재활전문병원 간호사의 환자안전문화에 대한 인식과 안전간호활동 수행의 관계 (Relationship between Perception of Patient Safety Culture and Performance for Safety Care Activity in Rehabilitation Hospital Nurse)

  • Purpose: This study was to examine the relationships between nurse's perception of patient safety culture and performance for safety nursing activities at rehabilitation hospitals. Methods: This study applied a descriptive research design. Participants were 194 nurses who have provided nursing services for more than 6 months at 4 rehabilitation hospitals located in B metropolitan city. Data were analyzed using descriptive statistics, t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson's correlation coefficients and multiple regression. Results: There was a positive correlation between the awareness of patient safety culture and safety nursing activity. Multivariate analysis showed that hospital work environment, experience of education, hospital climate, frequency of reported events, and marital status were significantly associated with the safety nursing activity. Overall, approximately 23.1% of total variability in the safety nursing activity could be explained by the 5 variables ($R^2=0.231$, p<.005). Conclusion: Nurses at rehabilitation hospitals are relatively positive about patient safety culture. Therefore, we need to develop safety education programs at the level of organization in order to improve patient safety through performing effective safety nursing activities in addition to increase awareness of patient safety culture among nurses. Furthermore, we need hospital's strategies at the system level for open communication and outcome reports regarding patient safety.

환자안전사건 소통하기에 대한 인식 및 교육 효과 분석: 간호사를 대상으로 한 예비 연구 (Perception and Effectiveness of Education Regarding Disclosure of Patient Safety Incidents: A Preliminary Study on Nurses)

  • Objectives: The purpose of this preliminary study was to identify the nurses' perception regarding disclosure of patient safety incidents (DPSI) and to evaluate the effectiveness of education for DPSI. Methods: DPSI education was conducted for nurses majoring in clinical nurse specialist at an university. Before and after the education, the nurses made a questionnaire to evaluate the perception of DPSI. The questionnaires were divided into four categories: first, overall perception of the DPSI; second, recognition evaluation of the DPSI using hypothetical case, third, opinion on legal and nonlegal measures for facilitating the DPSI; and fourth, socio-demographic factors. The Wilcoxon signed rank test was performed on the DPSI questionnaire response to compare the perceptions before and after the education. Results: A total of 10 nurses participated in the education. DPSI education showed the possibility of improving the overall perception, necessity, effect, obstacle, and promotion method of DPSI, although there were also several responses where there was no statistical significance. In particular, DPSI education led to statistically significance change in the perception of obstacles for DPSI. For example, the number of respondents who agreed to the item "DPSI will increase the incidence of medical lawsuits." was 7 before education but decreased to 3 after education (P-value: .025) Furthermore, nurses' perception of DPSI from this study was generally positive regardless of education. Conclusion:In the future, it will be necessary to carry out DPSI education and training and to evaluate its effectiveness for more nurses.

중소병원 간호사의 환자안전인식이 안전간호활동에 미치는 영향 (The Influences of the Awareness of Patient Safety Culture on Safety Care Activities among Nurse in small-medium Sized General Hospitals)

  • 본 연구는 중소병원의 간호사의 환자안전인식 및 안전간호활동을 조사하고 안전간호활동에 영향을 미치는 요인들을 분석함으로써 보다 안전한 의료서비스 제공의 기초자료로 삼고자 시행 되었다. 대상자는 2012년부터 4월 1개월 동안 3개의 중소병원 간호사 344명을 대상으로 실시하였다. 미국의 AHRQ에서 개발한 Hospital survey of patient safety culture라는 설문지와 의료기관인증문항 중 안전보장활동을 사용하였다. 자료분석은 SPSS 18.0 통계 패키지를 사용하여 기술통계 t-test 및 ANOVA와 Scheffe 사후 분석, Pearson's correlation coefficents, stepwise multiple regression으로 분석하였다. 환자안전인식 수준은 간호사의 경력, 근무시간, 근무부서에 따라 차이가 있었고, 안전간호활동은 간호사의 나이, 직위, 결혼, 간호사 경력, 근무시간에 따라 차이가 있었으며, 간호사의 안전인식수준이 높을수록 안전간호활동 수준이 높았다. 병원에서 환자안전인식 및 안전간호활동 수준을 개선하기 위해서 환자안전정책 및 근무환경개선제도가 요구된다. 근무조건 개선을 위해서 근무시간, 환자대 간호사의 비율을 정규화하는 정책 등이 요구된다.

군 병원 간호사의 의사소통능력, 비판적 사고성향, 환자안전문화 인식이 환자안전역량에 미치는 영향 (The Influence of Nurses' Communication Competency, Critical Thinking Disposition, and Perception of Patient Safety Culture on Patient Safety Competency in Armed Forces Hospitals)

  • Purpose: This study examined the influence of nurses' communication competency, critical thinking disposition, and perception of patient safety culture on patient safety competency in armed forces hospitals. Methods: A crosssectional design was employed with a convenience sample of 204 nurse officers from four armed forces hospitals in South Korea. Data were collected through a self-administered questionnaire from August 10~23, 2019, and analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients, and multiple regression. All analyses were conducted using SPSS for Windows 21.0. Results: Patient safety competency scores were 4.35±0.40 for patient safety attitudes, 3.63±0.64 for patient safety knowledge, and 3.94±0.51 for patient safety skill. Communication competency, critical thinking disposition, and perception of patient safety culture were positively correlated with patient safety competency. Communication competency (β=.30, p=.002) and perception of patient safety culture (β=.24, p=.001) were identified as particularly important factors influencing patient safety competency. Conclusion: This study's results suggest that education programs to enhance communication competency and patient safety culture could increase patient safety competency among nurses in armed forces hospitals.

  • 본 연구는 상급종합병원 환자와 간호사의 안전 인식을 비교하고, 간호사의 환자안전관리 직무수행에 미치는 영향요인을 확인하기 위해 수행된 서술적 조사연구이다. 경상남도 J시 G대학교병원에 입원한 환자 147명과 간호사 147명을 대상으로 하여 2018년 7월 24일부터 9월 23일까지 구조화된 설문지를 사용하여 자료수집 후 SPSS/WIN 23.0으로 통계 분석하였다. 분석 결과, 환자의 안전 인식이 간호사의 인식 정도보다 유의하게 높았으며, 간호사의 환자안전관리 직무수행에 미치는 유의한 영향요인은 교육수준, 근무형태, 의사소통, 환자안전 인식, 환자안전관리 중요성 인식이었다. 다중회귀분석 결과, 환자안전 지식의 하위영역인 '의사소통'과 환자안전관리 중요성 인식의 하위영역인 '환자안전관리에 대한 관심'이 간호사의 환자안전관리 직무수행을 54.8% 설명하는 것으로 나타났다. 본 연구 결과를 바탕으로 상급종합병원 간호사의 환자안전관리 직무수행 향상을 위해 간호사는 환자와 조직 구성원간 원활한 의사소통과 관심 정도를 높이는 전략을 개발할 필요가 강조된다.

수술실 간호사가 인지하는 조직건강, 안전분위기, 간호근무환경이 환자안전관리활동에 미치는 영향 (The Effects of Operating Room Nurses' Perceptions of Organizational Health, Safety Climate, and the Nursing Working Environment on Engagement in Patient Safety Management Activities)

  • Purpose: This study aimed to examine the relationship between organizational health (OH), safety climate (SC), the nursing working environment (NWE), and engagement in patient safety management activities (PSMA) among operating room nurses and identify the factors that predict engagement in PSMA. Methods: From August 10th to 25th, 2018, 176 operating room nurses who were working in tertiary and general hospitals responded to a structured questionnaire. Using SPSS/WIN 25.0, the collected data were subjected to independent-samples t-test, one-way analysis of variance, Scheffe?test, and Pearson's correlational and multiple stepwise regression analyses. Results: OH and SC were significantly correlated with engagement in PSMA. The factors that predicted engagement in PSMA were OH, NWE, participation in accreditation, years of work experience, and hospital size; together, they explained 17% of the variance in engagement in PSMA. Conclusion: This study revealed that OH has a significant influence on engagement in PSMA among operating room nurses. Therefore, hospitals should aim to create healthy working environments to promote engagement in PSMA among operating room nurses, actively delegate responsibilities to increase their level of participation in accreditation, and implement strategies that maintain high levels of nurse retention.

지방 중소병원 간호사의 환자안전문화 인식이 안전간호활동에 미치는 영향 (Effects of Perceived Patient Safety Culture on Safety Care Activities among Nurses in General Hospitals)

  • Purpose: An objective of this study was to investigate nurses' perceptions toward patient safety culture and to examine the factors affecting safety care activities. Methods: The participants were 429 nurses, at 6 hospitals located in regions, which have 150 to 300 beds, and HSOPSC (AHRQ, 2009) and questionnaire on safety care activities were used as measurement tools. Descriptive statistics, independent t-test, one way ANOVA, and stepwise multiple regression with SPSS/WIN version12.0 were used to analyze the data. Results: Supervisor manager expectations and actions promoting patients safety and frequency of events reported were the highest as positive responses, whereas staffing and nonpunitive response to errors showed the lowest scores as positive responses. Scores of medication surveillance is the highest while firefighting surveillance is the lowest in terms of safety care activities. Significant predictors influencing safety care activities were frequency of events report, handoffs and transitions, work unit a patient safety grade, organizational learning-continuous improvement, and teamwork across units. These predictors account for 23% of the variance. Conclusion: These results suggest that hospital policies and systems should be built to settle patient safety culture effectively. Development of standard manuals for safety care activities is another critical element for promoting patient safety.

중소병원 간호사의 직무 스트레스가 환자안전간호활동에 미치는 영향 (The Impact of Job Stress on the Patient Safety Nursing Activity among Nurses in Small-Medium Sized General Hospitals)

  • Purpose: The purpose of the study is to examine the impact of job stress on the patient safety nursing activities among nurses. Methods: The subjects of the study are 258 nurses working at 15 small-medium sized hospitals in D city. Data analysis was done using frequency, percentage, average and standard deviation, t-test, one-way ANOVA, $Scheff{\acute{e}}test$, Pearson's correlation coefficient, stepwise multiple regression. Results: The job stress scored average 3.67 and patient safety nursing activity scored average 4.35. Job stress was positively associated with patient safety nursing activity. It explains 9.49% of the variance. Conclusion: Universal and inevitable job stress among nurses should not exceed optimum level to assure patient safety. So, personal and organizational efforts to manage job stress are needed.

공공병원 간호사의 안전간호활동에 미치는 영향 요인 (Factors Influencing Safe Nursing Activities of Public Hospital Nurses)

  • Purpose: In this descriptive study, we aimed to identify factors influencing safe nursing activities of nurses working at public hospitals. Methods: We collected data in March 2021 through an online survey. The subjects were 199 nurses who had worked for at least six months at public hospitals. The self-reported questionnaire consisted of items pertaining to safe nursing activities, perception of patient safety culture, and the general characteristics of the subjects. To examine the differences in the safe nursing activities and perception of patient safety culture by general characteristics, we employed a t-test and ANOVA. We conducted a multiple linear regression analysis to explore the factors affecting safe nursing activities. Results: The mean score for safe nursing activity was 4.59±0.39. Perception of patient safety culture and age group were found to be the main factors affecting the safe nursing activities of nurses working at public hospitals (F=17.24, p<.001, Adj. R2=.40). When the score for perception of patient safety culture increased by 1 point, the score for safe nursing activities increased by 0.58 points. In addition, the score for safe nursing activity in the 40s age group was 0.16 points higher than for those in the 20s age group. Conclusion: Effective management and leadership in public hospitals is needed to improve the culture of patient safety at various levels so as to ensure safe nursing activities.
