관상정맥동 천정결손증 3례 보고

  • Unroofed coronary sinus syndrome is an uncommon anomaly, Unroofed coronary sinus syndrome is caused by incomplete formation of the left atriovenous fold, and it usually is associated with Left SVC. If it is not diagnosed, a residual reversed or bidirectional shunt will result, and its complications will reduce life expectancy. We experienced 3 cases of unroofed coronary sinus syndrome which combines TOF with PLSVC, partial ECD, primum type ASD. In case of Unroofed coronary sinus syndrome which combines TIF with PLSVC, preoperative diagnosis was not made. In corrective operation for TOF of this case, pump weaning was failed due to hypoxia and cardiac arrest, and he expired at operation room. At autopsy of this case, complete unroofed coronary sinus was found. In the other 2 cases, partial unroofed coronary sinus syndrome was found in operation field and corrective operation was performed successfully. Cytologic Findings of Colon Lavage Fluid in Colon Cancer (대장암에서 장세척물의 세포학적 소견)

  • Although exfoliative colonic cytology for the diagnosis of colorectal cancer has been largely abandoned due to the widespread use of colonoscopy, some authors still insiston the usefulness of colon lavage fluid. We tried evaluating the diagnostic feasibility of colon lavage fluid cytology using an orally administered balanced electrolyte solution. We collected colon lavage fluids in 106 patients prior to colonoscopy and reviewed the slides. Cytologic examination revealed neoplastic cells in 7 of 16(44%) cases of endoscopically proven adenocarcinoma patients. Therefore, we thin cytologic study of colon lavage fluid may be considered as one of the noninvasive diagnostic tools in colorectal cancer.

  • This paper describes an idea of design and construction for a radial pulse wave detector, In acquiring arterial pulse signal noninvasively, a new combinational fiber-optic transducer was used which has a detecting part and a sensing part. The mechanism of detecting part is composed of special form of structure that can detect changing pulses in contact with skin, and transmits arterial wall movements to the sensing part. It consists of elastic reflector and optical fibers, which are arranged in a fiber pair. Then, the intensity of the reflected light will be proportional to the displacement changed by inclined reflector. Using this transducer, it expected to make a clinically useful tool for arterial pulse wave diagnosis, especially on the application of "MACK- CHIN" in the field of Korean traditional medicine, since : his transducer has a simple structure and has an easy and clear signal acquisition method.on method.

  • Ultrasonographic examination for the rotator cuff disease is dynamic noninvasive study and the results is comparable to MRI on the behalf of recent development. It also can be used as a tool for guided injection and follow up study after rotator cuff repair. The development of equipment, learning curve and knowledge for the disease are the three most important factors for the critical role of the ultrasonography in the diagnosis and treatment of the rotator cuff disease.

  • The population of diabetes is continuously increasing because of the economic development and the lifestyle modification. If diabetes become chronic condition, it can cause various complications. Among many other complications, diabetic foot is the most fatal issue since it may require amputation of the legs. Diabetic foot has three different types such as neuropathic, neuro-ischemic and ischemic. Among these types, patients of neuropathic foot experience sensory abnormality. Nerve conduction velocity (NCV) is used for diagnosing neuropathic foot but this method uses strong electric stimulus to cause severe pain to the patients In this study, two channel photo-plethysmography was used as noninvasive screening tool for distinguish neuropathic foot and normal group by observing blood flow of both finger and toe simultaneously.

  • Transcutaneous immunization refers to the topical application of antigens onto the epidermis. Transcutaneous immunization targeting the Langerhans cells of the skin has received much attention due to its safe, needle-free, and noninvasive antigen delivery. The skin has important immunological functions with unique roles for antigen-presenting cells such as epidermal Langerhans cells and dermal dendritic cells. In recent years, novel vaccine delivery strategies have continually been developed; however, transcutaneous immunization has not yet been fully exploited due to the penetration barrier represented by the stratum corneum, which inhibits the transport of antigens and adjuvants. Herein we review recent achievements in transcutaneous immunization, focusing on the various strategies for the enhancement of antigen delivery and vaccination efficacy.

  • Constrictive pericarditis represents a rare cause of protein-losing enteropathy in children. Reported is an 11-year-old girl with protein-losing enteropathy (PLE) as the principal manifestations of constrictive pericarditis. After total pericardiectomy, symptoms and signs of PLE disappeared. Doppler echocardiography including tissue Doppler imaging is a useful noninvasive initial diagnostic tool for differential diagnosis of diastolic heart failure.

  • Scince $^{201}$TI was introduced as a myocardial perfusion imaging agent in the early 1970s, scintigraphic evaluation of myocardial perfusion for the diagnosis of coronary artery disease is a valuable noninvasive diagnostic imaging modality. Stress radionuclide myocardial perfusion imaging is widely accepted to have high diagnostic and prognostic use in the assessment of patients with known or suspected coronary artery disease. With wise use of this nonivasive imaging technique, more patients are referred for stress perfusion imaging. Until now various protocols for stress testing and myocardial imaging were developed and used in worldwide. This article presented various protocols of stress testing and myocardial imaging for clinical use.

  • Ultrasonography has increasingly moved from being a modality confined to the radiology department to an active diagnostic and therapeutic aid available to the head and neck at the point of patient care. However, the application of ultrasonography to the laryngeal disorder is very rare due to progressive age-related ossification of laryngeal cartilage and the presence of air in the lumen, which contribute to difficult conditions for transmission of the ultrasonic waves. The observation about the movements of larynx or surrounding structures is important to understand the physiology of phonation or swallowing and to diagnose the disease. Ultrasonography is a noninvasive and safe imaging technique that can be used to investigate the anatomic structures of the head and neck. Recently, the development of high-frequency ultrasonography makes it possible to apply the ultrasound in the evaluation of larynx.

  • The gallbladder is a pear-shaped, thin walled sac located on the inferior surface of the liver between the right and quadrate lobe, in a recess called fossa vesicae felleae. It is 7.5cm-12.5 cm in length, 3.5 cm in the largest width, and has a volume of about 45 ml with a remarkable capacity for expansion. There are many kinds of diagnostic methods to evaluate diseases of biliary tract including gallbladder-Plain abdomen, Oral cholecystography. Intravenous cholangiography, Percutaneous transhepatic cholangiography(PTC), Endoscopic retrograde cholangiography(ERCP), Operative or T-tube cholangiography, Ultrasonography, Radioisotope study, Computed tomography, and Angiography. Especially, ultrasonography is the most effective and noninvasive study in these days. Plain abdomen, oral cholecystography, intravenous cholangiography, and ultrasonography have been performed in our clinic. Methods and findings of above mentioned study are discussed with consideration of references.
