Attention in Child Psychiatry (소아정신과 장애에서 주의력의 문제)

  • This paper first reviewed the definition, criteria, and neurological theories concerning the etiology of AD/HD, and the empirical studies dealing with the comorbidity of AD/HD with other psychiatric disorders. Secondly, results of studies using various neuropsychological tests for assessing the cognitive and behavioral problems in AD/HD children were examined, which suggest the possibility that dysfunction may exist in neural pathways involving many areas of the brain in AD/HD. However, because most of neuropsychological test used in Korea for ADHD children had been developed abroad, further study involving AD/HD, normal control, and other psychiatric control groups is needed to obtain developmental norms for interpreting the results, and to make more accurate diagnosis, and to clarify comorbidity of AD/HD with other disorders.

Hepatotoxicity in an Adolescent with Black Iced Tea Overconsumption

  • Tea is the most widely consumed beverage after water in the world. The consumption of iced tea has increased in Western countries and spiked among teenagers for enjoyment, freshening up and alertness. A teenager presented with symptoms of hepatitis. Liver ultrasound revealed sludge in the gallbladder. Laboratory investigations excluded all known causes of hepatotoxicity. Detail nutritional history revealed that the patient had been drinking 1.5-2 liters of black iced tea per day for the last three months. He was immediately advised to stop drinking any tea. Gradually all symptoms disappeared and two months after discontinuation of the tea, all liver enzymes returned to normal and the sludge in the gallbladder disappeared. This case report underlines the importance of a meticulous assessment of a child's dietary behavior when investigating a case of hepatotoxicity and raises awareness about the potential side effects of tea overconsumption.

Adult Attachment Style in Mothers of Children with Selective Mutism (선택적 함구증 환아 어머니의 성인애착유형)

  • Objectives : The purpose of this study was to investigate the adult attachment style characteristics of mothers with selective mutism children and their relations to selective mutism. Methods :The subjects of this study were 15 mothers with selective mutism children who were diagnosed by DSM-IV criteria at psychiatry outpatient department of Kyungpook National University Hospital from March 1998 to February 2005. The controls of this study were 30 mothers with normal children who are in the second grade of elementary school in Daegu. We assessed the adult attachment style characteristics of these mothers by Revised Adult Attachment Scale, and Reciprocal Attachment Questionnaire-Korean version, self-report attachment style questionnaire. Results : 1) On the self-report attachment style questionnaire, it revealed that mothers with selective mutism children had both of the secure and the dismissing-avoidant types predominantly and the tendency that mothers with selective mutism children more commonly had dismissing-avoidant type than controls did. 2) On the comparison of attachment quality of mothers, although only the anxiety subscale difference was significant, it revealed that mothers with selective mutism children had generally lower score pattern in all of closeness, dependence and anxiety subscale than controls did. It was consistent with the consequence of self-report attachment style questionnaire in this study. Conclusion : This study showed that the distribution of adult attachment style of mothers with selective mutism children was different from those of controls. The dismissing-avoidant attachment style was predominant in mothers with selective mutism children. we suppose the possibility that the dismissing-avoidant attachment style of mothers with selective mutism children has relation with selective mutism.

Relationship between Communication Stance and Characteristics of Internet Use (청소년에서의 인터넷 중독 정도 조사 및 이에 따른 의사소통 유형과의 관계)

  • Objectives : Internet became essential component in these days. This study primarily tried to find out the characteristics of high risk internet users through using the communication scale. Methods : We investigated levels of internet addiction using Internet Addiction Scale in 1,193 high school and middle school students. Participants were divided into 3 groups (high risk user group, potential risk user group, normal user group) according to the results from internet addiction scale. We additionally surveyed characteristics of internet use, and patterns of communication and Communication Scale based on Satir theory in 614 from 1,193 participants. Results : Boys showed higher tendencies for internet addiction than girls in internet addiction scale. There were significant differences in mean computer using time especially during weekends among 3 internet user groups. The distribution of communication types in each internet user group was similar and this finding coincided with previous study results. However high risk user group showed higher scores in each inadequate communication pattern such as placating, blaming, super-reasonable and irrelevant stance. Conclusion : These results suggest that adolescents who use internet in addictive patterns may have more problematic communication styles and these may be associated with poor interpersonal relationships.

The Effects of Multidisciplinary Approach for Children with Feeding Disorder and Failure to Thrive on Their Mothers (섭식장애와 성장장애를 보이는 아동에 대한 다학제적 중재가 환모에게 미치는 영향)

  • Objectives : The aim of this study was to determine whether intervention using a multidisciplinary approach affects maternal mental health, parenting stress, and sense of parenting competence in children with feeding disorder and failure to thrive (FTT). Methods : Children with feeding disorder and FTT were randomized to the intervention group (N=11) or control group (N=8). We administered the Korean standardization of Parent Temperament Questionnaire for Children (K-PTQ) in both groups before intervention, and the Korean version of the Parenting Stress Index-Short Form (K-PSI-SF), Korean version of the Parenting Sense of Competence (K-PSOC), Korean version of the Beck's Depression Inventory (K-BDI), Korean version of the Beck Anxiety Inventory (K-BAI), and Korean version of the Mood Disorder Questionnaire (K-MDQ) in both groups before and after the intervention. Results : In the intervention group, the K-BDI (p=.068), K-BAI (p=.068), and K-MDQ (p=.066) scores tended to show a decline, the K-PSI-SF scores for stress related to child learning showed a significant decline, and the K-PSOC scores for sense of parenting competence showed significant improvement. However, no significant changes were observed in the control group. Conclusion : Use of a multidisciplinary approach improved maternal mental health, parenting stress, and sense of competence. Comparison of these results with those of normal control will be necessary in a future study.

Effect of Nutrition Education Pogram on Obese Middle Shoot Boys in Taegu (대구 지역의 일부 비만중학생을 위한 영양교육 프로그램의 효과)

  • This study was conducted to find the effect of in-class nutrition education and exercise program for obese adolescent boys in Taegu with the nutrition knowledge test before and after education program and anthropometric measurement. The subjects were consisted of a normal group (NG; n= 13) and an obese group (n=30). The obese group is divided into two groups; obese group A (OG-A; n=12), participated in a ten-week nutrition education only; and obese group B (OG-B; n=28), participated in a ten-week nutrition education and exercise program. The anthropometric data of the three groups were increased after the nutrition education program. However the obesity index(BMI, R hrer, WHR, Skinfold thickness) of OG-B were significantly decreased after the programs. The nutrition intake data were also changed after the nutrition education program. Especially the intake of carbohydrate was significantly decreased in OG-B from $421.5{\pm}155.9g$ to $349.0{\pm}41.0g$ (p<0.05) after the nutrition education program. It was noted that the intake of Ca was significantly decreased in all three groups after the nutrition education program. The mean score of the nutrition knowledge test also significantly increased from $13.7{\pm}3.1$ to $17.8{\pm}3.0$ after the nutrition education program. The result strongly suggested that nutrition education program should be successful if the obese adolescents and their parents especially mothers participated in the same class. In summary, nutrition education and exercise programs were effective on reducing obesity of obese adolescents. Nutrition knowledge of obese adolescent was significantly increased after nutrition education program.

A Study of Self-Concept and Body Image in High Risk Adolescents for Eating Disorders (식사장애 고위험 청소년의 자기개념과 신체상에 대한 연구)

  • Objects : This study was attempted to evaluate the body image and the self concept in high risk adolescents for eating disorders. Methods 589 middle school students were investigated with EAT(Eating Attitude Test) and BITE(Bulimic Investigatory Test Edinburgh) and Schematic Figures(Child/Adolescent Version) and Piers-Harris Self-Concept Scale. According to the score of EAT and BITE, three high risk groups(high EAT group, high BITE group, high EAT & BITE group) were identified. Three groups were examined in terms of sex, weight, self concept and body image. Each group was EAT group, BITE group, EAT & BITE group Results: The results of the study were as follows : 1) EAT & BITE group showed greatest body image distortion. 2) BITE group and EAT & BITE group showed lower score in self concept scale. 3) The majority of EAT group were boys, BITE group and EAT & BITE group were mostly girls. 4) All three high risk groups, especially Bin group, revealed overweight compared with normal control group. Conclusions : High BITE group and high EAT & BITE group could be considered as significantly high risk groups for eating disorders, showing overweight, severe body image distortion, and poor self concept. EAT score alone seemed not to discriminate high risk adolescents for eating disorders.

Internet Addiction, Depression and Suicidal Ideation among Adolescents (청소년들의 인터넷 중독과 우울 및 자살생각과의 연관성)

  • Purpose: This study was to assess the association of internet addiction and depression, stress and suicidal ideation among adolescents. Methods: The authors have surveyed 2 middle schools and 2 high schools located in Seoul and analyzed 525 questionnaires. The questionnaire included the Internet Addiction Self Diagnosis Index (K-index), Child Depression Index (CDI), Stress Index and Reynold's Suicidal Ideation Index. Structural equational model analysis was done to explore the association between internet addiction and suicidal ideation. Results: The adolescent internet addiction groups were classified as the high risk group 12 (2.3%), the potential risk group 64 (12.2%) and the normal group 449 (85.5%). When classified according to sex, the high risk group is consisted of 8 (3.9%) boys, 4 (1.3%) girls and the potential risk group is consisted of 41 (64.1%) boys and 23 (35.9%) girls which leads to the conclusion that boys are relatively more addicted compared with girls. The group with more addiction showed higher depression scores, stress scores and suicidal ideation scores. The correlation analysis showed the significant relations between internet addiction and depression (r=0.263), stress (r=0.265) and suicidal ideation (r=0.284). According to the structural equation model analysis, internet addiction had direct effects on the depression and indirect effects on suicidal ideation. Stress had the significant direct effects on both internet addiction and depression. Depression had a significant direct effect on suicidal ideation. Conclusion: Internet addiction affects directly and indirectly an depression and suicidal ideation among adolescent.

Screening Oppositional Defiant Disorder with the Korean Child Behavior Checklist : The Role of the Subscales of Aggressive and Delinquent Behavior (아동·청소년 문제행동평가척도를 이용한 반항성 도전장애의 선별 : 공격성 및 비행 하위척도의 역할)

  • Objectives : This present study examined the power of the Korean Child Behavior Checklist (K-CBCL) subscales to predict a DSM-IV diagnosis of oppositional defiant disorder (ODD). Methods : The sample included 37 children and adolescents with ODD and 46 normal controls. The participants and their parents were interviewed for clinical diagnosis using the Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version (K-SADS-PL) and the parents completed the K-CBCL. Logistic regression analysis was used to predict the diagnosis of ODD. Results : Among the CBCL subscales, Delinquent and Aggressive Behavior scales significantly predicted ODD diagnosis. The means of these CBCL subscales were significantly higher in the ODD group when compared to the controls. Conclusion:Two CBCL subscales (Deliquent and Aggressive Behavior) displayed good diagnostic efficiency for assessing ODD in children and adolescents. Through combining information from the CBCL, an empirical-quantitative approach to psychopathology in children and the DSM-IV diagnostic criteria, the results demonstrated that a clinical diagnostic approach is an effective diagnostic paradigm for children with ODD.

The Alcohol Drinking Pattern and the Related Factors in Problem Drinking among Adolescent's Parents in Korea (청소년 부모의 음주행태와 문제음주 관련요인)

  • Purpose: This study aimed to identify the alcohol drinking pattern of parents and to analyze drinking related factors in problem drinking among adolescent's parents. Method: This study was conducted through a structured questioning from November 5th to 22th in 2002. Those are 2,522 parents of the students of the 1st and 2nd grade who were selected randomly among 26 middle and high schools located in Socho-gu, Seoul. Data was analyzed using SAS program that included descriptive statistics, Chi-square test, and multiple logistic regression analysis. Result: 1. For frequency of drinking, 33.1% of the subjects responded to drink two-four times a month, 26.0% drink more than two times a week. 2. For the amount of drinking, 29.6% drink one or two glasses while 9.4% drink over ten glasses at a time. 3. The mean AUDIT score was 8.27 7.02(score of range: 0- 40). 4. Based on AUDIT score, drinkers with less than eight point from AUDIT(normal group) were 55.1%, from more than eight point to less than twelve point from AUDIT(problematic drinker) were 16.3%, and more than twelve point from AUDIT(alcohol abuse and dependency) were 28.6%. 5. Based on more than twelve point from AUDIT, female(mother), high school and, have religion, housekeeper were the significantly higher score than counter parts. Conclusion: Alcohol drinking pattern and level of drinking of their parents links to their children's drinking and results in family, social, and national loss. Active prevention is needed. Specially, pertinent education about drinking and public education for mothers who are housekeepers should be carried out with concentrated intervention programs for the problematic drinker so alcohol abuse and dependency can be reduced.
