어린이병원 간호사의 업무 스트레스와 공감만족, 공감피로, 소진 간의 관계 (Relationship between Job Stress and Compassion Satisfaction, Compassion Fatigue, Burnout for Nurses in Children's Hospital)

  • Purpose: Nurses experience burnout related to various factors. For this descriptive research job stress, compassion satisfaction, and compassion fatigue were examined as to their relationship to burnout in nurses from children's hospital. Methods: The participants were 305 nurses working in children's hospital. Self-report questionnaires were used to measure job stress, compassion satisfaction, compassion fatigue and burnout. Results: Nurses in children's hospital experienced a greater than moderate degree of job stress, compassion satisfaction, compassion fatigue and burnout, whereas differences existed according to general characteristics. Job stress, compassion fatigue and burnout showed a significant positive correlation and results of compassion fatigue and burnout were similar. Also, job stress, compassion satisfaction and compassion fatigue were associated with burnout in nurses working in children's hospital. Conclusion: Findings indicate that as longer work experience is accompanied by higher job stress and burnout, it is necessary to develop intervention programs to reduce burnout among career nurses exposed to greater job stress in children's hospital.

서울지역 가정간호사의 업무수행능력 실태조사 (A Study on the Nursing Performance of the Home Care Nurses in Seoul)

  • The purpose of this research was to find out the state of the nursing performance of the home care nurses in Seoul in order to provide the data for the practical work guide. The data were collected from fifty home care nurses working in 22 institutions located in Seoul. The research tool used for this research was modified and tested by Song Jong-Rye(1999) which was originally modified the standard tool of American Nurses Association (1998) and was partly supplement and adjusted for this research. The reliability of this tool was Cronbach's $\alpha$=.0982. The collected data were statistically processed using SAS for t-test, ANOVA. $Scheff\'{e}$ test. Among the surveyees, 50% have been working as a home care nurse more than three years and 74.0% of them were married. And 52% have B.S degree with certification of home care nurse from the certification program for Home Care Nurses affiliated at nursing schools. Half of the home care nursing organizations were based on the general hospitals, and most(78%) of the responsible senior personnels of those organizations were nurses. The following results are drawn from this research. 1) The level of nursing performance by the task sectors General performance level of home care nurses was relatively high in grade of 3.06 from total 4.00. Among the task sectors. the ethical field scored the highest points, and the next were nursing intervention, professional training, and data gathering, and the research sector scored the lowest point. 2) The level of nursing performance by general features of home care nurses Statistically relevant correlation between performance according to the duration of working experience as a nurse(p=0.8951) and performance according to the duration of working as home care nurse(p=0.2263) did not emerge. Also, performance by marriage status(p=0.2218), education(p=0.5733), and taking the certification program for home card nurse(p =0.1560) has no statistically meaningful correlation. 3) The level of nursing performance by the type of home care nursing organizations There exists a significant difference(p=0.002) between performances by the types of organizations. Most of the responsible senior personnels of the home care nursing organization were nurses. The level of nursing performance of the home care nursing organization under nurse management was relatively higher than that of home care nursing organizations led by medical doctors or non-medical professional, but this was not proved as statistically meaningful(p =0.3617). 4) The level of nursing performance by task sectors according to the characteristic of home care nursing service organization There exists a significant difference between nursing performances by task sectors according to the characteristics of home care nursing organization(p=0.002). In case of model research center of one College of Nursing, the nursing performance in the sectors of organization, theory, and data gathering were lower than that of in hospital based home care nursing service. And in case of local home care centers, performances in sectors of organization, theory, data gathering, nursing intervention. professional training, and research sectors were significantly low. Based on the obtained results, overall performance of home care nurses can be appraised as relatively good. Especially, performances in sectors of the nursing intervention, nursing plan. and data gathering including the in direct nursing were recorded high scores. From this, it can be concluded that high quality of nursing is relatively practicing for home patients at these days. Since the high quality of nursing for patients was directly related to the level of nursing performances of home care nurses, it is required to improve practical performance level of them by making constant evaluation and running continual education program and supplementing curriculum for the sectors with low scores.

간호에서의 여성적 윤리에 대한 비판적 탐구 (A critical inquiry on the feminine ethics in nursing)

  • Purpose : This study was to illuminate the main characteristics and limits of the feminine ethics of care when it applies to the nursing ethics, and suggested sufficient conditions of care ethics in nursing in order to actualize the autonomy for nurses. Method : This study inquired the relationship between the ethical trait of caring in nursing and Gilligan & Noddings's moral theory as feminine ethics in relation to supporting the nursing ethics. In contrast to traditional moral theories based on universal principles, Gilligan's moral theory emphasized the conscious of the interpersonal relationship on the basis of the empathy and the responsibility for the other's need in contextual situations, and Noddings developed her ethics based on mothering as a model for the caring relation, the moral feature of which was characterized as reciprocity, receptiveness and responsiveness. Result : The feminine ethics of care came to support nursing ethics considering the nterpersonal relationship and responsibility. However, it did not show a possible ideal of nursing ethics because it has some difficulties in actualizing the nurse's individual and professional autonomy in the health care system. Conclusion : Therefore, in order that ethics of care can be an ideal and universal nursing ethics, it should be studied in proper direction, that is, toward actualization of the autonomy of the universal ethical self in relation to the concern and responsibility for the other.

전북지역 산업장의 제특성에 따른 보건관리 수준에 관한 연구 (A Study on the Actual Conditions of Health Management, and Health Management Levels According to The Types of Industries)

  • The study was conducted with 27 health mangers working in manufacturing industries. The purpose of this study was to analyze the health manager's employment status, health examination, and it's follow. up health management level(about the industrial type). The results are as follows: 1. Characteristics of Manufacturing Industries: The ratio of nurse to employees is 1 : 552.6 and the percentage of physician employed was 51.9% All of the physicians were part time except one. 2. Health Examination and Follow Up ; 1) Periodic screening examinations were provided to 92.39% of the employees. Of these 11.56% required a detailed examination. Follow up on routine cases were done for 51.69%. Follow up on intensive cases were done 13.97%. 2) 62% of all employees working in hazardous conditions(noise, artificial light 74.1%) are required to receive a special health examination bi annually. Of these 96.66% were examined. 11.24% of these employees required a detailed follow up examination. 3. Relevancy between health management level, industrial type, and health manager's status 1) Health clinic operated separately except one case. Nursing activity level :. health diagnosis(0.27) Occupational condition (0.97) Health education(0.81) Health assessment(0.74) Health education level is higher at the industries working in environmental technician(P=0.017). The other's significance is not shown by any type of the staff.

간호사가 인식한 간호관리자의 팔로워십이 간호사의 직무만족에 미치는 영향: 임파워먼트 조절효과 중심으로 (Influence of Nursing Manager's Followership of Nurses' Perceptions on Job Satisfaction of Nurse : Focus on the control effect of Empowerment)

  • 본 연구의 목적은 간호사가 인식한 간호관리자의 팔로워십과 임파워먼트, 직무만족과의 관계를 파악하고, 간호관리자의 팔로워십이 조직구성원인 간호사의 직무만족에 미치는 영향에서 임파워먼트의 조절효과를 확인하기 위함이다. 자료수집은 2019년 3월 2일부터 3월 17일까지 전남 소재 500병상 이상 4개 종합병원에서 근무하는 간호사를 대상으로 하였으며, 자료 분석은 대상자의 일반적인 특성과 측정 변수의 정도는 빈도 분석과 기술통계분석으로 하였다. 일반적 특성에 따른 관련 요인의 차이는 t-test와 ANOVA로 분석하였고, 측정변수들 간의 상관관계 및 다중공선성은 Pearson's correlation coefficient로 분석하였으며, 임파워먼트의 조절효과의 검증을 위해 조절회귀분석을 실시하였다. 연구 결과는 일반적 특성에 따른 대상자가 인식한 간호관리자의 팔로워십과 임파워먼트, 직무만족의 차이에서는 성별에서 임파워먼트와 직무만족은 유의한 차이를 보였고, 교육정도에 따라서는 팔로워십과 임파워먼트, 직무만족에서 모두 유의한 차이를 보였다. 또한 직장 이동 경험에 따른 차이는 직무만족에서 통계적으로 유의한 차이를 보였다. 대상자가 인식한 간호관리자의 팔로워십과 임파워먼트, 직무만족 간에는 모두 정적인 상관관계가 있는 것으로 나타났으며, 팔로워십과 직무만족 간에 임파워먼트가 조절변수로 작용함이 확인되었다. 본 연구의 결론은 최근 대두되고 있는 팔로워십을 간호분야에 적용하여 그 영향력을 검증하였다는 점에서 의의가 있으며, 향후, 조직 구성원들의 리더십 개발 뿐 아니라 팔로워십 증진프로그램 개발과 다양한 변수들을 추가하여 팔로워십에 대한 광범위한 연구가 필요하다.

임상간호사의 간호사 윤리강령 인식의 영향 요인; 생명의료 윤리의식, 도덕적 민감성, 비판적사고 성향을 중심으로 (Factors influencing the recognition of clinical nurses' code of ethics; Focusing on consciousness of biomedical ethics, moral sensitivity, and critical thinking deposition)

  • 본 연구는 임상간호사의 생명의료 윤리의식, 도덕적 민감성, 비판적 사고성향 및 간호윤리강령인식 정도를 확인하고, 간호사 윤리강령 인식에 영향을 미치는 요인을 확인하고자 시도된 서술적 조사연구이다. 대상자의 일반적·윤리적특성은 빈도, 백분율, 평균과 표준편차로 산출하였고, 일반적·윤리적 특성에 따른 각 변수별 차이는 SAS 9.4를 이용하여, independent t-test, one-way ANOVA로 분석하였고, 이후 유의한 차이가 발견된 변수는 Scheffé 사후 검정을 이용하여 다중 비교하였다. 각 변수의 점수는 평균과 표준편차, 최소 값과 최대 값을 이용하였고, 변수 별 상관관계는 Pearson correlation coefficient로 분석하였다. 또한 간호사 윤리강령 인식에 영향을 미치는 요인을 파악하기 위하여 multiple regression으로 분석하였다. 대상자의 일반적·윤리적 특성과 변수 간의 상관관계는 모두 유의한 양의 상관관계를 보였다. 마지막으로 간호사 윤리강령 인식에 가장 크게 영향을 미치는 변수는 비판적 사고성향(p<.001)이었고, 다음으로 생명의료 윤리의식(p<.001), 도덕적 민감성(p<.001) 순으로 영향하는 것으로 나타났으며, 이에 대한 설명력은 27.4%이다. 따라서 임상간호사들에게 비판적 사고성향, 생명의료 윤리의식, 도덕적 민감성 등을 향상시킬 수 있는 교육 프로그램을 개발하고, 이를 적용시킴으로써 임상간호사들이 확고한 윤리적 가치관을 가지고 실무에 임할 수 있도록 도움을 주어야 할 것이다.

임상간호사의 분노표현양상: Q 방법론 적용 (Types of Anger Expression in Clinical Nurses: Q Methodology)

  • Purpose: The purpose of this study was to identify types and characteristics of anger expression in clinical nurses. Methods: Q methodology was used to analyze each individual participant's subjectivity on anger expression. A total of forty clinical nurses arranged 32 pre-selected Q statement cards on a 9 point scale, shaping the cards' pile into a normal distribution. Then the data from the arranged cards were analyzed using the QUANL PC Program. Results: Three types of anger expression were identified; 'reasonably overcoming', 'emotionally expressing', and 'internally mitigating' including both positive and negative aspects. Conclusion: Adequate strategies to resolve anger in clinical nurses needs to be developed according to anger expression types.

요양병원 간호사의 직무만족이 이직의도에 미치는 영향 (The Influence of Job Satisfation on Turnover Intention of Nurses in Geriatric Hospital)

  • This study aims to analyze the effects of job satisfaction on turnover intention among the nurses in geriatric hospitals. In terms of study methods, this study developed structured questionnaires, and distributed them to nurses working in 11 geriatric hospitals in metropolitan area. A total of 418 questionnaires (response rate: 87.0%) were used for final analysis. We conducted multiple regression analyses to analyze the influence of job satisfaction on turnover intention, adjusting for the characteristics of the research subjects. The results of this study are as follows. First, both job satisfaction and turnover intention were statistically significantly related to individual and hospital characteristics. Second, turnover intention was lower in older age group, and higher in smaller hospitals. As job satisfaction increases, turnover intention decreases. Third, the results of the regression analysis adjusting for subject's characteristics and job satisfaction's sub-categories show that the turnover intention is lower in nurses who are older, day worker, and work in bigger hospitals. Fourth, the result of subgroup analysis by hospital volume show that both of two groups are influenced by hospital ownership and satisfaction about compensation. In addition, professionalism had a significant effect on decreasing turnover intention in smaller hospitals. However, there was no statistically significant relationship between job satisfaction's sub-categories and turnover intention in larger hospitals. The results of this study confirms that job satisfaction lowers turnover intention among nurses in geriatric hospitals. It is suggested that the administrators of those hospitals continue to monitor critical factors associated with job satisfaction, and thus, reduce turnover intention, which helps improve quality of services.

정신요양 병동에서의 관리영역과 환자의 요양 공간영역의 관찰적 시선에 따른 병동구조와 요양 공간환경의 특성에 관한 연구 (Characteristics of Spatial Environment in Psychiatric Wards through the Analysis by Patient's Preferred Healing Environment in Observational Ward Structure Dividing Staff Area and Patient Area)

  • Purpose: The relationship between the staff area and the patient's private area is the key factor in designing the structure and the environmental characteristics of ward spaces in a psychiatric hospital. Recent research has found that for the purpose of treatment and securing privacy, psychiatric patients need to be in an open space of relief rather than closed confined environment and under the watch of nursing staffs. Methods: A survey at three kinds of wards in a private psychiatric hospital in Japan was conducted in October 2002. These wards include an acute ward, a psychiatric convalescence ward, and a stress care ward. All three kinds of wards have the same structure. At each ward, spatial preferences of the 145 psychiatric inpatients were surveyed and data concerning the patient's diagnostic category, symptoms, and activities of daily living were recorded. Results: The patients in the stress care ward prefer to stay in private spaces than public spaces. On the other hand, in the acute ward the patients seem to have a preference between managed public spaces where are monitored by nursing staffs and their private rooms where the nurse station is close. In addition, the patients in the psychiatric convalescence ward spend most of their time in the public space, such as the hallways or the day room. Implications: Base on this research, the spaces at the acute ward that could be monitored by the nursing station serves effectively as a safety space for patients was concluded. However, in the stress care ward, the patients may perceive the monitoring by the nursing staff as interruption or nuisance to their relaxation. In order to design an ideal healing environment for psychiatric patients in psychiatric ward, it is important to consider how environmental characteristics of space affect the environmental sense of patients in each ward.

간호사가 인식한 조직의 특성과 투약오류보고장애요인간의 정준상관관계 (Canonical correlation between organizational characteristics and barrier to medication error reporting of nurses)

  • 본 연구는 간호사들이 인식한 안전 분위기와 업무환경을 포함한 조직의 특성과 투약오류보고장애요인간의 관련성을 검증하기 위한 연구이다. 수정된 안전 분위기, 업무환경, 투약오류보고장애 문항을 활용하여 7개 병원의 334명의 간호사들을 대상으로 하여 조사하였다. 자료수집기간은 2012년 9월 한달 간이었고, 서술적 통계, Pearson correlation coefficient, canonical correlation을 적용하였다. 조직의 특성은 투약오류보고장애와 세 가지 유의한 상관이 있었다. 첫 번째 정준상관계수는 .50(Wilks' ${\lambda}$=0.61, df=32, p<.001)이었고, 두 번째는 .35(Wilks' ${\lambda}$=0.81, df=21, p<.001), 세 번째는 .22(Wilks' ${\lambda}$=0.93, df=12, p=.018)이었다. 첫 번째 정준상관은 높은 안전 분위기와 업무환경이 오류보고에의 두려움을 제외한 투약오류보고와 관련이 있었다. 두 번째 식은 '의료인간의 안전 분위기'의 높은 지각과 높은 '병원에의 간호사 참여'와 '인적 자원의 적절성'은 낮은 '불안감'과 '행정자들의 반응'과 관련이 있었다. 투약오류보고장애를 위한 전략과 안전 분위기와 업무환경과 같은 조직의 특성의 진보가 적용되어야 한다.
