최근 10년 동안 일개 상급종합병원의 칸디다혈증 환자에서 분리된 칸디다 균종의 빈도 (Frequency of Candida Strains Isolated from Candidiasis Patients at A Tertiary Hospital over the Last 10 Years)

  • 칸디다혈증(candidemia)은 이환율과 사망률을 높이는 주요 원인으로 입원 환자에서 심각한 위험으로 남아 있으며 의료비용을 증가시킨다. 2009년부터 2018년까지 S병원의 혈액배양 검사에서 분리된 칸디다 균주 3,533건을 대상으로 시기별 분리빈도, 연도, 성별, 나이, 병동 등에 따라 분석하였다. 전체 기간 중 혈액 배양 의뢰 건수 717,996 중에 54,739건이 배양 양성으로 7.6%의 양성율을 보였으며 칸디다 분리률은 1,036명의 환자에서 3,533건으로 6.4%였다. 균종의 분포는 C. albicans (33.8%), C. tropicalis (28.6%), C. glabrata (19.8%), C. parapsilosis (7.8%), C. krusei (4.0%) 이다. 전기/후기 분리에서는 C. tropicalis가 3.8% 감소하고 C. glabrata는 3.4% 증가하였다. 50세 이후 연령이 증가할수록 분리 빈도가 높았으며, 1~10대에서는 C. parapsilosis (31.3%), 41~50대에서는 C. tropicalis (30.3%), C. glabrata (27.6%) 순으로, 80대에서는 C. tropicalis (28.6%)가 상대적으로 자주 분리되었다. C. krusei 는 여성(60.9%)에서 상대적으로 높은 비율로 분리되었다. 따라서 Candida 균종의 분포를 지속적으로 감시하고 신속한 동정 결과를 제공하여 적절한 치료 및 항진균제 치료 지침을 위한 체계적이고 지속적인 병원감염관리 시스템이 이루어져야 할 것이다.

Factors Influencing Ventilator-Associated Pneumonia in Cancer Patients

  • Background: With increasing survival periods and diversification of treatment methods, treatment of critically ill cancer patients has become an important factor influencing patient prognosis. Patients with cancer are at high risk of infections and subsequent complications. This study investigated the incidence and factors contributing to the development of ventilator-associated pneumonia (VAP). Materials and Methods: This retrospective study investigated the incidence of VAP and factors leading to infection in patients admitted to the intensive care unit (ICU) of a cancer center from January 1, 2012 to December 31, 2013. Results: The incidence of VAP was 2.13 cases per 1,000 days of intubation, and 13 of 288 patients (4.5%) developed VAP. Lung cancer was the most common cancer associated with VAP (N=7, 53.9%), and longer hospital stays and intubation were associated with increased VAP incidence. In the group using a "ventilator bundle," the incidence was 1.14 cases per 1,000 days compared to 2.89 cases per 1,000 days without its use; however, this difference was not statistically significant (p=0.158). Age (${\geq}65$, OR=5.56, 95% confidence interval [CI]=1.29-23.95), surgery (OR=3.78, 95%CI=1.05-13.78), and tracheotomy (OR=4.46, 95%CI=1.00-19.85) were significant VAP risk factors. The most common causative organisms were methicillin-resistant Staphylococcus aureus (MRSA) and Pseudomonas aeruginosa (N=4, 30.8% each), followed by Acinetobacter baumannii and Candida albicans (N=2, 15.4% each). Conclusions: The incidence of pneumonia among critically ill cancer patients is highest in those with lung cancer, but lower than among non-cancer patients. The length of hospital stay and time on mechanical ventilation are important risk factors for development of VAP. Although not statistically significant, "ventilator bundle" care is an effective intervention that delays or reduces incidence of VAP. Major risk factors for VAP include age (${\geq}65$ years), surgery, and tracheostomy, while fungi, gram-negative bacteria, and multidrug-resistant organisms were identified as the major causative pathogens of VAP in this study.

Dectin-1 Stimulation Selectively Reinforces LPS-driven IgG1 Production by Mouse B Cells

  • Dectin-1, which specifically recognizes ${\beta}$-glucan of fungal cell walls, is a non-Toll-like receptor (TLR) pattern recognition receptor and a representative of C-type lectin receptors (CLRs). The importance of Dectin-1 in innate immune cells, such as dendritic cells and macrophages, has previously been well studied. However, the function of Dectin-1 in B cells is very poorly understood. To determine the role of Dectin-1 in B cell activation, we first investigated whether mouse B cells express Dectin-1 and then assessed the effect of Dectin-1 stimulation on B cell proliferation and antibody production. Mouse B cells express mRNAs encoding CLRs, including Dectin-1, and surface Dectin-1 was expressed in B cells of C57BL/6 rather than BALB/c strain. Dectin-1 agonists, heat-killed Candida albicans (HKCA) and heat-killed Saccharomyces cerevisiae (HKSC), alone induced B cell proliferation but not antibody production. Interestingly, HKSC, HKCA, and depleted zymosan (a selective Dectin-1 agonist) selectively enhanced LPS-driven IgG1 production. Taken together, these results suggest that, during fungal infection, ${\beta}$-glucan-stimulated Dectin-1 may cooperate with TLR4 to specifically enhance IgG1 production by mouse B cells.
