Implementation and Evaluation of Electroglottograph System (전기성문전도(EGG) 시스템의 개발 및 평가)

  • Electroglottograph(EGG) is a signal recorded from the vocal cord vibration by measuring electrical impedance across the vocal folds through the neck skin. The purpose of this study was to develop EGG system and to evaluate possibility for the application on speech analysis and laryngeal disease diagnosis. EGG system was composed of two pairs of ring electrodes, tuned amplifier, phase sensitive detector, low pass filter, and auto-gain controller. It was designed to extract electric impedance after detecting by amplitude modulation method with 2.7MHz carrier signal. Extracted signals were transmitted through line-in of PC sound card, sampled and quantized. Closed Quotient(CQ), Speed Quotient(SQ), Speed Index(SI), fundamental frequency of vocal cord vibration(F0), pitch variability of vocal fold vibration (Jitter), and peak-to-peak amplitude variability of vocal fold vibration(Shimmer) were analyzed as EGG parameters. Experimental results were as follows: the faster vocal fold vibration, the higher values in CQ parameter and the lower values in SQ and SI parameters. EGG and speech signals had the same fundamental frequency. CQ, SQ, and SI were significantly different between normal subjects and patients with laryngeal cancer. These results suggest that it is possible to implement portable EGG system to monitor the function of vocal cord and to test functional changes of the glottis.

Effects of a singing program using self-voice monitoring on the intonation and pitch production change for children with cochlear implants (자가음성 모니터링을 응용한 가창 프로그램이 인공와우이식 아동의 억양과 음고 변화에 미치는 영향)

  • The purpose of this study was to examine how a singing program using self-voice monitoring for children with cochlear implants (CI) influences on the intonation and the accuracy of pitch production. To verify and estimate the effectiveness, a program was conducted with participants of 7 prelingual CI users, whose aged between 4 years and 7 years. The program adopted three stages from the self-voice monitoring: Listen, Explore, and Reproduce (LER stage). All participants received 8 singing sessions over 8 weeks, including pre-test, intervention, and post-test. For the pre and post-test, participants' singing of an excerpt of a song "happy birthday" and speaking three assertive sentences and three interrogative sentences were recorded and analyzed in terms of the intonation slopes at the end of the sentences and the melodic contour. From the sentence speeches, we found that the intonation slopes of the interrogative sentences significantly improved as they showed similar patterns with that of the average normal hearing group. Also, in regard to singing, we observed that the melody contour had progressed, as well as the range of pitch production had extended. The positive result from the intervention indicates that the singing program was effective for children with CI to develop the intonation skill and accuracy of pitch production.

Phonetic improvement by adjusting the shape of the anterior palate of the maxillary complete denture: a case report (상악 총의치 전방 구개 부위 형태 조정을 통한 발음개선 증례)

  • Patients tend to return to normal pronunciation patterns after fitting new dentures. However, for some patients, it takes a long time to adapt the new complete denture. In this case, the patient came to the hospital at the address of wanting to remake dentures due to wear and tear. After diagnosis through clinical and radiological examination, the maxillary complete denture and mandibular removable partial denture were remade. The patient complained whistling /s/ sound at the first check-up after placement of the new denture. The anterior palatal area of polished surface of the new maxillary complete denture was concave comparing to old denture, and this was the cause of the whistling /s/ sound. A tissue conditioning material was applied to the maxillary complete denture and patient made /s/ sound. The tissue conditioning material was replaced with self-curing type denture base resin, and the patient was immediately satisfied with clear /s/ sound. As an objective assessment, palatogram and speech analytics software was applied. In this case, a patient who received denture treatment complaining of difficulty in pronunciation underwent immediate denture repair, which resulted in patient satisfaction and improved pronunciation through objective evaluation.

The Effect Of Neologism Ability Of Students With Mild Intellectual Disabilities On Peer Popularity (경도지적장애 학생의 신조어 능력이 또래인기도에 미치는 영향)

  • The purpose of this study is to investigate the relationship between the characteristics of using new words, ability to use new words, and peer popularity among students with mild intellectual disabilities and general students of the age-matched group. A total of 8 students, 4 students with mild intellectual disabilities aged 14 to 16 years of age and 4 normal students in the age-matched group, were compared between groups using a nonparametric test. In the case of new words, 60 new words were selected through expert content validity among 301 new words, and then recognition and background information on the 60 new words were collected. As a result of the study, first, there was a significant difference in understanding of new words between the student group with mild intellectual disability and the general student group of the same age. Second, the correlation between the use of new words and the popularity of peers was compared for each group of students with mild intellectual disabilities and a group of general students of the same age as possible. Therefore, when providing vocabulary instruction for students with mild intellectual disabilities, it suggests that it is necessary to teach new vocabulary in order to increase their relationship with their peers and their popularity.

Double-talk Control using Blind Signal Separation based on Geometric Concept in Acoustic Echo Canceller (음향반향제거기에서 기하학적 개념의 BSS를 이용한 동시통화 제어)

  • This paper describes an acoustic echo canceller with double-talk using BSS(: Blind Signal Separation) based on the geometric concept. The acoustic echo canceller may be deteriorated or diverged during the double-talk period. So we use the blind signal separation to detect the double talking by separating the near-end speech signal from the mixed microphone signal. In the closed reverberation environment, the blind signal separation extracts the near-end signal from unknown signals with the transformation and rotation based on the geometric concept. By this method, the acoustic echo canceller operates irrespective of double-talking. We verified performances of the proposed acoustic echo canceller by computer simulations. The results show that the acoustic echo canceller with this algorithm detects the double-talk periods thoroughly, and operates stably in the normal state without diverging of coefficients after ending the double-talking.

Knowledge Transfer Using User-Generated Data within Real-Time Cloud Services

  • When automatic speech recognition (ASR) is provided as a cloud service, it is easy to collect voice and application domain data from users. Harnessing these data will facilitate the provision of more personalized services. In this paper, we demonstrate our transfer learning-based knowledge service that built with the user-generated data collected through our novel system that deliveries personalized ASR service. First, we discuss the motivation, challenges, and prospects of building up such a knowledge-based service-oriented system. Second, we present a Quadruple Transfer Learning (QTL) method that can learn a classification model from a source domain and transfer it to a target domain. Third, we provide an overview architecture of our novel system that collects voice data from mobile users, labels the data via crowdsourcing, utilises these collected user-generated data to train different machine learning models, and delivers the personalised real-time cloud services. Finally, we use the E-Book data collected from our system to train classification models and apply them in the smart TV domain, and the experimental results show that our QTL method is effective in two classification tasks, which confirms that the knowledge transfer provides a value-added service for the upper-layer mobile applications in different domains.

Clinical Analysis about Complications of Stroke Treated Intensively in the Intensive Care Unit (뇌졸중으로 중환자실에서 집중치료한 환자들의 합병증에 관한 임상적 고찰)

  • Purpose : Cerebrovascular disease is the most frequent cause of death in Korea and it remains severe disabilities disturbing normal life, According to the previous studies, mortality of the stroke in the first one week is up to the 20% and 95% of stroke patients in the acute stage are accompanied by more than one complications. These complications affect not only the acute stage mortality but also the late stage rehabilitations. In Korea the oriental medicine is preformed in the treatment of stroke. Therefore it is important to recognize thoroughly the complications in the acute stage of stroke and to prevent them. But studies about complications of acute stage stroke is rarely presented in the fields of oriental medicine. So this study is prepared for investigating the characteristics and frequency of complications in the acute stage of stroke. And we are to assess the importance of theses acute complications by systemic reviewing the previous studies. Methods : Fifty one patients are included who had been admitted to ICU(intensive care unit) of Chenju Hospital of Wonkwang Oriental Medicine. Twenty nine patients are diagnosed as ischemic stroke and twenty one patients are diagnosed hemorrhagic stroke. Medical and neurological complications were investigated retrospectively based on medical notes excluding primary symptoms of stroke Le. motor weakness, sensory disturbance and speech disorder. And risk factors of stroke such as D.M. or hypertension are excluded. Results : Medical complications are more frequent than neurological ones. Most frequent medical complication is dysuria(61%) and constipation(45%), fever(30%) and aspiration pneumonia(22%) are followed in order of frequency. In Neurological complication dysphagia(56%), the exacerbation of infarction due to increased intracranial pressure(24%), irritability or insomnia(21%) is most frequently complicated in order of frequency. Conclusions : These complications are mostly caused by bed rest state in acute stage stroke. It is supposed that more aggressive management can prevent theses ones. And it is possible to improve the medical and neurological conditions by sticking theses study results.

Experience of Surgical Approach to the Pontine Lesions - Report of 4 Cases - (뇌교병변의 수술적 접근에 대한 증례보고)

  • Although direct surgical treatment of the lesion in the pons may cause severe neurologic morbidity, safe route to minimize injuries of the important structures in the pons should be considered. The authors operated four cases of intrapontine lesions via safe approach route without causing severe neurologic complications. Two cases were intrapontine tumors and other two were intrapontine hematoma. An anaplastic astrocytoma($3{\times}3{\times}3cm$) located bilaterally in the pons was approached via midline of the median sulcus, and a metastatic tumor($1.5{\times}1.5{\times}1.5cm$) located at the left posterolateral aspect in the upper pons was approached via suprafacial space. Two cases of hematoma were evacuated via median sulcus, and supra- and infrafacial spaces. Preoperatively, quadriplegia, swallowing difficulty, diplopia, speech disturbance, and nystagmus were noted in a patient with an anaplastic astrocytoma. A patient with metastatic tumor showed mild right hemiparesis, right hemisensory disturbance, diplopia, and dizziness. Two patients with hematoma in the pons were comatous, and had contracted, fixed pupils. Postoperatively, a patient with an anaplastic astrocytoma recovered and a patient with a metastatic tumor showed temporary hemifacial palsy. Mental status was fully recovered normal even though facial palsy, diplopia, severe ataxia, dizziness, and tremor persisted in both patients with pontine hematoma. Careful operation based on the anatomical knowledge of the floor of the 4th ventricle is of prime importance in appraoching to the intrapontine lesion with minimal injuries of the eloquent structures during surgery.

  • Rett syndrome is an X-linked dominant, progressive neurodevelopmental disorder, with a prevalence estimated to be one in 10,000-15,000 girls, which is thought to be the second most common genetic causes of mental retardation in females after Down syndrome. Patients with classic Rett syndrome show an apparently normal neonatal period, followed by developmental regression and deceleration of head growth, accompanied by gradual loss of speech and purposeful hand use, and development of microcephaly, seizures, autism, ataxia, intermittent hyperventilation and stereotypic hand movements. After regression between infancy and the fifth year of life, the clinical course stabilizes and patients usually survive into adulthood. It was recently discovered that Rett syndrome is caused by mutations in the methyl-CpG binding protein 2(MECP2) gene. Diagnosis of Rett syndrome is clinically difficult before three years of age, especially in atypical cases, but molecular analysis of the MECP2 gene could assist correct diagnosis in some patients. Recently, we diagnosed a case of Rett syndrome in a two year-old girl by mutational analysis of the MECP2 gene and want to report this case with brief review of literature.

Development of a Bone Conduction Telephone for Conductive Hearing Impaired Persons and its Performance Test (전음성 청각장애인용 골도 전화기 개발 및 성능 평가)

  • This paper describes characteristics of a bone conduction telephone which was developed for conductive hearing impaired persons to call without additional devices and results of its performance test. Not only the hearing impaired but also normal hearing persons can use this telephone because we developed a bone conduction vibrator with which they can perceive speech signal using functions of air conductive hearing as well as bone conductive hearing. It also has tone control function compensating hearing losses for the hearing impaired originating from their hearing characteristics, and using this function together with received volume control it has received volume range of 20dB in loudness rating, which is similar effect as what a telephone set with built-in received amplifier has. From results of articulation and intelligibility tests for 19 hearing impaired persons, we can see that if their bone-conduction hearing loss is 61dB or less, they can understand words or sentences and response well with this telephone.
