Vitamin B-6 nutrition has been shown to be inadequate in many population groups including pregnant and lactating women, and in infants. Vitamin B-6 intake was measured in 98 pregnant mothers and a total of 172 cord blood samples of their and other new born infants were analyzed for erythrocyte alanine aminotransferase(EALAT) activities with or without the addition of pyridoxal-5-phosphate to assess vitamin B-6 status of the infants. The average daily vitamin B-6 intake of the pregnant mothers was 1.79mg$\pm$0.88(81.4% of the Recommended Dietary Allowances ; RDA) and vitamin B-6 to protein intake ration was 0.017mg vitamin B-6/g protein. Thirty-eight percent of the pregnant women consumed diets which provided less than the RDA for vitamin B-6 during pregnancy. Seventy-two percent of the dietary pyridoxine intake was provided by the plant food source whose bioavailability was reported to be lower when compared to that of the animal food. The average activity coefficient(AC) values of the cord blood EALAT was 1.41$\pm$0.11, and 32% of the blood samples had EALATAC values greater than 1.25, suggesting that vitamin B-6 status of the newborns might be less than adequate.

Purpose: The purpose of this study was to develope nursing activities record to improve home care nursing for postpartum women following a Cesarean section and newborns. Method: This study for instrument development had three phases: first, selection of nursing activities according to intervention, second, validation of the preliminary home care nursing activities, and third. application of the home care nursing activities. The subjects for validaton were 137 home care nurses and clinical nurses in department of maternity. Result: By Fehring's method, 116 nursing activities according to 19 interventions were included in the preliminary nursing activities record. Among them, 51 critical nursing activities and 65 supporting nursing activities were chosen. During the final process of validation, 121 nursing activities were included. Conclusion: In order to have systemic standardization of this record forms, replication and application in the various home nursing area is need in the future.
Purpose: The aim of this study was to analyze Neonatal Intensive Care Unit nurses' behaviors while soothing newborns with bronchopulmonary dysplasia. Methods: An observational study was used to assess nurses' soothing behaviors. Data were collected from September, 2012 to March, 2013 using an audio-video recording system. Participants were eight babies and 12 nurses caring for those babies. After obtaining parental permission, the overall process of each episode from nurses' engagement in soothing to the end of soothing was recorded. Then a researcher interviewed each participating nurse. Data from 18 episodes were transcribed as verbal and nonverbal nursing behaviors and then categorized by two researchers. Results: There were 177 observed soothing behaviors which were classified with the five sensory-based categories (tactile, oral, visual, auditory, vestibular). Most frequently observed soothing behavior was 'Gently talking' followed by 'Removing irritant', and 'Providing non-nutritive sucking'. Nurses' perceived soothing behaviors were similar to the observed soothing behaviors except for 'Gently talking'. Conclusion: Nurses used diverse and mixed soothing behaviors as well as recognizing those behaviors as essential nursing skills. Nurses' soothing behaviors identified in this study can be used to comfort babies and to enhance their developmental potential in accordance with individual characterstics or cues.
Purpose: The purpose of this study was to compare birth outcomes between Korean women and immigrant women. Methods: Medical records were reviewed retrospectively from 201 immigrant women and 201 Korean women who delivered babies at K women's hospital in U city from January 2006 to December 2009. Maternal outcomes related factors included nationality, age, obstetric history, delivery type, indications of cesarean section, and complications of pregnancy and delivery. Principal neonatal outcomes were birth weight, Apgar scores, and complications of newborns. Results: Immigrant women were younger and had fewer pregnancies, abortions, and surviving children than Korean women. The rate of primary cesarean section and its indication in immigrant women were not significantly different from Korean women. However, immigrant women's newborn were more likely to have low birth weight and meconium staining. Conclusion: The results of this study indicate less equity of immigrant women in women's health care, although immigrant women's babies had lower Apgar score and more meconium staining. Nurses should help immigrant women cope with labor process effectively to prevent adverse health outcomes for their newborns.
Purpose: The purpose of this study was a comprehensive understanding about maternal transition in mothers with high risk newborns according to the degree of situational meaning. Method: A methodological triangulation that combines qualitative and quantitative methods was used. The situational meaning of a high risk newborn mother was identified using a Family Meaning Attribution Scale. According to the degree of situational meaning, in-depth interviews were conducted at 3 time periods postpartum: between 3-10 days after childbirth, around the time of the newborn's discharge, and between 10-12 weeks after childbirth. Quantitative data was analyzed using descriptive statistics and t-test. Qualitative data was analyzed using Tutty, Rothery, & Grinnell's methodology. Result: The average score of the situational meaning in high the risk newborn mother was 53.57(possible score is between 0-96) and the average score of each item was 1.67. A Maternal transition process in the mother that has a positive situational meaning was conceptualized in three distinctive phases: confusion, accepting, and shaping phases. The Maternal transition process in the mother that has a negative situational meaning was also conceptualized in three distinctive phases: avoiding, conflicting, and accepting phases. Conclusion: It is necessary that the nurses provide high risk newborn mothers with individualized care considering both the situational meaning that is attributed to them and the maternal transition phase that they are faced with.

The ventricular myocardia of 14, 16, 18 and 20-day-old rat fetuses and newborns have been studies by light and electron microscopic morphometrics. The volume density of the myocyte and interstitial compartments as well as volume, surface and numerical density of nuclei were estimated by light microscopic morphometrics. Whereas, the volume density of myofibrils and glycogen granules as well as the volume, surface and numerical density of mitochondria were assessed by electron microscopic morphometrics. The volume density of myocyte compartment of the ventricular myocardia in developing fetuses decreased, but increased in newborn rats. On the other hand, the volume density of the interstitial compartment increased in growing fetuses and decreased in newborns. In all groups the volume, surface and numerical density of nuclei decreased gradually with elongation of myocytes. Conversely, the volume, surface and numerical density of mitochondria and volume density of myofibrils and glycogen granules in ventricular myocytes incresed. The increase in numerical density of mitochondria probably reflects an increase in metabolic activity. Sarcomere length also increased during development.
Hirschsprung's disease (HD) is usually diagnosed in the newborn period and early infancy. The common presentation of HD in newborns consists of a history of delayed passage of meconium within the first 48 hours of life. The differential diagnosis in newborns is one of the clinical challenges of this disorder. A number of medical conditions which cause functional obstruction of the intestines are easily excluded. Neonates with meconium ileus, meconium plug syndrome, distal ileal atresia and low imperforate anus often present in a manner similar to those with HD in the first few days of life. Abdominal radiographs may help to diagnose complete obstruction such as intestinal atresia. Microcolon on contrast enema can be shown in cases with total colonic aganglionosis, ileal atresia or meconium ileus. Suction rectal biopsy or frozen section biopsy at operation is essential for differential diagnosis in such cases. HD is also considered in any child who has a history of constipation regardless of age. Older children with functional constipation may have symptoms that resemble those of HD and contrast enema is usually diagnostic. However, children with other motility disorders generally referred to as chronic idiopathic intestinal pseudoobstruction present with very similar symptoms and radiographic findings. These disorders are classified according to their histologic characteristics.; visceral myopathy, visceral neuropathy, intestinal neuronal dysplasia (IND), hypoganglionosis, immature ganglia, internal sphincter achalasia. Therefore, the workup for motility disorders should include rectal biopsy not only to confirm the presence of ganglion cells but also evaluate the other pathologic conditions.
Purpose: In this study changes were observed in body temperature, heart rate and arterial oxygen saturation (SaO2) of newborns after bathing and to determine the effects of covering their heads with cotton hats after bathing. Methods: Participants were 58 newborn infants, 31 in the experimental group had their heads covered with cotton hats after their bath while 27 in the control group did not. Body temperature, arterial oxygen saturation and heart rate were measured at 8 consecutive times after bathing. Data were analyzed using t-test and repeated measures ANOVA. Results: Body temperature declined shortly after bathing. The experimental group showed faster recovery (p<.001). Heart rate increased after bathing in both groups. Heart rate in the experimental group decreased for 120 minutes and gradually increased to baseline (p<.001). In the control group, heart rate decreased for 180 minutes and then increased but did not reach the baseline (p<.001). Arterial oxygen saturation decreased shortly after bathing and recovery to the baseline was more rapid in the experimental group (30 minutes vs. 60 minutes) (p<.001). Conclusion: With significant changes observed in newborns' body temperature, arterial oxygen saturation and heart rate, covering the head right after bathing is effective in stabilizing infants' physiological system.
Purpose: This study was done to examine the incidence of pressure ulcers and associated factors, by inspecting the skin of newborn babies in a newborn unit or newborn intensive care unit (NICU). Methods: The participants were 101 newborn babies in either a newborn unit or NICU in D general hospital. The incidence of pressure ulcer was measured using the skin inspection tool for pressure ulcer, suggested by Agency for Health Care Policy and Research. Results: Incidence rate of pressure ulcer was 19.8%, and 80% of the newborns with pressure ulcers were premature babies. The commonest region of onset was the ear (36.8%), followed by the foot (31.6%), occipital region (15.8%) and knee (15.8%). Those are the regions related to external medical devices like nasal Continuous Positive Airway Pressure and Pulse Oximetry. Factors related to pressure ulcers were gestational period of 37 weeks or less, hospitalization for 7 days or more, birth weight under 2,500 g and a low level of serum albumin. Conclusion: The results of the study show that the skin and underlying tissues of premature infants is at risk for pressure-related skin breakdown. As most pressure ulcers are caused by medical devices, nursing interventions are required to prevent further aggravation of the lesions.
Purpose: The purpose of this study was to identify the current status and needs in neonatal emergency training for nursing students in Korea and to obtain preliminary information to develop a simulation based educational program on neonatal emergency care for nursing students. Methods: Structured questionnaires were distributed to five hospitals and ten nursing schools during April and May, 2012. Data were collected from 59 nurses who worked in the nursery or Neonatal Intensive Care Units and 13 nursing educators who had specialized in pediatric nursing. Results: Most nurses (86.4%) reported that they had experienced an emergency situation with newborns. Most nursing educators (84.6%) claimed that more intensive training with newborns is needed for nursing students. In particular, training in neonatal resuscitation (72.2%), respiratory distress (59.7%), and neonatal seizures (18.1%) were highly recommended as simulation based training for nursing students. Conclusion: A significant need for neonatal emergency educational programs was found. More efforts should be made to provide nursing students with knowledge and skills for working with neonates. The findings of this survey will ultimately provide a basis for developing a simulation based educational program on neonatal emergency care for nursing students.
