, attitude, and condition and to prepare for the basic data needed for the development of a sexual education program suitable to them. The methods of the survey of this study are applied by modifying or supplementing those of precedented studies. They are used after analyzing reliance of items according to Cronbach's $\alpha$ figure calculating law, and modifying or supplementing items lower in reliance. The questionnaire survey was conducted among 1,000 randomly selected from teachers working in middle and high schools in Korea from June 14th in 2004 to June 30th. The survey analyzed 632 questionnaires suitable to it. The outcomes of the survey of this study are like the following: First, the objects of the survey consist of 64.6% men and 35.4% women. Among them, 41.1% are under fifty and the rest are over fifty. As for the periods of marriage, 15.5% are unmarried, 16.3% are under ten years, 18.0% are between ten and twenty, and 50.2% are over twenty. Second, with reagard to sex, 25.6% admit the necessity of a heterosexual friend. 32.0% say that they are conservative to sex. 54.1% are taught sexual education. 67.1% attribute sexual education to nursing teachers. Third, among those who answer that they enoughly know the concept of sex, seenig by age, 25.0% are under fifty and 58.1% are over fifty. Seening by the periods of marriage, 9.2% are unmarried, 28.2% are under ten, 49.1% are between ten and twenty, and 59.0% are over twenty. Among those who answer that they enoughly know the concepts of sexual harassment and sexual violence, seenig by age, in the turns by above-mentioned order, each 47.7% and 76.3%. Seening by the periods of marriage, each 9.2%, 28.2%, 49.1%, and 59.0%. Fourth, among those who answer that "teachers also need sexual education and sexual counsel," by age, each92.7% and 91.4%. By the periods of marriage, each 89.8%, 95.2%, 89.4%, and 92.4%. As a matter of course, we can infer the necessity of the sexual education and sexual counsel for teachers. Fifth, among those who answer that they are satisfied with their sexuality, by age, each 41.1% and 61.3%. By the period of marriage, each 4.1%, 63.1%, 64.1%, and 61.5%. As for the sexual desire like spiritual intercourse, physical intercourse containing sexuality, by age, each 71.9% and 93.5%. By the periods of marriage, each 54.0%, 81.5%, 90.3%, and 93.0%. The survey shows that those over fifty and having long marriage are feeling the stronger desire towards sexuality. Through the outcomes of this survey, a few suggestions are possible: First, it is necessary to check of the teachers' view of sexual value and to study deep about the sexual tendency of them in twenties, thirties, and forties. It is to make them teach their students on the firm basis, and also to develop suitable sexual education and counsel program.

출생 및 영아사망 신고체계 및 전산정보체계 개발 (Birth and Infant Death Reporting System via Computer Network)

  • Accurate vital statistics are essential for a national health planning and evaluation. Among various vital statistics, birth and death rates, and infant and matemal mortality rates together with the causes of death are the very basic ones for above purposes as well as for the maternal and child health management. These statistics are based on the birth and death reports. It is required by law to report every birth and death within one month after its occurrence. However, in case of a neonatal death occurring prior to the birth report, most of the birth and death are not reported. Thus accurate infant and maternal mortality rates are not available yet in Korea. The main objective of this study is to develop a birth and infant death reporting system via computer network. We designed a new birth report form based on the current form and data from the analysis of medical record forms of 14 hospitals. A new form is basically addition of essential medical information to the current birth report form. Since a revision of the rules and regulations related wtih the birth report is necessary to use a new form, we kept the current from intact to make it acceptable to the government office for a field trial. We also developed computer programs for data input for birth and death reports at a medical faciltiy, data processing for production of maternal and child health indices at a health center, and management of maternal and child health services including immunization and postantal care at health center. The birth certificate and birth report can be printed out at a medical facility. The computer packages were programmed by Borland Delphi 3.0 and can be run under Windows 95 system. We proposed a new birth and death reporting system via computer network after a field trial for data input, transmission, and processing. The medical and demographic data o birth and death at medical facilities will be sent to health centers directly via computer network. The health center will retain the medical data for analysis and forward only the data for birth and death reports required by current regulations to the Dong, Up, or Myun Office. Once the birth or death is reported via computer network to the Dong Office, then the Dong Office will notify the baby's mother of the birth report and request to submit the baby's name by mail. When the baby's name its submitted. the Dong Office will forward the birth reports to the Common Court and Statistics Agency in the same way as the current system, Upon the completion of birth registration of the Common Court, the court will issue the birth certificate to mother which will be used in lieu of the family record. The advantages of proposed birth and death reporting system via computer network ar as follows ; I) The accuracy, timing, and completeness of reporting will be improved and more accurate maternal and child health indices can be obtained, ii) The maternal and child health services of health center will be obtained, iii) Epidemiologic data for pregnancy and birth can be obtained, iv) Manpower for birth and death reporting will be saved.

건강식품에 대한 소비자 인식 연구 (II) -건강식품에 관한 소비자 의식구조- (A Survey on the Consumer Attitude Toward Health Food in Korea (II) -Consumer Perception on Health Foods-)

  • 건강식품 및 원료의 유효성분 및 위해요소 조사분석에 관한 연구의 기초조사로 건강식품에 대한 소비자의 의식조사를 실시하였다. 건강식품에 관한 이용실태 조사와 건강식품에 대한 객관적 지식정도 조사로 구성된 설문지를 이용하여 서울, 경기 지역에 거주하는 방문 및 협조가 가능한 20세 이상의 일반 소비자 1000명을 대상으로 1995년 10월부터 96년 2월에 걸쳐 직접 방문하고 설문지를 배부하여 설명한 후 조사대상자가 직접 기입하게 하는 방법으로 조사하였다. 882부를 회수(회수율 88%)하였으나 이중 불완전한 응답 23부를 제외한 859부(유효회수율 86%)를 SAS프로그램을 이용하여 통계처리하였다. 조사대상자의 사회경제적 여건에 대하여는 단순빈도와 백분율을 구하였고 교차표를 이용하여 결혼 여부, 월수입, 교육정도 등의 사회 경제적 여건과의 통계적 연관성을 파악하였다. 건강식품에 관한 이용실태 조사결과로는 응답자의 58.8%가 건강식품을 사용한 경험이 있었으며 68.2%는 건강식품의 효능을 믿는다고 응답하고 있어 건강식품에 대한 관심도가 매우 큰 것으로 나타났다. 건강식품에 대한 가장 큰 기대효과는 전체적 건강유지에 도움을 주는 것(59.8%)이라고 응답하였으며 가장 큰 부정적 견해로는 과대선전(52.1%)이라고 응답하였다. 건강식품에 대한 정보 선택 경로로는 주로 친지의 권유나 소개(30.6%)로 이용하고 있었으며 우리나라 식품법에서 건강보조식품의 등록관리사실은 63.7%가 모르고 있다고 응답하여 응답자의 과반수 이상이 건강식품의 행정적 관리사실에 무지한 것으로 나타났다. 건강식품의 등록관리 사실을 알고 있는 건강식품의 종류로는 로얄제리(22.7%), 스쿠알렌(16.0%), 정제어유(15.1%), 유산균(10.6%), 알로에(8.8%)순으로 나타났다. 건강식품과 약품(또는 한약)과의 차별인식 조사결과에서는 응답자의 84.2%가 건강식품과 한약이 다르다고 생각하면서도 가장 잘 알고있는 건강식품의 종류와 약품(또는 한약)의 종류를 묻는 두 문항에 모두 인삼(22.7%, 41.7%)을 가장 많이 응답하여 전체적으로 건강식품과 약품의 구별을 제대로 하지 못하는 것으로 나타나 이를 위한 계몽과 교육이 시급함을 알 수 있었다.

한방병원의 침도 시술 동의서의 현황 조사와 델파이 기법을 활용한 표준 시술 동의서 개발 (Current Status of Informed Consent Form for Acupotomy in Korean Medicine Hospitals and Development of a Standard Informed Consent Form Using Delphi Method)

  • Objectives: This study was conducted to develop a standard acupotomy consent form that takes into account the unique characteristics of Korean Medicine. The study was motivated by the increasing importance of patient autonomy and the growing number of legal disputes related to medical malpractice in the clinical field of Korean Medicine. Methods: The analysis phase of the study involved a survey of the current status of acupotomy consent forms in Korean Medicine hospitals nationwide. The items of each form were analyzed based on the contents of the Medical law and the standard contract for medical procedures of the Fair Trade Commission (FTC). In the development and evaluation phase, the items and contents of the acupotomy consent form were evaluated using a 5-point Likert scale and content validity was assessed through two rounds of Delphi surveys. In the improvement phase, the contents of the consent form were revised based on the results of a survey of inpatient and outpatient patients in the Department of Acupuncture and Moxibustion at Pusan National University Korean Medicine Hospital, and real-time online meeting. The final version of the standard acupotomy consent form was completed after undergoing proofreading and corrections by a linguistics expert. Results: Only 30% of Korean Medicine hospitals have implemented acupotomy consent forms. The items of the consent forms did not fully include the items presented in the Medical act and the standard contract for medical procedures of the FTC. To address this issue, two rounds of Delphi surveys and a real-time discussion were conducted with a panel of 12 experts on 27 preliminary items of consent forms. The items and contents that met the criteria for content validity ratio, convergence, and consensus were derived. Based on the derived items and content, a standard acupotomy consent form was developed. Conclusions: The standard consent form for acupotomy is anticipated to ensure patient autonomy and enhance transparency and liability in acupotomy. Furthermore, it is expected to serve as evidence in case of medical disputes related to acupotomy and contribute as a reference document for the development of standard consents forms for various procedures of Korean Medicine. However, the limitations of the study include that the survey of consent forms was limited to only training hospitals of Korean Medicine, and the standard consent form is only applicable to adults in Korea. Future studies are needed to address these limitations.
