노인요양시설의 간호인력과 비간호인력간 상처관리에 대한 병원중심 가정간호 요구도 비교 (Wound Care Needs of Hospital-based Home Care Nursing at Elderly Care Facilities)

  • Background: The purpose of this study is to investigate the needs for wound care services in the elderly care institutions by analyzing hospital-based home care nursing in select facilities in Korea. Methods: A total of 92 staffs at the elderly care institutions, located throughout the country, completed self-report questionnaires between February 1 and November 31, 2009. SPSS ver. 17.0 was used for data analysis, regarding frequency and chi square. The 5% level of significance was the critical level for acceptance of the study's hypotheses. Results: Results were as follows; group A and group B were rated as 'high' and 'very high', respectively, in care of simple pressure ulcer, care of complicated pressure ulcer, stitch out, care of simple surgical wound, ostomy care, complicated ostomy care, simple dressing and complicated dressing. Conclusion: Political decisions intended to promote visits by hospital- based home care nurses to the elderly care institutions would be a prudent course.

대학병원 간호사의 노인에 대한 공감능력, 간호근무환경 및 인간중심돌봄이 노인간호실천에 미치는 영향 (Effects of Empathy for the Elderly, the Nursing Work Environment, and Person-centered Care on Geriatric Nursing Practice Among University Hospital Nurses)

  • Purpose: We investigated the association between empathy for the elderly, the nursing work environment, person-centered care, and geriatric nursing practice among university hospital nurses and factors that affect geriatric nursing practice. Method: We administered questionnaires to 178 nurses between May 23 and June 21, 2022 at a university hospital in city D. We used descriptive statistics, the t-test, one-way analysis of variance, the scheffé test, Pearson correlation coefficients, and stepwise multiple regression analysis for data analysis. Results: We observed a statistically significant positive correlation between nurses' geriatric nursing practice and empathy for the elderly, the nursing work environment, and person-centered care. Empathy for the elderly was positively correlated with the nursing work environment and person-centered care, and the nursing work environment showed a statistically significant positive correlation with person-centered care. Empathy for the elderly and the nursing work environment significantly affected geriatric nursing practice. The overall explanatory power of the regression model was 33.4%. Conclusion: Greater awareness and implementation of various educational programs that promote empathy for the elderly are necessary to improve nursing care for the elderly population. Furthermore, adequate manpower and material support are important to improve the nursing work environment among nurses employed at university hospitals.

노인요양시설 내 의료서비스 발생빈도와 병원중심 가정간호 요구도 조사 (Incidence of Medical Services and Needs for Hospital-based Home Care Nursing in Elder Care Institutions)

  • Purpose: This study aimed to clarify the needs for hospital-based home care nursing medical services in elder care institutions by analyzing the details and frequency of medical services provided by, and the needs for, hospital based home care nursing in select institutions in Korea. Methods: Seventy-seven staffs at elderly care institutions located throughout the country completed self-report questionnaires between February 1 and May 31, 2009. SPSS ver. 14.0 was used for data analysis regarding frequency and percentage, mean and standard deviation. Results: Forty-eight hospital-based home care nursing medical services in eight domains were identified as being needed in elderly care institutions. The most commonly used medical services were providing instruction in oral drug administration, checking drug beneficial/adverse effects, and administering blood glucose test, while the most needed medical services requiring hospital based home care nursing were complex pressure ulcer care, followed by diabetic foot ulcer management and nutrient injection. Conclusion: The present results should provide fundamental data for better healthcare services with hospital based home care nursing at elderly care institutions as part of a 'win-win' strategy through which medical expenses are reduced, insurance costs are kept stable, and safe and high-quality medical services are provided for residents of elder care institutions. Political decisions intended to promote visits by hospital based home care nurses to elder care institutions would be a prudent course.

병원중심가정간호를 받는 만성질환 노인의 삶의 경험에 관한 내러티브 탐구 (A Narrative Inquiry of Experience of the Elderly's Living with Chronic Disease in Hospital-based Home Care Services)

  • Purpose: The purpose of this study is to document the experiences of elderly persons with chronic diseases who are under hospital-based home care services and to find out how they describe the experiences using a narrative inquiry method. Methods: The participants are 7 elderly patients over the age of 70. Data collection and analysis were conducted through Jeong Gwang Soon's six-stages: observing phenomena, selecting participants, talking, talking again, writing, and confirming. Results: The results of data analysis were classified into 4 themes of overall experience related to chronic diseases: response to home health care, overcoming chronic disease, life reconstruction, and 22 categories as common denominators extracted from the elderly persons' lives. Conclusion: This research makes the experiences of the elderly living with chronic diseases more understandable, and it can be utilized as a effective nursing praxis to improve the quality of elderly persons with chronic diseases in hospital-based home care services.

  • According to the rapid increase of the elderly population, especially frail older population, many kinds of elderly care facilities have been supplied within a relatively short period. Among them, elderly hospitals and elderly welfare facilities have occupied a major portion. The elderly hospital, which had emerged from 1994, has recorded sharp increase in facility numbers and bed numbers by the support of Korean government together with the increase of care demand. However, the concept and fundamental planning criteria of elderly hospitals have not yet been set up. This paper has derived the concept of the elderly hospital from the Medical Law and Elderly Welfare Law, and prospected the supply of it from domestic and international statistics. Also this paper has explored the fundamental design issues of elderly hospitals by analysing precedent studies and designs, and by surveying some facilities.

노인환자의 수술 후 섬망에 대한 회복실 간호사의 간호수행과 영향요인 (Factors Influencing Performance of Delirium Care for Postoperative Delirium of Elderly Patients among Recovery Room Nurses)

  • Purpose: The purpose of this study was to identify factors associated with performance of nursing care for postoperative delirium of elderly patients among recovery room nurses. Methods: The research was a cross-sectional, descriptive design using questionnaires. The participants were 99 nurses from five urban recovery rooms. Data were analyzed using descriptive statistics, t-test, ANOVA and multiple regression with SPSS WIN 21.0. Results: The performance level of delirium nursing care was low because its mean score was 3.80 out of 5. Performance of nursing care was significantly positively correlated with the importance of nursing care and self-efficacy. Stepwise multiple regression analysis for performance of nursing care revealed that the most powerful predictor was the importance of nursing care. The importance of nursing care and self-efficacy explained 32.3% of the variance. Conclusion: The results indicate a need to enhance the performance of nursing care for postoperative delirium of elderly patients among recovery room nurses. The findings also suggest that consideration be given to strategies for improving the importance of nursing care and self-efficacy in developing programs to enhance the performance level of nursing care for elderly patients with postoperative delirium.

노인대상 의료기관 가정간호사업의 운영실태 (An Analysis of the Elderly Care and Management in Hospital-Based Home Care Agencies)

  • Purpose: To analyze the home care services provided to the elderly aged 65 and older by a hospital-based home care agencies and to investigate the effects of long-term care insurance for the elderly. Method: The subjects were the home care service recipients aged 65 and older in 172 hospital-based, home care agencies registered in Health Insurance Review & Assessment Service in January, 2007. The data were collected using a questionnaire from March 16 to April 15, 2007. The questionnaire return rate was 43.8%. Result: The hospital-based home care agencies were able to visit 66.5% of the national administrative districts. Of the home care service recipients, over 50% were 65 years old and older. About 43% of the agencies reported that over 50% of their patients would be subject to the long-term care insurance. They expressed concern that home care services would be withdrawn once the insurance system is initiated. Conclusion: This study suggests that hospital-based home care agencies need to manage home care services with long-term care insurance. It also recommends developing guidelines for the use of services and referrals.

장기요양시설 노인에 대한 간호서비스 제공시간 분석 (Care Time of Elderly in Long-Term Care Facilities)

  • Purpose: This study was to define the care time of elderly in long-term care facilities and to compare and analysis the care time by staff and facility types. Method: The data were collected from 530 elderly over sixty, residing in two long-term care hospitals for dementia, three long-term care hospitals for the elderly and two skilled nursing facilities. Care time for individual residents was measured the total time spent by nurses, aides. Result: The average care time measured by nursing staff was 158.6 minutes a day. The average care time for one resident by nurse was 40.4 minutes, and by aides, 118.2 minutes. The difference of the care time showed statistical significance between long-term care facility types(p<0.001) : average care time for dementia hospital(199.1min) was twice as long as that of skilled nursing facility(94.1min). Conclusion: The results of the study showed that the care time differentiates care time provided to elderly in long-term care facilities in Korea. The study suggests the need to emphasize the importance of standardization of level of staff and service programs by the long-term facilities.

노인 심부전 환자의 건강 문해력에 따른 심부전 지식과 자가 간호 (Relationships between Health Literacy, Knowledge and Self-Care Behaviors in Elderly Patients with Heart Failure)

  • Purpose: This study was to investigate the relationships between among health literacy, knowledge and self-care behavior in elderly patients with heart failure(HF). Methods: 166 patients (age ${\geq}65$ years) with HF were recruited in a cardiovascular center of an university affiliated hospital. The structured questionnaire included Health literacy, Dutch Heart Failure Knowledge Scale, European Heart Failure Self-care Behavior, Self-Care of Heart Failure Index. Data were analyzed by descriptive statistics, Chi-test, Pearson correlation analysis, t-test and ANCOVA using SPSS/WIN version 21.0. Results: Participants with the high level of health literacy were more likely to be younger (p=.001), men (p=.001), with more education (p<.001), and have a job (p=.004), and with a higher economic status (p=.005). The positive correlations between the level of health literacy, knowledge, and self-care behavior were confirmed (p<.001). Participants with the high level of health literacy showed higher level of knowledge, more self-care behavior for health maintenance, and confidence. Conclusion: This study shows that the level of health literacy may influence knowledge and self-care behavior in elderly patients with HF. In order to improve self-care behaviors in elderly patients, a strategic nursing approach based on the level of patients' health literacy needs to be considered.

피부관찰기록지 이용여부에 따른 요양병원 간호 인력의 욕창간호 지식과 수행도 및 욕창발생률 (Knowledge, Performance, and Incidence Rate of Pressure Injury Using Skin Observation Records in Long-Term Care Hospitals)

  • Purpose: To demonstrate the importance of comprehensive skin observation as an effective intervention for pressure injury prevention in elderly long-term care hospital patients. Methods: The survey was conducted with 70 nursing staff members working at two long-term care hospitals with 200 beds or less in D city. Data were collected from October 16 to October 23, 2019 and analyzed using descriptive statistics, the chi-square test, and the independent t-test with the SPSS 25.0 program. Results: Nursing care knowledge for pressure injury was similar between the two groups. Conversely, nursing care performance for pressure injury degree of the nursing staff in the hospital using skin observation records performed better than those who did not (t=6.11, p<.001). Furthermore, comprehensive skin assessments in long-term care hospitals using skin observation records showed a lower incidence rate of pressure injury than that showed using general skin assessments (t=-5.28, p=.006). Conclusions: Comprehensive skin assessment is important for pressure injury prevention in elderly long-term care hospital patients. To implement this effectively, it is necessary to devise institutional regulations, guidelines, and systematic education programs.
