Chemokines Expression in Children with a Non-productive Cough (소아기 단순 기침 환아에서의 케모카인 발현 양상 연구)

  • Purpose : To evaluate the chemokine expression in children with a non-productive cough. Materials and Methods : Six children with a non-productive cough who visited Yeungnam University Hospital were evaluated for the mRNA expression of interferon-${\gamma}$-inducible protein 10(IP-10), macrophage cationic protein 1 and 3 (MCP-1, 3), interleukin (IL)-8, regulated upon activation in normal T cells expressed and secreted (RANTES), eotaxin and growth-related oncogene-${\alpha}$ (Gro-${\alpha}$) using the reverse transcription polymerase chain reaction. Results : The chemokines IP-10 and MCP-3 were expressed in all samples. The chemokine RANTES was expressed in five cases, and IL-8 was expressed in three among them. However, eotaxin, Gro-${\alpha}$ and MCP-1 were not expressed at all. The expression of chemokine MCP-3, RANTES and IL-8 were suppressed after the resolution of coughing in just one available case. Conclusion : The chemokines MCP-3, RANTES and IL-8 may contribute to airway inflammation in children with a non-productive cough, whereas IP-10 is of secondary importance in this condition.

Clinical Trials of Cofrel for Cough in Infants and Young Children (유유아(乳幼兒)의 기침에 대(對)한 Cofrel 사용경험(使用經驗))

  • We tried $Cofrel^{\circledR}$ ($Pyrexyl^{\circledR}$ 1-(2-benzyl phenoxy)-2-piperidino-propane phosphate) which is non-narcotic, to evaluate its clinical effect on cough due to respiratory tract diseases-35 cases. 1) Thirty two cases among 35 cases disclosed remarkable effectiveness (91.4% of efficacy) 2) It was almost equally effective in dry cough, productive cough and barking cough. 3) Speed of response was very rapid in 21 cases(60.0) moderately rapid in 12 cases(34.3%). 4) Palatability were excellent in all cases and side effect was not observed. It is very difficult to evaluate effectiveness of anti-tussive drug. However, we concluded that Cofrel is a quite effective medicament for cough in respiratory diseases in children.

  • Background: The purpose of this study was to examine the causes and pathologic process of chronic non-productive cough as an isolated symptom with a normal spirometry and chest radiograph by investigating clinicopathologic findings. Method: We studied 25 adults with chronic non-productive cough over a 3-week period with a normal chest radiograph and pulmonary function tests without any other symptoms. Clinical assessment, cough score, chest and sinus radiograph, pulmonary function tests, methacholine challenge, allergic skin prick test, and bronchoscopy for bronchial biopsies were performed. Subjects were then treated with prednesolone 20 to 30 mg/day for 1 to 2 weeks. Results: The experimental group was divided into two subgroups-those infiltrated with eosinophils, and those infiltrated with lymphocytes depending on eosinophil and lymphocyte counts, both of which were respectively higher than those of the control group. Eosinophils infiltrated group had mean numbers of eosinophil of 89.8 $cells/mm^3$ while control group's mean was 0.4 $cells/mm^2$(p=0.005). Lymphocyte infiltrated group was 4 patients whose mean was 84.3 $cells/mm^2$ with 28.4 $cells/mm^2$ of control group(P=0.026). In addition, the mean thickness of the basement membrane of experimental group was $14.20{\pm}5.20{\mu}m$ in contrast of control group whose mean was $3.50{\pm}1.37{\mu}m$(P=0.001). With the methacholine challenge test, 7 of the 21 eosinophil infiltrated subjects were diagnosed with cough variant asthma ; the other 14 with eosinophilic bronchitis. Three subjects with eosinophilic bronchitis were atopic positive (21.4%) with the skin prick test In the lymphocyte dominant group, all four subjects were diagnosed with lymphocytic bronchitis. Cough score was improved after steroid treatment in 22 of 25 subjects in the experimental group (88.0%). Conclusion: These results suggest chronic non-productive cough as an isolated symptom with a normal spirometry and chest radiograph was associated with airway inflammation by eosinophil and lymphocyte infiltration. The causes for chronic non-productive cough were eosinophilic bronchitis, cough variant asthma, and lymphocytic bronchitis(written in frequency). They further suggest that therapeutic treatment with steroids can provide effective symptomatic relief.

  • Interstitial pneumonitis may be the presenting manifestation of polymyositis-dermatomyositis (PM-DM), or may occur later in the evolution of disease. The clinical picture is characterized by non-productive cough, dyspnea and hypoxemia. The chest radiograph demonstrates interstitial infiltrates with predilection for the lung bases, often with an alveolar pattern in addition. We experienced a case of polymyositis associated with diffuse alveolar damage(DAD) that was proven in open lung biopsy. The patient was a 52 year-old woman who was presented with 6 months' duration of generalized ache, edema on ankle and wrist, non-productive cough and mild dyspnea. She had typical symptoms and physical findings of interstitial pneuminitis, and elevated muscle enzyme levels in serum with characteristic histologic findings of myositis on muscle biopsy. She also had typical interstitial lung disease pattern on high resolution CT and restrictive pattern on pulmonary function tests. The findings of open lung biopsy was compatible with diffuse alveolar damage(DAD). She failed to respond to the therapeutic trials with corticosteroid and cyclophosphamide, and finally expired due to acute respiratory distress syndrome.

Intralobar Pulmonary Sequestration A Case Report (내엽형 폐격리증 수술 1례 보고)

  • Pulmonary sequestration is an uncommon congenital malformation characterized by the presence of non-functioning lung tissues which receives its blood supply from an anomalous systemic artery instead of a pulmonary arterial branch. We present a case of intralobar pulmonary sequestration experienced lately. The patient was 7 years old girl with the complaints of chronic productive cough and right lower chest pain. Serial chest films showed a large cyst with or without a air-fluid level on the right lower lung field. Aortography revealed an aberrant artery originating from thoracic aorta just above the diaphragm and that drained into the right inferior pulmonary vein. During operation, a large abscess cavity measuring 6.5x5x5 cm in dimension at the right lower lobe was noted. And the two aberrant arteries, measuring 3 mm in diameter, arising from thoracic aorta 5 cm above the diaphragm was noted. After division and ligation of the aberrant arteries, right lower lobectomy was performed and the patient`s postoperative course was uneventful.

  • Endobronchial metastases from extrathoracic primary malignancies are uncommon. Breast, renal, and colonic carcinomas are primary sites most likely to give rise to endobronchial metastases. A number of other tumours have been reported as being complicated by endobronchial metastasis, including ovarian, thyroid, uterine, adrenal, testicular and prostatic carcinomas. The incidence of endobronchial metastasis has been estimated at 2% in patients who died of metastatic disease. Lung parenchymal metastases are common manifestations in patients with rectal cancer, however spread to the major airway is extremely rare. We herein report a case of endobronchial metastasis from rectal adenocarcinoma. A 69-year-old male patient who had been previously treated with surgical resection with rectal cancer presented with a 8-month history of gradually increasing dyspnea and non-productive cough. Clinical and radiological investigations revealed endobronchial metastasis involving, and penetrating, the lower carina and the left main bronchus. We confirmed endobronchial metastasis from the rectal carcinoma by bronchoscopic biopsy.

Solitary Papilloma of the Right Upper Lobe Bronchus: Report of a Case and Review of the Literature (우측 폐상엽 기관지에 발생한 고립성 유두종: 1례 치험 및 문헌 고찰)

  • Papilloma of the upper respiratory tract, particularly larynx and vocal cords are relatively common disease. However, solitary papilloma of the bronchus is extremely rare condition and only a handful cases were reported in the literature. The patient is a 39 year old housewife who has been suffering from productive cough and occasional hemoptysis in the past one year. X-ray of the chest showed complete atelectasis of the right upper lobe. Bronchography revealed a hemispherical protruding mass in the right main bronchus with complete occlusion of the upper lobe bronchus. Bronchoscopy showed a whitish friable mass in the lumen of the right main bronchus biopsy of which was reported as benign papilloma. Right upper lobectomy together with wedge resection of the portion of right main bronchus to include the tumor was done. Cut-edges of the bronchus were stitched together with interrupted fine dacron sutures. During this procedure, right main bronchus was gently clamped with non-crushing Satinsky type clamp. Patient has had uneventful recovery from surgery and was discharged without symptom. Patientis doing well three months following the operation.

  • Acinetobacter calcoaceticus var anitratus, a non-fermentative gram negative bacillus, has been infrequently reported as a cause of community-acquired pneumonia. Bacteremic community-acquired pneumonia caused by Acinetobacter is a fulminant disease with a high mortality (above 40 per cent in the reported case). The onset of the illness is rapid but nonspecific with fever, productive cough, pleuritic pain and rapid prostration. And the patients frequently present with respiratory distress, severe hypoxemia, leukopenia and septic shock. We experienced a case of community-acquired Acinetobacter pneumonia who underwent respiratory support with mechanical ventilator for about 4 weeks and survived. We report the case with review of literature.

  • Dark-blue pigmentation has been thought to be related to smoking or occupational exposure and has been regarded as anthracotic pigmentation. It is also frequently observed in non-smokers without occupational exposure, but there is no proven mechanism of pigmetation. To investigate clinical features and to find other causes of dark-blue pigmentaion, retrospective analysis was done in 59 patients who showed anthracotic pigmentation on bronchoscopy during recent 5 years in Seoul National University Hospital. The results were as follows; 1) Forty cases were non-smokers, while smokers were 19 cases. 2) Fifteen cases had history of tuberculosis, but there was no history of environmental exposure. 3) Mediastinal calcification was observed in 89.7%. 4) There was significant bleeding without exception when biopsy was done at the pigmentation site. 5) In patients with pigmentation only, hemoptysis and productive cough were main chief complaints, and chest X-ray showed atelectasis, infiltration, mass, or pleural change. 6) The number of patients whose lesion of X-ray corresponds to pigmentation site were 19/30 in tuberculosis, 4/30 in DILD and 7/30 in other diseases.

A Case of Interstitial Pneumonitis in a Patient with Rheumatoid Arthritis Treated with Leflunomide (Leflunomide로 치료중인 류마티스 관절염 환자에서 발생한 간질성 폐렴 1예)

  • Leflunomide, a disease-modifying antirheumatic drug (DMARD) for the treatment of rheumatoid arthritis has been available in Korea since 2003. Leflunomide-associated interstitial pneumonitis has been appearing recently. A 25-year-old woman with a 12-month history of seronegative rheumatoid arthritis (RA) presented with acute respiratory insufficiency. She developed fever, dyspnea, and non-productive cough. Her medication history included methotrexate (15 mg/week. commencing 1 year prior) and leflunomide (20 mg/day, no loading dose, commencing 4 months prior). She was diagnosed with leflunomide-associated interstitial pneumonitis based on history, physical examination, laboratory and radiologic findings. She recovered quickly after leflunomide was withdrawn and steroids and cholestyramine were initiated quickly. We report a case of leflunomide-associated interstitial pneumonitis treated successfully with intravenous high-dose steroid and cholestyramine.
