미숙아 호흡곤란증후군에서 폐표면활성제의 조기 투여와 연성 환기요법이 만성폐질환의 예방에 미치는 영향에 관하여 (The effects of early surfactant treatment and minimal ventilation on prevention of bronchopulmonary dysplasia in respiratory distress syndrome)

  • 목 적 : 폐표면 활성제의 조기 투여와 연성 환기 요법에 의한 인공호흡기 치료 단축에도 불구하고 장기간 산소 보충이 필요한 만성 폐질환은 여전히 해결해야 할 난제이다. 저산소의 사용, 연성 환기에 의한 호흡기 치료 기간의 단축이 폐 손상을 줄이는데 기여하나 예방에는 한계가 있다고 한다. 이에 저자들은 폐표면 활성제의 조기 사용 및 연성 환기, 공격적인 기도 발관을 통한 인공호흡기 치료 기간의 단축이 만성 폐질환의 발생에 미치는 영향에 관해서 알아보고자 하였다. 방 법 : 2001년 1월부터 2006년 12월까지 본원 신생아실에 입원한 재태 연령 36주 이하 출생체중 1,500 g 이하의 환아 139예를 대상으로 병력지를 기초로 후향적으로 조사하였다. 2단계 이하인 경한 호흡곤란 증후군이 있는 환아를 대상으로 폐표면 활성제는 2시간 이내에 조기 투여하였으며 인공호흡기 치료 기간은 대부분 3일 미만으로 상태가 안정되는 즉시 공격적인 기도 발관을 시행하여 만성 폐질환의 발생률을 조사하였다. 결 과 : 총 139예의 환아 중 2단계 이하인 경한 호흡 곤란 증후군에서 조기 폐표면 활성제 치료를 시행한 대상 환아는 총 82예(59%)였으며 인공호흡기 치료 기간이 길수록 RDS의 정도가 심할수록 출생당시의 상태가 좋지 않을수록 만성 폐질환 발생이 증가하였다(P<0.001). 경증의 호흡 곤란 증후군에서 조기 폐표면 활성제를 사용하고 2일 이하로 조기 발관을 시행한 그룹에서도 상당한 수에서 경증 만성 폐질환은 발생하는 것을 알 수 있다. 출생당시 상태가 좋지 않아 기도 삽관을 시행한 그룹에서 출생 당시 상태가 비교적 양호한 군에 비해서 인공호흡기 치료 기간이 길었음을 알 수 있었다(P=0.020). 결 론 : 조기 폐표면 활성제의 투여와 연성 환기에 의한 인공호흡기 치료 기간의 단축만으로는 여전히 저 농도의 산소가 필요한 경증의 만성 폐질환 예방에는 한계가 있으며 출생 당시의 초기 처치가 만성 폐질환의 발생에 영향을 미치는 것으로 보아 향후 분만실에서 조건 및 nasal canula를 통한 초기 치료의 질적 향상과 nasal CPAP의 적극적 활용이 필요할 것으로 사료되며 nasal canula를 통한 저 농도 산소 공급역시 사용기간을 줄이는데 많은 노력을 해야 할 것으로 생각된다. 하지만 본 연구에서는 만성 폐질환에 영향을 줄 수 있는 여러 요인들을 복합적이 데이터 분석이 이루어지지 못하였으며 대조군을 통한 비교 연구가 되지 않은 점은 문제점이라 생각되며 앞으로 이에 대한 더 많은 연구가 필요할 것으로 사료된다.

자연 기흉의 개흉례에 대한 임상적 고찰 (Clinical Evaluation of Open Thoracotomy Cases in Spontaneous Pneumothorax)

  • 자연기흉은 장측 늑막 기포의 공기 유출로 인해 폐가 허탈된 상태이다. 폐쇄식 흉강삽관술,흉강천자술 및 단순한 관찰이 효과적인 치료이다. 그러나 이러한 치료가 비효과적일 때, 개홉술을 하게된다. 저자들은 자연 기흉이 흉부외과적 영역에서 매우 흔하게 접하는 질환이기에, 개흉례의 수술 적응증에 따른 병리소견을 비교함이 향후 수술적응에 도움이 될것으로 생각되어 본 연구를 시작했다. 1988년부터 1996년까지 9년동안 마산삼성병원 흉부외과에 입원 치료한 기흉 환자 중에서 개흉술을 시행한 242례(236명)에 관한 임상적 고찰을 하였다. 결과는 다음과 같다. 1. 남녀비는 11.7:1로 남자가 현저히 우세했다. 2. 가장 빈발한 연령층은 10대와 20대이었다(10대=29.3%,20대=30.2%). 3. 가장 흔한 증상은 흉통과 호흡곤란이었다(흉통=41.7%,호흡곤란=36.8%). 4. 자연기흉의 원인은 원발성 자연기흉(86.4%)이 제일 많았고, 속발성 자연기흉으로는 결핵(9.1%),폐기종 및 만성 폐쇄성 폐질환(3.7%) 등이 있었다. 5. 자연기흉의 발생위치는 우측 52.1% 좌측 45.4%, 양측 2.5%이었다. 6. 개흉술의 가장 흔한 적응증은 재발(44.2%), 지속적 공기유출(31.8%) 및 奴舅\ulcorner폐확장이었다(15.7%). 7. 수술수기는 후흉회측절개 및 정중흉골절개를 통한 개흉술을 주로 시행했다. 8. 수술시 기포가 가장 많았던 곳은 우상엽 첨구역(35.1%)와 좌상엽 후첨구역(41.3%)이었다. 9. 수술시 기포수는 대개 1~5개(88%)이었고, 수술 적응증에 따른 차이는 없었다. 10. 수술시 기포 크기는 대개 5cm(81%) 기하였으며, 수술적응증이 재발된 기흉이었던 경우는 작은 기포(1cm 이하)가 많았고, 지속적 공기유출 및 불완전 확장이었던 경우는 비교적 큰기포(5cm초과)가 많았다. 11, 늑막유착은 54.5%에서 있었고,수술적응증이 재발인 경우 64.1%, 지속적 공기 유출인 경우 11.9%,불완전 확장인 경우 47.4%이 었다

BERTopic을 활용한 불면증 소셜 데이터 토픽 모델링 및 불면증 경향 문헌 딥러닝 자동분류 모델 구축 (Topic Modeling Insomnia Social Media Corpus using BERTopic and Building Automatic Deep Learning Classification Model)

  • 불면증은 최근 5년 새 환자가 20% 이상 증가하고 있는 현대 사회의 만성적인 질병이다. 수면이 부족할 경우 나타나는 개인 및 사회적 문제가 심각하고 불면증의 유발 요인이 복합적으로 작용하고 있어서 진단 및 치료가 중요한 질환이다. 본 연구는 자유롭게 의견을 표출하는 소셜 미디어 'Reddit'의 불면증 커뮤니티인 'insomnia'를 대상으로 5,699개의 데이터를 수집하였고 이를 국제수면장애분류 ICSD-3 기준과 정신의학과 전문의의 자문을 받은 가이드라인을 바탕으로 불면증 경향 문헌과 비경향 문헌으로 태깅하여 불면증 말뭉치를 구축하였다. 구축된 불면증 말뭉치를 학습데이터로 하여 5개의 딥러닝 언어모델(BERT, RoBERTa, ALBERT, ELECTRA, XLNet)을 훈련시켰고 성능 평가 결과 RoBERTa가 정확도, 정밀도, 재현율, F1점수에서 가장 높은 성능을 보였다. 불면증 소셜 데이터를 심층적으로 분석하기 위해 기존에 많이 사용되었던 LDA의 약점을 보완하며 새롭게 등장한 BERTopic 방법을 사용하여 토픽 모델링을 진행하였다. 계층적 클러스터링 분석 결과 8개의 주제군('부정적 감정', '조언 및 도움과 감사', '불면증 관련 질병', '수면제', '운동 및 식습관', '신체적 특징', '활동적 특징', '환경적 특징')을 확인할 수 있었다. 이용자들은 불면증 커뮤니티에서 부정 감정을 표현하고 도움과 조언을 구하는 모습을 보였다. 또한, 불면증과 관련된 질병들을 언급하고 수면제 사용에 대한 담론을 나누며 운동 및 식습관에 관한 관심을 표현하고 있었다. 발견된 불면증 관련 특징으로는 호흡, 임신, 심장 등의 신체적 특징과 좀비, 수면 경련, 그로기상태 등의 활동적 특징, 햇빛, 담요, 온도, 낮잠 등의 환경적 특징이 확인되었다.

다제내성 결핵 균주에서 리팜핀과 리파부틴간의 교차내성률 및 rpoB 유전자 돌연변이와의 연관성 (Cross-resistance Between Rifampicin and Rifabutin and Its Relationship with rpoB Gene Mutations in Clinically Isolated MDR-TB Strains)

  • 목 적 : RFP과 RBU 사이의 교차 내성률은 다양하게 보고되고 있으며 rpoB 돌연변이가 이에 관여하는 것으로 알려져 있다. 본 연구에서는 본원에서 동정되어 보관중인 다제내성 결핵균주를 대상으로 하여 두 약물간의 교차 내성률 및 rpoB 돌연변이와의 연관성을 조사함으로써 다제내성 결핵의 치료에 있어 RBU의 효용성에 대하여 알아보고자 하였다. 방 법 : 2004년 한 해 동안 본원 검사실에서 다제내성 결핵균으로 동정되어 보관중인 130균주를 대상으로 하였다. RFP과 RBU의 내성검사는 L-J 배지를 이용한 절대농도법으로 시행하였으며 추가로 다음과 같은 다섯 가지의 농도를 이용하여 RBU의 MICs를 조사하였다; 10, 20, 40, 60, $120{\mu}g/ml$. 내성기준농도는 RFP의 경우 $40{\mu}g/ml$, RBU의 경우 $20{\mu}g/ml$로 하여 내성유무를 판정하였다. rpoB 돌연변이는 LiPA법을 이용한 REBA $MTB-Rifa^{(R)}$검사로 조사하였으며 염기서열분석을 의뢰하여 그 결과를 검증하고 구체적인 개별 돌연변이양상을 알아보았다. 결 과 : RFP은 모두 내성으로 확인되었고 RBU의 $MIC_{50}$$80{\mu}g/ml$, $MIC_{90}$${\geq}160{\mu}g/ml$였으며 RFP과 RBU간의 교차내성률은 70.5%였다. REBA $MTB-Rifa^{(R)}$검사 결과 rpoB 돌연변이는 대부분 코돈 524-534 사이에서 발생하였고 검사를 시행한 100균주 가운데 98개의 균주에서 돌연변이가 확인되어 RFP내성을 진단할 수 있는 진단율은 약물감수성검사와 비교하여 98%의 일치율을 보였다. 염기서열분석결과 코돈 531과 513의 돌연변이는 돌연변이의 양상에 관계없이 항상 RBU내성과 관련되어 있었던 반면 코돈 526의 돌연변이는 돌연변이의 양상에 따라 내성 혹은 감성과 관련되어 있었다. 가장 흔한 돌연변이는 Ser531Leu로 전체의 45.5%를 차지하였다. 약물감수성검사에 비추어 His526Gln, His526Leu, Leu533Pro, Gln513Glu, Leu511Pro가 감성 돌연변이로 판단되었다. 결 론 : 두 약제간의 내성률을 고려하여 볼 때 RBU은 일부 다제내성 결핵의 치료에 있어서 효과가 있겠다. 우선 RBU에 대한 전통적인 약물감수성 검사를 도입하여 적절한 내성기준농도를 확립하는 노력이 필요하며 현재까지 rpoB 유전자 검사법은 임상에 적용하기에 한계가 있는 것으로 사료된다.

비디오 흉강경을 이용한 심낭막 개창술 (Video Assisted Thoracic Surgery(VATS) of Pericardial Window Operation)

  • 심낭삼출은 결핵 등의 감염성 질환, 악성 신생물, 개심술, 요독증 등 다양한 병변들로 인해 발생되며 이에 대한 진단 및 치료적 방법으로서 심낭천자술 및 심낭막 개창술 등이 시행되고 있다. 최근 비디오 흉강경술(VATS)은 심낭삼출을 비롯한 흉강내 여러 질환에 대해서 새로운 효과적인 수술방법으로 각광받고 있다 개흉술시의 제한적 수술시야를 비디오 흉강경으로서 극복할 수 있다는 점, 심낭막 및 심낭내 병변에 대한 전체적 관찰이 용이하고 수술 후 통증이 적다는 점, 미용적 효과와 재원 기간의 단축 등이 그 이유이다. 울산대학교 서울아산병원 흉부외과에서는 1995년 3월부터 2001년 8월까지 심낭막 개창술을 시행한 환자 55례에서 VATS로 시행한 40례 중 기존 상병의 악화로 30일 이상 흉관을 거치한 3례를 제외한 37례(A군)와 개흉술 15례 중 1례를 제외한 14례(B군)를 구분하여 수술시간, 흉관 거치기간, 재발율 및 재원기간 등을 비교 고찰하였다. A군은 남자18명, 여자19명이었으며 나이는 20세부터 80세까지 평균 56.4세였고 B군은 남자 8명 여자 6명으로 나이는 34세에서 77세까지 평균 58.4세였다. A군의 수술 시간은 18분부터 155분까지 평균 61.2분이었으며 흉관제거는 수술 후 2일부터 24일 까지 평균 9.3일만에 제거하였고 재발율은 2.7%였으며 재원기간은 5일에서 39일까지 평균 16.2일이었다. B군의 수술시간은 30분부터 100분까지 평균 58.4분이었고 흉관 제거는 수술 후 6일에서 28일 까지 평균 12.2일만에 제거하였고 재발율은 7.1%였으며 재원기간은 8일에서 34일 까지 평균 17.3일이었다. 통증의 정도는 술 후 진통제를 근주 및 정주 한 횟수로 비교해 볼 때 A군은 평균 4.2회 B군은 평균 6.3회로 A군에서 더 진통제의 사용이 적었다. 평균 재원 기간, 흉관 거치기관을 비교해 볼 때 비디오 흉강경술의 경우에서 짧았으나 두 군간의 유의한 차이는 보이지 않았다. 수술시간 및 재발율 또한 두 군간에 유의한 차이를 보이지 않았다. 본 저자들은 심낭삼출의 치료로서 비디오 흉강경술은 안전하고 입원기간, 흉관 거치기간, 통증의 정도가 적으며 피부 창상이 적어 개흉술을 대치할 수 있다고 사료된다.

High Resolution HC$_3$N Observations toward the Central Region of Sagittarius B2

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500,550,570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500,550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100,300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. 치과코디네이터의 업무수행 및 인식도에 관한 조사연구 (A Study on the Job Performance of Dental Coordinators and Their Perception)

  • 서울, 경기, 인천 지역을 중심으로 치과코디네이터가 근무하는 치과병 의원 선정하여 현직 치과코디네이터들을 대상으로 치과코디네이터의 업무수행 정도와 인식도를 조사하여, 보다 효율적인 인력활용 방안을 마련하는 기초를 제공하고자 2005년 5월 1일부터 8월 8일까지 설문지를 통하여 자료를 수집한 후 회수된 108부를 분석한 결과는 다음과 같다. 1. 응답한 치과코디네이터들의 치과근무기간은 5년 이상이 43.5%, 2년 미만이 19.5%, 3년 이상 5년 미만이 19.4%의 순으로 나타났고, 치과코디네이터로서의 업무기간은 2년 미만이 39.8%, 2년 이상 3년 미만과 5년 이상이 각 19.4%의 순으로 나타났다. 그리고 현재 불리워지는 명칭으로는 실장(팀장)이 38%, 코디네이터가 30.6%이었으며, 치과코디네이터로 담당하는 세부 업무로는 리셉션이 30.6%로 가장 높았고, 소속된 부서는 진료지원팀이 57.4%로 가장 높게 나타났다. 2. 교육관련 사항으로는 치과코디네이터가 되기 위해 가장 많이 교육을 받은 기관으로는 45.4%가 사설기관이고, 응답자의 73.1%가 공인된 치과코디네이터 자격시험이 필요하다고 응답하였다. 또한 자격인정을 위한 적절한 공인기관으로는 중앙부처라고 응답한 율이 43.5%로 가장 높았고, 응답자의 70.8%는 이수한 업무교육 내용이 직무수행에 적합했다고 응답하였다. 치과코디네이터 업무능력 향상을 위한 지속교육 필요 여부는 96.3%가 "예"라고 응답하였고, 그 이유는 능력향상을 위해서가 63.9%, 체계적인 교육을 위해서가 22.2였다. 교육비 부담은 근무기관에서 총 교육비의 일정액 보조가 29.6%, 전액 자비 부담이 25.9%였다. 치과코디네이터 교육과정 중 필수 이수항목에서는 의료서비스 마케팅이 66.7%, 치과코디네이터 이론과 실무가 65.7%, 치과의료기초 57.4%의 순이었고, 보완을 희망하는 교육항목은 치과의료서비스 마케팅이 46.3%, 건강보험실무가 35.2%였다. 3. 치과코디네이터로서 현재 수행하는 업무는 고객관리 분야에서는 예약관리가 88.9%, 자기관리 분야에서는 서비스기본매너 갖추기가 87.9%, 원무관리 분야에서는 수납이 81.3%로 높게 나타났다. 4. 치과코디네이터의 수행업무에 대한 인식으로는 '현재 수행하고 있는 직종에 자부심을 가지고 있다($3.99{\pm}0.76$)', '치과코디네이터 업무는 경영 기여도가 높다고 생각한다($3.92{\pm}0.70$)', '내가 수행하는 업무는 전체 치과병 의원 업무에서 차지하는 비중이 크다($3.91{\pm}0.84$)', '나는 직원들과 직급에 관계없이 잘 지낸다($3.86{\pm}0.74$)', '업무를 통하여 환자의 구강건강 증진에 많은 도움이 되고 있다고 생각한다($3.76{\pm}0.75$)', '내 직업은 미래 전망이 밝다($3.74{\pm}0.86$)' 순으로 높게 나타났다. 5. 치과코디네이터의 연령별로 인식을 살펴보면 대체적으로 모든 항목에서 연령이 높을수록 업무에 대한 인식도가 높은 것으로 나타났고, '내가 수행하는 업무는 전체 치과병 의원업무 차지하는 비중이 크다'(P < 0.001), '수행하는 업무에 대하여 경영자의 인정과 신뢰를 받는다'(P < 0.01), '현재 수행하고 있는 직종에 자부심을 가지고 있다', '내 직업에 대한 사회적 인지도가 높다', '스텝들은 치과코디네이터들이 하는 일에 대해 이해하고 인정한다', '치과의사들은 치과코디네이터들이 하는 일에 대해 이해하고 인정한다', '현재 불리워지는 직명에 만족한다', '내 직업은 나이의 제한을 받지 않는다고 생각한다', '치과 코디네이터 업무는 경영 기여도가 높다고 생각한다'(P < 0.05)의 항목에서는 연령별로 통계적 유의성이 유의한 차이를 보였다. 6. 치과코디네이터의 직종별로 업무에 대한 인식을 살펴보면 대부분의 항목에서 치과위생사, 간호조무사, 기타 순으로 업무에 대한 만족도가 높은 것으로 나타났다. 그리고 '업무를 수행함에 있어서 업무관련 결정을 내가 하고 있다'(P < 0.001), ' 내가 수행하는 업무는 전체 병원업무에서 차지하는 비중이 크다', '내 업무는 나의 능력을 향상시켜 준다', '업무를 통하여 환자의 구강건강건강 증진에 많은 도움이 되고 있다고 생각한다', '현재 받고 있는 보수에 만족한다', '스텝들은 치과코디네이터들이 하는 일에 대해 이해하고 인정한다'(P < 0.01), '내 직업에 대한 사회적 인지도가 높다', '업무 수행시 스텝과의 갈등이 없다', '치과병 의원에서는 치과코디네이터의 능력향상을 위한 자기개발 기회를 주고 있다'(P < 0.05)의 항목에서 통계적으로 유의한 차이를 보였다.

우리나라 기생충병관리의 현황(現況)과 효율적방안에 관(關)한 연구(硏究) (The Present Status and a Proposal of the Prospective Measures for Parasitic Diseases Control in Korea)

  • 우리나라에서 공중보건상(公衆保健上) 중요(重要)한 연충류감염에 대(對)하여 1969년(年) 현재(現在) 실시(實施)한 관리(管理) 현황(現況)을 조사(調査)하였으며, 그 결과(結果)를 보면 1966년(年)부터 1969년(年)까지의 학생기생충검사 및 회충구충 사업(事業)에 있어서 그 간(間) 회충감염률의 저하(低下)를 별(別)로 가져오지 못하였음이 나타났으며 따라서 앞으로 본(本) 사업(事業)의 개선(改善) 또는 보다 적극적인 사업(事業)의 추진(推進)이 요망(要望)된다. 보건사회부(保健社會部)에서 지정(指定)한 지역(地域)에서 $1965{\sim}1968$년(年)의 4년간(年間)에 실시(實施)한 폐(肺) 및 간(肝) 디스토마 집단치료사업(集團治療事業)에 있어서는 추정환자수(推定患者數)의 감소(減少)를 크게 가져오지 못하여 사업(事業)의 부진(不振)함을 나타내었다. 구충제제조에 있어서 너무 많은 제조회사(製造會社)에서 많은 종류(種類)의 상품(商品)이 제조(製造)되어 난맥(亂脈)을 이루고 있었으며 따라서 구충제 제조허가(製造許可)에 있어서의 재고(再考)가 요구(要求)된다. 인분처리(人糞處理) 상황(狀況)에 있어서 도시지역(都市地域)에 있어서는 위생적(衛生的)인 하수도(下水道)가 미비(未備)되고 하수종말처리장(下水終末處理場)이 전연(全然) 없고 수거처리(收去處理)되고 있으며 한편 농촌지역(農村地域)의 인분처리(人糞處理)는 거의 수임상태(收任狀態)로 비위생적(非衛生的)이어서 기생충 전파(傳播)의 가장 큰 요인(要因)의 하나가 된다. 인분사용금지구역(人糞使用禁止區域)으로 지정된 전국(全國) 55개(個) 지역중(地域中) 34개(個) 지역(地域)에서 총(總) 170건(件)의 토양(土壤)(밭흙)을 채취하여 검사(檢査)한바 그중(中) 44%에서 회충난이 검출(檢出)되었다. 청정채소보급소(淸淨菜蔬補給所)와 일반시장(一般市場)에서 판매하는 각종 채소(菜蔬) 각(各) 64건(件)씩을 구입하여 검사(檢査)한바 회충난검출률은 보급소(補給所)의 것이 25%, 일반시장(一般市場)의 것이 36%로 나타났다. 전국(全國) 보건소(保建所)에서의 기생충병관리사업 실적(實績)과 기생충 검사능력(檢査能力)은 저조(低調)하였다. 보건사회부(保健社會部)의 기생충병 관리(管理) 예산(豫算)은 아주 적으며, 세로판후층도말법(厚層塗抹法)에 소요(所要)되는 실비(實費)를 산출(算出)한바 1건당(件當) 8원이었다. 기생충병 관리원칙(管理原則)은 기생충 자체(自體)의 생활사(生活史)를 중심(中心)으로 하는 요인(要因)과 이에 영향을 주는 제환경적(諸環境的) 요인(要因), 그리고 감염(感染)과 관계되는 인간숙주측(人間宿主側) 요인(要因)등의 여러가지 요인(要因)에 의(依)해서 발생(發生)되는 기생충감염의 생성과정중(生成過程中)의 어느 한 부분(部分) 또는 그 이상(以上)을 제거하는 방향으로 이루어지며, 그 방법에는 다음과 같은 사항들을 들수 있다. (1) 감염자(感染者)에 대(對)한 집단치료(集團治療)로서 감염(感染)의 근원(根源)이 되는 병원소(病原巢)를 제거(除去)한다. (2) 보충동물을 제거(除去)함으로써 인체감염(人體感染)과 관계되는 동물병원소(動物病原巢)를 제거(除去)한다. (3) 보충숙주의 배설물(排泄物)에 대(對)한 위생적(衛生的) 처리(處理)인바 여기에서 주요(主要)한 것은 도시지역(都市地域)에서의 완비(完備)된 하수도(下水道)와 하수종말처리장(下水終末處理場), 그리고 농촌지역(農村地域)에서의 비흡취식(非吸取式)인 위생적편소(衛生的便所)의 설치(設置) 등이며 이것은 우리나라에서의 인분비료사용(人糞肥料使用)의 폐지(廢止)와 국가경제(國家經濟)의 향상(向上)으로서 그 목적(目的)을 달성(達成)할 수 있게 된다. (4) 매개체(媒介體) 및 중간숙주(中間宿主)의 관리(管理)로서 토양(土壤)의 기생충오염방지, 식품위생(食品衛生), 곤충관리 그리고 패류관리(貝類管理) 등을 들 수 있다. (5) 비위생적(非衛生的) 태도(態度)와 폐습(弊習)의 개선(改善)으로서 기생충감염과 관계 있는 입분비료(入糞肥料)의 사용(使用), 식습관(食習慣) 및 개인위생(個人衛生) 등이 문제된다. (6) 화학예방(化學豫防) 및 예방접종(豫防接種)은 전술(前述)한 바와같은 전파예방(傳播豫防) 조치(措置)가 실패(失敗)하는 경우 체내(體內)에 침입(侵入)되는 기생충체를 체내(體內)의 방어력(防禦力)에 의(依)하여 그 감염(感染)의 성립(成立)과 성충에의 발육(發育)을 방지(防止)하려는 조치(措置)로서 이에 대(對)한 앞으로의 보다 적극적인 연구(硏究) 개발(開發)이 요망(要望)된다.

  • The prime object of the study is to evaluate how much all the students of the Nursing Schools throughout the nation are in comprehension toward the application of nursing process to clinical experience as means of systematic solution of nursing problems. An effort has been made to find out the actual state whether they are in practice of clinical experience in accordance with application of nursing process, over the period of four weeks managing from December 1st to 28th, 1980 and centering on 36 nursing schools, and meanwhile and evaluation, employing the assessment tool used by Bertuccietal, has been made on the nursing process notes recorded by 200 senions out of 21 nursing schools where application of nursing process to clinical experience being in practice. The assessment tool is composed of 5 different criteria in view of patient nursing and authors made an attempt to find out the result of clinical experience on application students in accordance with 5 different scoring criteria and further evaluating all the findings thereof. The findings were disposed of accordance with practice duration and criteria of the specific sudents subject to this finding as to verify the scoring difference in significance and of which the results are as follows: 1) as of now, in 21 (58.2%) out of 36 nursing Schools nursing process in being appliced in clinical experience. 2) Schools that started the application of nursing process to clinical experience amount to - for more than 4 yrs -6 (28.6%) - for 2 to 3 yrs-11s(52.4%) - for 1 yr -4 (19.0%) 3) As for the response upon application of nursing process. To clinical etperience, the largest voice (61.9%) heard was that it is rather difficult beyond the lecturing thereof, to practically apply it outs patients and the second voice (19.1%) turned out to be that it is hard to put in practice owing to uninformed nurses of the process serving in the clinical field. 4) The response. Of the processors assigned to instruction as to the most difficult problem in criteria of nursing process, the largest voice (38.2%) centered on the problem assessment while the second voice (17.7%) on the indirect nursing activity and the objective data respectively and considered to be the easiest was the indirect nursing activity (11.7%). 5) In order for a satisfactory. application of nursing process to clinical experience hence-forth, it has been pointed out that sufficient number of nurses should be supplemented in clinical field (44.1%) and at the same time supplementory education (35.3%) centered around professors be necessary. 6) Of the criteria that record result of nursing process, a significant difference in comprehension of subjective and objective data has been revealed according to the degree of the practice duration of application to clinical experience. For instance, while although poor it may seen, only 74.9% in subjective data and 71.1% in objective data represent the student group in practice for more than 4 years and only 56.3% in subjective data and 66.8% in objective data represent the student group in practice for 2 to 3 years but they still surpass in comprehension over the student group in practice for 1 year attaning only 19.6% in subjective data and 16.8% in objective data (P < 0.005). 7) As for problem assessment, the student group who started application of nursing process for 4 years stand for 37,7% the group for 2 to 3 years started for 25.3% and the group for 1 year started for 5.4%, revealing no significant difference according to duration (P < 0.5) and as poor as to indicate only 22.8% on an overage is in comprehension. 8) On direct and indirect nursing activity, the student group of for more than 4 years in appling nursing process (representing 49.5% in direct nursing activity, 21.4% in indirect nursing activity). Know more about it than the student group of for 2 to 3 years (representing 36.3% in direct nursing activity, 20.8% in indirect nursing activity) but revealed no significant difference. (P < 0.5) 9) The student group applying nursing process for more than 4 years subjective data (74.9%) comprehend were more than objective data (71.1%) but shown no significant difference (P < 0.5). 10) However, the student group applying nursing process for 2 to 3 years comprehend objective data (66.8%) well ever subjective data (55.5%) indicating that 40.9% in average is in comprehension, thereby revealing a significant difference (P < 0.005). 11) On the other hand, the student group applying nursing process to clinical experience for 1 year had revealed themselves as poorly as to comprehend only 11.7% are an average of it, revealing no significant difference (P < 0.5). In consequence of the fore going, I the conductor of the present study, hereby suggest the following points: 1) Application of nursing process to clinical experience be practiced in all the Nursing Schools all over the nation at the earliest possible date in order that scientific nursing be prevailed (as of now only 58.0%), 2) In teaching nursing process, it is desirable to teach specific method of applying to practical clinical situations. 3) In order to meet the end of satisfactory application of nursing process to clinical experience, sufgecient nursing man power be sysplemented in clinical field and at the save time supplementary education by professors is necessary. 4) Sinces the students whose application duration of nursing process to clinical experience is longer comprehend more about it, it is reguired that the schools not yet in practice of the application be promptlyurged to follow. 5) Of the criteria recording nursing process, since it is comparatively hard to comprehend“assessment”and“Direct and indirect nursing activity”, a concentrated instruction is desirable. 6) The students whose duration of application of nursing process to clinical experience falls short of 1 years be put in a concentrated guidance program on individual criterion.
