This study attempted to analize nurse's experiences about care giver including nurse's recognition of occupational care giver's existence, the helping part of occupational care giver's activities, problems between occupational care giver and nurse, and correlations between reasons of problems and each variable. The subjects of the study are 218 nurses who are working at general hospitals. The study informations gathered by using structured questionnaire which consisted of 38 items. The reliability of the instrument was cronhach's 0.8310. The collected data was analized by spss program for the statistics of percent, t-test, ANOVA, and Pearson's correlation. The study results were as follows : 1. For the nurse's recognition of occupational care giver's existence 70.2% of nurses(153 nurses) who felt occupaitional care giver's existence was helpful. The result showed 2.20 when transfered to score. 2. Occupational care giver's activities which were helpful to nurses were in order of following : simple serving, personal hygiene, companion of talking, room arrangement, and saftey management. 3. Among 10 items of the problems which nurses having experiences with occupational care givers, 4 items were related with nurses directly. 88.4% of the reasons of the problems were due to lack of occupational care giver's eduction. 4. For the relationships of general characteristics and nurse's reason of problems there was the significant difference in the nurse's status and education. There was the significant difference showed in the group of over status of charge nurse and over college education(T=-2.08, P<0.05, T=-2.13, P<0.01) 5. Correlation between nurse's recognition of occupational care giver's existence and the problems between care giver and the nurse revealed weak relationship (r=0.2002, P<0.01). However, correlation between nurse's recognition of occupational care giver's existence and degree of occupational care giver's helping and also relationship of degree of care giver's helping and problems between these two groups were not statistically significant.
Purpose: The purpose of this study was to examine job involvement according to working pattern and daytime sleepiness in hospital nurses. Methods: At 2 hospitals in affiliation of university, after obtaining participant's consent form, data were collected from October to November, 2007. Twohundred fifty nurses participated in the study. Questionnaire consisted of Epworth Sleepiness Scale (ESS), Job Involvement. Collected data was analyzed with SPSS 14.0 program, which was used for frequency, percentage, mean, standard deviation, t-test and Pearson correlation coefficients. Results: Major findings of this study were as follows 1) The nurses for 3 shift work was 172 (68.8%), the nurses for 2 shift work was 3 (1.2%) and the nurses for day fixation was 75 (30.0%). 2) Mean of ESS was 5.94 (3.28), daytime sleepiness was 13.2% and Job involvement had a mean of $21.27.{\pm}4.61.3$) There were significant differences between shift work and day fixed work on ESS (t=4.33, p<.001), job involvement (t=6.54, p<.001). Higher ESS were significantly related to lower job involvement (r=-.185, p=.003). Conclusion: The finding of this study gives useful informations about sleep and work involvement of hospital nurses. It is need to develop systemic management for shift work nurses by hospital, nurse organization, and government.

The purpose of this study is to compare the educational needs and perception of mothers of normal neonates and high risk infants. This research was designed as a descriptive study. Data were collected for two months from April 2002 to March 2002. Subjects were 41 mothers of high risk infants and 60 mothers of normal neonates in one general hospital in Seoul participated in the study. Measurement tools used in this study were the educational needs scale developed by Cho Kyoul Ja et al and the neonatal perceptive inventories scale developed by Broussard. They ask mothers to rate each item on a four point Likert type scale. The collected data were analyzed using SPSS 8.0 program. The variables were listed as frequency, mean, standard deviation, X2 test, t-test, ANOVA. The results were as follows: The educational needs of the mothers of primi pregnancy was higher than the mothers of multi pregnancy. The educational needs on management of diseases such as prevention of infection, symptoms of disease, mental development, attachment promotion, congenital metabolism test, management of convulsion, care of vomiting and fever were higher than general care of infants such as immunization, measurement of temperature, hiccough care, follow up care. The informations must be included in nursing intervention program to reduce the mothers' stress level. In conclusion, in order to promote positive mother infant relationship, nurse need to give information and educate the mothers of high risk infant and normal neonate.

While the socioeonomic status of Koreas has been dramatically increasing in recent years, chronic and geriatric diseases have also been on the rise, bringing about many changes in our health care system. The basic goals of the home health care are to reduce health care costs, to increase the attrition rate in general hospitals, and to care for patients effectively and conveniontly at home. The purpose of this paper is to review and examine the current status of the home health care in Korea throughout the reports, surveys, other informations and education system of home health nurse. We identified the various types of home health care services programs, such as hospital-based home health care operated in public sector(demonstration project) and community-based home health care in health centers or in private sector, that is, Korean Nurse Association. Hospital based home heatlh care model was established as an alternative to traditional in-patiet services. Quality assurance and client satisfaction is an important measure of care received and establishment of payment and reimbursement for home health care services is important in promotng the home health care. We found out a fee-per-visit system composed of three kinds of fees : a basic service fee(16,000 Won), a travel fee(5,000 Won), and per-service fees (variables). Like fees paid for in-patient care, insureds pay 20% and insurers pay 80% of the basic and per-service fee. The travel fee is borne totally by the insured. Home health care continues to be viewed as not only the most preferred way to provide care to clients, but also the most cost effective. Home health care is that component of a continuum of comprehensive health care whereby health services are provided to individuals and families in their places of residence for the purpose of promoting, maintaining, or restoring health, or of maximizing the level of independence, while minimizing illness. Services appropriate to the needs of the individual patient and family should be planned and provided, nursing is to be a force for positive change and enhanced the nursing professionalism. Whatever type of involvement of home health care, it is essential to remember that home health care is highly service-oriented and highly touch health car deilvery system.
The purpose of this study was to investigate the relationships among variables of transtheoretical model for exercise in the elderly. Method: A hypothetical model explaining the stage of change was constructed based on a transtheoretical model. Empirical data for testing the hypothetical model was collected from 246 old adults over 65 years old in a community settings in Seoul, Korea in June and July, 2000. Data were analyzed by descriptive statistics and correlational analysis using pc-SAS program. The Linear Structural Modeling(LISREL) 8.0 program was used to find the best fit model which explain causal relationship of variables. Result: The fitness of modified model to the data was X2= 96.75(df=49, p=.00005), GFI= .98, NFI=.99, AGFI=.95, NNFI=.99. The the predictable variables of stage of change explained 64% of stage of change for exercise Conclusion: Results are consistent with the studies of application of the transtheoretical model, which has been used to understand how people change health behaviors. The findings of this study give useful informations to construct exercise intervention program for the elderly about relationships among variables influencing to the stage of change of exercise.
Studies are needed to determine the standard norms for sleep/activity patterns in new born baby and there have been no established reports of discrepancy of sleep/activity patterns in new born baby among various races. The purpose of this study is to determine whether the NCASA(Nursing Child Assessment Sleep Activity Record) would be suitable to evaluate and to provide the basic resources for a Korean model of sleep/activity patterns during the new born baby, and to provide a basis for nursing intervention for mothers of new born baby. The subjects of this study were 38 normal new born baby who visited the postpartum care center and two general hospitals located in pusan from January 1 to April 28, 2000. They all agreed to participate in this study. The method of data collection was through convenient sampling. Data were collected through questionnaires including demographic data, birth hi story, and general informations concerning the infant, mother and family. The instrument of this study was the NCASA translated by the Korean parent Child Health Academic Association. The collected data were analysed by mean. standard deviation, frequency, and percentage by use of SPSS/PC. The conclusions obtained from this study are summarized as follows: 1. The mean amount of daytime sleep was 10.23 hours. The mean amount of nighttime sleep was 5.53 hours. The mean amount of total daily sleep was 15.77 hours. The mean of the longest sleep period was 4.49 hours. The mean regularity of daytime sleep was 49.69%. The mean regularity of nighttime sleep was 66.98%. The mean regularity of total daily sleep was 55.81%. The mean frequency of nighttime wakenings was 3.09 times. 2. The mean amount of daytime awake periods was 6.12 hours. The mean amount of nighttime awake periods was 2.11 hours. The mean amount of daily total awake periods was 8.23 hours. The mean of the longest awake periods was 3.76 hours. 3. The mean frequency of daytime feeding was 5.71 times. The mean frequency of nighttime feeding was 2.65 times. The mean frequency of total daily feeding was 8.36 times. The mean frequency regularity of feeding was 62.50%. 4. The mean amount of a mother's day time was 16.36 hours. The mean amount of a mother's night time was 7.64 hours. In conclusion, The new born baby slept more during the daytime compare than nighttime and more active during the daytime. On the other hand. although nighttime sleeping the length of the mother was normal, but sleep was interrupted by the infant over 3 times on average. Therefore this research study will contribute to nursing practice and nursing research by its implication through postnatal educational nursing programs at hospital and nursing intervention programs that would help individual caring of early infant mothers at home.
The purpose of this study is to review the Home and Community Based Care Program in the United States and to examine the ways these programs help the rural elderly to maintain independent living in their own home in the community instead of moving into the nursing home. First, the overall welfare policy for the aged and service delivery systems in the United States are reviewed. Second, basic assumptions and programs of HCBC are reviewed. Third, using the informations about the elder support program in Madison, Dane County of Wisconsin as a case, various kinds and contents of specific programs are introduced. Applicability of these programs to rural Korea is discussed.
Benzodiazepines are widely used for a variety of conditions in psychiatric field. In this article, the author reviewed the indications, the effects on anxiety and sleep disorders, the side effects, the drug-drug interactions, and the tapering strategies. Benzodiazepines were relatively safe and useful for the treatment of anxiety and sleep disorders. However, used clinically, benzodiazepines can induce many adverse effects (e. g. over-sedation, cognitive impairments, paradoxical effects, dependence and withdrawal symptoms, and so on). Currently available informations about their effects on the developing fetus is controversial. For this reason, pregnant women and nursing mother should be cautioned against the use of benzodiazepines. Drug-drug interactions have to be considered in combination treatments. For the tapering issues, gradual tapering was important to prevent the withdrawal symptoms. Especially, the tapering schedules have to be individualized for the each long-term benzodiapzepine users.
The purpose of this study was to provide the elderly and their family kowledge about senile dementia and to get them to have positive attitude about senile dementia. The subjects were 204 elderly who lived in the whole country. The data were collected from Sep. to Dec., 1998, using a 36 items questionnaire and analyzed by SAS program for t-test, ANOVA. Scheffe test, Pearson Correlation Coefficients. The results were as follows: 1. The mean score of knowledge about senile dementia was $9.33\pm2.68$(range 0-15), The elderly who was unschooled, lived in Kyung Sang province, didn't have any hobby or interest, didn't access to informations about senile dementia got lower score than the others. 2. The examples of knowledge test items about senile dementia that the subjects above fifty percents answered uncorrectly were 'the patient of senile dementia doesn't die soon', 'senile dementia is uncurable disease', 'the symptoms of senile dementia is apparent in new and strange circumstances', 'the pood and lonely elderly is apt to have senile dementia'. 3. The mean score of attitude about senile dementia was $15.87\pm2.25$(range 0-20) and attitude about senile dementia was relatively positve. Attitude about senile dementia by general characteristic was not significantly different. 4. The examples of attitude test items about senile dementia to which the majority of subjects agreed were 'It is a pity to see the patient suffered from senile dementia', 'It is difficult to take care of senile dementia patient' and so on. 5. The correlation between kowledge and attitude about senile dementia was very low. Therefore this study suggests that it is crucial to educate the elderly and their family to induce positive attitude about senile dementia.

Purpose: Long-term care facilities have a responsibility to provide care service that enables residents to maintain their maximal functional capacity and quality of life. Also their needs must be reflected to the service programs. In oder to provide an adequate service, we should assess the elderly's physical, psychological and social health status and the need. In addition to this, the long-term care facilities must be defined clearly by the type of services. This study would contribute to conduct appropriate services in public long-term care policy for the older population in the future. This study would provide informations of long-term care facilities' services and older persons' needs for long-term care. Method: To achieve this objectives, this paper investigates the types, service programs of long-term care institutes and job descriptions of workers. The subjects were consisted of 150 long-term care institutes. 150 institutes of long-term care facilities were drawn from all over the country by a nonrandom, convenience sampling. The data were analyzed by frequency, percentage, $x^2$-test using SPSS program. The instruments of this study were self-reported questionnaires for long-term care institutes. The data were collected from March 1, 2004 to may 31, 2004. Results: Service programs of long-term care institutes were not enough for residents' demands. The job descriptions among nurse, social worker and physical therapist were not clearly defined. The nurse's main role was medication and checking vital sign(49.7%), that of social worker's was observation and supervising (31.2%). The most significant problems were lacking of diverse service programs for residents. Conclusion: Considering these findings and conclusion, the needs of long-term care services should be provided by individual physical and psychological level. And the professional manpower for elderly should be educated in multi disciplines.
