A Study on the Application Situation and Improvement Plan for the Universal Design of Nursing Home (노인요양시설의 유니버설디자인 적용현황 및 개선방안에 관한 연구)

  • Recently, the service for nursing home has been transforming in an overall way, and along with that, the supply of the recuperation facilities for the elderly has been expanding. However, the quantitative expansion of those facilities has not been accompanied by the qualitative supply. In consideration of those shortcomings of the existing facilities, it may be necessary to ensure an improvement in qualitative terms. This study investigated and analyzed the Nursing home, examined the current situation, reconsidered the state of recuperation facilities for the elderly as part of effort to prepare for the silver society, and present measures for improvement in an attempt not to repeat mistakes that have been observed in the existing facilities in planning Nursing home that will be open and built in the upcoming days, which aimed to provide basic data that can be used in the future and explore the direction of relevant design.

Nurses' Experience of Practice in Nursing Homes (노인요양시설 간호사의 실무 경험)

  • Purpose: This article aimed to identify nurses' experience of practice in nursing homes. Method: Using phenomenological approach for qualitative research, the participants were 8 nurses who were working at nursing homse in C city. The data was analyzed by using phenomenological methodology by Colaizzi. Results: Five theme clusters were derived from the data. They were: burden (independent decision making in clinical situations, tension of as a multifunctional member); lowered self-esteem (exhaustion, relative self-evaluation is low, comparing with others); conflict (negative image of elderly, communication break with elderly family, monitoring of nursing home staff); worth (slight improvement of elderly health state, trust and recognition, intimacy); and self-reflection (need for knowledge extension, effort towards enhancing virtue, perception of importance in their health management). Conclusion: The results showed that nursing home nurses experienced negative aspects of various nursing practice, suggesting the ways to solve their negative experience.

The Effects of Foot Reflexology on Sleep, Depression and Skin Temperature of the Female Elderly at Home (발반사 마사지가 재가 여성노인의 수면, 우울 및 피부온도에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of foot reflexology on sleep, depression and skin temperature of the female elderly at home. Methods: This research used the non-equivalent control group pretest-posttest design. The participants were 45 elders residing at home: 23 in the experimental group and 22 in the control group. The experimental group received 30-minute foot reflexology 8 times for 2 weeks. The results were analyzed using ${\chi}^2$ test, t-test with the SPSS/WIN 12.0 program. Results: The results showed that foot reflexology was significantly effective in improving the quality of sleep, reducing depression, and raising the temperature of both feet. However, the temperature of both palms was not statistically significantly different between the two groups. Conclusion: The results of this study indicated that foot reflexology is an effective nursing intervention in improving the quality of sleep, reducing depression, and raising the temperature of both feet. Therefore, it is recommended to use foot reflexology as a complementary nursing intervention for elderly women at home in community.

Oral health knowledge among nursing home employees and needs to educate on elderly oral care (일부지역 노인요양기관 종사자의 구강보건지식 및 교육 필요도)

  • Objectives: The purpose of this study is to analyze the oral health knowledge among workers at nursing homes, and needs to educate them on elderly oral care. Methods: A self-reported questionnaire was filled out by 241 workers at nursing homes in Gwangju and Jeonnam, Korea from May to June, 2016. The data were analyzed with the SPSS 19.0 program for independent t-test or one-way ANOVA analysis. Results: The data analysis showed that workers at nursing homes had lower knowledge about periodontal diseases (65.1%) than about denture management (78.5%). The knowledge of periodontal disease and denture management of workers with experience of oral care education for elderly was significantly higher than that of non-experienced ones (p<0.01). On the question whether those workers should get oral health care education is necessary or not, respondents answered positively (They scored 4.10 points on the 5-point Likert scale from 1 point - Not at all to 5 points - definitely yes). Conclusions: It is necessary to provide elderly oral care education to workers at nursing homes. Furthermore, oral health professionals should be deployed for systematic and practical oral care for elderly.

Effects of Empathy for the Elderly, the Nursing Work Environment, and Person-centered Care on Geriatric Nursing Practice Among University Hospital Nurses (대학병원 간호사의 노인에 대한 공감능력, 간호근무환경 및 인간중심돌봄이 노인간호실천에 미치는 영향)

  • Purpose: We investigated the association between empathy for the elderly, the nursing work environment, person-centered care, and geriatric nursing practice among university hospital nurses and factors that affect geriatric nursing practice. Method: We administered questionnaires to 178 nurses between May 23 and June 21, 2022 at a university hospital in city D. We used descriptive statistics, the t-test, one-way analysis of variance, the scheffé test, Pearson correlation coefficients, and stepwise multiple regression analysis for data analysis. Results: We observed a statistically significant positive correlation between nurses' geriatric nursing practice and empathy for the elderly, the nursing work environment, and person-centered care. Empathy for the elderly was positively correlated with the nursing work environment and person-centered care, and the nursing work environment showed a statistically significant positive correlation with person-centered care. Empathy for the elderly and the nursing work environment significantly affected geriatric nursing practice. The overall explanatory power of the regression model was 33.4%. Conclusion: Greater awareness and implementation of various educational programs that promote empathy for the elderly are necessary to improve nursing care for the elderly population. Furthermore, adequate manpower and material support are important to improve the nursing work environment among nurses employed at university hospitals.

Medication Status and Adherence of the Elderly under Home Care Nursing (가정간호 노인대상자의 처방약물복용 실태 및 복용 이행도 영향요인)

  • Purpose: This study is a descriptive research intended to clarify the medication status of community-dwelling elders and to identify factors affecting their medication adherence. Methods: Data were collected using questionnaires and interviews from 101 subjects who had taken prescribed drugs for at least 7 days sampled among elderly people using home care nursing at a general hospital in Seoul. Results: According to the results of this study, medication adherence measured by pill counting was 88.3% and that measured by self-reporting was 94.6%. There were statistically significant differences in medication adherence according to major disease (p=.006), the number of admissions (p=.032), the number of drugs (p=.051), the frequency of medication (p=.026), and depression (r=-.205). In addition, depression was found to be a significant variable explaining the medication adherence with explanatory power 3.8% (p=.035). Conclusion: The presence of depression affected the elderly subjects' the medication adherence. Therefore, more concern and educational approaches are required to encourage elderly people to comply correctly with medication regimens particularly for elderly patients who have a malignant or long-lasting disease or who have to take multiple drugs or maintain a daily dosing frequency.

Factors Influencing the Nursing Performance of Delirium in Elderly Patients among Ward Nurses at a University Hospital (대학병원 병동간호사의 노인섬망 간호수행에 영향을 미치는 요인)

  • Purpose: This research aims to identify the relationship between the knowledge of delirium in elderly patients, nursing stress of delirium in elderly patients, emotional intelligence, and nursing performance of delirium in elderly patients among word nurses at a university hospital. Method: Data were collected from September 28 to October 13, 2020, by distributing questionnaires to 185 ward nurses at a university hospital located in city D. A data analysis was performed using descriptive statistics, a t-test, one-way ANOVA, and Pearson correlation coefficients, and a multiple regression analysis. Results: Nursing performance of delirium in elderly patients showed statistically significant differences according to nursing experience with these patients. Nurses' knowledge of delirium in elderly patients had a statistically significant positive correlation with emotional intelligence and nursing performance of delirium in elderly patients. Also, nurses' emotional intelligence had a statistically positive correlation with nursing performance of delirium in elderly patients. The factors that significantly affected nursing performance delirium in elderly patients were emotional intelligence and knowledge of delirium in elderly patients. The total explanatory power of the regression model was 12.5%. Conclusion: For nursing performance of delirium in elderly patients by ward nurses, it is necessary to develop a geriatric delirium nursing education program that can improve the knowledge of delirium in elderly patients. In addition, guidelines in consideration of the emotional intelligence of nurses should be included in the development of educational programs.

A Survey on the Characteristics of Living Environment on Nursing Homes for the Elderly in Korea (한국 노인요양시설 거주환경의 특성에 대한 연구)

  • The purpose of this study is to analyze the characteristic of living environment on nursing homes and to examine the current status and problems of the nursing homes by looking into the differences in the spatial uniqueness of living environment in relation to the characteristics of the nursing home. This study utilized a mail survey during July 29 - August 31, 2006 that included a structured questionnaire. The questionnaire largely dealt with the general feature of the facility and the elderly who live there, and the spatial feature of the facility. The study limited its survey to those facilities that were registered to the Ministry of Health and Welfare, which opened after 1995 and that had the capacity of more than 30 people. As a result, the questionnaire was sent to the directors of 344 facilities who were in charge of the nursing home operation and planning. 182 facilities returned the questionnaire and the study used'182 questionnaires for data analysis. The study used the SPSS 12 program to draws the frequency, percentage of the answers and utilized the cross-tab, t-test, ANOVA to analyze the differences.

Local Community and Nursing-home of the resident elderly in the Comparative Study of the Time Use for Occupational balance (작업균형에 의한 시간 사용 비교 연구: 지역 사회 거주 노인과 시설 거주 노인을 기준으로)

  • Objective : Local Community and Nursing-home of the elderly of the general characteristics and time use compared with the Occupational balance of the two groups is to look at what is being done. Method : The study conducted by the Time Use Survey 2009 of National Statistical office, the results were statistically analyzed 246 by based on more than 65 years of age and older living in Daejeon. The Comparison subjects of this study compared the Daejeon nursing homes in two locations were selected and 146 people of use on the general characteristics and time use data were collected. Analysis using SPSS 12.0 for correlation analysis and multiple logistic regression analysis for the study were analyzed. Result : Local Community and Nursing-home of the elderly are compared. Each classified behavior correlation showed significant results in a difference in a time-use, respectively and also Nursing-home of the elderly, non-activity time in the form of the activity time was higher than the results. Conclusion : Through this research, according to the living environment for elderly people with a time use to find out that there was significant, the elderly living in Nursing-home more than the elderly living in the community and show the imbalance of the Occupational could be.

A Study on Degree of Cognitive Impairment and Health Status of the Aged in Seoul Area (서울지역 일부 양노시설 노인과 재가노인 인지장애정도와 건강상태에 관한 연구)

  • This study was carried out to assess the degree of cognitive impairment and health status for the aged people in seoul area. The subjects were selected 117 aged at the nursing home and 101 residents from the community. To assess the cognitive function, Mini-Mental Status Examination(MMSE-K) and the assessement tool of health status for the elderly which was developed by Lee(1989) used to check the health status. The followings are the results of the Survey Study; 1. The elderly who were assessed as a cognitive impairment status was $47.9\%$ of the nursing home people and $42.6\%$ of the ordinary community residency elderly. 2. Health status of the community residency elderly was much better condition than the status of the nursing home residency elderly. 3. There was a significant relationship between the degree of cognitive impairment and the physical health status.
