  • This study examined the evaluation of the information effects of the teaching on knowledge and daily activities of open heart surgery patients between 2 and 6 weeks after discharge. The subject was 29 patients being taught with teaching materials at discharge as experimental group, 20 patients who received no education as control group among the patients who had undergone open heart surgery in S.N.U.H. And research method was non-equivalent control group non-synchronized quasi-experimental design. As the tool of this study, 30 items of knowledge measurement scale which was extracted among the content of teaching materials to evaluate the effect of education and 28 items of which were designed to measure the daily activities of patients with myocardial infarction for the estimation of the degree of observance in daily activities were used. For data analysis, frequency, t-test, Pearson's correlation coefficient and Cronbach's $\alpha$ were used. The result were as follows; 1. Informations given through teaching materials were effective for increasing the knowledge of the patient with open heart surgery. The knowledge of patients increased to the top level (p<0.05) in 2 weeks after discharge. In control group, the knowledge level of patients did not increase after discharge. 2. The knowledge level daily activity of the experimental group was somewat higher than that of the control group, but there was no significant difference. The score .of the experimental group was 69.66 in 6 weeks after discharge much less than the top level score 112. 3. The correlation between knowledge and daily activities was not significant, suggesting the fact that the increase of knowledge did not influence the daily activities significantly. Recommendation was suggested that; 1) Further studies might be .needed with the increasing numbers of the subjects. 2) Daily activities of the patients with open heart surgery should be investigated for long term period until they recovered normal activities.

  • Surveys were carried out from April to December of 2003 to collect the information on hypertension patients for the detailed analysis. Totally, 1,423 patients, who have been registered to the public health center or its branch located in a small city with both urban and countryside characteristics in Kyungsangbuk-Do area, were the subjects of this investigation. The purpose of this research was to examine the health situation of the hypertension patients and to understand the official state of their management program and their own efforts to control the blood pressure, so that we could develop the better program to protect the patients more efficiently and systematically. Eventual purpose was to provide the patients at high risk with the essential informations to prevent the onset of the disease and to promote their health conditions by early diagnosis. Most patients, registered to the public health center, were relatively obedient to the prescription, however, the incidence of being checked up regularly was low. It was turned out that the patients showed a low tendency to practice to maintain the healthy life style such as the proper eating habit or the effort to control the stress. So. it is necessary to develop the strategy and the management program to enhance the self-nursing activities to control the patients' blood pressure at the proper level. In addition, multidisciplinary approaches should to be done for efficient and effective care for hypertension patients. Finally, systematic management of hypertension patients is required to classify the duty and role for caring patients among those health care system.

  • The purpose of this study was to identify the barrier factors of health behaviors of urban and rural elderly and to compare the health behaviors and level of barriers between two groups, and finally to get the basic informations about the adequate nursing strategies to promote the health state of urban and rural elderly. The subjects of this study were 177 over the age of 65, 81 elderly lived in Seoul and 96 elderly lived in rural areas. The instruments for this study were the health behavior scale(14 items) and the barrier scale (118 items) developed by Gu et al(2003). For the data analysis, SPSS PC program was utilized for descriptive statistics, ${\chi}^2$- test, t-test, Pearson correlation. The results of this study were ; 1. The mean score of health behaviors (range 1-4) was 2.69 in urban elderly and 2.33 in rural elderly ; there was significant difference(t=5.03, P=.00). 2. There were significant differences in levels of barriers(range 1-3) between the two groups, such as calcium intake(t=-3.16, P=.00), regular exercise(t=-3.80, P=.00), exercise time(t=-5.54, P=.00), use of stress reduction method(t=-3.45, P=.00), regular check up(t=-3.89, P=.00), vaccination(t=-3.83, P=.00). Higher levels of barriers were found in rural elderly than in urban elderly. 3. Lack of habituation, lack of will power and lack of knowledge in calcium intake; lack of time, lack of habituatuion, lack of family support, lack of will power and lack of environment in exercise; lack of perceived benefit, lack of time, lack of will power and lack of knowledge in use of stress reduction method; lack of time, lack of interest, lack of habituation and lack of will power in disease prevention were significantly higher in rural elderly than in urban elderly. In the conclusion, nursing interventions should be planned based on the social environment of elderly. To promote the health state of elderly, interventions to decrease the barrier levels and to reduce the barrier factors to health behaviors should be implemented.

Nursing research has related stress during hospitalization of adolescent patients to adaptation to a new environment, isolation from friends, limitation due to illness, over protection of parents and communication with member of the medical team. The investigator conducted this study in the hope that an understanding of adolescents responses to hospitalization, their perceptions, the kinds and levels of stress, and the relationships between stressors and individual characteristics would contribute to the improvement of adolescent patient care. The objective of the study was to obtain informations related to the adolescents psychological stress experience during hospitalization, specifically stress from interpersonal relationships and communication, isolation from the family, social or economic problems, illness and from the treatment environment and nursing care. An interview schedule adopted from Holmes and Rahe's Social Readjustment Rating Scale and selected items from Voicer's instrument on stress-producing events was used with 120 adolescent inpatients aged 13 to 18 years three general hospitals in Seoul during Aug. 10, to Sep. 30, 1975. 1. The sample consisted of 66 male and 54 female patients. Sixty-six percent were late adolescents, aged 16 to 18 years: 4% were early adolescents, aged 13 to 15 years. The primary cause for hospitalization was for orthopedic problems (35.8%). More than half of these (54.4%) were due to injury or accident. 2. Stress eclated to illness revealed the highest score (4.97), followed by stress related to treatment environment and nursing care (4.34) , isolation from family and social or economic problems (4.01) and interpersonal relationships and communication (3.96). 3. The perceived indifference of doctors and nurses was a serious cause of stress (mean=4.83). Fellow patients and visitors caused least stress (mean=2.06). 4. Discontinuation of education or unemployment were major stressful events (mean=4.71). Least stressful was isolation from the family (mean=3.47). 5. More than 94% of the respondents expressed fears related to body image (mean=4.97) 6. Within the category of treatment environment and nursing care, items related to restrictions because of treatment, discomfort because of treatment, inadequate explanation from nurses about procedures were rated as severe stress events (mean=4.6). Items related to the ward environment and to having a relative stay with them were seen by the group as less serious events (mean=3.7). 7. Stress related to interpersonal relationships and communication was correlated positively with female patients and those preferring passive activities. (P〈0.05) 8. Stress related to family problems was positively related to female and early adolescent patients (P< 0.05). Stress related to social problems was positively , elated to students and those preferring active pursuits (P< 0.05). 9. There were no correlation between the high stress related to disease and any of the characteristic items. (P> 0.05) 10. Stress related to treatment environment and nursing care was positively related of early adolescent and female and student patients. (P< 0.05) This group of hospitalized adolescents reported high level of stress related to treatment environment and nursing care, due to lack of consideration of normal growth and development and individual characteristics. • This descriptive study was done to detect the possibilities of the development of the advocacy concept in nursing. The subjects consisted of 3 nurses who had agreed to participate to the study, working at nursery room in a general hospital. Data was collected from May 22 to June 13 in 1995 through tape recorded intensive-interview, and written down, then content analyzed qualitatively related to the infant advocacy. 1. Derivation fo the meaning about advocacy concept 1) The situations in need of infant advocacy were 12 categories : malpractice, overcharing, negligence, unnecessary services, and services without consent, and so on. 2) Fourteen categories of advocacy activities percived by nurses were derived from participants' statements. Protesting infant or his /her families against their counterpart, providing informations to families, cooperating with medical staffs for her patient, then calling medical staff not so as to be maleficient to her patient in its rank. 3) The expected result of advocacy activities perceived by nurses was respectively positive to her patient or families, but negative to nurse. 4) The feelings of nurses in the sitution of advocacy were expressed in to concern, comprehension, regret, powerlessness, charity, desire, and so on. 5) Nurses perceived that advocacy activities could be influenced positively by factors related to nurses' qualification and negatively by factors related to doctor's overdo and nurse's underdo. 2. Categorization of the meaning and their relationships In case of antecedental situation in need of infant advocacy, nurse perceives her patients need the advocacy to get a benefit through nurse's information, intervention speaking, building cooperations. The expected factors to influence advocacy activities perceived by nurses, are the power imbalance between medical staffs, the nurse's qualification, and the nurse's feeling from the situation. The above results suggest that the infant care situation will be recommendable field work place for concept development of advocacy with hybride model when it involves infant's families.
