금연프로그램에 대한 인지도, 이용의도 및 영향요인 (Factors Related to the Recognition and Behavioral Intention for Smoking Cessation Programs)

  • This study was conducted to evaluate factors related to the recogniton and behavioral intention for smoking cessation programs. Five effective smoking cessation programs were considered: acupuncture, nicotine patch, clinic program, mass education, and alliance programs. To explain the health behavior for smoking and smoking cessation programs, a five-stage behavioral intention model was built, and 500 questionnaires were completed through a telephone survey. Stages of the model included recogniton of the programs, past experiences, present smoking status, intention for smoking, and behavioral intention for smoking cessation programs. The results showed that the recogniton rate of the programs were low in general, therefore strategies of education, public relations, and advertisement need to be pursued. Nicotine dependency resulted in the fact that success rates were low although trial rates of smoking cessation were high among smokers. The necessity for smoking cessation programs was suggested. And the significant factors related to the intention for smoking cessation were individual attitude and reluctancy to pay time and money. Others' attitude was insignificant to subjects' smoking cessation. Purchase rates for nicotine patches were 11.3% for male and 27.3% for female, those for acupunture were 7.6% for male and 10.0% for female. There were very low purchase rates for clinic, mass education, and alliance programs. In conclusion, evidence-based and effective smoking cessation programs need to be promoted by medical doctors. Strategies in education, public relations, and advertisement also need development. In addition, continuing legal and systematic support for smoking cessation would lower the smoking rate and ultimately contribute to the nation's health.

심혈관계 질환자의 금연성공 예측 요인 (Factors which Differentiate Ex-smokers from Smokers among Patients with Cardiovascular Disease)

  • Purpose: The purpose of this study was to identify factors which differentiate ex-smokers from smokers among patients with cardiovascular disease. Ex-smokers were patients who abstained from smoking for at least 6 months. Method: The subjects were 92 male cardiovascular patients who try to quit. The stepwise discriminant analysis was used to identify predictors of smoking cessation. Result: 1. The ex-smokers(n=47) and smokers(n=45) were significantly different regarding the age, duration after diagnosis, nicotine dependence, family support for smoking cessation, and intention-to-quit. 2. The family support for smoking cessation, intention-to-quit, nicotine dependence, and age were the predictors which differentiate ex-smokers from smokers among cardiovascular patients. The percentage of grouped cases correctly classified was 92.4%. Conclusion: The higher the family support for smoking cessation, intention-to-quit, age and the lower the nicotine dependence, the more likely these smokers were to success smoking cessation.

PC방에서의 간접흡연에 따른 요중 코티닌의 농도

  • Many adolescents stay long time in the PC game room and are exposed to much of tobacco smoke. To evaluate the effect of passive smoking in the PC game room, airborne nicotine concentrations and urinary cotinine concentrations were measured for 20 adolescents at 2 PC game rooms in Sung-nam city. And the subjects were interviewed for duration and time in PC game room and smoking pattern. Subjects are composed of each of 10 smokers(5 males and 5 females) and 10 non-smokers(5 males and 5 females). They stayed for three hours in the PC game room without smoking. Concentrations of nicotine in smokers and non-smokers were 129.72 $\mu\textrm{g}$/㎥ and 99.99 $\mu\textrm{g}$/㎥, respectively. Urinary cotinine concentrations were increased as time goes on after exposure to nicotine and showed maximum value at 9.45 hours after nicotine exposure and were 32.21 and 110.66 $\mu\textrm{g}$/L for non-smoker and smokers. Urinary cotinine has a tendency to be increased by passive smoking. Therefore, it is recommended that the effective control for indoor air quality and extensive research be needed to reduce nicotine concentration by passive smoking in the PC game room.

Uninary Nicotine and Cotinine Levels in Smokers and Nonsmokers Related to Smoking Habit in Korea

  • This study was conducted to determine the urinary nicotine and cotinine concentration in 126 smokers and 143 nonsmokers. While urine samples were being collected, personal characteristics related to smoking habit such as sex, age, number of years since a person has been a smoker, average number of cigarettes consumed per day, and number of smokers in the family were surveyed. Urinary nicotine and cotinine concentration were analyzed by GC/NPD. The smokers smoked an average 17.0 cigarettes per day and the average concentration of nicotine and cotinine was 3.88 $\mu\textrm{g}$/ml and 3.64 $\mu\textrm{g}$/ml, respectively. The average number of smokers in the family was 0.72 persons and the average concentration of nicotine and cotinine were 0.11 $\mu\textrm{g}$/ml and 0.02 $\mu\textrm{g}$/ml in the urine of non-smokers, respectively. The concentration of nicotine and cotnine in smoker\`s urine was dependent on the number of cigarettes smoked per day(p<0.01). The number of years since a person had been a smoker, and the number of smokers in the family were not associated with the concentration of nicotine and cotinine. Also there was no significant effects of passive smoking on the family of smokers by the level of nicotine and cotinine concentration. We describe the relationship between smoking habit as assessed by urinary nicotine and cotinine excretion. This study provides an evidence for the assessment of active and passive exposure to tobacco smoke.

Frequency of Cigarette Smoking Among Psychiatric Inpatients Evaluated by the Fagerstrom Test for Nicotine Dependence

  • Background: In this study our aim was to determine the rate of smoking in a sample of psychiatric in-patients with diagnoses of schizophrenia, bipolar disorder and major depression and to examine factors related to smoking status and the level of dependence in this population. Materials and Methods: A total of 160 people were included in this descriptive study. 80 were inpatients with schizophrenia, bipolar disorder and major depression and 80 people without any psychiatric diagnoses were included as a control group. The participants were interviewed face-to face using a semi-structured questionnaire and Fagerstrom Test for Nicotine Dependence was used to define smoking habits. Results: The mean age of the participants was $37.24{\pm}12.19$ years ranging from 18 to 81 years, 54.4% of the participants were (n=87) female, and 45.6% cases (n=73) were male. 70% (n=56) of the patients and 55% of the control group were smoking and the difference was statistically significant (p<0.01). Total score of Fagerstrom Test for Nicotine Dependence in the patient group was statistically significantly higher than in the control group (p<0.01). Conclusions: In our sample, the frequency of cigarette smoking and nicotine dependence among psychiatric inpatients was high, posing a high risk for smoking related diseases including cancers; therefore there should be counseling on tobacco control and smoking cessation programming targeting this population.

Policy Effects of Secondhand Smoke Exposure in Public Places in the Republic of Korea: Evidence from PM2.5 levels and Air Nicotine Concentrations

  • Objective: The purpose of this study was to evaluate secondhand smoke (SHS) exposure inside selected public places to provide basic data for the development and promotion of smoke-free policies. Methods: Between March and May 2009, an SHS exposure survey was conducted. $PM_{2.5}$ levels and air nicotine concentrations were measured in hospitals (n=5), government buildings (4), restaurants (10) and entertainment venues (10) in Seoul, Republic of Korea, using a common protocol. Field researchers completed an observational questionnaire to document evidence of active smoking (the smell of cigarette smoke, presence of cigarette butts and witnessing people smoking) and administered a questionnaire regarding building characteristics and smoking policy. Results: Indoor $PM_{2.5}$ levels and air nicotine concentrations were relatively higher in monitoring sites where smoking is not prohibited by law. Entertainment venues had the highest values of $PM_{2.5}$(${\mu}g/m^3$) and air nicotine concentration(${\mu}g/m^3$), which were 7.6 and 67.9 fold higher than those of hospitals, respectively, where the values were the lowest. When evidence of active smoking was present, the mean $PM_{2.5}$ level was 104.9 ${\mu}g/m^3$, i.e., more than 4-fold the level determined by the World Health Organization for 24-hr exposure (25 ${\mu}g/m^3$). Mean indoor air nicotine concentration at monitoring sites with evidence of active smoking was 59-fold higher than at sites without this evidence (2.94 ${\mu}g/m^3$ vs. 0.05 ${\mu}g/m^3$). The results were similar at all specific monitoring sites except restaurants, where mean indoor $PM_{2.5}$ levels did not differ at sites with and without active smoking evidence and indoor air nicotine concentrations were higher in sites without evidence of smoking. Conclusion: Nicotine was detected in most of our monitoring sites, including those where smoking is prohibited by law, such as hospitals, demonstrating that enforcement and compliance with current smoke-free policies in Korea is not adequate to protect against SHS exposure.

대구지역 고등학생들의 흡연실태 및 니코틴 의존도 (Smoking Status and Nicotine Dependency of High School Students in Daegu)

  • The purpose of this study was to surveyed the smoking status and pattern, nicotine dependence and factors associated with smoking and nicotine dependence for high school students in Daegu. In March. 2000, 1,593 students from 7 male and 6 female high schools located in Taegu were assessed with self-completing questionnaire. The major findings were as follows; Percentage of male smokers was 15.0% in general high school, 30.9% in business high school and that of female smokers was 5.0% in whole woman's high school. The median score of FTQ was 4.0 among male smokers and female smokers. the mean of FTQ score was $4.07{\pm}1.96$ in male and $3.94{\pm}1.63$ in female. Percentage of students with high FTQ score(FTQ$\geq$7) was 13.3% in male and 5.9% in female. In multiple logistic regression analysis, smoking had significant association with general characteristics such as personal-specifics factors, family environment factors and school environment factors. In multiple logistic regression analysis, nicotine dependence had significant association with school-age and duration of smoking On consideration of above findings, to prevent students from smoking and to reduce the smoking rate, we have to try together in house, school and society.

남자 대학생을 위한 금연프로그램의 효과 - 니코틴 의존도와 자기효능감을 중심으로 - (Effects of Smoking Cessation Program for Male University Students: Perceived Nicotine Dependency and Self-efficacy)

  • Purpose: The purpose of this study was to evaluate the effectiveness of 8 weeks smoking cessation program(SCP) developed by researchers. Method: One group pretest-posttest design was used in this study. The subjects were 88 male university students who consented to participate in the 8 weeks SCP. The SCP in this study consisted of a stop-smoking class, e-mail, SMS(short message service), cellular phone, internet cafe, displayed panel, and patch or acupuncture for quitting smoking. Korean version of questionnaire for nicotine dependency by Ahn et al.(2002), the smoking self-efficacy scale translated by Choi(1999) and subjects' opinion about the SCP were measured. The data were analyzed by using frequency, Chi-Square test, paired t-test and repeated measures ANOVA with SPSS WIN 10.1 Program. Results: 1) The rate of quitting smoking was 31.8% after 8 weeks SCP. 2) There was no significant decrease in the nicotine dependency score among smoking subjects after SCP. 3) There was a significant increase in self-efficacy after SCP. The mean score of self -efficacy in the quitting smoking group increased significantly, but did not significantly change in the smoking group. 4) The stop-smoking class was the most helpful among contents of the SCP. Contents of the SCP except for internet cafe were useful for quitting smoking in this study. Conclusion: The above results indicated that the 8 weeks SCP would be a helpful intervention to quit smoking for male university students.

금연 자기효능감 증진 프로그램이 흡연 급성관상동맥증후군 환자에게 미치는 효과 (Effects of the Smoking Cessation Self-efficacy Improvement Program on Smoking Patients after Acute Coronary Syndrome)

  • Purpose: The purpose of this study was to examine the effects of the program to improve on smoking patients' after acute coronary syndrome (ACS) smoking cessation rate, smoking cessation related self-efficacy, carbone monoxide (CO), nicotine dependence, and pain sense. Methods: This study used a non-equivalent control group pretest-posttest design. The participants of this study were 60 ACS patients: experimental group (30), control group (30), who received percutaneous coronary intervention. The study lasted from Aug 16, 2016 to Jan 13, 2017. Smoking cessation rate, smoking cessation related self-efficacy, CO, nicotine dependence, and pain sense were measured using the structured questionnaires and CO monitor Results: An examination of the effects of this program revealed the experimental group to have a significant increase in smoking cessation rate, smoking cessation related self-efficacy than the control group, and a significant decrease in CO, nicotine dependence and pain sense than in the control group. Conclusion: The findings indicate that this smoking cessation self-efficacy improvement program is effective for hospitalized patients after ACS.

Smoking Cessation Strategies Targeting Specific Populations

  • Quitting smoking helps smokers maintain their health and extend their lifespan by 10 or more years. Treatment strategies for smoking cessation should be tailored to individual smokers with special needs based on their specific circumstances. It is recommended that pregnant women adopt smoking cessation through counseling and behavioral interventions because the safety of medications has yet to be established. Counseling is the main strategy for smoking cessation in adolescents and nicotine replacement therapy can be used with caution in individuals with serious nicotine dependence. It is important for smokers with psychiatric diseases to quit smoking following accurate assessment of their depression status. Nicotine replacement therapy, varenicline, and bupropion can be used for smoking cessation in smokers with psychiatric disorders. The incidence of cardiovascular disease decreased according to the smoking status and the duration of smoking cessation. In smokers with chronic obstructive pulmonary disease (COPD) who used a combination of counseling and pharmacotherapy the quitting rate was more than twice as high as subjects who used behavioral interventions alone. Varenicline can be used as the most effective anti-smoking drug by most smokers including those with psychiatric disorders, cardiovascular disease, and COPD.
