Influence of Maternal Environmental Tobacco Smoke Exposure Assessed by Hair Nicotine Levels on Birth Weight

  • Background: While the perinatal outcomes of active maternal smoking are well documented, results of the effects of environmental tobacco smoke (ETS) exposure during pregnancy are inconsistent. We aimed to examine the effect of ETS exposure, assessed by maternal hair nicotine levels at $35^{th}$ week of gestation, on birth weight and the risk of small for gestational age (SGA) and low birth weight (LBW). Materials and Methods: A total of 871 non-smoking healthy pregnant women were recruited by one Korean hospital between 1 October 2006 and 31 July 2007. Hair samples were collected and anthropometric questionnaires administered at $35^{th}$ week of gestation. The primary outcome was birth weight and secondary outcomes were the risk of babies being SGA and LBW. Results: Log-transformed hair nicotine concentrations were inversely related with birth weight after adjusting for confounding variables (${\beta}=-0.077$, p=0.037). After stratifying hair nicotine levels by tertiles (T1, low [0.0-0.28 ng/mg]; T2, medium [0.29-0.62 ng/mg]; and T3, high [0.63-5.99 ng/mg]), the mean birth weight in each groups were 3,342g (T1) 3,296g (T2) and 3,290 g (T3), respectively. However the difference between groups was not statistically significant by analysis of co-variance (ANCOVA) adjusting for covariates (p=0.062). In logistic regression analysis, the risk of SGA was higher in the T3 (OR=1.59, 95%CI 1.05-2.42) than in the reference group (T1), after controlling for confounding variables. The risk of low birth weight (<2,500g, LBW) was not significantly higher (OR=1.44, 95%CI 0.95-2.19), but the risk of babies being below 3,000g birth weight was increased in the T3 group (OR=1.53, 95%CI 1.00-2.36) compared with that in the T1 group. Conclusions: Maternal ETS exposure during pregnancy was inversely related with birth weight. The risk of SGA increased in the highest ETS exposure group compared with in the low exposure group. To prevent ETS exposure during pregnancy, more comprehensive tobacco control policies are needed.

경남지역 일부 남자대학생의 문제음주에 영향을 미치는 요인 (Factors affecting Problematic Drinking of Male College Students in the Gyeongnam Area)

  • This study investigated the factors affecting problematic drinking amongst the male university students of the Changwon area (n=367). The participants were divided into 3 groups, and undertook the self-report questionnaire survey. Based on the AUDIT (Alcohol Use Disorders Identification Test) guidelines, problematic drinking were rated as 'no problem' (28.8%), 'at-risk drinking' (41.7%), and 'alcohol abuse' (29.5%). Our study revealed that students living alone, indulged in more club activities, and drank more than 3 cans of carbonated drinks a day; this was significantly high on the AUDIT score (p<0.05). Participation in education was significantly higher (p<0.05), but intention to reduce the monthly alcohol consumption was significantly lower (p<0.001) in the alcohol abuse group. Recognition level of drinking cultures, drinking habits, AUDIT, and nicotine dependence were significantly higher in the alcohol abuse group (p<0.001). Recognition level of drinking cultures and nicotine dependence by the FTND (Fagerstrom Test for Nicotine Dependence) positively correlated with problematic drinking, whereas dietary guidelines and self-esteem showed a negative correlation with problematic drinking (p<0.05). Based on the results of multiple linear regression analysis, the factors affecting problematic drinking were recognition level of drinking cultures (${\beta}=0.47$, p<0.001) and nicotine dependence (${\beta}=0.23$, p<0.001). We conclude that implementation of health education for university students, would aid in rectifying the incorrect perception of drinking. Furthermore, both drinking and smoking should be considered simultaneously.

Shisha Smoking and Associated Factors among Medical Students in Malaysia

  • Background: The aim of the study was to determine the prevalence of shisha smoking and associated factors among medical students in Malaysia. Materials and Methods: A cross-sectional study was conducted at the Management and Science University from December 2011 until March 2012. The questionnaire consisted of five sections including socio-demographic, social environment, knowledge about shisha, psychosocial factors, and personal shisha smoking behavior. Obtained data were analyzed using Statistical Package for the Social Sciences (SPSS 13). T-test was used to determine the relationships between shisha smoking and socio-demographic characteristic. Results: A total number of 300 medical students participated in this study. Mean age was $22.5{\pm}2.5$ years. The majority were female, Malay, single, from urban areas (67%, 54%, 97%, 73%; respectively). The prevalence of shisha smoking among medical students was found to be 20%. The study revealed that many students believed that shisha does not contains nicotine, carbon monoxide, does not lead to lung cancer, dental problems and does not lead to cardiovascular diseases (25%, 20.7%, 22.3%, 29%, 26.7%; respectively). Age and sex were found to be significantly associated with smoking shisha status among medical students (p=0.029, p<0.001; respectively). Furthermore, having parents, siblings and friends smokers of shisha were found to be significantly associated with shisha smoking status (p<0.001, p<0.001, p<0.001; respectively). Furthermore, family problems, problems with friends, financial problems and university life were found to significantly associated with shisha smoking status among medical students (p<0.001, p=0.002, p<0.001, p=0.002; respectively). Conclusions: There is a high prevalence of shisha smoking and a poor knowledge about its impact on health among medical students. More attention is needed to focus on medical education in this regard. The policies that are currently employed in order to reduce the cigarettes smoking should be applied to shisha smoking and shisha products.

  • 보건소 '금연클리닉사업'은 흡연자들에게 6개월 동안 9회 이상의 금연상담서비스와 CO측정, 니코틴 패치, 니코틴 껌, 니코틴 사탕 등의 니코틴보조제를 제공한다. 또한 행동 강화 물품과 금연 성공 기념품을 제공하는 등, 금연 실천을 유도하고 흡연율을 감소시켜 지역주민의 건강증진을 도모한다. 흡연과 과음 등의 생활습관은 고지혈증 및 비만 등 만성질환을 유발시키는 주요인자이며, 경제적인 손실, 나아가 건전한 문화교양인으로서의 삶을 파괴하는 한 요인이 된다. 흡연, 과음 등의 생활습관이 건전한 삶이라는 문화적 가치와 더불어 의료비라는 경제적 가치와도 밀접한 관계가 있음을 고려해 볼 때, 흡연 예방 및 금연과 절주 등의 보건교육에 동참하는 사업장과 기관에 대한 제도적 지원이 병행되어야 할 것으로 사료된다.

비흡연 대학생의 전자담배 간접흡연에 대한 흡연태도와 흡연대처행동의 관계 (The Relationship between Attitude and Coping Skills toward Secondhand E-cigarette Smoking among Nonsmoking College Students)

  • Purpose: The purpose of the study was to examine the relationship between smoking attitude and coping skills toward secondhand e-cigarette smoking among nonsmoking college students. Methods: This study was conducted with 148 students from a University in Seoul, Korea. Participants completed questionnaires and the data were analyzed using a T-test, ANOVA, and Pearson's correlation coefficients. Results: The mean score of smoking attitude is $51.35{\pm}4.73$. The mean score of coping skills is $13.89{\pm}2.53$ in verbal skills, $10.16{\pm}2.37$ in behavioral skills, and $8.32{\pm}2.30$ in assertive skills. Coping skills toward secondhand smoking are noted as being different by current cohabitation smoker. A significant relationship between smoking attitude and behavioral coping skills(r = .23, p= .004) was found, which means that a better smoking attitude was correlated with more active behavioral coping skills. Conclusion: The results showed that nonsmoking college students with better smoking attitude react to the E-cigarette smoking with more active behaviors. Therefore, interventions are needed to be developed to encourage better smoking attitude and coping skills toward secondhand E-cigarette smoking.

  • 흡연의 주성분중의 하나인 니코틴은 인체내에 다양한 전신적 및 국소적인 질환의 원인으로 보고 되어지고 있다. 전신적인 질환에 있어 특히, 호흡기와 순환기 조직세포에 대한 세포분열효과가 많은 연구의 초점이 되어왔으며, 국소적인 효과에 대한 연구에서는 조직파괴나 치료후 치유지연에 대해 보고하고 있다. Platelet-Derived Growth Factor(PDGF)와 Insulin-like Growth Factor(IGF)는 치주인대세포의 세포분열을 촉진하는 주요 성장인자로 알려져있다. 본 연구의 목적은 니코틴이 사람의 치주인대세포에 미치는 세포분열효과를 알아보기 위하여 니코틴 처리된 치주인대세포로부터 추출한 PDGF-${\alpha}\;and\;{\beta}$ receptor 및 IGF-l 수용기의 mRNA변화를 Northern분석을 이용해 확인해 보고자 함이다. 실험군은 각기 다른 농도의 니코틴(100ng/ml, 1000mg/ml)과 배양액내 혈청농도($1\%,\;10\%$)로 나누었으며 이를 각각 니코틴 처리 시간에 따라 분류하였다. 본연구의 결과로 $10\%$ 혈청의 배양액과 100ng/ml 니코틴 농도군에서 모든 성장인자 수용체의 mRNA가 증가됨을 보였으며 이는 흡연자의 체내 축적 가능한 니코틴 농도에서 치주인대세포의 세포분열을 촉진한다는 추측을 가능케 한다.

동기면담 금연 프로그램이 청소년의 금연에 미치는 효과 (Effectiveness of a Motivational Interviewing Smoking Cessation Program on Cessation Change in Adolescents)

  • Purpose: This study examined the effectiveness of an Adolescent Motivational Interviewing Cessation program on smoking cessation change. The study was done with a nonequivalent control group pretest-posttest design. Methods: The participants were 39 high school students from G city, who were in school from September 1 to October 30, 2009. The students were assigned to the experimental group (20) and participated in the motivational interviewing cessation program or to the control group (19) who did not participate. Data analyses involved ${\chi}^2$-test, independent t-test, Repeated Measures ANOVA, and utilized the SPSS program. Results: The experimental group had significantly less daily smoking, nicotine dependence and smoking temptation in comparison to the control group. The experimental group had significantly higher stage of change in comparison to the control group. Conclusion: The results of the study indicate that a motivational interviewing cessation program delivered to adolescents who smoke is an effective method of encouraging cessation, and can be utilized as an effective nursing intervention for adolescents who smoke.

Smoking Cessation Intervention in Rural Kerala, India: Findings of a Randomised Controlled Trial

  • Background: Prevalence of tobacco use is higher in the rural than urban areas of India. Unlike tobacco cessation clinics located in urban areas, community-based smoking cessation intervention has the potential to reach a wider section of the community to assist in smoking cessation in the rural setting. The present study aimed to assess the effectiveness of a cessation intervention in rural Kerala state, India. Materials and Methods: Current daily smoking resident males in the age group 18-60 years from four community development blocks in rural Kerala were randomly allocated to intervention and control groups. The intervention group received multiple approaches in which priority was given to face-to-face interviews and telephone counselling. Initially educational materials on tobacco hazards were distributed. Further, four rounds of counselling sessions were conducted which included a group counselling with a medical camp as well as individual counselling by trained medical social workers. The control group received general awareness training on tobacco hazards along with an anti-tobacco leaflet. Self-reported smoking status was assessed after 6 and 12 months. Factors associated with tobacco cessation were estimated using binomial regression method. Results: Overall prevalence of smoking abstinence was 14.7% in the intervention and 6.8% in the control group (Relative risk: 1.85, 95% CI: 1.05, 3.25). A total of 41.3% subjects in the intervention area and 13.6% in the control area had reduced smoking by 50% or more at the end of 12 months. Lower number of cigarettes/ bidi used, low nicotine dependence and consultation with a doctor for a medical ailment were the statistically significant predictors for smoking cessation. Conclusions: Rigorous approaches for smoking cessation programmes can enhance quit rates in smoking in rural areas of India.

금연클리닉 프로그램을 통한 금연침 효과의 후향 연구 (Effects of the Acupuncture on the Smoking-cessation Clinic Program of Public Health Center in Urban Area: Retrospective Case Control Study)

  • Objectives : This study identified the effects of the acupuncture associated with successful cessation at a smoking-cessation clinic of public health center in an urban area. Methods : The subjects of this study were 1121 patients to the smoking cessation clinic of the Anjung Public Healthcare Center in Pyeongtaek, from June 1, 2007 to December 31, 2008. The 123 patients of them were treated by the auricular acupuncture. The experimental group were treated by the acupuncture, and the control only participated in the smoking-cessation clinic program. The data were analysed by SPSS(version 17.0). Chi-square analysis were conducted to see the different of effects between experimental and control group. Also, to find out the factors that affect the success of smoking cessation, logistic regression and Fisher's exact test were done. Results & Conculusions : 1. As the result of the analysis, it showed that 73.2% of the experimental were able to quit smoking for 4 weeks and 6months. 2. Logistic regression and Fisher's exact test showed that number of Cigarette per day, Nicotine Dependence, CO figure(p=.004<.01), and Number of Acupuncture(p=.000<.001) were the factors that affect the successful smoking cessation. 3. The smoking cessation were more likely to success as patients have lower CO figure(B=-.074), more number of the acupuncture(B=1.718). The more number of acupuncture increased 1 time, the more smoking cessation success did 5.576 times(e$^{(1.718{\times}x)$=e$^{(1.718{\times}1)$ = 5.576). 4. As the result of the analysis between experimental and control group, the success rate have the significance difference(p=.000<.001). 73.2% of the experimental were able to quit smoking, but 45.1% of the control did.

Impact of Admission Diagnosis on the Smoking Cessation Rate: A Brief Report From a Multi-centre Inpatient Smoking Cessation Programme in Singapore

  • Objectives: Few studies have been published regarding the relevance of the admission diagnosis to the smoking cessation rate. We studied smoking cessation rates in relation to admission diagnoses in our inpatient smoking cessation programmes. Methods: This retrospective study included all patients recruited into our inpatient smoking cessation programmes at 2 institutions in Singapore between June 2008 and December 2016. Patients were given individualized intensive counselling and were followed up via phone interviews for up to 6-month to assess their smoking status. Multivariable logistic regression was used to analyse potential associations between admission diagnoses and 6-month abstinence. Results: A total of 7194 patients were included in this study. The mean age was 54.1 years, and 93.2% were male. In total, 1778 patients (24.7%) were abstinent at the 6-month follow-up call. Patients who quit smoking tended to be of Chinese ethnicity, have initiated smoking at a later age, be better educated, and have lower Fagerström Test of Nicotine Dependence scores. After adjusting for these factors, patients with a cardiovascular admission diagnosis had a significantly higher probability of quitting tobacco use than patients with a respiratory or other diagnosis. Conclusions: In patients acutely admitted to the hospital, a diagnosis of cardiovascular disease was associated with the highest quit rate. Smoking cessation interventions need to be incorporated into all cardiovascular disease treatment pathways to leverage the patient's motivation and to improve the quit rate. In addition, patients in groups with lower quit rates may benefit from more intensive programmes to increase the rate of successful cessation.
