Safety Culture, A New Challenge to Human Factors Engineering for 21st Century

  • Objective: This paper discusses the recent challenges to human factors engineering due to the safety culture. Background: As incidents occurring in specific fields such as logistics, plant, energy and medical sectors in Korea, as well as in the public sectors including railway, road, aviation and shipping, are recently raised as social issues from the disaster dimension, those incidents are dealt with as man-made disasters in many cases. The trend regarding all accidents as man-made disasters has been expanded in the active perspective that the controllability of all incidents should be ensured in technology development, due to change from a fatal point of view regarding disasters as random occurrence of uncertainties in the past. Method: Man-made disasters are concluded as human errors, and safety culture stands out as a cause of human errors or a new cause item recently. Because safety culture, however, is a very comprehensive term, of which true nature is obscure, although many definitions of safety culture have been presented, the safety culture may make avoid the true nature and responsibility of an incident, or make the main player and subject obscure. Raising safety culture as a cause without presenting a specific countermeasure will be just a wisdom of hindsight. Results and Conclusion: This study reviews the fundamental discussions on "Is safety culture a task of human factors engineering?" and the existing approach carried out from various perspectives in order to seek an effective approach on the new task of safety culture in the human factors engineering field. This study discusses an engineering approach to meet a precondition that safety culture is not just an added factor through a review of the approaches in the proactive fields such as nuclear power and aviation, and the traditional approaches of human factors engineering. Application: This study especially defines the perspective of socio-technological system that has expanded the existing man-machine system, and discusses a systemic approach embracing various interactions, and several overriding tasks.

Effects of Roscovitine on Nuclear Maturation, Spindle Configuration, and Chromosome Alignment in Porcine Oocytes

  • In the present study, effects of concentration and time of culture in presence of roscovitine on nuclear maturation and meiotic spindle configuration, chromosomal alignment were examined in porcine oocytes. In experiment 1, porcine cumulus oocyte complexes (COCs) were cultured at $39^{\circ}C$ in a 5% $CO_2$ atmosphere in North Carolina State University 23 (NCSU-23) supplemented with 25, 50, 75 or $100\;{\mu}M$ roscovitine for 22 h and then were cultured for additional 22 h after removal of roscovitine. Nuclear maturation and morphology of the meiotic spindle and chromosomal alignment were examined to determine the optimal concentration of roscovitine in oocyte maturation. In experiment 2, COCs were cultured in NCSU-23 supplemented with $50\;{\mu}M$ roscovitine for 17, 20, 27 or 42 h and then an additional 22 h without roscovitine was followed to determine the optimal time of culture. The optimal concentration of roscovitine to arrest and resume meiosis of porcine oocyte was $50\;{\mu}M$ by examining nuclear status (p<0.05) and normal spindle and chromosome configuration. The optimal time of culture in presence of roscovitine to arrest meiosis of porcine oocyte was 17 h (p<0.05), although MII rates and normal morphology of the meiotic spindle and chromosomal alignment were not significantly different among various times of culture. In conclusion, the optimal concentration and time of culture in presence of roscovitine to arrest porcine oocytes are $50\;{\mu}M$ and 17 h, respectively.

A Study of Methodology to Examine Organizational Root Causes through the Retrospect Error Analysis of Railroad Accident Cases

  • Objective: This study proposes a systematic process to present the analysis methods and solutions of organizational root causes to human errors on the railroad. Background: In fact, organizational root cause such as organizational culture is an important factor in the safety concerns on human errors in the nuclear power plant, railroad and aircraft. Method: The proposed process is as follows: 1) define analysis boundary 2) select human error taxonomy 3) perform accident analysis 4) draw root causes with FGI 5) review root causes analysis with survey 6) chart analysis of root causes, and 7) propose alternatives and solutions. Results: As a result, root causes of the organizations like railroad and nuclear power plant came from the educational problems, violations, payoff system, safety culture and so forth. Conclusion: The proposed process does predict potential railroad accident through retrospect error analysis by building new human error taxonomies and problem solution. Application: This study would contribute to examination of the relationship between human error-based accidents and organizational root causes.

국내 의료 방사선 안전문화 활동 현황 : 의료방사선안전문화연합회 중심으로 (Report for Spreading Culture of Medical Radiation Safety in Korea : Mainly the Activities of the Korean Alliance for Radiation Safety and Culture in Medicine(KARSM))

  • 2011년 3월 일본 동북해안의 지진과 해일, 그리고 연이은 후쿠시마 원전사고에 따른 국내의 방사선 피폭에 대하여 심각하게 우려되고 있다. 후쿠시마 원전에서 누출된 방사성 물질이 미량이나마 국내에도 검출되고 있어 방사성 물질에 대한 시민들의 불안감이 고조됨과 동시에, 국내 언론에서는 과학적인 접근 보다는 실시간으로 시민의 불안감에 편승하여 대중들을 방사선에 대한 공포로 몰아가는 상황이 자주 연출되고 있다. 이에 현상을 올바르게 직시하고 대안을 제시하기 위해 세계 각국의 보건, 의료기관 및 학술 단체에서는 의료방사선 안전문화 확산을 위해 다양한 정보를 제공하고, 의료방사선이 안전하다는 인식을 일반인에게 확산시키기 위한 캠페인을 실시하고 있다. 이에 국내에서도 지난 2011년 6월 초 식품의약품안전처의 제안으로 '의료용 방사선 관련 유관학회의 공동 심포지엄'의 개최를 위하여 마련된 유관학회 전문가 회의에서 공동 심포지엄의 개최 결정과 더불어, 의료용 방사선 안전을 위한 연합회 구성을 공식화하여 '의료방사선안전문화연합회(KARSM, the Korean Alliance for Radiation Safety and Culture in Medicine)'를 창립하게 되었다. 따라서 본 보고에서는 국외 의료방사선 안전문화 활동들에 대한 소개와 더불어 국내 의료방사선 안전문화 확산을 위한 의료방사선안전문화 연합회의 사업을 중점적으로 소개하고자 한다. 의료방사선 안전문화 확산과 관련한 여러 활동들을 통하여 일반 대중들에게 의료방사선의 막연한 공포감을 해소하고 바람직한 인식 형성에 기여하고자 한다.

원전 종사자 및 조직 특성이 사고에 미치는 영향 (A Study on the Effects of Nuclear Power Plant Workers and Organizational Characteristics on Accidents)

  • The purpose of this study is to analyze the effects of nuclear power plant workers and organizational characteristics on accidents, and seek the combination of policies for reducing accidents. For this purpose, it grasped, by using System Dynamics and Causal Loop Diagram, how various factors that cause accidents in nuclear plants are interrelated, and performed simulations after building simulation models. As a result of the analysis, first, the fundamental cause of safety accidents in nuclear power plants turned out to be the time constraint on the workload. Second, it turned out that as the workload of workers increases, physical and mental stresses increase, thereby increasing accidents due to errors and violations. Third, it turned out that as the levels of educational training and compliance with regulations and procedures are enhanced, accidents are reduced. Lastly, it turned out that as the combination of policies (reliance level + working environment + educational training + compliance with regulations and procedures) is increased, accidents are reduced. Especially, it turned out that if the combination of policies is increased by 30%, more than 10 cases of accidents are reduced.
