Invasive Lobular Carcinoma: MRI Features and Clinicohistological Characteristics According to the ER, PR, and HER2 Statuses

  • Purpose: To investigate correlations of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor type 2 (HER2) statuses with magnetic resonance imaging (MRI) features and clinicohistological characteristics in patients with invasive lobular carcinoma (ILC). Materials and Methods: Data from 64 histologically confirmed ILCs were analyzed retrospectively. Preoperative breast MRI was reviewed for morphology and dynamic contrast-enhanced kinetics of the tumor. Pathologic reports were reviewed for ER, PR, and HER2 positivity, tumor size, lymph node metastasis, and the number of metastatic lymph nodes. Furthermore, there was an investigation of the MRI features and clinicohistologic characteristics, according to the ER, PR, and HER2 statuses. Results: A significant difference in MRI features and clinicohistological tumor characteristics were observed only in relation to PR status. Of the 64 ILCs, 10 (15.6%) were PR negative. PR negative cancers, compared with PR positive cancers, were more likely to present as non-mass enhancement (P = 0.027); have a significantly larger mean tumor size ($5.00{\pm}1.05cm$ vs. $2.57{\pm}0.21cm$, P = 0.021); and have significantly more metastatic lymph nodes (P = 0.010). Conclusion: PR negative ILC presented more frequently as non-mass enhancement on MRI, with larger tumors and increased numbers of metastatic lymph nodes. Therefore, the PR status plays an important role in determining MRI features and clinicohistological characteristics of ILC.

Molecular Imaging Using Sodium Iodide Symporter (NIS) (Sodium Iodide Symporter (NIS)를 이용한 분자영상)

  • Radioiodide uptake in thyroid follicular epithelial cells, mediated by a plasma membrane transporter, sodium iodide symporter (NIS), provides a first step mechanism for thyroid cancer detection by radioiodide injection and effective radioiodide treatment for patients with invasive, recurrent, and/or metastatic thyroid cancers after total thyroidectomy. NIS gene transfer to tumor cells may significantly and specifically enhance internal radioactive accumulation of tumors following radioiodide administration, and result in better tumor control. NIS gene transfers have been successfully performed in a variety of tumor animal models by either plasmid-mediated transfection or virus (adenovirus or retrovirus)-mediated gene delivery. These animal models include nude mice xenografted with human melanoma, glioma, breast cancer or prostate cancer, rats with subcutaneous thyroid tumor implantation, as well as the rat intracranial glioma model. In these animal models, non-invasive imaging of in vivo tumors by gamma camera scintigraphy after radioiodide or technetium injection has been performed successfully, suggesting that the NIS can serve as an imaging reporter gene for gene therapy trials. In addition, the tumor killing effects of I-131, ReO4-188 and At-211 after NIS gene transfer have been demonstrated in in vitro clonogenic assays and in vivo radioiodide therapy studies, suggesting that NIS gene can also serve as a therapeutic agent when combined with radioiodide injection. Better NIS-mediated imaging and tumor treatment by radioiodide requires a more efficient and specific system of gene delivery with better retention of radioiodide in tumor. Results thus far are, however, promising, and suggest that NIS gene transfer followed by radioiodide treatment will allow non-invasive in vivo imaging to assess the outcome of gene therapy and provide a therapeutic strategy for a variety of human diseases.

Cerebral blood flow enhancement device using Blood Oxygen Level Sensor (Blood Oxygen Level Sensor를 이용한 대뇌혈류증가 장치)

  • Surgery to increase cerebral blood flow is one of the treatment methods of cerebral infarction. In order to supplement this invasive method, non-invasive devices have been introduced that use human blood pressure to pressurize the extremities to increase cerebral blood flow. However, the problem of poor speed and accuracy was raised. In this paper, the perfusion index of each arm is measured by applying pressure to both arms using Blood Oxygen Level Sensor to improve the accuracy of measurement and measurement time. The pressure applied to the arm is calculated by using the pressure value obtained from the arm. Like the existing blood pressure measuring cerebral blood flow increasing device, the blood flow can be increased by more than 20% and the measurement time can be shortened, so that it can be selectively used for the patient with cerebral infarction.

  • In this study, an implementation of a system for measuring more accurate blood pressure by non-invasive methods of oscillometric was performed to reduce errors and weaknesses of the existing invasive blood pressure measurement methods. The system is composed of pressure control, signal measurement and blood pressure signal processing units. To verify the validity of the system, tests of characteristics evaluations for pressure measurement unit, extraction of characteristic ratios for blood pressure estimation, blood pressure tracking by oscillometric method were performed. A group of five adult male was selected for the clinical test of the implemented system. The results of the oscillometric method in comparison with auscultatory method are that the maximum ratios of PAD of average, systolic and diastolic arterial pressure are 1.38%, 1.63% and 2.97% with SEP of 5.00, 3.72 and 4.34.

A Case of Septic Arthritis due to Streptococcus intermedius in a Healthy Child (특이 과거력이 없는 소아에서 발생한 Streptococcus intermedius 화농 관절염 1예)

  • Streptococcus intermedius is a small, non-motile, Gram-positive, non-sporeforming, and aerotolerant anaerobic coccus. It is a part of the normal microflora in the oral cavity and upper respiratory, gastrointestinal and female urogenital tracts. It is an opportunistic pathogen that causes serious infections in patients with immunocompromised states or cardiac diseases as a result of trauma or invasive procedures. We describe a case of septic arthritis of the hip caused by S. intermedius in an immunocompetent healthy 7-year-old boy without a history of periodontal disease or invasive procedures. He had hip joint pain three weeks ago, and the fever began on the day of the visit. He had been healthy and had not undergone any invasive procedures recently. Septic arthritis of the hip was indicated in the magnetic resonance imaging of the hip. S. intermedius was identified in the hip joint fluid aspiration and blood culture. He was successfully treated with surgical intervention and antibiotic therapy with ceftriaxone followed by amoxicillin for five weeks.
