A Comparative Study on the Eating Behavior in Inchon : The Elderly Living in Home and the Elderly Nursing Home (인천 지역 가정 거주 노인과 시설 거주 노인의 식행동 비교 연구)

  • This study is investigate the eating behavior between the elderly living at home and the elderly at nursing home in Inchon City. This survey was carried out by questionaires. The result are summarized as the followings: 1. Both the elderly living at home and the elderly at nursing home are not smoking and drinking. They are in good health. 2. The elderly living at home and the elderly at nursing home have diseases like neuralgia, hypertension, diabetes etc. 3. Both the elderly living at home and the elderly at nursing home have methods of health care of a regular eating and a good sleeping. The elderly at nursing home have a regular medical examination but the elderly living at home do not. 4. Health giving drugs are depression of blood pressure, a medicine for the stomach and bowels, an anodyne. And health giving sports are walking and jogging. 5. The elderly living at home and the elderly at nursing home are significant relationship on knowledge of nutrition. 6. The elderly living at home and the elderly at nursing home prefer to taste sweet, boiling, korean foods. 7. elderly living at home have meats once a week and the elderly at nursing home have twice a week. Both the elderly living at home and the elderly at nursing home have fish, fruits, vegetables twice a week. The elderly at nursing home have milks twice a week. 8. The frequency of snack intake is higher the elderly at nursing home than the elderly living at home. 9. The elderly living at home and the elderly at nursing home are satisfied their dietary life.

The Cost of Long-Term Care Alternatives for the Elderly (미국 노인환자들의 만성질환관리형태에 대한 비용분석)

  • The purpose of this study was to explore whether there is a point within the range of physical impairment after which the cost of home care exceeds the cost of nursing home care among the elderly who require long-term care. The provision of long-term care for the elderly is a major health policy issue, in part due to the aging of the American population and dramatic increase in health care costs. The framework for this study was guided by Pollak's(1973)model of costs of alternative care settings for the elderly. This study used a retrospective, descriptive correlational design. Physical impairment was measured by the modified Index of Activities of Daily Living(Katz et al. 1963). Cost of care was measured by the average cost per patient per day. The sample for this study included 67 patients receiving long-term care at home from the Long-term Home Health Care Programs (LTHHCPs) and 67 patients receiving long-term care in nursing homes. Data were collected on patient characteristics. including activities of daily living and cognitive impairment. and on the number of physician visits. emergency room visits. and hospitalization from the patient records. For each patient. Medicaid cost data for home care services/or nursing home services were collected from the financial department of each home care agency or nursing home. The living costs and informal care costs were estimated for home care patients. The results indicated that the home care sample and the nursing home sample were similar in terms of gender. ethnic background. and marital status. The elderly patients in the home care sample were: however. younger and less physically impaired than those in the nursing home sample. The hypotheses of this study were supported: For elderly persons with physical impairment scores below 12(possible range of 0 to 14), cost of care was lower in home care than in the nursing home care setting. However, for elderly persons with physical impairment scores above 12. the cost of care was higher in home care than in the nursing home care setting. Thus. in this sample for elderly patients with extreme physical impairment, the cost of home care exceeded the cost of nursing home care.

Comparison of Sleep Patterns between Home and Institute Elderly People (재가노인과 시설노인의 수면양상에 대한 비교연구)

  • This study was attempted to show basic data for search of nursing intervention about sleep improving in home and institute elderly peoples. Research design was cross-sectional survey method for comparison of sleep patterns and another variables between home and institute elderly peoples. Subjects were sampled by purpose as home and institute elderly peoples at age of 65 or older in Seoul or nearly. The survey questionnaire was used by modification of sleep questionnaire which Kim, O, Song & Bak(1997) developed. Data were collected between December, 1999 and April, 2000. And data were analyzed by SPSS PC+ for purpose of research. Results are as follows: 1. Home and institute elderly peoples, all were change of sleep patterns and sleep pattern was showed early sleeping down. But institute elderly peoples slept earlier than home elderly peoples. 2. As rest of morning, difficulty of falling asleep and disturbance of re-asleep, subjective thinking and feeling were showed more negative thing home elderly peoples than institute elderly peoples. As frequency of falling asleep within 5 min in 1 week, frequency of falling asleep over 30 min in 1 week and frequency of awakening in a day, objective frequency were showed more insomnia institute elderly peoples than home elderly peoples. Home elderly peoples didn't nap most and institute elderly peoples napped on very short time. 3. Regarding sleep-disturbing factors, physical factor was popularly joint-disturbance in all, home and institute elderly peoples. Environmental factor was popularly noise, particularly institute elderly peoples. Emotional factor wasn't all. 4. Practiced strategies for better sleep was popularly TV/Radio in home elderly peoples and were popularly regular sleep and religious action in institute elderly peoples. From the results of this study, home elderly peoples need nursing intervention of improving self-satisfaction on sleep and institute elderly peoples need nursing intervention of decreasing falling asleep-disturbing and decreasing frequency of awakening in sleep.

Precedent survey for development of nursing home clothes according to aged society

  • The purpose of this study was to identify the preferred materials and design characteristics of nursing home clothes in order to collect the basic data necessary for the elderly nursing home clothes. It was to utilize the elderly nursing home clothes considering elderly body shape, hand function, illness. This study was conducted by questionnaire method and SPSS ver. 20.0 program was used. The preference for nursing home clothes material was high for cotton, and it was found that they prefer soft, stretch material, warm feeling material, and lightweight material. In the hygienic aspect, they favored sweat-absorbent materials, and preferred laundry- care-resistant materials with poor wrinkles and dirtiness. The nursing home clothes preferred a two piece form consisting of a round neckline, two pockets on both sides, a waistband of rubber band, long lengths, and a waistline pants. The most important function in the nursing home clothes was recognized as wearing comfort. Aesthetics, symbolism, color and print pattern were recognized as not important functions. Therefore, it is necessary to focus on the functional part such as wearing comfort in the design of the nursing home clothes. The material is also hygienic and comfortable to wear. In the case of the elderly, it is necessary to provide convenience for the wear of clothes through the development of stretchable material and detachment device since the movement range of muscles, arms, and legs is reduced. Based on this study, we will utilize it for the development of nursing home suit considering the characteristics of elderly person in the elderly society. It is to develop functional materials for the elderly in need of nursing home, to develop the pattern considering the elderly body shape, and to develop the desorption device considering the movement of the hand.

Estimation of nursing home needs in elderly people (노인인구의 간호요양원(Nursing Home) 필요예측에 관한 연구)

  • The purpose of this study is to estimate the population requiring nursing home services among elderly people in Korea. This study identifies the need of nursing home services determined by health care professionals and estimates the proportion of elderly people requiring nursing home service according to the admission criteria. Surveys were conducted on health care professionals including medical doctors, home care nurses, and nurse practitioners. They were asked to assess nursing home need based on four content areas: Physical function (Activities of Daily Living), chronic disease, Physical symptoms (incontinence), mobility, eating, and sensory function. Based on the professionally determined need criteria the proportion of elderly people requiring nursing home services was estimated using secondary data from the 1994 Survey on the Living Status of the Korean Elderly. The number of study subjects to estimate nursing home need who were 60 and older totaled 2,058. The most important factor contributing to the admission eligibility criteria was the elderly living alone. Other factors related were the elderly being unable or having difficulty carrying out activities, and having insufficient help from other our activities, and having insufficient help from other members of the household. Using only physical function, the proportion of elderly people requiring nursing home was $8{\sim}9%$. When only chronic disease was used, proportions varied widely; for the doctor's group, the proportion was over 30%. Using all areas, the proportions of elderly people requiring nursing home were between 13% and 38%. The estimate using chronic disease and physical function was similar to the on using all areas.

Activity of Daily Living(ADL) and Instrumental Activity of Daily Living(IADL) of Elderly in Home (재가노인의 일상생활활동(ADL)과 수단적 일상생활활동(IADL) 실태)

  • Purpose: The purpose of this study was to identify the ADL and IADL of elderly in home. Method: This was a descriptive study. Data were collected from convenient samples of 76 elderly in home using ADL and IADL scales among MDS-HC V2.0 instrument from October to November. The data were analyzed using SPSS/Win 12.0 with frequency, t-test, and ANOVA at a significant level of=.05. Result: The elderly in home had performance with difficulty at bathing, locomotion outside of home, and transfer. On the other hand, the elderly in home had performance with easy at mobility in bed, dressing lower body and eating. Relation to the IADL the elderly in home had performance with difficulty at transportation, shopping, and managing finance. Conclusion; Based on the results, it is necessary to develop of a nursing intervention program and rehabilitation plan for the bathing and locomotion of the elderly in home.

A Study on the Supply Estimation and Allocation of Healthcare Facilities for the Elderly People in Seoul (서울시 노인의료복지시설의 공급과 배치에 관한 연구)

  • Recently, as the population of the elderly rapidly increases, the number of the special care facility for the elderly is a big social issue. Especially, special care facilities(nursing home, skilled nursing home, etc) for the elderly in Seoul are insufficient now. So this study quantitatively estimates nursing home beds needed in seoul in 2007, 2012 and 2017, and proposes an allocation of these facilities in Seoul. This study is to clarify problems in supply of long-term care facilities and present a solution for them. The main outcome of this study can be summarized into three parts; first, as architectural planning and supply estimation of long term care facility, an aged population of cities and countries in Seoul should be considered. Second, when the allocation of long term care facility in Seoul, the type of facilities, regional equilibrium etc. should be considered. Third, nursing home and residential care home are linked with the other area.

  • The Japanese elderly welfare policy has focused on facility policy for the aged and preventive care service for healthy elderly people. This paper has conducted a comparative analysis on Geriatric Gealth Services Facility and Special Nursing Home for the Elderly. For this, each service function has been divided into six categories; daily life / nursing and caring / medical service / management / supply / miscellaneous. Then the change in real structure by category has been analyzed through a plan analysis on case facilities. In the Geriatric Health Services Facility, the biggest change was observed in 'livelihood' among six categories. In the Special Nursing Home for the Elderly, 'the nursing and care parts' and 'medical service part' are decreased since 1999. At that time, the facilities started to be individualized and divided into a unit. To pursue home-like care instead of unit care, there was a change in construction planning to help the aged with dementia live a self-sufficient life.

Scale Development of Free Nursing Home-Adjustment for the Elderly (노인의 무료요양원 적응 척도 개발)

  • Purpose: The purpose of this study was to develop a scale of free nursing home adjustment for the elderly in Korea. Method: The developmental processes were construction of a conceptual framework, development of preliminary items, verification of content validity, twice data gathering, verification of construct validity, and reliability of final items. The first 53 preliminary items were obtained through review of literatures about nursing home adjustment and in depth interviews with 5 staffs of nursing home. These items were reviewed by five specialists for content validity and 39 items were chosen. The first data was collected from 107 elderly residing in 3 nursing homes and the second survey was from 147 residents who were living in five nursing homes. Finally this data was analyzed for construct validity and reliability. Result: There were 23 final items which were sorted into 5 factors. The factors were identified as 'Relocation distress symptom'(8items), 'Making friends'(4items), 'Acceptance of new residence'(6items), 'Difficulty in group life'(3items), and 'Having self-worth'(2items). The cumulative percent of variance was 53.466%. The reliability of the scale, Cronbach's alpha was .837. Conclusion: The result of this study could be used for measuring nursing home adjustment of the elderly. However, for further validity and reliability, repeated researches are needed.

Development and Clinical Use of Assessment Indicators for Improving Medication Adherence of Elderly Residents in Nursing Home (노인요양원 입소자의 복약순응도 향상을 위한 평가지표의 개발 및 임상활용)

  • Background: South Korea is rapidly being an aging-society and the demand of long-term care insurance services for elderly patients is rising. In addition, because the elderly taking multiple medicines, the adherence is lowered and the adverse events are easily occurred. Therefore, many are interested in introducing the geriatric pharmacy specialist to manage this situation. Purpose: By applying a similar program such as the geriatric pharmacy specialist working in nursing home, we conducted this study to evaluate the potential contribution to both the health insurance services and financial savings for the elderly patients. Methods: We conducted a trial in an elderly nursing home to collaborate between doctor and pharmacist making a checklist for improving medication adherence and establish a consultation system. Also we applied a smart phone application in the pharmaceutical care processes. Results: Thereby completing the drug therapy related checklist apply to nursing facilities in South Korea. And we got a performance that improves medication adherence when used in the clinical practice settings. Conclusion: By introducing a training program of pharmacy care managers and geriatric professionals such as the United States and Japan, we revealed the elderly nursing home residents and vulnerable elderly people living alone were improved the medication adherence and it will contributed to the health and quality of life of the patients.
