A Case of Atopic Dermatitis with Nummular Eczema (화폐상습진을 동반한 성인형 아토피피부염의 한방 치험 1례)

  • Introduction : Atopic dermatitis(AD) is chronic eczematous dermatitis which is related strongly to a family history of atopic disease and hereditary disposition. Adolescence and adult atopic dermatitis (AAD) patients have been overlooked because they were relatively small population and AAD were considered merely consecutive of infant and childhood AD. Objective : The purpose of this study was to estimate the efficacy of oriental medical treatment and management on chromic AAD with nummular eczema and breast eczema. Methods : We tented this case with herb-medication, herb-irrigation, acupuncture, negative therapy, full spectrum visible lay therapy (by carbon arc) and external application (SA1, Siwon Ilhwang-Go). We observed for 7 months. Results : The grades of the severity of AD with nummular eczema, breast eczema were lower only by the oriental medical treatments and managements. Conclusion : Considering the above results, more study is needed on the case with AAD with diffuse nummular eczema, effectiveness of external application and correlation between immunological and results of oriental media treatments.

A Case of Acute Allergic Contact Dermatitis with Multiple Nummular Eczema (화폐상 습진에 병발한 급성 알레르기성 피부염 한방 치험 1례)

  • Objective : The purpose of this report is to know the effect of oriental medical treatments on acute allergic contact dermatitis with multiple nummular eczema. Methods : We treated a 34-years-old woman patient with acute allergic contact dermatitis on face and multiple nummular eczema on both arm, right ankle and left inguinal region by Korean medicine. After Korean medical treatment for 42 days in hospital, we measured the decrease of exudation and pruritis as well as recovery of skin damages. We recorded pictures of changes on symptoms. Results & Conclusion : The symptoms of ACD and nummular eczema were significantly improved except scars. Exudation and pruritis were disappeared and skin damages were almost recovered until discharge from hospital. After 20 days we could confirm the symptoms were clearly disappeared. Thus Korean medical treatments are effective on ACD and nummular eczema.

  • Objectives : The purpose of this study is to transcribe the effect of Korean medical treatment on nummular dermatitis with allergic dermatitis.Methods : A 42-year-old man patient suffering from nummular dermatitis with allergic dermatitis was treated with herbal medicine, acupuncture, etc. We observed progress of patient with Visual Anologue Scale(VAS) and used comparative observation through camera photographing.Results : After Korean medical treatment, patient's chief complaints with erythema, edema, pruritus, and keratin were significantly declined.Conclusions : Korean medical treatments were an effectual cure for nummular dermatitis with allergic dermatitis.

A Case of Chronic Nummular Dermatitis Improved by Hwangryunagyo-tang (황련아교탕(黃連阿膠湯) 투여로 호전된 만성 동전 모양 습진 1례)

  • Objectives: This study aims to report a case of chronic nummular dermatitis improved by Hwangryunagyo-tang. Methods: The patient in this study was a 15-year-old female, who visited our hospital with symptoms such as redness, erythema, papule, excoriation, scale, exudation, crusting, lichenification, and pruritus of the extremities and trunk. Considering that the skin lesions were coin-shaped and persisted for more than 6 months, the patient's symptoms were diagnosed as chronic nummular eczema. Based on the patient's symptoms and overall condition, Hwangryunagyo-tang was prescribed. Results: Approximately 4 months after administration of Hwangryunagyo-tang, the exudate disappeared, and overall skin symptoms improved, from objective score 33.7 to 6.6, leaving only slight scale, dryness, and pigmentation. As the pruritus of the skin lesion decreased from numeric rating scale 8 to 1, the patient's sleeping condition also improved from numeric rating scale 9 to 0. Conclusion: This study is significant in that the symptoms of chronic nummular dermatitis were effectively improved after administration of Hwangryunagyo-tang.

Nummular Eczema Treated by Hoeyeok-tang Based on the Shanghanlun Disease Pattern Identification Diagnostic System (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 회역탕(回逆湯) 투여 후 호전된 화폐상 습진 증례 1례)

  • Objective: This study aimed to report the improvement of a patient with nummular eczema treated with Hoeyeok-tang based on Shanghanlun provisions. Methods: According to the disease pattern identification diagnostic system (DPIDS) based on the Shanghanlun provisions, the patient was diagnosed with lesser yin disease according to the 324th provision. The patient was treated with Hoeyeok-tang for 90 days. The severity of pruritus decreased, and changes in symptoms were estimated by a visual analog scale, Scoring Atopic Dermatitis Index and the Validated Investigator Global Assessment for Atopic Dermatitis. Results: The patient's symptoms were improved. The severity of pruritus decreased and they recovered from abrasions. Conclusions: This study suggests that the 324th provision of Shanghanlun refer directly to nummular eczema.

A Case of Atopic Dermatitis Accompanying Seborrheic Capitis and Nummular Eczema (두피 지루피부염과 화폐상 습진을 동반한 아토피피부염 환자 치험 1례)

  • Objectives : The objective of this study is to report a case of a male atopic dermatitis patient accompanying seborrheic capitis and nummular eczema improved by Korean medicine therapy and lifestyle modification. Methods : A male patient was hospitalized for eczematous lesions in the head, face, and both hands which relapsed on March 2021. For 15 days, he took Korean medicine therapy including acupuncture, Bangpungtongseongsan-gagam, pharmacopuncture, and wet dressing with Hwangryunhaedok-tang. Simultaneously, lifestyle correction also conducted during administration. On the other hand, corticosteroid and antihistamine were prescribed from internal medicine of our hospital for the first 10 days because of severe skin lesions. As an outpatient, he was continuously treated by the same Korean medicine therapy except herbal decoction weekly for about 7 months after discharge. To assess symptoms, scoring atopic dermatitis(SCORAD) index, taking photos, and numerical rating scale(NRS) were used. Results : After 15 days of hospitalization, the SCORAD index decreased to 30.0, which was about a half of the initial SCORAD index(61.2). NRS score also dropped from 6 to 3. Despite stopping western medicine administration, skin lesions and subjective symptoms of the patient were steadily improved without aggravation. For 7 months of continued outpatient treatment, atopic dermatitis were steadily ameliorated despite temporary aggravation and improvement of symptoms, and seborrheic capitis was not relapsed. The final SCORAD index and NRS on November 9th, 2021 were 24.7 and 1, respectively. Conclusions : These results suggests that Korean medicine therapy contributes to improving SCORAD index, subjective symptoms, and skin lesions of the patient. Furthermore, lifestyle modification is also important as much as proper treatment for caring atopic dermatitis patients.

  • Nummular eczema is well-shaped, coin-like lesion. Usually it is surrounded by the papules and vesicles, and is likely to be the impetigo. It's said that nummular eczema is combined with atopic dermititis, contact dermatitis, bacterial infection and so on. Recently, we experienced a nummular eczema with impetigo and that was successfully treated by the herb-medication and the ceramic resonance therapy. The medications taken by the patient were yunwhasagnatang and gamisopungsan which is used to treat allergic disease and make immune system be strong. The former effects the liver to be cool down and healthy and it also has anti-imflamatory reactions like the corticosteroid hormone. Ceramic resonance therapy utilises irradiation within the frequency range of the radiation emitted by the human body. So we report this case with a brief review of the oriental medical and medical literatures.

  • "Chimumchang" seems to be a kind eczema. The cause and symptom of eczema is similar to that of chimumchang. so, I investigated the literatures of oriental medicin and recent medicine. The results are summarized as follows. 1. Chimumchang seems to be a kind of eczema. Allergic contact dermatitis, atopic dermatitis, seborrheic dermatits, neurodermatitis, nummular dermatitis, xerotic eczema and so on are similar to chimumchang on cause & symptom. 2. The cause of chimumchang are summer vital energy excess(夏脈太過), sehwa excess(歲火太過), herat meridian wind-heat(心經風熱), heart fire(心火), wind taken spleen dampnees(脾濕受風), and so on. The cause of eczema are contact of external agent, immunological problem, heridity, infection, abnormalities of sebaceous glands, neurologic dysfuction, and so on. 3. The symptoms of chimumchang have been described to the three pattern on literature of oriental medicine until now. 1) scratching slightly, It has exudation, and combination. 2) Vital energy on skin is irritable by psychological problem. At the acute stage the patient is complain of itching & pain, and gradually the exudation expands in whole body. 3) early peorid it's size is very small, and at first the patient is complain of itching, later complain of pain. The exudation change to ulcer and expands in the whole body. 4. In the investigation of prescription(Total: 34, Oral medicine: 4, Ointment: 30), the most frequently used oral medicine is sungmatang(4 times), and the most frequently used ointment are kohosan, korvunsan and kegwanhyuldobang.

A Case Study on Xerotic Eczema Treated by Oriental Medicine (수부(手部) 건성습진(乾性濕疹)에 대한 한방치료(韓方治療) 증례(證例) 보고(報告))

  • Eczema is a general term for skin diseases showing itching, vessiculopapular eruption, and erythema when acute, or lichenification and scale if chronic. It is classified as irritant dermatitis, atopic dermatitis, seborrheic dermatitis, neurodermatitis, nummular eczema, statis dermatitis, infective dermatitis, xerotic eczema, and pompholyx by afflicted regions and symptoms. Therapy for eczema is allopathic, and includes application of ice- pack to erythema, and application of emollients and steroids to the affected parts during subacute or chronic stages. Eczema is understood as ulceration(瘡), tinea(癬), wind syndrome(風證), and dryness syndrome(燥證) in terms of oriental medicine. Xerotic eczema is understood in dryness syndrome. Nourishing blood(養血) therapy is used. This is a case study on which xerotic eczema occurred on a patient's afflicted left palm and fingers after stroke. The patient's xerotic eczema was attributed to dryness syndrome. Therefore, Samul-tang(四物湯) and Saenghyeoryunbu-eum(生血潤膚飮) were used for nourishing blood. This therapy was effective on eczema and accompanying symptoms such as dizziness and numbness of the left hand.

A Study on the Skin Disease of Sanghanron 《傷寒論》 and Geumgweyoryak 《金匱要略》 (《상한론(傷寒論)》, 《김궤요략(金匱要略)》의 피부증상(皮膚症狀) 및 질환(疾患)에 대한 연구(硏究))

  • Objective : The purpose of this study is to investigate the skin disease in Sanghanron"傷寒論" and Geumgweyoryak"金匱要略". Methods : We conducted a study on the original text paragraphs of Sanghanron and Geumgweyoryak containing the skin disease and analysis of Woo, Oh, Sung, Zhang, etc. We drew a parallel between skin disease from Sanghanron and Geumgweyoryak and matching diagnoses from western medicine. Results : The results were as follows. 1. In Sanghanron and Geumgweyoryak pruritus was related to psychogenic pruritus and similar to skin disease caused by decrease of sweating, dry skin in cholinergic urticaria, atopic dermatitis and eczema, etc. in western medicine. 2. In Geumgweyoryak ichthyosis was caused by woman's disease, after menopause, chronic disease and malnutrition and related to winter itch and xerotic eczema in western medicine. 3. In Sanghanron Goose bumps, keratosis follicularis, acute and chronic urticaria and cold urticaria were occurred because of poorly managed fever care and side effect of antibiotic. 4. In Geumgweyoryak red face was similar to face flushing and side effects, which is after drug and laser treatment on face, in western medicine. 5. In Geumgweyoryak urticaria was, in a broad sense, skin disease with pruritus and secondary infection resulted from scratch. Skin diseases with pruritus are similar to urticaria, dermatitis herpetiformis, lichen planus, atopic dermatitis, contact dermatitis, psoriasis, nummular eczema, lichen simplex chronicus, prurigo nodularis, neurodermatitis and internal disease with pruritus in western medicine. 6. In Geumgweyoryak spots in the skin were caused by fever and similar to allergic purpura and SLE symptom in western medicine. 7. In Geumgweyoryak bullous disease with yellowish discharge was similar to eczema, pustule, atopic eczema, etc. in western medicine. 8. In Geumgweyoryak repeated inflammation of oral cavity, genitals, eyes and skin was similar to Behcet's disease in western medicine. 9. In Sanghanron and Geumgweyoryak boil was similar to abscess and acute lymphadenitis in western medicine caused by oily foods and infections. 10. In Geumgweyoryak swelling and boil were occurred in a poor health. Damage from metalic material could cause a convulsion and was similar to tetanus in western medicine. Conclusion : We analyzed the original text paragraphs of Sanghanron and Geumgweyoryak and explanations about skin disease. As a results, we found out etiology, pathogenesis, treatments of the skin disease in Sanghanron and Geumgweyoryak. Further we compared with western medicine to develop better understanding of the skin disease.
