Objectives: In this case report, we will show the effectiveness of herbal medicine with neurofeedback treatment on social phobia. Methods: First, we diagnosed the patient by interview and questionnaire as having social phobia and suffering from anxiety, shaking and palpitation in public places and fear to meet new people. Beside, we examined the pattern of brainwaves by QEEG-8 system. Second, we treated the patient by herbal medicine with neurofeedback treatment for 4 months. Then we conducted new assessments by SCL-90-R, BDI, BAI and QEEG-8 system. Results: The patient's psychological and physiological symptoms were significantly improved. Conclusions: Herbal medicine with neurofeedback can be effective in the treatment of social phobia. Especially, it can be helpful to improve the patient's sensitivity to its physiological symptoms.

We propose Sequential Patient Recruitment Monitoring (SPRM), a new monitoring procedure for patient recruitment in a clinical trial. Based on the sequential probability ratio test using improved stopping boundaries by Woodroofe, the method allows for continuous monitoring of the rate of enrollment. It gives an early warning when the recruitment is unlikely to achieve the target enrollment. The packet data approach combined with the Central Limit Theorem makes the method robust to the distribution of the recruitment entry pattern. A straightforward application of the counting process framework can be used to estimate the probability to achieve the target enrollment under the assumption that the current trend continues. The required extension of the recruitment period can also be derived for a given confidence level. SPRM is a new, continuous patient recruitment monitoring tool that provides an opportunity for corrective action in a timely manner. It is suitable for the modern, centralized data management environment and requires minimal effort to maintain. We illustrate this method using real data from two well-known, multicenter, phase III clinical trials.
Design scheme drawings of the two major corporate hospitals - Asan Medical Center and Samsung Medical Center - are analysed in the aspect of floor area distributions and their proportions for various users and user-functions. Primary users of hospitals are categorized into three ; patients, employees and others. Then, patient spaces are subdivided into patient private, patient/medical, patient/support staff, patient/nursing, patient/guardian, patient common ; employee spaces into doctors, trainees, doctors common, nurses, support staffs, non-medical staffs, employee common ; others spaces into subcontractors, general publics, non-living, and internal circulation. The purpose of this research is to develop a user-focused insight into hospital spaces and a new method of hospital programming and design.
Recent research suggested that hospitals have not responded to consumer demand for the improved aesthetic and comfort of their patient garments. Thus a new patient garment has been developed considering human factors based on the Functional-Modesty-Aesthetics model. Based on the reviewed literature and the model, necessary design requirements were identified. Then the patient garment was evaluated in comparison to existing ones with regard to potential patients' satisfaction levels. As a result, the developed patient garment received better evaluations in the aspects of overall aesthetic satisfaction, color satisfaction, and willingness to wear than the existing patient garments.
The present study examines the relative roles of external recommendations and internally felt satisfaction in influencing patient loyalty to a doctor. It establishes that recommendations do result in preliminary loyalty formation in the patient to the doctor, but only until the formation of own experiences; the significance of external sources of recommendation in the determination of patient loyalty becomes insignificant thenceforth. Implications for the doctors are that they should strive at fostering bonds of emotional attachment in their present patients so that they become strongly loyal and spread positive word-of-mouth which could result in the doctor getting new patients as well. Probable extensions of this research are also discussed later in the paper.
1. Objectives We analyze Doctor-Patient Relationship in anthropological medicine, medical sociology Oriental Medicine and find out solutions about problems of Modern Medicine which are dehumanized through the meaning of Doctor-Patient Relationship in Sasang Constitutional Medicine. 2. Methods We consider various studies which are related with doctor-patient relationship from models of the Doctor-Patient Relationship, anthropological medicine, medical sociology and Oriental Medicine. And we compare and analyze Sasang Constitutional Medicine's meaning of doctor and patient through Sasang Constitutional Medicine's view of treatment and philosophy. 3. Results and Conclusions 1) In various studies, Doctor-Patient Relationship have been dualistic perspective which is divided into doctor and patient, and lay emphasis on relationship between them. 2) In Sasang Constitutional Medicine, anyone might be a patient because they are human being, anyone might be a doctor as cure their own disease. In other words, doctor and patient are not separated but united. A point of regarding human is the core of illness and cure can present new direction that solves problems of dualistic perspective Modern Medicine.
Background : Theoretically as the waiting-time of patient is estimated in queueing, many men and much money are needed. But being the estimation of patient s waiting-time very important in hospital service, so the continuous monitoring of waiting-time is inevitable. To verify that the estimation of waiting-time using parking time is economical, effective and continuous monitoring method and to develop utilizing the method, this study was done. Method : In parking confirmation office, the personnel of parking office wrote parking confirm time, chart number and whether medical examination and treatment finish or not in parking ticket. The next day the parking tickets were gathered and the above data were input. The input parking data were connected with the hospital outpatient file indexing chart number. Then the patient' s data for department, new patient or not, reservation or not, receipt time and payment time were retrieved. The group for finishing medical treatment were compared with that for not finishing in average time lag between confirmation and out-time for hospital. And In-time for hospital, receipt time, payment and out-time for hospital were also analyzed. Result : Confirming parking ticket, the group for finished medical treatment left hospital after 7 minutes. This result showed that the patient for finished medical treatment left hospital immediately. So parking time was reasonable to estimation of hospital-time was concluded. The time for medical treatment, diagnosis and test was constant for all patients and short for waiting time, Then I concluded that the parking time was reasonable for estimation patient's waiting time. Overall patient's waiting time was 113 minutes and new patient's time was 149 minutes, old patient's times was 109 minutes. Waiting time for reservation patient was 98 minutes and for non reservation patient was 122 minutes. The time from hospital arrival to payment was 50 minutes for the group of reservation patient and 69 minutes for non-reservation group. The time from payment to hospital leaving was 51 minutes and 56 minutes for non reservation group. The short time difference between reserved group and not reserved group from payment to hospital leaving time was due to bottle neck effect. Conclusion : The estimation of patient's waiting time using parking time was reasonable because the possession of car was common and the time for medical treatment was equal and the patient after treatment left hospital immediately. Using this method, timely, fast evaluation and continuous monitoring of the intervention effect were possible.
A group of studies revealed that family members of the seriously ill patient had some needs during the period of patient's hospitalization. Needs of the seriously-ill patient family could be classified into three aspect, i. e. needs on the prognosis of patient, needs on the well-being of family members themselves and needs on hospital environment. Several instrument were developed to measure the needs of the seriously ill patient, but their content tended to overemphasize the aspect of the prognosis of patient. The purpose of this study was to develop a new instrument to measure the needs in the aspect of the seriously ill patient's family themselves in details to increase the cummulative Percentage of the scale. Subjects were 134 family members of the seriously-ill patients, who were bung cared in ICU of seven university hospitals and data were collected from march 16, 1987 to April 11, 1987. The instrument used in this study was made by the author on the basis of results of literature review. Content valitity of the instrument was tested by a professor majoring in nursing and reliability by calculation of Cronbach's α with data of the respondents. Data was analyzed as follows, using SAS, computer system. Factor structures of the seriously ill patients' family needs were elicited by factor analysis. The programme was the Principal Component Factor Analysis Method of factoring employing Varimax Orthogonal Rotation. The influences of the demographic variables on the degree of the seriously-ill patients' family need were analyzed by t-test and P-test. Results were as follows. 1. Needs of the seriously-ill patients' family elicited and their cummulative percentage were: Needs to be supported emotionally, 29.2% Needs to be informed on facilities available, 9.6% Needs to be informed on the patient's prognosis, 7.7%. Needs to be supported spiritually, 5.1%. Needs to be informed on hospital environment, 4.2%. Needs to be helped to spare time, 3.9%. Needs to be informed on their role, 3.5%. Needs to be present near patient, 3.3%. 2. Educational background was found to influence on some factors. College-graduate group had higher Bevel of needs to be . informed on facilities available to be informed on the patient's prognosis, needs to be supported spiritually, needs to be helped to spare time, needs to be informed on their role than high school graduate group. 3. Among the parents, sons and daughters and the relative groups, the parents of the seriously ill patient had highest level of needs to be present near patient. Suggestions for further studies were as follows. 1. As the instruments used in the previous studies had high cummulative percentages in the aspect of the prognosis of patient and that in this study in the aspect of the prognosis of patient and that in this study in the aspect of the seriously ill patient's family themselves, development of a new instrument which combined the items of both of them is needed. 2. A study to clarify the influence of type and number of admission on need to be supported emotionally is needed.
Purpose: The purpose of this study was to investigate perception of patient safety culture among hospital nurses. Methods: There were four steps in this study; education about patient safety culture, pre-test, nursing activities for patient safety, post-test. A questionnaire was distributed twice to all nurses in one hospital. Pretest data were collected from April 1 to April 20 and posttest from November 15 to November 25, 2013. For the pretest data, 302 data sets were analyzed and for the posttest, 266. SPSS 12.0 was used for descriptive analysis. Results: Overall perception of nurses on patient safety culture was "moderate"(3.27). For general characteristics, there was a significant difference in patient safety culture according to work unit and length of employment. Attitude to leaders was significantly different according to nurses' age, position and work unit. Organizational culture was significantly different according to nurses' age and work unit. System of patient safety was significantly different according to work environment. In the posttest, the mean score improved. Conclusion: Results indicate that patient safety cultural perception is related to safety during nursing activities and systematic strategies to increase perception should be expanded through research and the development of new educational programs on patient safety culture.
Patient safety remains one of the most important health care issues in Korea. To improve patient safety, we have introduced concepts from the field of safety science such as the Swiss cheese model, and adopted several methodologies previously used in other industries, including incident reporting systems, root cause analysis, and failure mode and effects analysis. This approach has enabled substantial progress in patient safety to be made through undertaking patient safety improvement activities in hospitals that are systems-based, rather than individual-based. However, these methods have the shared limitation of focusing on negative consequences of patient safety. Therefore, the paradigm shift from Safety I to Safety-II in safety science becomes the focus of our discussion. We believe that Safety-II will complement, rather than replace, Safety-I in the discipline of patient safety. In order to continuously advance patient safety practices in Korea, it is necessary that Korea keeps abreast of the recent global trends and development in safety science. In addition, more focus should be placed on testing the feasibility of new patient safety approaches in real-world situations.
