Influence of Community Contextual Characteristics on Community Differences of Mental Health (지역사회 상황적 특성이 지역사회 간 정신건강 차이에 미치는 영향)

  • The purpose of this paper was to identify the relationship between the characteristics of community mental health, and to identify if mediatable contextual property acts as factor influencing community mental health. Data of 229 municipal population groups were collected from KNSO and The 6th Community Health Plan. The data were analyzed using ANOVA, Spearman correlation, and multiple regression analysis. The perceived stress showed significant difference depending on health budget, mental health budget, number of mental health personnel, and was negatively correlated with proportion of the elderly, employment rate, percentage of NBLS recipients, health budget, and number of mental health personnel. The depressive mood differed depending on the number mental health personnel, and was negatively correlated with percentage of NBLS recipients and number of mental health personnel. Mental health budget and number of mental health personnel were respectively influencing factor of perceived stress and depressive mood. Following the above result, in order to reduce the gap between the mental health of communities, the researcher suggests that mental health budget as well as overall health budget and the number of mental health personnel be enforced.

The effects of Leisure activity, Self esteem, Achievement on Mental Wellbeing among the Aged (노인의 여가활동, 자아존중감, 성취감이 정신적 웰빙에 미치는 영향)

  • This study examined the effects of leisure activity, self-esteem, and the achievement on mental wellbeing among the elderly. The participants in this cross-sectional, descriptive study were 149 elderly who completed a self-report structured questionnaire. The data were analyzed using descriptive statistics, correlation and multiple linear regression analyses with SPSS statistics 20 program. The mental well-being averaged 51.5 (${\pm}15.1$); leisure activity averaged 26.2 (${\pm}6.5$); self-esteem averaged 30.2 (${\pm}5.2$), and self-esteem averaged 44.4 (${\pm}8.3$). The mental well being of the elderly was positively correlated with all three variables. The leisure activity, self-esteem, and achievement were strong predictors of the mental well-being among the elderly. These variables explained 42.7% of the variance in mental wellbeing. The results suggest that the elderly are satisfied with their own situations and abilities, have a positive mind by accepting their present state of being, continue social interactions through active leisure activities, and set specific goals. The mental well-being of the elderly can be improved.

The Effect of Stress Coping Ability and Recovery Resilience on Retention Intention of Nurses in Medium-Sized Hospitals (중소병원 간호사의 스트레스 대처능력과 회복탄력성이 재직의도에 미치는 영향)

  • This study was designed to investigate the impact of stress coping ability and recovery resilience on nurses' retention intent in medium-sized hospitals. For this descriptive study, a survey was conducted with 265 nurses from 5 medium-sized hospitals with over 150 sickbeds located in G and I province; the data were collected from May 19 to May 25, 2018. The collected data were analyzed by t-test, ANOVA, Pearson's correlation coefficients, and multiple regression. The average score was $2.55{\pm}0.25$ for stress coping ability, $3.47{\pm}0.49$ for recovery resilience, and $2.59{\pm}0.29$ for retention intent. Retention intent was positively correlated with stress coping ability (r=0.285, p<0.01) and recovery resilience (r=0.457, p<0.01). The factors affecting retention intent were gender (${\beta}=0.117$, p=0.027), job satisfaction (${\beta}=0.345$, p<0.001), stress coping ability (${\beta}=0.142$, p=0.008), and recovery resilience (${\beta}=0.238$, p<0.001). Furthermore, the model explained 37.8% of the retention intent (F=11.686, p<0.001). In conclusion, effective strategies for improving job satisfaction, stress coping ability, and recovery resilience for nurses need to be developed and investigated.

Validity and Reliability of Korean version of Professional Quality of Life Scale-Short Form(ProQOL-K-SF) for Staff of Long-term Care Insurance for Elderly (한국어판 전문직 삶의 질 측정도구-간편형(ProQOL-K-SF)의 타당도와 신뢰도: 장기요양요원을 중심으로)

  • This study was conducted to test the validity and reliability of the Korean version of the Professional Quality of Life Scale-Short Form (ProQOL-K-SF). Survey data were collected from 191 staff involved in long-term care insurance who worked for National Health Insurance Services. Construct validity using exploratory factor analysis and the internal consistency reliability were determined using SPSS/WIN (21.0). Factor analysis of the ProQOL-K-SF demonstrated that it has a two-factor structure (compassion satisfaction and compassion fatigues) and 13 items that support construct validity. Factor loadings of the compassion satisfaction ranged from 0.70-0.86, factor loading of the compassion fatigues ranged from 0.71-0.82 and the total variance was 60%. The ProQOL-K showed interanal consistency based on Cronbach's ${\alpha}$, with a total scale and two sub scale values of 0.76-0.90. Compassion fatigue and compassion satisfaction of 30 items of the ProQOL-K were positively correlated with those of 13 items of the ProQOL-K-SF. The results support that the ProQOL-K has satisfactory construct validity and is a reliable measure of professional quality of life in Korea.

Socioeconomic and Sociodemographic Factors related to Allergic Diseases in Korean adolescents based on the 14th Korean Youth Risk Behavior Survey (청소년의 알레르기성 질환과 관련된 사회경제적 및 사회인구학적 요인: 제14차 한국청소년건강행태조사를 기반으로)

  • Various international reports have shown strong proof that socioeconomic and sociodemographic variables are correlated with allergic diseases, yet little is known about how these variables affect Korean adolescents. This study was conducted to identify the socioeconomic and sociodemographic risk factors for allergic diseases of Korean adolescents, and to provide information for preventing and managing such conditions. Data from the 2018 Korean Youth Risk Behavior Survey (KYRBS) of 60,040 adolescents was used for this study. An anonymously administered online survey was conducted to collect information on the dependent variables. The independent variables were asthma, allergic rhinitis and atopic dermatitis. Multivariate logistic regression analysis was performed to analyze the correlations between the dependent variables and the independent variables. Low economic status, living with mother, high education level of parents, high school record, obesity, drinking and smoking were the risk factors for asthma. Low economic status, the place of residence, living with mother, high education level of the parents, gender, high school record, obesity, drinking and smoking were the risk factors for allergic rhinitis. Low economic status, living with mother, high education level of parents, genderhigh school record, obesity and drinking were the risk factors for atopic dermatitis. We found that low socioeconomic status and unhealthy behavior were the risk factor for allergic diseases of Korean adolescents. Balanced economic growth in the community and controlling unhealthy behavior can help reduce the prevalence of allergic diseases.

Hepatotoxicity and nephrotoxicity of saponin-enriched extract of Asparagus cochinchinensis in ICR mice

  • The inhibitory effects of Asparagus cochinchinensis against inflammatory response induced by lipopolysaccharide (LPS), substance P and phthalic anhydride (PA) treatment were recently reported for some cell lines and animal models. To evaluate the hepatotoxicity and nephrotoxicity of A. cochinchinensis toward the livers and kidneys of ICR mice, alterations in related markers including body weight, organ weight, urine composition, liver pathology and kidney pathology were analyzed in male and female ICR mice after oral administration of 150, 300 and 600 mg/kg body weight/day saponin-enriched extract of A. cochinchinensis (SEAC) for 14 days. The saponin, total flavonoid and total phenol levels were found to be 57.2, 88.5 and 102.1 mg/g in SEAC, respectively, and the scavenging activity of SEAC gradually increased in a dose-dependent manner. Moreover, body and organ weight, clinical phenotypes, urine parameters and mice mortality did not differ between the vehicle and SEAC treated group. Furthermore, no significant alterations were measured in alkaline phosphatase (ALP), alanine aminotransferase (ALT), aspartate aminotransferase (AST), lactate dehydrogenase (LDH), blood urea nitrogen (BUN) and the serum creatinine (Cr) in the SEAC treated group relative to the vehicle treated group. Moreover, the specific pathological features induced by most toxic compounds were not observed upon liver and kidney histological analysis. Overall, the results of the present study suggest that SEAC does not induce any specific toxicity in the livers and kidneys of male and female ICR mice at doses of 600 mg/kg body weight/day.

Related Factors of Smoking Cessation Success and Maintenance of Patients using Smoking Cessation Clinic at a Public Hospital : focused on patient demographics and smoking related factors (일개 공공병원 금연클리닉 이용 흡연 환자의 금연 성공 및 유지 요인 : 일반적 특성과 금연관련 특성 중심으로)

  • This study was conducted to identify the factors related to the patient demographics and smoking factors on smoking cessation success and maintenance of subjects at the 6th and 12th week at a public hospital. This study utilized secondary data collected through the smoking cessation program of the 2014 public health care program, which is mediated for 6 weeks and checked for maintenance on the 12th week. The data were analyzed by the chi-square test, t-test and logistic regression analysis. There were 90 subjects in total, 85 men (94.45%) and 5 women (5.55%). The smoking success group and smoking failure group showed significant differences in family type (${\chi}^2=4.496$, p=0.037), education (${\chi}^2=12.253$, p=0.002), smoking amount per day before program (t=-2.906, p=0.005), and nicotine dependence before program (t=-3.081, p=0.003) after 6 weeks. Factors influencing smoking cessation success were religion (OR=3.719, 95% CI=1.076-12.859) and nicotine dependence before program (OR=0.713, 95% CI=0.520-0.976). Factors influencing smoking cessation maintenance were family type (OR=0.017, 95% CI=0.001-0.507) and no-smoking willingness (OR=2.062, 95% CI=1.059-4.015). For smoking cessation to be successful and sustainable, it is necessary to develop programs specific for the characteristics of the subjects; accordingly, continuous research and support are needed.

Analysis of Factors Influencing the Utilization Rate of Public Health Centers in Korea (한국의 보건소 이용률에 영향을 미치는 요인 분석)

  • This study was conducted to identify the utilization of public health centers, as well as the individual characteristics and regional characteristics that affect their utilization based on data from the 2016 Community Health Survey, National Statistical Portal, and National Institute of Environmental Research. Independent samples t-tests, variance analysis, and multiple logistic regression analysis were used for analysis. Hierarchical multiple regression was used to analyze individual and regional characteristics. The results of hierarchical multiple regressions revealed that aged regions, women, older age individuals, respondents with lower education level and income level, walking practitioners, nutrition label readers, individuals experiencing depression, those who have received health checkups, those who are not covered by essential care, those who have spouses, and basic livelihood beneficiaries have increased use of public health centers. However, the use of public health centers decreased in stressors, and regions in which the population per 1,000, number of health care workers, health and welfare budget, fiscal independence, and unemployment rate were above the national average. As above, the central government and local governments need to analyze not only individual characteristics such as health behavior and psychological factors, but also regional characteristics, when establishing local health care policy.

Utilization and Expenditure of Health Care and Long-term Care at the End of Life: Evidence from Korea (장기요양 인정자의 사망 전 의료 및 요양서비스 이용 양상 분석)

  • Purpose: This study empirically investigates the utilization and expenditure of health care and long-term care at the last year of life for long-term care beneficiaries in Korea. Methods: This study used National Health Insurance and Long-term Care Insurance claims data of 271,474 LTCI beneficiaries, who died from July 2008 to December 2012. Their cause of death, place of death, health care costs, and the provision of aggressive care were analyzed. Results: Cardio-vascular disease(29.8%) and cancer(15.3%) were reported as their major cause of death, and hospital(64.4%), home(22.0%), social care facility(9.2%) were analyzed as the place of death. 99.3% of subjects used both health care and long-term care during the last 1 year of life. The average survival period were 516.2 days after they were LTCI beneficiaries. The health care expenditure gradually increased near the death, and the last month were three times more rather than the first month. Furthermore, 31.8% experienced some aggressive cares(CPR, blood transfusion, hemo-dialysis, etc.) at the last month of life. Conclusion: The results of this study suggest that it is important to develop the end of life care policies(for example, hospice, advanced care directives) for the LTCI beneficiaries. They might contribute to the improvement of quality of life and the reduction of health care expenditure of the elderly at the end-of-life.

The Relations between metabolic syndrome, physical activity, and dietary patterns in Korean adults (한국 성인의 대사증후군과 신체활동, 식이 형태의 관계)

  • This study involved a secondary analysis of 2016 Korean National Health and Nutrition Examination Survey data to identify the relations between metabolic syndrome, physical activity, and dietary patterns in Korean adults. The ${\chi}^2$-test, t-test, and logistic regression analysis of complex samples were conducted on the data of 3,352 subjects aged 19 to 64 years with metabolic syndrome. The prevalences of metabolic syndrome among men and women were 29.7% and 16.7%, respectively. In men, the high physical activity group showed a 0.8-fold increase in the likelihood of metabolic syndrome (OR=0.8, p=.038), and higher fat intake increased the risk of metabolic syndrome 0.98-fold (OR=0.98, p=.007). In women, the high physical activity group showed a 0.7-fold increase in the likelihood of metabolic syndrome (OR=0.71, p=.034), higher carbohydrate intake increased the risk of metabolic syndrome by 1.03-fold (OR=1.03, p<.001), and that higher protein and fat intakes reduced the risk by 0.96-fold (OR=0.96, p=.009) and 0.95-fold (OR=0.95, p<.001), respectively. The study shows that to meet the challenge presented by the increasing prevalence of metabolic syndrome, efforts are needed to screen for adults at risk of the metabolic syndrome by regular physical examination and to develop and provide intervention programs that involve regular high intensity physical activity and controlled carbohydrate, protein, and fat intakes.
