Comparison of the accuracy of neutrophil CD64 and C-reactive protein as a single test for the early detection of neonatal sepsis

  • Purpose: Early identification of neonatal sepsis is a global issue because of limitations in diagnostic procedures. The objective of this study was to compare the diagnostic accuracy of neutrophil CD64 and C-reactive protein (CRP) as a single test for the early detection of neonatal sepsis. Methods: A prospective study enrolled newborns with documented sepsis (n=11), clinical sepsis (n=12) and control newborns (n=14). CRP, neutrophil CD64, complete blood counts and blood culture were taken at the time of the suspected sepsis for the documented or clinical group and at the time of venipuncture for laboratory tests in control newborns. Neutrophil CD64 was analyzed by flow cytometry. Results: CD64 was significantly elevated in the groups with documented or clinical sepsis, whereas CRP was not significantly increased compared with controls. For documented sepsis, CD64 and CRP had a sensitivity of 91% and 9%, a specificity of 83% and 83%, a positive predictive value of 83% and 33% and a negative predictive value of 91% and 50%, respectively, with a cutoff value of 3.0 mg/dL for CD64 and 1.0 mg/dL for CRP. The area under the receiver-operating characteristic curves for CD64 index and CRP were 0.955 and 0.527 ($P$ <0.01), respectively. Conclusion: These preliminary data show that diagnostic accuracy of CD64 is superior to CRP when measured at the time of suspected sepsis, which implies that CD64 is a more reliable marker for the early identification of neonatal sepsis as a single determination compared with CRP.

Transfusion of red blood cells in neonatology

  • Blood transfusion in the neonatal period remains a therapeutic act, that no one dares to consider it as perfectly innocuous and that it is advisable to make rare, considering its risks as well immunological as infectious. The present work is a retrospective study, conducted in neonatal intensive care unit, in the University Hospital MOHAMED VI, Marrakech, during the period from January 1st to December 31st, 2019. All newborns were included in this work, who received one or multiple transfusions of red blood cells (RBC). Our study covered 60 neonates with a total of birth: 794 neonates, with a prevalence of 7.55%, hospitalized for various indications (60% of newborns were at term, 31.7% premature and 8, 33% were post-mature. The majority of transfusion procedures were performed during the first week of life. This is explained by the frequency of haemolytic anemias by fœto-maternal incompatibility. The main indications for transfusion were haemolysis, anemic syndrome and haemorrhagic syndrome. The average number of transfusion episodes was 1.95 +/- 1.47 per patient. Newborns were polytransfused in 18.33% of cases. A single transfusion accident was found in our study. However, we did not observe a correlation between the maternal diseases, the state of the newborn, and the transfusional indication.

Haptoglobin Concentration in the Cord Blood of Uninfected Korean Newborns

  • Objective: Highly sensitive haptoglobin measurement should be used in neonates because the haptoglobin concentration in neonates is lower than that of adults. The aim of this study was to establish the reference values of haptoglobin levels in the cord blood of uninfected neonates. Methods: The cord blood of 29 preterm and 51 term babies was collected, and data from the mother and the newborn were recorded. The haptoglobin concentrations of 80 cord blood samples were simultaneously measured by enzyme-linked immunosorbent assay (ELISA; Assaypro, St Charles, MO, USA) and immunoturbidimetry assay (Roche Diagnostics, Basel, Switzerland). C-reactive protein (CRP) was also measured by immunoturbidimetry assay (Roche Diagnostics, Switzerland). Results: Mean values of CRP and ELISA haptoglobin were not significantly different between preterm and term babies. The 2.5 percentile and 97.5 percentile values of ELISA haptoglobin concentration were as follows: 80 neonates, 0.01 mg/dL and 0.59 mg/dL; 29 preterm babies, 0.08 mg/dL and 0.18 mg/dL; and 51 term babies, 0.07 mg/dL and 0.23 mg/dL. There were no differences in ELISA haptoglobin concentration according to maternal underlying diseases, delivery method, usage of antibiotics or steroids before delivery, gestational age, gender of baby, or twin gestation. Conclusion: A highly sensitive haptoglobin method should be used to determine the haptoglobin concentration in Korean newborns because the reference values of cord blood haptoglobin concentration in Korean newborns are less than the lower detection limit for commonly used immunoturbidimetric haptoglobin measurement methods.

고위험 신생아 부모가 지각한 간호사의 의사소통과 부모 스트레스와의 관계 (The Relationship between Parental Stress and Nurses' Communication as Perceived by Parents of High-risk Newborns)

  • Purpose: This study aimed to characterize the relationship between parental stress and nurses' communication as perceived by parents of high-risk newborns in a neonatal intensive care unit (NICU). Methods: The participants were 54 parents of high-risk newborns in a NICU. Data were collected from January to March 2018. Parental stress and parents' perceptions of nurses' communication ability and styles were measured using a questionnaire. Results: The average scores for parental stress and nurses' communication ability were 3.39 and 4.38 respectively, on a 5-point scale. Parents most commonly reported that nurses showed a friendly communication style, followed by informative and authoritative styles. Mothers and fathers reported significantly different levels of parental stress. Parental stress showed a negative correlation with nurses' perceived verbal communication ability. Higher scores for nurses' verbal communication ability and for friendly and informative communication styles were associated with lower parental stress induced by the environment, the baby's appearance and behaviors, and treatments in the NICU. Conclusion: The findings of this study suggest that nurses need to offer proper information for parents and to support parents by encouraging them to express their emotions of stress and by providing parents with therapeutic communication and opportunities to participate in care.

Incidence of congenital hypothyroidism by gestational age: a retrospective observational study

  • Background: Congenital hypothyroidism (CH) is the leading cause of preventable physical and intellectual disabilities. This study aimed to assess the incidence and clinical characteristics of CH in newborns. Methods: We retrospectively reviewed the medical records of all newborns delivered at the Pusan National University Hospital between January 2011 and March 2021. The incidence of CH was compared according to gestational age, birth weight, and small for gestational age (SGA). The patients aged ≥3 years who could not maintain normal thyroid function and required levothyroxine treatment were diagnosed with permanent CH. Logistic regression analysis was performed to compare CH risks. Results: Of 3,722 newborns, 40 were diagnosed with CH (1.07%). Gestational age and birth weight were significantly associated with CH incidence. The odds ratios (ORs) of CH in infants delivered at 32-37, 28-31, and <28 weeks were 2.568 (95% confidence interval [CI], 1.141-5.778), 5.917 (95% CI, 2.264-15.464), and 7.441 (95% CI, 2.617-21.159) times higher, respectively, than those delivered at term. The ORs of CH in infants weighing 1,500-2,499 g, 1,000-1,499 g, and <1,000 g were 4.664 (95% CI, 1.928-11.279), 11.076 (95% CI, 4.089-29.999), and 12.544 (95% CI, 4.350-36.176) times greater, respectively, than those in infants weighing ≥2,500 g. The OR of CH was 6.795 (95% CI, 3.553-13.692) times greater in SGA than in non-SGA infants. Conclusion: The CH incidence in South Korea has increased significantly compared with that in the past. Gestational age, birth weight, and SGA were significantly associated with CH incidence.

신생아 Apgar Score의 시간적 변동에 관한 임상적 관찰 (Clinical Observation on Apgar Score Changing of the Newborn Infants)

  • This study was attempted for clinical investigation regarding with Apgar score changing phase of newborns and their sucking power related the score and weight. The data for this study obtained from 545 newborns who were born at Dept. of obstetric WooSok Hospital, Korea University from Jan. to Sept.. 1373. It is hoped that this study will contribute to nursing care planning of newborns in nursery. The Apgar score was checked in the delivery room at 1 minute after delivery by Doctor and rechecked at 30 minutes and 1 hour after delivery by nurses. The results obtained were as follows: 1. The total newborn numbers at 1 minute after delivery were 545, at 30 minutes were 542 (3 dead) and at 1 hour were 540. The babies were divided into 3 groups by the Apgar score. The frequency of group 1 (score 1 to 4) at each times were 5.0%, 0.9%, 0.4%, group 2 (score 5 to 7) 11.2%, 4.1%, 1.7% and group 3 (score 8 to 10) 83.8%, 95.0%, 97.9%. 2. The group I was found most frequently among the cases of less than 2, 500g and group 3 was found among the cases over 2.500g. It was found that there was a tendency of delaying in recovery process of low score among the premature and low birth weight newborns. 3. The type of delivery, breech and other I e was found worst and C-Section was the second, however normal spontaneous delivery was found the best. 4. In observation of sucking power of the newborn related its score and weight, good, fair, poor marks were given. But for convenience of statistical analysis. good and fair was collected together and poor alone evaluated. The result of group 1.2 examined statistically by F.E.T shown no relation between newborn weight and sucking power. however group 3, examined by X$^2$test shown very significant relation between newborn weight and sucking power. 5. The mortality rate of newborn while they were in hospital was found 2.6% to 545 of birth and all cases of dead belonged to group 1 at 1 minutes after delivery. 64.3% of the dead was found among the cases less than 2, 500g. 6. It was found that in this observation premature and Newborn asphyxia had influence to low score at birth. Conclusively, good anthemata carr should be emphasized in order to prevent main causes of these and should avoid abnormal delivery as possible.

정상 신생아의 체위에 따른 수면양상 및 행동변화 비교 (The Comparison of Sleep Characteristics According to the Sleep Positions in Healthy Newborns)

  • The purpose of this study was to compare the sleep characteristics between the prone and the supine position in healthy newborns. The 48 newborns were observed in the prone position and the supine position respectively on the 2nd day after birth. The data were collected from January to May, 1999. The state of a newborn was classified and categorized to 6 states (deep sleep. light sleep, drowsy, quiet alert, active alert, crying) by Barnard. The movements of eyes, face and extremities, pulse and arterial oxygen were observed and recorded continuously from the start of sleep after feeding until the time of being woken for the next feeding by a trained nurse The data was analyzed by using paired t-test. The results of this study were as follows; 1. There was no significant difference in the length of sleeping time between the prone and the supine position. 2. There was no significant difference in the length and frequencies of each states(deep sleep, light sleep, drowsy, quiet alert, active alert, crying) between the prone and the supine position But the frequency of light sleep in the supine position was significantly higher than that of the prone position. 3 There was no significant difference In the numbers or eyes movements between the prone and the supine sleep position. But the amount of facial and extremity movement in the supine position was significantly higher than those in the prone position 4. There was no significant difference in the arterial oxygen content between the prone and the supine sleep position. 5. There was no significant difference in the heart rates between the prone and the supine sleep position. The above results indicated that the newborns in the prone Position moved less and slept deeper than those in the supine position. though there was no difference in the length of sleep or arterial oxygen content between the prone and the supine sleep Position. But. Nurses and mothers should consider the relationship between the sleep Position and SIDS suggested by previous researches. The infant's 'awakening' during sleep is a normal process and rather valuable because it can provide an opportunity to promote a stronger relationship between mother and baby. So, It is suggested that the supine sleep position is better than the prone sleep position for infants.

  • Population screening of newborns is an extremely important and informative diagnostic approach that allows early identification of babies who are predisposed to the development of a number of serious diseases. Some of these diseases are known and have effective treatment methods. Neonatal screening enables the early diagnosis and subsequent timely initiation of therapy. This helps to prevent serious complications and reduce the percentage of disability and deaths among newborns and young children. Primary immunodeficiency diseases and primary immunodeficiency syndrome (PIDS) are a heterogeneous group of diseases and conditions based on impaired immune system function associated with developmental defects and characterized by various combinations of recurrent infections, development of autoimmune and lymphoproliferative syndromes (genetic defects in apoptosis, gene mutation Fas receptor or ligand), granulomatous process, and malignant neoplasms. Most of these diseases manifest in infancy and lead to serious illness, disability, and high mortality rates. Until recently, it was impossible to identify children with PIDS before the onset of the first clinical symptoms, which are usually accompanied by complications in the form of severe coinfections of a viral-bacterial-fungal etiology. Modern advances in medical laboratory technology have allowed the identification of children with severe PIDS, manifested by T- and/or B-cell lymphopenia and other disorders of the immune system. This review discusses the main existing strategies and directions used in PIDS screening programs for newborns, including approaches to screening based on excision of T-cell receptors and kappa-recombination excision circles, as well as the potential role and place of next-generation sequencing technology to increase the diagnostic accuracy of these diseases.

미숙아에서의 선천성 대엽성 폐기종 수술치험 1례 (Congenital Lobar Emphysema in Premature Infant)

  • Congenital lobar emphysema is an uncommon disease affecting newborns and infants with respiratory distress. This report describes a 1.9kg premature baby of 31weeks gestation who developed respiratory distress symptoms of congenital lobar emphysema involved left upper lobe.Left upper lobe lobectomy was carried out and good result was obtained. So, we present one case report with literature review.
