Purpose: The purpose of this study was to identify the effects of job stress, depression and fatigue on sleep quality of care workers employed in long-term care facilities. Methods: The participants were 213 care workers who worked in 11 long-term care facilities. The collected data was analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient and multiple linear regression with using the SPSS 23.0 program. Results: We found that patient-related job stress was the greatest stressor among care workers' job stresses. A significant difference in sleep quality was observed according to the self-perceived health status of the care workers. The care workers' sleep quality was negatively correlated with job stress (r=-.16, p=.018), depression (r=-.31, p<.001) and fatigue (r=-.29, p<.001). The influencing factors of sleep quality were depression (${\beta}=-0.22$, p=.001) and fatigue (${\beta}=-0.15$, p=.047). Conclusion: It is necessary to improve the health status and sleep quality of care workers by developing and applying health promotion programs to reduce the depression and fatigue of the care worker. This will ultimately improve the quality of service care for the patients.

Purpose: Long-term care facilities have a responsibility to provide care service that enables residents to maintain their maximal functional capacity and quality of life. Also their needs must be reflected to the service programs. In oder to provide an adequate service, we should assess the elderly's physical, psychological and social health status and the need. In addition to this, the long-term care facilities must be defined clearly by the type of services. This study would contribute to conduct appropriate services in public long-term care policy for the older population in the future. This study would provide informations of long-term care facilities' services and older persons' needs for long-term care. Method: To achieve this objectives, this paper investigates the types, service programs of long-term care institutes and job descriptions of workers. The subjects were consisted of 150 long-term care institutes. 150 institutes of long-term care facilities were drawn from all over the country by a nonrandom, convenience sampling. The data were analyzed by frequency, percentage, $x^2$-test using SPSS program. The instruments of this study were self-reported questionnaires for long-term care institutes. The data were collected from March 1, 2004 to may 31, 2004. Results: Service programs of long-term care institutes were not enough for residents' demands. The job descriptions among nurse, social worker and physical therapist were not clearly defined. The nurse's main role was medication and checking vital sign(49.7%), that of social worker's was observation and supervising (31.2%). The most significant problems were lacking of diverse service programs for residents. Conclusion: Considering these findings and conclusion, the needs of long-term care services should be provided by individual physical and psychological level. And the professional manpower for elderly should be educated in multi disciplines.
Purpose: The purpose of this study was to explore the reported good service experiences from the perspective of elderly residents of long-term care facilities. Methods: Of those residents who are 65 years old or older, 14 residents whose length of stay were one month or longer and scores of the K-Mini Mental State Examination were 15 or higher were interviewed. The interview data formed the basis for the empirical statements about the reported nature of patients' experiences as residents of long-term care facilities. These data were used in concept mapping. Results: Through multidimensional scaling analysis and hierarchical cluster analysis, 62 core statements, two dimensions, and six clusters of good service experiences were derived. The two dimensions were classified as 'care centered-participation centered services' and as 'physical-emotional services.' Six cluster themes emerged as good service experiences: 'safety of care and treatment', 'responsible and supportive staff', 'comfort of living environment', 'mental well-being', and 'respect and communication'. Conclusion: The result of the study provides information about what experiences are important to older adults with cognitive impairment. The concept map can be used to develop a patient experience index for the elderly residents of long-term care facilities.

This study examined the effect of the distance Infection Control Education Program (ICEP), developed based on the ADDIE model, on infection control knowledge, attitude, and performance among care workers in long-term care facilities nationwide. The program, developed based on the ADDIE model, was applied to 173 care workers directly responsible for nursing care of elderly residents in lomg-term care facilities. The distance ICEP for care workers was conducted through the website and lasted 30 minutes for each of the eight topics. To determine the effectiveness of the education, infection control knowledge, attitude, performance, and satisfaction were surveyed before and four weeks after the program. Differences in infection control knowledge, attitude, and performance before and after the distance ICEP were assessed by a t-test. A significant difference was observed in knowledge and infection control performance after the distance ICEP was administered to care workers. In the sub-domains of infection control performance, overall understanding of infection, regular infection control education, infection control by special pathogen (multidrug-resistant bacteria, tuberculosis, tick-borne infectious diseases), and detailed infection control education by infection site (pressure ulcers and urinary tract infections) were significantly improved. Infection control knowledge and performance improved through the distance ICEP applied to care workers. Satisfaction also displayed high scores on most items and indicated that it was helpful for infection control in facilities, confirming the effectiveness of infection control education. Based on the survey of care workers nationwide, the infection education program can be effectively used for care workers in the future.

Purpose: The first thing to be done in promoting community care is local diagnosis. Therefore, this study attempted to derive the physical infrastructure to be diagnosed, and to develop diagnostic items and diagnostic indicators applicable to this. Methods:: First, the physical infrastructure related to the community care is derived. And the diagnosis items are derived using the checklist of 'community support and health services' in the WHO Guide for Global age-friendly cities. Next, by analyzing previous studies, we develop diagnostic indicators for each diagnostic item and explore their applicability. Results: As a result of deriving the physical infrastructure for each area of housing, health service, and nursing care for community care, 22 facilities were derived for 9 types. Diagnosis items for the facilities are 1)regional equity, 2)proximity between facilities, 3)transportation access, 4)regional use, 5)barrier-free design, 6)diversity of facilities, and a total of 14 diagnostic indicators was derived. We reviewed and suggested the applicability of diagnostic items and indicators by each physical infrastructure. Implications: For the realization of community care, local diagnosis should not be limited to sim- ply grasping the presence or absence of facilities and the total amount. Instead it should strengthen capabilities by conducting diagnosis to understand the performance of facilities.
Presented here are policy alternatives for understanding home health care for the long-term health care insurance system which is being developed for elderly people starting 2007. The summary of issues concerning home health nursing care under the long-term care insurance system include; 1) absence of comprehensive and systematic policy in home health care deliverly systems; 2) absence of community based home health agencies that are considered as the community residents in general. In order to overcome these problems and Issues, policy alternatives of home health care should 1) establish a comprehensive home health care policy for homebound persons; 2) establish the foundations for home health care nurses and community based home health care systems; 3) establish home health care facilities and infrastructure; and 4) promote research and development concerning home health care. Conclusively, a home health care system should be built on a comprehensive policy vision based on health policy, especially long-term care insurance system in the near future. Every homebound residents service has to be constructed systematically under suitable facilities considering the consumer characteristics and health conditions. By doing this, the consumer based comprehensive community home health care delivery system can be constructed in view of the long-term health care insurance system for elderly people.
The purpose of this study is to investigate the experience of the mother-daughter relationship among middle-aged women in depth through the process of admission to nursing home facilities of their dementia mother. For the purpose, 8 middle-aged women whose dementia mothers were admitted to nursing care facilities were selected as research subject, and data collection was carried out through individual interviews. As a result of this research based on the phenomenological method of Colazzi, 3 categories, 6 sub-categories, and 17 subject bundles were derived. Research results can be summarized as follows: First, the mother-daughter relationship was dramatically reversed in the process of mother's admission to the nursing care facilities. Second, in the process of dementia mother's admission to the nursing care facilities, it was confirmed that daughters rather than sons were in an independent position regarding care and management. Third, in the process of dementia mother's admission to the nursing care facilities, it turned out that the entire family experiences depression, anxiety, and feeling of loss due to a sudden change in mother-daughter relationship as well as further transformation in family relationship, suggesting that there is a need to operate education or consultation programs for the family members to help them accommodate and deal with the sudden change.
Purpose: The study was done to identify factors influencing the quality of life of elderly in non-paid or paid assisted living facilities. Methods: Data were collected by questionnaires from 634 elderly in Seoul and Gyeonggi province. Measures were self-esteem, activities of daily living, depression, family support, health status, facility adaptation, care facility's quality, and quality of life. Data were analyzed using $x^2$-test, independent t-test, ANCOVA, Pearson correlation coefficients, and stepwise multiple regression with SPSS/WIN 15.0 version. Results: The significant factors influencing quality of life of elderly in non-paid assisted living facilities were care facility's quality, depression, self-esteem, family support, education, and activity of daily living, which explained about 40.4% of the variance. The significant factors influencing quality of life of elderly in paid assisted living facilities were activity of daily living, depression, family support, education, care facility's quality, and facility adaptation, which explained about 83.9% of the variance. Conclusion: The results suggest that type of assisted living facilities in developing nursing interventions to improve quality of life of elderly should be considered.
Purpose: This study was done to examine the effects of 4% hypertonic saline solution mouthwash and tooth brushing education on the oral health of elders living in long term care facilities. Methods: In this quasi-experimental study, the participants were assigned to a 2% experimental group (n=20), a 4% experimental group (n=20), and a control group (n=20). Data were analyzed using ANOVA, repeated measures ANOVA, Fisher exact test, Chi-square test, Kruskal-Wallis test and multiple response analysis with the SAS program. Results: Regular tooth brushing and use of 4% hypertonic saline solution mouthwash by elders provided better oral health by decreasing xerostomia, oral tongue plaque, halitosis, and the number of oral bacteria. Conclusion: The results indicate that regular tooth brushing with continuous 4% hypertonic saline solution mouth washing education promotes oral health for elders in long term care facilities, thus the dental care described in this study is recommended for elders in long term facilities.

The demand for long-term care is continuously on the rise as number of elders among the population increases. Due to the rapid growing demand for long-term care in Korea, there have been discussions for the introduction of new long-term care system into Korean society. The purpose of this study in to analyze changes of elderly care facilities in Japan after the beginning of long-term care system in 2000. The functional and architectural changes of Japanese facilities were researched to be used as references for predicting changes in Korean facilities. In Japan, after the execution of long term care system, the alterative aspect of facilities is divided into some categories; in sum, the change of environment for long term care, the effort to spread specialized facilities in a whole community, and the tendency to complex a variety of function of facilities such as an facility for home care service, and the like.
