Aesthetic Design of Skin-Sparing Mastectomy Incisions for Immediate Autologous Tissue Breast Reconstruction in Asian Women

  • Background The advent of skin-sparing mastectomy has allowed for the reconstruction of the breast and nipple with improved cosmesis. However, the nipple-areolar complex (NAC) in Asian patients is more pigmented and scars easily. Therefore, commonly described incisions tend to result in poor aesthetic outcomes in Asian patients with breast cancer. Methods We describe an algorithmic approach to skin-sparing mastectomy incisions in Asian patients on the basis of the location of the biopsy scar and the tumor site and size. Four incision types are described: peri-areolar, a peri-areolar incision with a second distant skin paddle, "racquet handle," and peri-areolar with adjacent skin excision. Results 281 immediate breast reconstructions were performed between May 2001 and February 2012 after skin-sparing mastectomy. The mastectomy incisions used included the peri-areolar design (n=124, 44%), peri-areolar design with a second distant skin paddle (n=39, 14%), "racquet handle" (n=21, 7.5%), and peri-areolar design with adjacent skin excision (n=42, 14%). The traditional elliptical incision and other variants where the NAC outline was not preserved were performed in the remaining 55 patients. The average follow-up was 44.7 months during which there was 1 case of total flap loss and 7 cases of partial flap necrosis; all remaining flaps survived. 24% of the patients (68/281) underwent subsequent nipple reconstruction. Conclusions Our algorithm avoids breast incisions that are randomly placed or excessively long and prevents the unnecessary sacrifice of normal breast skin. This allows skin-sparing mastectomy and immediate breast reconstruction to be performed with a consistently achievable aesthetic result in Asian women without neglecting oncological safety.

Study of Real Somatotype and Perceived Body Classified by Body Index of College Women (여대생(女大生)들의 지수치(指數値)에 의한 체형(體型) 집단(集團)별 실제체형(實際體型)과 신체인식(身體認識)에 관한 연구(硏究))

  • The purpose of this research is to study the recognition of real somatotype and body of college women. The study was carried out by classifying somatotypes into three categories based on the body index; slim, normal, and obese somatotype. The subject of this study was 106 college women, and real measurement and survey were used. Data was analyzed by SPSS 12.0 and cross tabulation analysis, t-test, Anova, and correlation analysis were operated. The result is as follows. 1. The height of the subjects was 163.5cm, weight was 53.3kg, Rohrer index was 1.2, BMI was 19.9, Vervaeck index was 84.4 and the classification based on the body index belonged to the normal group. 2. The characteristics of groups divided by the body index are that slimmer groups is longer in length factor and smaller in the factors related with volume than other groups. The characteristics of groups classified by self-consciousness shows significant difference in all girth factors, back intersyce breadth, front intersyce breadth, neck to nipple length, and nipple to nipple breadth. 3. Conscious about the somatotype with using body index generally shows similarity. In the length factor including height, slim somatotype group is perceived as longer than normal somatotype group, and in the girth factor including weight, normal somatotype group is recognized as bigger and thicker than others.

A Study on Desirable Breast Type of Women in Their Twenties Based on the Ratio of Breast in Works of Art and Cup Design According to Breast Type (미술작품의 유방 실루엣 비율에 근거한 20대 여성의 바람직한 유방형태와 유형별 컵 설계에 관한 연구)

  • This paper analyzes desirable breast types using measurements from 31 selected female subjects based on the ratio of breast in works of art and to design a brassiere cup design according to the breast type of 182 female subjects in their 20s. The subjects selected for this paper are somewhat larger than a previous study on aesthetically perfect breasts of foreigners according to chest breadth, center neck point to breast point, breast circumference, and breast volume. However, the aesthetic breasts of foreigners are larger than the subjects selected for this study according to chest depth, under-bust circumference, and bust point to bust point. Comparing various breasts types and aesthetic breasts, padding is necessary to complement the form of flat breasts to increase the volume and diameter. Brassiere cups for cone-shaped breasts should be designed to increase breast volume through an increase in nipple height. Hemisphere breasts should be designed to increase the overall volume-leaving diameter. Protrusion breasts should enhance the functions of aggregating and supporting without any increase or decrease of the breast volume. Drooping breasts require the ability to support a large volume to stabilize the breast. Subjects were selected depending on the ratio of breast silhouette as works of art and who have large breasts in disproportional to a slender trunk. Three items, the circumferential length of breast, height of the nipple, and the depth of inner breast using the anthropomorphic measurements of 182 subjects were measured through regression equations for breast volume. Breast volume = -394.86 + 27.52 ${\times}$ (the circumferential length of breast) + 18.73 ${\times}$ (height of the nipple) + 12.85 ${\times}$ (the depth of inner breast). Regression equations to extract the aesthetic breast volume in measurements irrelevant to breasts using the anthropomorphic measurements of 31 subjects were as follows. Aesthetic breast volume = -611.30 + 17.67 ${\times}$ (bust circumference) -24.29 ${\times}$ (under-bust circumference) + 16.31 ${\times}$ (neck point to breast point to waistline) + 22.83 ${\times}$ (bust breadth) + 12.22 ${\times}$ (waist depth) -8.34 ${\times}$ (interscye- front). This prediction equation is useful to develop a breast type brassiere pattern, complement breast enhancement surgery, or minimize the effect a mastectomy.

Prevention of Implant Malposition in Inframammary Augmentation Mammaplasty

  • Background Implant malposition can produce unsatisfactory aesthetic results after breast augmentation. The goal of this article is to identify aspects of the preoperative surgical planning and intraoperative flap fixation that can prevent implant malposition. Methods This study examined 36 patients who underwent primary dual plane breast augmentation through an inframammary incision between September 1, 2012 and January 31, 2013. Before the surgery, preoperative evaluation and design using the Randquist formula were performed. Each patient was evaluated retrospectively for nipple position relative to the breast implant and breast contour, using standardized preoperative and postoperative photographs. The average follow-up period was 10 months. Results Seven of 72 breasts were identified as having implant malposition. These malpositions were divided into two groups. In relation to the new breast mound, six breasts had an inferiorly positioned and one breast had a superiorly positioned nipple-areolar complex. Two of these seven breasts were accompanied with an unsatisfactory breast contour. Conclusions We identified two main causes of implant malposition after inframammary augmentation mammaplasty. One cause was an incorrect preoperatively designed nipple to inframammary fold (N-IMF) distance. The breast skin and parenchyma quality, such as an extremely tight envelope, should be considered. If an extremely tight envelope is found, the preoperatively designed new N-IMF distance should be increased. The other main cause of malposition is failure of the fascial suture from Scarpa's fascia to the perichondrium through an inframammary incision. As well, when this fixation is performed, it should be performed directly downward to the perichondrium, rather than slanted in a cranial or caudal direction.

  • The high pressure hoses are widely used for the vehicles, aircraft, and overall industries. The hose assembly is generally composed of a nipple, a socket and a hose with reinforcement layers to increase the tensile strength. To produce the hose assembly, crimping or swaging process is usually used to clamp its components to ensure the prevention of fluid leakage. Crimping is a cold-working technique to form a strong bond between the workpiece and a non-metallic component. The crimping stroke is a primary parameter to be determined in the metalworking process, and it plays an important role in hose performance. This study aims at investigating the optimal crimping stroke according to the size of aircraft high pressure hose by using MSC/MARC. It is supposed that the results can be useful to get the information about the crimping stroke in manufacturing process, even with the different size of a hose.

A Study on the Upper Body Shapes of Late Elementary Schoolgirls (학령후기 여아의 상반신 체형 연구)

  • This study is done to classify the upper body shapes for late elementary schoolgirls. The sampling was done for 11~12 years-old-girls resident in Busan and Kyungnam. Based on the somatometric charateristics of them, 33 anthropometic and 7 photogrphic measurment data were acquired from every girl. These data are statistically analyzed with the following methods; Factor Analysis, Cluster Analysis, and Discriminant Analysis. Resulting from the factor analysis, it is shown that 79.95% of the whole variances can be explained with 8 factors. Through the cluster analysis, 3 types of upper body shapes can be categorized as follows: Type I has average horizontal size, big vertical size and lots of protruded chest ; Type III has big horizontal size, the mean vertical size, and big upper angle of the back ; Type II has small horizontal and vertical size and long surface length of the upper body. Through the discriminant analysis, the high discriminative items in discriminant function are follows: Upper chest circumference, arm length and waist front length of discriminant function I and waist depth, front length, back breadth, nipple to nipple breadth and upper chest circumference of discriminant function II have large coefficient values.

A Predictive Model using Decision Tree Method on Demand for Alternative Feeding Education by Nurses (의사결정나무분석법을 이용한 간호사의 대체수유교육요구 예측모형)

  • Purpose: One of the main reasons why mothers quit breast feeding is that the volume of breast milk is inadequate due to insufficiency in suckling. We believe suckling experience may be a factor affecting nipple confusion. So an alternative feeding method, namely cup, spoon, finger, or nasogastric tube feeding may be needed to prevent nipple confusion. The purpose of this study was to construct a predictive model for demand for alternative feeding education by nurses. Methods: A descriptive design with structured self-report questionnaires was used for this study. Data from 175 nurses working in hospitals in Busan were collected between April 1 and 15, 2009. Data were analyzed by decision tree method, one of the data mining techniques using SAS 9.1 and Enterprise Miner 4.3 program. Results: Of the nurses, 81.1% demanded alternative feeding education and 5 factors showed that most of them expressed intention to pay, desire to know about alternative feeding, age, and learning experience. From these results, the derived model is considered appropriative for explaining and predicting demand for alternative feeding education. Conclusion: This confirms that knowledge and compliance in alternative breast feeding for newborn babies should be correct and any inaccuracies or insufficient information should be supplemented.

The Effects of Antenatal Breast Care on Breast Feeding practice (임신중 유방간호가 산모의 모유수유 실천에 미치는 영향)

  • The purpose of this research was to study the effects of antenatal breast care on decreases in breast discomfort and increases in the breast feeding rate during the postpartum period. A nonequivalent control group posttest research design was used. The experimental group consisted of fifty -one pregnant women(primigravida) who were receiving antenatal care in the OBGYN clinics of four hospitals between March 5 and May 30, 1991. The control group was made up of seventy - five postpartum women who delivered at two hospitals OBGYN clinic and one midwife clinic between May 4 and June 15, and between September 5 and October 15, 1991. Data were collected via telephone interviews on the seventh postpartum day and at the end of the second month. Data analysis methods used frequencies and the x$^2$- test. The results were as follows : 1. The rate of breast feeding practice at two months was higher in the experimental group(70.6%) than in the control group(25.3%) (p<.01). 2. Nipple soreness in the early breast feeding period was lower in the experimental group(14.6%) than in the control group(25.3)(p<.01). 3. Severe breast discomfort in the early breast feeding period was lower in the experimental group (12.5%) than in the control group(39.2%)(P<.01). 4. There was a significant relationship between the breast feeding practice and the planned feeding method(p<.05), and between breast feeding practice and nipple soreness(p<.01) in the experimental group, and the presence of a job(p<.01), the sex of the infant(p<.05), and the first feeding time(p<.05) in the control group. 5. The reasons for unsuccessful breast feeding were a deficiency of breast milk (66.7% in the experimental group, 30.4% in the control group), poor sucking on the part of the baby(13.3% in the experimental group, 21.4% in the control group).

Design of a combined cartilage graft crusher, morselizer and holder for use in rhinoplasty

  • A combined cartilage holder and crusher is described that allows the surgeon to hold, crush, morselize, and suture a single piece or stack of cartilage graft without letting it slip. The customized slit-shaped jaws allow adequate room for the suture needle, while the serrated surfaces hold the cartilage firmly. The use of this instrument is advocated primarily in rhinoplasty for manipulating and suturing a small cartilage graft or a stack of grafts. The use of this instrument may be extended to aesthetic or reconstructive cases where cartilage grafts need to be sutured or shaped, as in eyelid, ear, and nipple reconstruction.

  • Vivienne Westwood could be placed in a unique position in the contemporary fashion design. She has never belonged in a core group of mainstream fashion designer but not been always in an outsider position. She got from mainstream fashion designer circles enormous appreciation, even established fashion industry was influenced by her fashion design. She began her fashion design with rebellious T-shirts like 'destroy' T-shirt, chicken-bone T-shirt, and nipple-zipper T-shirt, all of which revealed her disgust against establishment. All these T-shirts testify her total negation tendency in her youth. However, she did not continue to keep such a kind of total negation attitude against establishment In 1980s she changed her political attitude towards establishment, and this change also found a reflection in her fashion design. In her pirate-collection the dark image of her fashion in the 1970s changed into a totally different bright image with full of gold colour. Although this collection had radiated brightness, it contained still outsider character from mainstream fashion design. The following fashion design in 1980s and 1990s evolved further on the line of Pirate-collection, but they continued to contain certain outsider characteristics. Vivienne Westwood kept some kind of anti-establishment attitude, and this attitude more or little came to the surface of her design. She was always political and critical to the establishment. In 2005, in her 64, she designed a liberty T-shirt, which showed her political attitude and her engagement in social issues. Also it showed the change from the early total negation to the constructive critical affirmation. In this paper the relation between the fashion design of Vivienne Westwood and her political attitude and the influence of her political attitude to her fashion design is analyzed.
